N Engl J Med 1986; 314:1547-52Background Into the mid-1980's, digoxin and diuretics were the mainstay of chronic disease management for congestive heart failure. Vasodilator agents were also commonly used based on limited data of their favorable hemodynamic effects. No sufficiently powered trials in this space had been performed to assess whether administration of vasodilators or any other agents improved long-term morbidity or mortality for heart failure patients. The V-HEFT trial was undertaken to test the hypotheses that 2 widely used vasodilator regimens (prazosin or a combination of hydralazine and isosorbide dinitrate) were superior for reducing death versus placebo. The trial was sponsored by the Veterans Administration and only enrolled men.Patients Men between the ages of 18 and 75 were recruited from 11 participating Veterans Administration hospitals and had to have chronic congestive heart failure based on either evidence of cardiac dilatation or left ventricular dysfunction (EF 0.7 ng/mL and euvolemic volume status. Clinical evaluations and exercise-tolerance tests on 2 consecutive visits, two weeks apart, had to reveal clinical and exercise stability before randomization could occur. Following randomization, patients continued to receive the optimal dose of digoxin and diuretic along with 1 of 3 study regimens. The placebo group was given placebo tablets and placebo capsules and instructed to take them 4 times a day. The prazosin group took 2.5 mg prazosin capsules and placebo tablets 4 times a day. The hydralazine-isosorbide dinitrate group took 37.5 mg hydralazine capsules and 20 mg isosorbide dinitrate tablets 4 times a day.In all groups, therapy began with 1 capsule and 1 tablet to be taken 4 times a day. In the absence of side effects, this was increased to 2 capsules and 2 tablets 4 times a day for a total of 20 mg of prazosin or 300-160 mg of hydralazine-isosorbide dinitrate. If drug-related side effects occurred, the dose could be reduced to half a tablet 4 times per day or to one capsule 2 times per day. If the dose was reduced, an attempt was made later to reinstitute the full dose.In order to limit dropouts, rigorous criteria were established for “treatment failures.” Physicians were advised to hospitalize patients with worsening symptoms, and, if appropriate, to use temporary intravenous vasodilator or inotropic interventions for stabilization. Physicians were encouraged to resume study medications upon discharge. At least 2 such hospitalizations were required, along with objective evidence of deterioration, before the study medications were discontinued and replaced with known therapy.Endpoints The primary endpoint was all-cause mortality.Results 642 patients were enrolled (273 in placebo group, 183 in prazosin group and 186 in the hydralazine-isosorbide dinitrate group). Excluding discontinuations that took place within 1 month before death, 47 patients (17%) discontinued one or both types of placebos, 43 patients (23%) discontinued prazosin, and 60 patients (32%) discontinued either one or both drugs in the hydralazine-isosorbide group. Six months after randomization, the average prescribed doses were 18.6 mg per day of prazosin, 270 mg per day of hydralazine, and 136 mg per day of isosorbide dinitrate. More than 85% of the prescribed drugs were taken in each treatment group.The mean follow-up was 2.3 years (range 6 months to 5.7 years). Only 4 patients were lost to follow up (2 in placebo group, 1 in prazosin group, and 1 in hydralazine-dinitrate group). There were 120 deaths in placebo group (44%; 19 per 100 patient years), 91 in the prazosin group (50%; 22 per 100 patient years), 72 in the hydralazine-dinitrate group (39%; 17 per 100 patient years). A reduction in mortality over the entire follow-up period was observed in the hydralazine-nitrate group compared with placebo (p = 0.093 on the log-rank test and p = 0.046 on the generalized Wilcoxon test, which gives more weight to treatment differences occurring in the earlier part of the mortality curves and less weight to the latter part, where the numbers are smaller). The absolute difference in mortality between these groups increased during three years and then began to diminish. The absolute difference in mortality between the placebo group and hydralazine-isosorbide groups at years 1 through 4 was 7%, 9%, 11% and 4%, respectively.Prespecified subgroup analysis in CAD vs no CAD stratification showed no significant treatment effect heterogeneity for hydralazine-nitrate among those with CAD although the absolute difference in mortality between groups was numerically higher for patients with CAD.At 8 weeks and 1 year, SBP (-4.1 and -4.6 mmHg) and DBP (-3.2 and -2.7 mmHg) decreased the most in the prazosin group compared to placebo. Hydralazine-nitrate was not associated with a statistically significant nor clinically significant difference in BP with exception of DBP at 8 weeks. The EF rose significantly at 8 weeks and 1 year in the hydralazine-nitrate group (+2.9 and +4.2) compared to placebo but not in the prazosin group.Side effects were reported in 4.0% of placebo patients, 11% of prazosin patients and 19% of hydralazine-nitrate patients, respectively. The most common side effects were headache and dizziness. Headache was reported in 12% of hydralazine-nitrate patients.Conclusions This study compared the combination of hydralazine-isosorbide dinitrate or prazosin to placebo in patients with chronic congestive heart failure who were optimized on digoxin and diuretic therapy. In what