The following question refers to Section 9.5 of the 2022 AHA/ACC/HFSA Guideline for the Management of Heart Failure. The question is asked by Keck School of Medicine USC medical student & former CardioNerds Intern Hirsh Elhence, answered first by Vanderbilt Cardiology Fellow and CardioNerds Academy Faculty Dr. Breana Hansen, and then by expert faculty Dr. Javed Butler. Dr. Butler is an advanced heart failure and transplant cardiologist, President of the Baylor Scott and White Research Institute, Senior Vice President for the Baylor Scott and White Health, and Distinguished Professor of Medicine at the University of Mississippi The Decipher the Guidelines: 2022 AHA / ACC / HFSA Guideline for The Management of Heart Failure series was developed by the CardioNerds and created in collaboration with the American Heart Association and the Heart Failure Society of America. It was created by 30 trainees spanning college through advanced fellowship under the leadership of CardioNerds Cofounders Dr. Amit Goyal and Dr. Dan Ambinder, with mentorship from Dr. Anu Lala, Dr. Robert Mentz, and Dr. Nancy Sweitzer. We thank Dr. Judy Bezanson and Dr. Elliott Antman for tremendous guidance. Enjoy this Circulation 2022 Paths to Discovery article to learn about the CardioNerds story, mission, and values. Question #31 Mrs. Hart is a 70-year-old woman who was admitted to the CICU two days ago for signs and symptoms consistent with cardiogenic shock. Since her admission, she has been on maximal diuretics, requiring greater doses of intravenous dobutamine. Unfortunately, her liver and renal function continue to worsen, and urine output is decreasing. A right heart catheterization reveals elevated biventricular filling pressures with a cardiac index of 1.7 L/min/m2 by the Fick method.   What is the next best step? A Continue current measures and monitor for improvement B Switch from dobutamine to norepinephrine C Place an intra-aortic balloon pump (IABP) D Resume guideline directed medical therapy Answer #31 Explanation The Correct answer is C – Place an intra-aortic balloon pump. This patient is between the SCAI Shock Stages C and D with elevated venous pressures, decreased urine output, and worsening signs of hypoperfusion. She has been started on appropriate therapies, including diuresis and inotropic support. The relevant Class 2a recommendation is that in patients with cardiogenic shock, temporary MCS is reasonable when end-organ function cannot be maintained by pharmacologic means to support cardiac function (LOE B-NR). Thus, the next best step is a form of temporary MCS. IABP is appropriate to help increase coronary perfusion and offload the left ventricle. In fact, for patients who are not rapidly responding to initial shock measures, triage to centers that can provide temporary MCS may be considered to optimize management (Class 2b, LOE C-LD).    The guidelines further state that in patients presenting with cardiogenic shock, placement of a pulmonary arterial line may be considered to define hemodynamic subsets and appropriate management strategies (Class 2B, LOE B-NR). And so, if time allows escalation to MCS should be guided by invasively obtained hemodynamic data via PA catheterization. Several observational experiences have associated PA catheterization use with improved outcomes, particularly in conjunction with short-term MCS. Additionally, PA catheterization is useful when there is diagnostic uncertainty as to the cause of hypotension or end-organ dysfunction, particularly when the patient in shock is not responding to empiric initial measures, such as in this patient. There are additional appropriate measures at this time that are more institution-dependent. An institutional shock team would be very helpful here as they often comprise multidisciplinary teams of heart failure and critical care specialists,

Thursday's shooting had students at Perry High School barricading themselves in offices, ducking into classrooms and fleeing. Authorities say one of the wounded was a school administrator and the suspect was found dead with an apparent self-inflicted gunshot wound. investigators are looking at possible motive and reviewing the suspect's social media posts. Two friends and their mother who spoke with The Associated Press said the suspect was a quiet person who had been bullied for years. PRETORIA, South Africa (AP) — Officials say South African athlete Oscar Pistorius has been released from prison on parole after serving nearly nine years in prison for killing his girlfriend. The Department of Corrections gave no more details of Pistorius' release. The announcement came around 8:30 a.m., indicating that officials released the world-famous double-amputee Olympic runner early in the morning. Pistorius has served nearly nine years of his almost 13-and-a-half-year murder sentence for killing model and law graduate Reeva Steenkamp on Valentine's Day 2013. He was approved for parole in November. Serious offenders in South Africa are eligible for parole after serving at least half their sentence. BERLIN (AP) — Police say four people have died after a fire broke out at a hospital in northern Germany and spread to several patients' rooms. Emergency workers saw flames on the fourth floor of a wing of the hospital in Uelzen, southeast of Hamburg, when they arrived at the scene late Thursday evening. They could hear calls for help and evacuated several people, and were able to stop the fire spreading any further. Police said Friday that three patients died at the scene and a fourth after being taken to another hospital. It wasn't immediately clear how many people were injured, nor what caused the fire. WASHINGTON (AP) — The Labor Department is expected to report that employers added a solid 160,000 jobs last month, according to the data firm FactSet. That would mean that the economy had added 2.7 million jobs in 2023. Federal Reserve Chair Jerome Powell warned of hard times ahead after the Fed began jacking up interest rates in the spring of 2022 to attack high inflation. Economists predicted that the much higher borrowing costs would cause a recession. SYDNEY (AP) — The largest male specimen of the world's most venomous spider that boasts fangs powerful enough to pierce a human fingernail has found a new home after