Research scientist Nichole Cubbage discusses her article, "The silent grief of vanishing twin syndrome: Why words matter in health care." She explains that Vanishing Twin Syndrome (VTS), the loss of one fetus in a multiple pregnancy, often leads to unique and isolating grief, frequently unacknowledged by health care providers. Nichole shares findings from her research indicating significant dissatisfaction among patients regarding provider communication and the information received about VTS, with over half reporting negative experiences. The conversation highlights how provider language—using terms like "insignificant" or medically unclear phrasing—profoundly affects patients' emotional processing and underscores the lack of standardized clinical guidelines for VTS diagnosis and counseling. Do churches need to be more exclusive? For years, both Evangelical and Progressive churches have lowered expectations to get people in the door. In this episode, Loren Richmond Jr. talks with Dr. Tricia Lyons, a leader in evangelism at Virginia Theological Seminary, about why churches should raise the bar to reclaim a compelling, transformative vision for sharing the gospel. Dr. Lyons challenges seeker-sensitive and all-are-welcome models, questioning whether they truly form disciples. Instead, she advocates for a high-bar Christianity—one that calls people into deep commitment rather than passive inclusion. The conversation explores church decline, the limits of hospitality, the power of formation, and why high expectations create stronger faith communities. Dr. Lyons currently teaches evangelism, works with the Lifelong Learning team, and serves as the Senior Advisor to the Dean for Evangelism Initiatives at VTS. She also directs the EvangelismFormationLab@VTS, a digital portal on YouTube and Facebook offering original content from the Seminary as well as thoughtful curation of resources in evangelism and formation for the wider church. Dr. Lyons was a lay chaplain and teacher of religion in Episcopal schools for 20 years before being ordained a priest and then serving parishes in Washington, DC, eventually serving as Canon for Evangelism in the Episcopal Diocese of Washington. Currently, Tricia serves as a non-stipendiary priest at the Church of Saint Clement in Alexandria, VA. Tricia is a member of the Presiding Bishop's Strategic Cabinet on Evangelism and one of the original writers of the “Way of Love.” Tricia is an honors graduate from Harvard College, the Harvard Divinity School, and received her doctorate from the Virginia Theological Seminary. Dr. Jason T. Jacobson, MD, FHRS, Westchester Medical Center, New York Medical College is joined by Melissa Robinson, MD, FHRS, Providence Heart Institute of Montana, and Dr. Sei Iwai, MD, FHRS, Westchester Medical Center, New York Medical College to discuss: BACKGROUND Factors determining hemodynamic stability during human ventricular tachycardia (VT) are incompletely understood. OBJECTIVES This study aimed to characterize sinus rate (SR) responses during monomorphic VT in association with hemodynamic stability and prospectively assess the effects of vagolytic therapy on VT tolerance. METHODS This is a retrospective analysis of patients undergoing scar-related VT ablation. Vasovagal responses were evaluated by analyzing sinus cycle length before VT induction and during VT. SR responses were classified into 3 groups: increasing ($5 beats/min, sympathetic), decreasing ($5 beats/min, vagal), and unchanged, with the latter 2 categorized as inappropriate SR. In a prospective cohort (n . 30) that exhibited a failure to increase SR, atropine was administered to improve hemodynamic tolerance to VT. RESULTS In 150 patients, 261 VT episodes were analyzed (29% untolerated, 71% tolerated) with a median VT duration of 1.6 minutes. A total of 52% of VT episodes were associated with a sympathetic response, 31% had unchanged SR, and 17% of VTs exhibited a vagal response. A significantly higher prevalence of inappropriate SR responses was observed during untolerated VT (sustained VT requiring cardioversion within 150 seconds) compared with tolerated VT (84% vs 34%; P < 0.001). Untolerated VT was significantly different between groups: 9% (sympathetic), 82% (vagal), and 32% (unchanged) (P < 0.001). Atropine administration improved hemodynamic tolerance to VT by 70%. CONCLUSIONS Nearly one-half of VT episodes are associated with failure to augment SR, indicative of an underrecognized pathophysiological vasovagal response to VT. Inappropriate SR responses were more predictive of hemodynamic instability than VT rate and ejection fraction. Happy holidays from BITM! We are delighted to share this episode with you this week. Kirk and Amelia sat down with Dr. Donald Carter III, assistant professor at Mercer School of Medicine. Dr. Carter earned his Doctorate in Bioethics from Loyola University after earning a Masters in Divinity from Vanderbilt and an MBA from Tennessee State. He is a Sadler Scholar with the Hastings Center and the co-chair of the Race and Culture and Ethnicity (RACE) affinity group of the American Society for Bioethics and Humanities (ASBH). Dr. Carter began his career as a musician after earning a BA in music from Fisk University, one of the many Historically Black Colleges and Universities (HBCUs) in the United States. This episode is a great one for those who are curious about pursuing a career in Bioethics. Dr. Carter shared how he explored several career paths before ultimately finding his professional home in Bioethics. Dr. Carted discussed his work exploring the impacts of the 1910 Flexner report, which, while contributing to creating uniform standards for medical education, also resulted in the closing of 5 of the 7 medical schools within HBCUs. This is one of the reasons for the shortage of Black physicians that persists today. He discussed the importance of diversifying the field of Bioethics and the positive changes he has observed within ASBH over the past few years. He shared how he views HBCUs as an important resource to tap to help diversify the field. He recommends building up bioethics education at HBCUs as a way of fostering lasting change. He also discussed the importance of humanities in medical education, sharing his experiences using visual training strategies (VTS) in medical education, reminding us that the arts have a way of returning us to a place where we can see each other as full humans again. Dr. Carter pointed out that one of the superpowers of the field of Bioethics is that we have a large tent. He examined specific ways in which racial, cultural and professional diversity can enhance the value our field brings to patients and communities.