appears to be a young (58 years) and highly selected population of clinically stable, male veterans with dilated cardiomyopathies and low symptom burdens, the combination of hydralazine-isosorbide reduced death by 2 per 100 patient years, increased EF by 4% at 1 year and did not significantly alter BP compared to placebo. Side effects were reported in approximately 1 out of 5 patients with the most common being headache and approximately 1 out of 3 discontinued 1 or both study drugs. Prasozin did not reduce death or increase EF but did reduce BP compared to placebo. The internal validity of the study is high with only a few minor imbalances in baseline characteristics, which do not appear clinically relevant nor to consistently favor any one group. Less than 1% of patients were lost to follow up with no significant imbalances between groups. The external validity is limited by the fact that this is a population of male veterans and the etiologic distribution of cardiomyopathy and heart failure is likely different from a general heart failure population; etiologic causes of death are also likely to be different. Furthermore, the population is highly selected and its unclear how many patients from the general heart failure population would meet study criteria.Cardiology Trial's Substack is a reader-supported publication. Nếu có một nước nào phải theo dõi sát sao và với đầy nỗi lo lắng một trục Nga – Trung ngày càng được củng cố, thì đó chính là Nhật Bản. Bị giới hạn về địa lý và chịu sức ép từ gánh nặng lịch sử, Nhật Bản có nguy cơ đối mặt cùng lúc với hai đối thủ cường quốc hạt nhân, thậm chí có thể là ba, nếu bao gồm cả mối đe dọa Bắc Triều Tiên trong vùng Đông Á. Nỗi lo này đã được thể hiện rõ trong Sách Trắng quốc phòng thường niên « Quốc phòng Nhật Bản 2024 », được công bố ngày 12/07/2024, cho rằng Nhật Bản « đang phải đối mặt với một môi trường an ninh nghiêm trọng và phức tạp nhất kể từ khi kết thúc Đệ Nhị Thế Chiến ».Lần đầu tiên Tokyo cảnh báo trực tiếp về « khả năng một tình huống nghiêm trọng tương tự như cuộc xâm lược Ukraina của Nga xảy ra ở khu vực Ấn Độ - Thái Bình Dương trong tương lai, đặc biệt là ở vùng Đông Á », ám chỉ đến những hoạt động quân sự của Trung Quốc xung quanh đảo Đài Loan.Đài Loan và mối lo an ninh cho Nhật BảnTheo nhận định của Yoshinaga Kenji, cựu sĩ quan tình báo của Cơ quan Điều tra An ninh và Lực lượng Phòng vệ Biển Nhật Bản, với trang The Diplomat, « phần đầu của Sách Trắng quốc phòng năm nay, mô tả tình hình an ninh xung quanh Nhật Bản, là phần căng thẳng nhất trong lịch sử Sách Trắng, khi xét đến cuộc chiến xâm lược kéo dài của Nga tại Ukraina, áp lực quân sự ngày càng lớn của Trung Quốc đối với Đài Loan và việc Bắc Triều Tiên triển khai vũ khí hạt nhân chiến thuật ».Lập trường đối ngoại cứng rắn cùng với đà trỗi dậy mạnh mẽ năng lực quân sự của Bắc Kinh đặt Tokyo trước một thách thức chiến lược lớn chưa từng có. Bộ trưởng Quốc Phòng Nhật Bản Kihara Minoru mở đầu Sách Trắng với đánh giá « không quốc gia nào có thể tự bảo vệ an ninh của mình ». Ông nhấn mạnh đến sự cần thiết tăng cường hợp tác với các quốc gia có cùng chí hướng mà Tokyo chia sẻ các giá trị phổ quát và lợi ích chiến lược.Nhật Bản bày tỏ « quan ngại nghiêm trọng » về các hoạt động quân sự của Trung Quốc trên toàn khu vực xung quanh Nhật Bản, từ biển Hoa Đông – đặc biệt xung quanh quần đảo Senkaku đang tranh chấp chủ quyền với Trung Quốc – cho đến biển Nhật Bản, Tây Thái Bình Dương, nhất là xung quanh Đài Loan, trước nguy cơ « xảy ra căng thẳng quân sự giữa Trung Quốc và Đài Loan, do những hoạt động quân sự ngày càng tăng từ phía Bắc Kinh ».Về điểm này, nhà nghiên cứu địa chính trị Valérie Niquet, chuyên gia về Trung Quốc thuộc Quỹ Nghiên cứu Chiến lược FRS, trong một chương trình tranh luận trên đài phát thanh France Culture (18/05/2024), đưa ra một số phân tích :« Đài Loan có một vị thế quan trọng cả về mặt ý thức hệ, cho thấy một mô hình dân chủ vẫn có thể tồn tại trong thế giới Trung Hoa, điều mà Tập Cận Bình hoàn toàn phủ nhận, và cả về mặt chiến lược, vì Đài Loan là chốt chặn lối ra Thái Bình Dương và do vậy không thể để cho Đài Loan rơi vào tay Bắc Kinh. Đó là chưa nói đến người dân.Câu hỏi lớn được đặt ra ở đây là mối quan hệ chiến lược quốc phòng giữa Nhật Bản với Hoa Kỳ. Các căn cứ chính của Mỹ ở châu Á đều nằm ở Nhật Bản và chủ yếu ở quần đảo Okinawa, nằm ở tuyến đầu đối diện với Đài Loan, (với 53.000 lính Mỹ).Hòn đảo cuối cùng, Yonaguni, nằm gần nhất với đảo cực tây của Nhật Bản, chỉ cách bờ biển Đài Loan khoảng 100 km, nên thực sự Nhật Bản ở trên tuyến đầu. Hơn nữa, khi Trung Quốc tập trận để gây áp lực với Đài Loan, vào lúc bà Nancy Pelosi thăm Đài Loan cách đây không lâu, 5 hay 6 quả tên lửa, tôi quên mất con số chính xác, đã rơi vào vùng đặc quyền kinh tế ở Biển Nhật Bản. Đây rõ ràng còn là lời cảnh báo đối với Nhật Bản, gây áp lực buộc nước này không được đi quá xa. »Nga - Trung hợp tác quân sự : « Cơn ác mộng » cho Nhật BảnNhưng điều làm Tokyo đặc biệt lo lắng là khả năng Bắc Kinh và Matxcơva siết chặt hơn nữa quan hệ hợp tác quân sự. Tokyo xem những cuộc tập ném bom chung và tuần tra hải quân chung thường xuyên giữa hai cường quốc này là « nhằm mục đích phô trương sức mạnh chống Nhật Bản ».Đây không phải là lần đầu tiên Nhật Bản cho thấy nỗi bất an về quan hệ hợp tác Nga - Trung. Trang South China Morning Post ngày 27/11/2023 trích dẫn một báo cáo an ninh của Viện Nghiên cứu Quốc Phòng (NIDS), một tổ chức tư vấn trực thuộc bộ Quốc Phòng Nhật Bản, cho thấy một trong những nỗi lo sợ lớn nhất của Tokyo là hai cường quốc này thực sự trở thành đồng minh quân sự.Dưới thời thủ tướng Shinzo Abe (2012-2020), Nhật Bản đã nỗ lực thiết lập một chính sách đối ngoại hòa dịu với Nga. Cố thủ tướng Abe đã có gần 30 cuộc gặp với tổng thống Vladimir Putin nhằm gầy dựng một mối quan hệ tin cậy, một mặt là để giải quyết các tranh chấp chủ quyền trên quần đảo Kuril, dưới sự kiểm soát của Nga từ năm 1945.Vùng « Lãnh thổ phương bắc » này, theo cách gọi của Nhật Bản, có một vị trí chiến lược, nằm ở phía bắc đảo Hokkaido và là chốt chặn cửa biển Okhotsk. Nhưng nỗ lực này của ông Abe chẳng mang lại kết quả : Nga không những không nhượng một tấc đất lãnh thổ nào mà còn tăng cường sự hiện diện quân sự trên quần đảo, bố trí 3500 binh lính và nhất là lắp đặt các hệ thống tên lửa địa đối không S-300 vào cuối năm 2020.Mặt khác, theo phân tích của nhà địa chính trị học Céline Pajon, Viện Quan hệ Quốc tế Pháp (IFRI) trên báo Libération ngày 25/03/2022, ông Abe còn nhắm đến mục tiêu chiến lược xa hơn. Khi tìm cách xích lại gần Putin, thủ tướng Nhật Bản hy vọng tránh để Nga rơi vào quỹ đạo Trung Quốc, hay chí ít là ngăn chặn việc hình thành một mặt trận Nga - Trung chống Nhật Bản trong các vấn đề lãnh thổ và lịch sử.Một lần nữa ông Abe lại gặp thất bại. Matxcơva lệ thuộc ngày càng nhiều vào Bắc Kinh về kinh tế và hai bên có những cam kết hợp tác quân sự chưa từng có. Quân đội Nga và Trung Quốc tiến hành nhiều cuộc tập trận quy mô lớn tại vùng Viễn Đông, tổ chức các cuộc tuần tra chung xung quanh Nhật Bản, khiến Tokyo phải lo lắng.Trước nguy cơ phải đối mặt với cùng lúc hai cường quốc hạt nhân Nga – Trung trong các vấn đề tranh chấp lãnh thổ, và thậm chí có thể có bên thứ ba là Bắc Triều Tiên, tháng 12/2022, chính phủ của thủ tướng Fumio Kishida công bố ba tài liệu chiến lược quan trọng : Chiến lược An ninh Quốc gia (NSS), Chiến lược Phòng thủ Quốc gia (NDS) và Chương trình Tăng cường Phòng thủ (DBP).Theo nhiều nhà quan sát tại Pháp, cuộc chiến xâm lược Ukraina do Nga phát động đã có những tác động đáng kể đến việc vạch ra chiến lược an ninh cho Nhật Bản. Tài liệu NSS đánh giá Trung Quốc như là một « thách thức chiến lược chưa từng có » và xem Bắc Triều Tiên là một « mối đe dọa nghiêm trọng hơn và sắp xảy ra ». Nhưng, nước Nga, không giống như trong phiên bản NSS năm 2013, cùng sự phối hợp chiến lược của Matxcơva với Bắc Kinh, giờ được xác định là « mối quan tâm sâu sắc về an ninh ».AUKUS: Giải pháp để thoát gọng kềm Nga – Trung ?Nhật Bản còn thông báo tăng đáng kể ngân sách quốc phòng với mục tiêu từ đây đến năm 2027 đạt mức chi tiêu quân sự 2% GDP như các nước thành viên Liên minh Quân sự Bắc Đại Tây Dương NATO. Đáng chú ý là lần đầu tiên Nhật Bản cho biết tham vọng sở hữu năng lực phản công, để nước này thực hiện các cuộc tấn công trả đũa nhằm vào lãnh thổ đối thủ.Giáo sư Tsuyoshi Goroku, ngành Quan hệ Quốc tế và Kinh tế, trường đại học Nishogakusha, trong cuộc trả lời phỏng vấn dành riêng cho Quỹ Nghiên cứu Chiến lược (FRS) của Pháp hồi tháng 3/2024, lưu ý bước tiến lịch sử này, « không phải là một sự đảo ngược đột ngột các nguyên tắc chính sách quốc phòng truyền thống của đất nước. Đúng hơn đó là một sự thừa nhận của Tokyo về môi trường an ninh ngày càng xấu đi của Nhật Bản. Điều này là kết quả của một loạt những thay đổi tăng dần trong suốt hai thập niên qua. Tuy nhiên, việc Nga vô cớ xâm lược Ukraina là một lời cảnh báo cho Nhật Bản, củng cố những đánh giá ảm đạm và thúc đẩy hơn nữa những thay đổi này ».Vị giáo sư Nhật Bản thừa nhận đất nước ông đã hưởng được một nền an ninh và sự thịnh vượng trong khuôn khổ trật tự quốc tế dựa trên các luật lệ. Nhưng việc Nga và Trung Quốc có tham vọng thay đổi trật tự đó khiến Nhật Bản cảm thấy bất an. Trong suốt hai năm qua, thủ tướng Fumio Kishida nhiều lần cảnh báo « Ukraina hôm nay rất có thể sẽ là Đông Á ngày mai » và « an ninh của châu Âu và vùng Ấn Độ - Thái Bình Dương là không thể tách rời ».Và cảm giác bất an này dường như được đông đảo công luận Nhật Bản chia sẻ. Giáo sư Tsuyoshi Goroku giải thích :« Sau cuộc xâm lược của Nga, một số cuộc thăm dò dư luận cho thấy công chúng Nhật Bản ngày càng lo sợ an ninh của Nhật Bản đang bị đe dọa. Ngoài ra, một cuộc khảo sát do tờ Nikkei thực hiện vào tháng 3 năm 2022 cho thấy 77% số người được hỏi "lo ngại" nếu cộng đồng quốc tế không ngăn chặn được cuộc xâm lược của Nga và việc thay đổi biên giới, điều này sẽ dẫn đến việc Trung Quốc sử dụng vũ lực chống Đài Loan. Liệu cuộc xâm lược của Nga và thành công có thể của cuộc chiến này có khuyến khích Trung Quốc sử dụng vũ lực hay không, đó vẫn còn là vấn đề tranh luận, nhưng công chúng Nhật Bản đã thể hiện rõ những lo ngại này. »Trong hai ngày 16 - 17/05/2024, tổng thống Nga Vladimir Putin đã đến thăm Bắc Kinh gặp chủ tịch Trung Quốc Tập Cận Bình, cuộc gặp lần thứ 43 kể từ khi ông Tập Cận Bình lên cầm quyền năm 2012. Trong chuyến thăm này, tổng thống Nga đã đến thăm Harbin, nơi có Viện Công nghệ Quân sự Trung Quốc. Một chuyến thăm mang tính biểu tượng cao, một tín hiệu mạnh mẽ gởi đến Nhật Bản và các nước phương Tây : Nga và Trung Quốc đang thắt chặt quan hệ hợp tác, kể cả về công nghiệp vũ khí.Trong bối cảnh này, Nhật Bản quan ngại về các hoạt động quân sự của Nga và sự phối hợp chiến lược của Nga với Trung Quốc trong vùng Ấn Độ - Thái Bình Dương. Chiến tranh Ukraina kéo dài, Bắc Triều Tiên có thể sẽ can dự với việc cung cấp tên lửa và đạn dược cho Nga. Mối quan hệ hợp tác quân sự Matxcơva – Bình Nhưỡng càng làm cho môi trường an ninh Nhật Bản thêm phần u ám.Đối diện với những mối đe dọa an ninh chưa từng có này, thủ tướng Fumio Kishida đã gia tăng các hoạt động ngoại giao, thiết lập các mối quan hệ chiến lược với nhiều nước từ châu Á đến châu Âu, từ việc mở rộng quan hệ đối tác với NATO, cho đến việc để ngỏ khả năng tham gia liên minh quân sự AUKUS, hiện quy tụ ba nước Anh, Mỹ và Úc. Về điểm này, nhà địa chính trị học Valerie Niquet nhận định :« Ưu điểm lớn của AUKUS so với các đối tác khác: đây là một liên minh Anglo-Saxon và do vậy sẽ không làm Washington tức giận, vì Úc và Anh là những đồng minh rất thân thiết của Mỹ và Nhật Bản đã ký kết những thỏa thuận hợp tác trao đổi, chẳng hạn như tạo điều kiện thuận lợi cho các cuộc tập trận chung với cả Úc và Anh.Hơn nữa, chúng ta không thể quên một thực tế là việc tham gia một phần vào liên minh AUKUS, không liên quan đến khía cạnh hạt nhân, mà có lẽ là trụ cột đầu tiên như người ta nói về hợp tác công nghệ đối với Nhật Bản, cũng có yếu tố uy tín theo nghĩa Nhật Bản trở lại với trường quốc tế với tư cách là một tác nhân chính, cả trong lĩnh vực công nghiệp quân sự, điều vẫn chưa có trên phương diện xuất khẩu vũ khí ».

Lancet 1990;336:65-71.Background Large trials up to this point had established the role of thrombolytic therapy and aspirin in patients with acute MI. The next question centered on the different types of thrombolytic agents as well as the merits of adding high dose heparin to aspirin following revascularization. Data from smaller studies showed that alteplase (tPA) had a higher recanalization rate at 90 minutes compared to streptokinase (SK). “Recanalization” is a surrogate endpoint – a person doesn't necessarily care whether their artery is open or not at 90 minutes, but instead, whether they live or die, and how they live in the aftermath of a heart attack. The GISSI-2 trial sought to test the hypothesis that tPA would reduce the composite hard endpoint of mortality and extensive LV dysfunction compared to streptokinase (SK) and that heparin plus aspirin compared to aspirin alone would do the same.Cardiology Trial's Substack is a reader-supported publication. To receive new posts and support our work, consider becoming a free or paid subscriber.Patients Patients were eligible who presented with chest pain and ST segment elevations of ≥1 mm in any limb lead of the ECG and/or ≥2 mm in any precordial leads and were admitted to the CCU within 6 hr from the onset of symptoms. Absolute contraindications included: recent or current bleeding, stroke within the previous 6 months, a surgical or invasive procedure or trauma within the previous 2 weeks, uncontrolled hypertension defined as SBP ≥200 mmHg or DBP ≥110 mmHg, or previous treatment with SK within 6 months.Baseline characteristics The number of patients who were admitted to CCUs with acute myocardial infarctions (MI) over the study period was 38,086 and 12,490 (33%) were ultimately enrolled. Reasons for exclusion included: more than 6 hr from onset of symptoms (61%), contraindications to fibrinolysis (15%), unlikely to have acute MI (13%), administrative reasons (5%), ST depressions (4%) and 1% were not reported. Similar to GISSI-1, which also provided information on patients enrolled versus those who were not, women were more likely to be excluded (30% of excluded vs 20% of enrolled) as were patients >70 years of age (41% of excluded vs 22% of enrolled). The death rate of those excluded was higher (11% of excluded vs 9% of enrolled).Approximately 80% of patients enrolled were men under the age of 70. Patients with inferior (34%) and anterior STEMI's (31%) composed more than half of the cohort. 72% of patients presented within 3 hours of symptom onset and over 95% had Killip scores of II or below with the vast majority being Killip I (78%).Procedures Immediately following randomization half of all patients received either 1.5 MU of SK infused over 1 hour or 100 mg of tPA infused over 3 hours. Since this was a 2x2 factorial design, half of all patients also received 12,500 U of subcutaneous heparin twice daily starting 12 hours after the beginning of the tPA or SK infusion and to be continued until hospital discharge.All patients without specific contraindications were recommended to receive oral aspirin 325 mg/day and 5-10 mg of IV atenolol, even before randomization, as soon as evolving MI was diagnosed.Endpoints The combined primary endpoint consisted of all-cause mortality plus the number of patients who had late (day 4 or later) clinical congestive heart failure, or extensive LV damage (LVEF

Lancet 1986;397-401Background This study by the Italian Group for the Study of Streptokinase in Myocardial Infarction (GISSI) is the first large trial to test the hypothesis that opening blocked heart arteries, in this case via use of the intravenous thrombolytic agent Streptokinase, reduces death in patients presenting with acute myocardial infarction (MI). Opening blocked arteries or revascularization is now a foundational element of cardiovascular medicine that arguably spurred much of the growth in the field over the 1990's and early 2000's. Therefore, it is critical to revisit this historic trial and appreciate the lessons it taught us.Patients Patients were eligible who presented with chest pain and ST segment elevations of ≥1 mm in any limb lead of the electrocardiogram and/or ≥2 mm in any precordial leads and were admitted to the cardiac intensive care unit (CCU) within 12 hours from the onset of symptoms. Absolute contraindications included recent or current bleeding, stroke within the previous 2 months, a surgical procedure or trauma within the previous 10 days, invasive procedures within the previous 10 days, SBP ≥200 mmHg, DBP ≥110 mmHg, previous treatment with streptokinase, or any other life-threatening condition.Baseline characteristics The number of patients who were admitted to CCUs with acute MIs over the study period was 31,826 and 11,806 (37%) were ultimately enrolled. Reasons for exclusion included being more than 12 hr from onset of symptoms (51%), contraindications to Streptokinase (21%), unlikely to have acute MI (19%), administrative reasons (8%) and 2% were not reported. Notably, women were more likely to be excluded (27% of excluded vs 20% of enrolled) as were patients >75 years of age (21% of excluded vs 11% of enrolled).Not a lot of demographic information is provided on the cohort in the main manuscript but it was composed primarily of men (80%) who were ≤65 years of age (65%). Executive Director Alice Clark and Program Director Lindsey Payne Johnstone joined us to tell about the role DBP plays in keeping our downtown core vibrant and thriving.  The Downtown Bellingham Partnership (a 501(c)(3) nonprofit) is famous for is events - First Fridays, Wine Walk, and especially the iconic Downtown Sounds, a free-admission outdoor concert series which draws thousands of people and closes three city blocks over five Wednesdays in the summer.  But the DBP crew is responsible for so much more: administrating micro grants for storefront improvements, public art efforts, graffiti cleanup, spring floral baskets, and safety patrols. Over the past three years, they've also seen their role expand further into business development support and advocacy, including networking with other downtown organizations down the I-5 corridor. Potatoes have developed a reputation for causing weight gain and an increased risk for type 2 diabetes, and often find themselves on a list of foods to avoid, especially for individuals with insulin resistance. However, a new study from Pennington Biomedical Research Center at Louisiana State University, published in the Journal of Medicinal Food, says that potatoes actually did not increase that risk, are filled with key nutrients, and packed with health benefits. Candida Rebello, PhD, an assistant professor at Pennington Biomedical, said, “We demonstrated that contrary to common belief, potatoes do not negatively impact blood glucose levels. In fact, the individuals who participated in our study lost weight.” “People tend to eat the same weight of food regardless of calorie content in order to feel full,” Rebello explained. “By eating foods with a heavier weight that are low in calories, you can easily reduce the number of calories you consume. The key aspect of our study is that we did not reduce the portion size of meals but lowered their caloric content by including potatoes. Each participant's meal was tailored to their personalized caloric needs, yet by replacing some meat content with potato, participants found themselves fuller, quicker, and often did not even finish their meal. In effect, you can lose weight with little effort.” The study involved 36 participants between the ages of 18 and 60 who were overweight, had obesity, or insulin resistance. Insulin resistance refers to a health condition in which the body's cells do not respond well to insulin and glucose does not enter into the cells to make energy. Insulin resistance is linked to obesity, high blood pressure, high cholesterol, and type 2 diabetes Participants were fed precisely-controlled diets of widely available common foods including either beans, peas, and meat or fish, or white potatoes with meat or fish. Both diets were high in fruit and vegetable content and substituted an estimated 40% of typical meat consumption with either beans and peas or potatoes. Previous studies have shown that eating beans and peas improves blood glucose levels in individuals with newly diagnosed type 2 diabetes. To increase the dietary fiber component of the potatoes, they were boiled with the skin intact and then refrigerated between 12 and 24 hours. Potatoes were incorporated into the main lunch and dinner entrées, and served together with sides such as mashed potatoes, oven-roasted potato wedges, potato salad, and scalloped potatoes with lunch and dinner entrees. “We prepared the potatoes in a way that would maximize their fiber content. When we compared a diet with potatoes to a diet with beans and peas, we found them to be equal in terms of health benefits,” Rebello said. “People typically do not stick with a diet they don't like or isn't varied enough. The meal plans provided a variety of dishes, and we showed that a healthy eating plan can have varied options for individuals striving to eat healthy. In addition, potatoes are a fairly inexpensive vegetable to incorporate into a diet.” (NEXT) Celery Seed Extract Lowers High Blood Pressure in Human Study University of Chicago Medical Center, November 8, 2022 Drugs to lower blood pressure are among the most commonly prescribed medicines in the U.S. More than 678 million blood pressure prescriptions were filled in 2010. All of the current classes of blood pressure lowering drugs possess significant side effects.A recent study with a special extract of celery seed extract indicates that it may produce clinical results without producing the side effects that plague current drug treatment. Celery seed extract contains an important compound known as 3-n-butylphthalide, or 3nB for short, that is also responsible for the characteristic flavor and odor of celery. 3nB was discovered as the active component of celery in response to investigations by researchers seeking to explain some of the medicinal effects of celery, including the lowering of blood pressure and the relief of arthritis. 3nB first drew significant scientific attention when researchers identified it as the factor in celery responsible for the blood pressure lowering effect of celery. The research was prompted by one of the researcher's father, who after eating a quarter-pound of celery every day for one week observed his blood pressure dropped from 158 over 96 to a normal reading of 118 over 82. A recent human study evaluated the efficacy of a standardized extract of celery seed supplying 85 percent 3nB in 30 patients with mild to moderate hypertension. The dosage was 150 mg per day. The results showed a statistically significant decrease in both systolic (SBP) and diastolic blood pressure (DBP) compared to baseline measurements. The change at week six for the SBP was 8.2 mmHg and for the DBP was 8.5 mmHG. No side effects were reported. These results indicate that celery seed extract may produce the greatest blood pressure-lowering effects in natural products available on health food store shelves. A major advantage of celery extract over conventional drugs used in high blood pressure is that drugs like beta-blockers, angiotensin converting enzyme (ACE) inhibitors, and calcium channel blockers tend to significantly lower the blood flow to the brain While this effect is helpful in reducing the likelihood of stroke, it often leaves patients taking these drugs feeling tired, depressed, dizzy, and forgetful. Celery extract on the other hand has actually been shown to not only help prevent stroke in animal studies, but also improve blood flow as well and act to protect the brain and enhance energy production with the brain. (NEXT) Exercise during pregnancy gives newborn brain development a head start University of Montreal, November 10, 2022 As little as 20 minutes of moderate exercise three times per week during pregnancy enhances the newborn child's brain development, according to researchers at the University of Montreal and its affiliated CHU Sainte-Justine children's hospital. This head-start could have an impact on the child's entire life. “Our research indicates that exercise during pregnancy enhances the newborn child's brain development,” explained Professor Dave Ellemberg, who led the study. “While animal studies have shown similar results, this is the first randomized controlled trial in humans to objectively measure the impact of exercise during pregnancy directly on the newborn's brain.We hope these results will guide public health interventions and research on brain plasticity. Most of all, we are optimistic that this will encourage women to change their health habits, given that the simple act of exercising during pregnancy could make a difference for their child's future.” Not so long ago, obstetricians would tell women to take it easy and rest during their pregnancy. Recently, the tides have turned and it is now commonly accepted that inactivity is actually a health concern. “While being sedentary increases the risks of suffering complications during pregnancy, being active can ease post-partum recovery, make pregnancy more comfortable and reduce the risk of obesity in the children,” Curier explained. “Given that exercise has been demonstrated to be beneficial for the adult's brain, we hypothesized that it could also be beneficial for the unborn child through the mother's actions.” To verify this, starting at the beginning of their second trimester, women were randomly assigned to an exercise group or a sedentary group. Women in the exercise group had to perform at least 20 minutes of cardiovascular exercise three times per week at a moderate intensity, which should lead to at least a slight shortness of breath. Women in the sedentary group did not exercise. The brain activity of the newborns was assessed between the ages of 8 to 12 days, by means of electroencephalography, which enables the recording of the electrical activity of the brain. “We used 124 soft electrodes placed on the infant's head and waited for the child to fall asleep on his or her mother's lap. We then measured auditory memory by means of the brain's unconscious response to repeated and novel sounds,” Labonté-LeMoyne said. “Our results show that the babies born from the mothers who were physically active have a more mature cerebral activation, suggesting that their brains developed more rapidly.” (NEXT) Keeping indoor humidity levels at a ‘sweet spot' may reduce the spread of COVID-19 Massachusetts Institute of Technology, November 14, 2022 We know proper indoor ventilation is key to reducing the spread of COVID-19. Now, a study by MIT researchers finds that indoor relative humidity may also influence transmission of the virus. Relative humidity is the amount of moisture in the air compared to the total moisture the air can hold at a given temperature before saturating and forming condensation. In a study appearing in the Journal of the Royal Society Interface, the MIT team reports that maintaining an indoor relative humidity between 40 and 60% is associated with relatively lower rates of COVID-19 infections and deaths, while indoor conditions outside this range are associated with worse COVID-19 outcomes. To put this into perspective, most people are comfortable between 30 and 50% relative humidity, and an airplane cabin is at around 20% relative humidity. The findings are based on the team's analysis of COVID-19 data combined with meteorological measurements from 121 countries, from January 2020 through August 2020. Their study suggests a strong connection between regional outbreaks and indoor relative humidity. In general, the researchers found that whenever a region experienced a rise in COVID-19 cases and deaths prevaccination, the estimated indoor relative humidity in that region, on average, was either lower than 40% or higher than 60% regardless of season. Nearly all regions in the study experienced fewer COVID-19 cases and deaths during periods when estimated indoor relative humidity was within a “sweet spot” between 40 and 60%. “There's potentially a protective effect of this intermediate indoor relative humidity,” suggests lead author Connor Verheyen, a Ph.D. student in medical engineering and medical physics in the Harvard-MIT Program in Health Sciences and Technology. “However, we find that maintaining an indoor relative humidity in that sweet spot—of 40 to 60%—is associated with reduced COVID-19 cases and deaths.”In all, they focused on 121 countries where COVID-19 outbreaks occurred. For each country, they also tracked the local COVID-19 related policies, such as isolation, quarantine, and testing measures, and their statistical association with COVID-19 outcomes. In warmer times, both outdoor and indoor relative humidity for each country was about the same, but they quickly diverged in colder times. While outdoor humidity remained around 50% throughout the year, indoor relative humidity for countries in the Northern and Southern Hemispheres dropped below 40% in their respective colder periods, when COVID-19 cases and deaths also spiked in these regions. (NEXT) Why Are Kids So Great at Learning? GABA Brown University, November 16, 2022 GABA is the abbreviation for the neurotransmitter γ-aminobutyric acid. In the study, published in the journal Current Biology, researchers explain GABA's crucial role in helping children process new information and prepare their brains to learn and store even more. “What we found is a rapid increase in GABA in children, associated with learning,” says lead study author Takeo Watanabe, a professor of cognitive and linguistic sciences at Brown University. The neurotransmitter GABA plays an important role in helping the brain consolidate new info, Watanabe says. It “stabilizes” the network so that subsequent learning doesn't override what was already there and defends knowledge against retrograde interference. Yet this kind of GABA inhibitory processing is not fully matured in children, he says. Kids have lower levels of GABA—it's why they have less inhibitory abilities and weaker impulse control than adults. So if GABA is necessary to set the brain up to learn consecutive items, and children have less GABA than adults, then how are children able to, as Watanabe puts it, “learn and learn and learn and learn?” Using an advanced imaging technique called functional MRS, they measured the concentration of GABA in early visual cortical areas before, during and after learning sessions. They then compared the concentrations between children (ages 8 to 11) and adults (ages 18 to 35). They found that before learning begins, the overall amount of GABA in children is indeed smaller than in adults, Watanabe says. However, the researchers found that children exhibited a rapid boost in GABA concentration in the second round of learning, while the concentration of GABA in adults did not change.The results of the experiments suggest that compared with adults, children exhibit more dynamic GABA-associated inhibitory processing, which more rapidly adapts to stabilize learning than in adults, the researchers conclude. (NEXT) Being comfortable with aging can benefit sex life University of Missouri, November 17, 2022 Researchers have long known that having a positive outlook can benefit a person's health.Now, a new study by the University of Missouri has found older adults who feel positively about aging have a healthier sex life—a finding that didn't surprise the researcher, who's been studying the benefits of the positive perceptions of aging.”We know positive perceptions of aging can be really beneficial, but when they are negative, they can be really detrimental. Negative perceptions of aging are linked to higher likelihood of cognitive decline, higher likelihood of cardiovascular disease and even shorter lifespans. We wanted to see how it would affect people's sexual relationships. As expected, thinking positively about the way you age can also lead to a healthy sex life.” “We found this relationship to be strong with both men and women,” Skoblow said. “With data from more than 1,100 couples, we were able to find that participants with a more positive perception of aging also had sex more frequently as well as increased satisfaction.” “It's possible that if people expect a steep physical decline as an inevitable part of aging, they could have anticipatory inhibitions. They stop enjoying themselves in the moment and could have less satisfying sexual encounters,” Skoblow said. “We also know that western cultures often have many youthful beauty ideals, so maybe people with more positive perceptions of aging don't buy into them as much, leading to a more satisfying sex life as their body begins to change.”

But what if you could stop this degenerative disease in its tracks? A world-first study from the University of South Australia could make this a reality as new genetic research shows a direct link between dementia and a lack of vitamin D. Investigating the association between vitamin D, neuroimaging features, and the risk of dementia and stroke, the study found: low levels of vitamin D were associated with lower brain volumes and an increased risk of dementia and stroke genetic analyses supported a causal effect of vitamin D deficiency and dementia. in some populations as much as 17% of dementia cases might be prevented by increasing everyone to normal levels of vitamin D (50 nmol/L). Flaxseed supplements linked to improved blood pressure: Meta-analysis University of Medicine and Pharmacy (Romania), June 9, 2022 Supplements of flaxseed may effectively management blood pressure, says a new meta-analysis of 15 clinical trials. Data from 1,302 participants indicated that flaxseed supplements are associated with significant reductions in both systolic and diastolic blood pressure of about 2.85 mmHg and 2.39 mmHg, respectively. “The results obtained in the present meta-analysis – a decrease of 2.85/2.39 mmHg after flaxseed supplementation – might be valuable for the hypertension management using nutraceuticals, since Heart Outcome Evaluation study demonstrated that a 3.3/1.4 mmHg reduction was associated with a 22% decline of relative risk of cardiovascular mortality,” wrote scientists from Romania, Iran, Australia and Poland in Clinical Nutrition . The scientists also report that supplementation for longer than 12 weeks resulted in even greater reductions in systolic and diastolic blood pressure of 3.10 mmHg and 2.62 mmHg, respectively, compared to trials of shorter duration. The potential biological mechanism are not completely understood, said the researchers, but could be linked to the lignan content of flaxseed. Specifically, a lignan named SDG is known to be an angiotensin-converting enzyme (ACE) inhibitor. ACE inhibitors work by inhibiting the conversion of angiotensin I to the potent vasoconstrictor, angiotensin II, thereby improving blood flow and blood pressure. Finally, an arginine-rich protein fraction (KCl-F1) from flaxseed may also impact blood pressure. Flaxseed powder supplements were found to affect systolic blood pressure (SBP) and diastolic blood pressure (DBP), whereas flaxseed oil preparations only affected DBP. On the other hand, lignan extracts was not associated with any changes in SBP and DBP, they said. Keep calm and carry on — for the sake of your long-term health Penn State University, June 9, 2022 Reacting positively to stressful situations may play a key role in long-term health, according to researchers. In a study measuring adults' reactions to stress and how it affects their bodies, researchers found that adults who fail to maintain positive moods such as cheerfulness or calm when faced with the minor stressors of everyday life appear to have elevated levels of inflammation. Furthermore, women can be at heightened risk. Nancy Sin, in the Center for Healthy Aging, Penn State and her colleagues showed that the frequency of daily stressors, in and of itself, was less consequential for inflammation than how an individual reacted to those stressors. “A person's frequency of stress may be less related to inflammation than responses to stress,” said Sin. “It is how a person reacts to stress that is important.” In the short-term, with illness or exercise, the body experiences a high immune response to help repair itself. However, in the long term, heightened inflammatory immune responses may not be healthy. Individuals who have trouble regulating their responses may be at risk for certain age-related conditions, such as cardiovascular disease, frailty and cognitive decline, Sin said. “To our knowledge, this paper is the first to link biomarkers of inflammation with positive mood responses to stressors in everyday life,” said Jennifer E. Graham-Engeland, associate professor of biobehavioral health, Penn State. For breast cancer prevention, diet quality matters Healthy, plant-based diet linked with lower cancer risk for postmenopausal women  Paris-Saclay University (France), June 14, 2022 Research shows that what we eat can influence our cancer risk, but it's not always clear which foods or dietary patterns are best for cancer prevention. Results from a new study suggest that the quality or overall healthiness of a person's diet may be key. The study, based on data from over 65,000 postmenopausal women who were tracked for more than two decades, found that a healthy plant-based diet was linked with a 14% lower risk of breast cancer while an unhealthy plant-based diet was linked with a 20% higher risk of breast cancer. A research group has investigated how important the motivational factors for becoming really proficient at a skill change over the years. “We wanted to see how the passion, grit and belief that you'll succeed at getting better – your growth mindset – change with age and in relation to gender,” says Hermundur Sigmundsson, a professor at the Norwegian University of Science and Technology's (NTNU) Department of Psychology. “The passion for what you used to burn for declines. And so does the belief that you can succeed at becoming really good at it,” says Professor Sigmundsson. But this is where it is important to take hold of yourself and not give up. You might not become a world champion in anything, but you could still get really proficient if you go for it. Or at least better. “That's when it's important to maintain a growth mindset. You can't stop believing in growth even though you're getting older,” says Sigmundsson. “Passion and a growth mindset decrease with age, but the willingness to persevere increases if we look at the elderly population as a whole,” says Sigmundsson.