being discovered north of Sydney, Australia. The Sydney funnel-web spider, found only in Australia, has been donated to the Australian Reptile Park to help save lives by contributing to the park's antivenom program. The spider, given the moniker “Hercules.” was initially delivered by a member of the public to a local hospital. But park experts retrieved it and soon realized it was the largest male specimen ever received by the public in Australia. “Hercules” measures 7.9 centimeters (3 inches) from foot to foot, surpassing the park's previous record-holder from 2018, the male funnel-web named “Colossus”. WASHINGTON (AP) — Donald Trump plans to spend the third anniversary of the Capitol riot by campaigning in Iowa as he tries win back the White House. The former Republican president has two events planned for Saturday in the leadoff voting state Iowa as he marks the anniversary of what he has called “a beautiful day.” President Joe Biden will be at a site near Valley Forge, Pennsylvania, on Friday where he's expected to press his case that Trump and top supporters of his “Make America Great Again” movement are threats to American democracy. Biden's visit has been moved up from Saturday due to weather concerns. The Capitol riot is increasingly viewed differently along partisan lines. New York (AP) — New York City is suing more than a dozen charter bus companies for their role in Texas Gov. Greg Abbott's operation to send tens of thousands of migrants to urban areas. The lawsuit, filed Thursday, claims the 17 bus companies “knowingly implemented” Abbott's busing plan in violation of a New York law that limits transferring “needy persons” across state lines. It seeks more than $700 million in damages to recoup the cost of caring for an estimated 33,000 migrants who have arrived in the city on charter buses since April 2022. Abbott said in response that the suit was a violation of the commerce clause, which guarantees the constitutional right to travel. DALLAS (AP) — The family of former U.S. Rep. Eddie Bernice Johnson says the trailblazing Texas congresswoman died after getting an infection and accused a Dallas rehabilitation facility of neglect. Johnson, the first registered nurse elected to Congress, died on Dec. 31 at the age of 89. Les Weisbrod, her family's attorney, said at a news conference that her death was caused by an infection in her spine that developed after she was left in her own feces at Baylor Scott & White Institute for Rehabilitation following back surgery. Over on in the US, Wall Street closed lower on Tuesday as the release of home sales and consumer confidence data in the region sparked investor concerns over the state of the US economy.  The Dow Jones is fell 1.14% in its worst session since March, the Nasdaq retreated 1.57% and the S&P500 is lost 1.47% at the closing bell on Tuesday.August new home sales data missed expectations with homes under contract totalling 675,000 for the month, down 8.7% on July and below economists' expectations of 695,000 which would have represented a lesser decline from July. The Conference Board's consumer confidence index fell to 103 points in September, down from 108.7 in August and also below economists' expectations of 105.5 points. Both of these readings falling short of economists' expectations and being greater declines than expected indicate the greater impact interest rate hikes are having on the US economy to date.Over in Europe, markets closed lower for a fourth straight session as negative investor sentiment impacted global stocks. The STOXX600 fell 0.6% on Tuesday weighed down by technology and automaking stocks falling 2% and 1.2% respectively, while Germany's DAX lost just shy of 1%, the French CAC fell 0.7% and, in the UK, the FTSE100 closed flat. The muted day in the UK follows signs that the Bank of England and European Central Bank will hold rates steady while the Federal Reserve in the US may have another hike in store. Rising bond yields in the region are also weighing on European markets as investors opt for safer returns alternatives to equities in the current market environment.The ASX fell 0.54% yesterday as interest-rate sensitive sectors weighed on the key index with the REIT and Tech sectors shedding 1.35% and 1.93% respectively as the market prepares for interest rates to remain higher for longer across not only locally, but among international economies too. Rising bond yields are also weighing on the ASX as investors turn to bonds over the higher-risk equities in the current environment.Pro Medicus popped 9% on Tuesday after the leading health imaging company announced its wholly owned US subsidiary, Visage Imaging, has signed a $140m, 10-year contract with the largest not-for-profit healthcare system in Texas, Baylor Scott & White Health (BSWH). What to watch today:Ahead of the local trading session here in Australia the SPI futures are expecting the ASX to open 0.34% lower following the slide in global investor sentiment overnight.On the commodities front, oil is trading 1.07% higher at US$90.64/barrel, gold is down 0.83% at US$1899/ounce and iron ore is down 3.64% at US$119/tonne.AU$1.00 is buying US$0.64, 95.29 Japanese Yen, 52.73 British Pence and NZ$1.08.Trading Ideas:Bell Potter has raised the price target on Pro Medicus (ASX:PME) from $70/share to $75/share and maintain a hold rating on the leading health imaging IT provider after the company announced the signing of its largest ever deal in the US through a 10-year contract with Baylor Scott & White Health. With a shortage of radiologists and ongoing cuts to Medicare re-imbursement for radiology, companies like Pro Medicus are also expected to continue working with AI providers and researchers to facilitate the provision of AI tools in radiology, however the company is not developing its own applications as understood by Bell Potter.And Trading Central has identified a bearish signal on Charter Hall Long WALE REIT (ASX:CLW) following the formation of a pattern over a period of 40-days which is roughly the same amount of time the share price may fall from the close of $3.28 to the range of $2.50 to $2.65 according to standard principles of technical analysis.

Pearls and Quotes - Atrial Fibrillation: Situational Assessment of Stroke and Bleeding Risk In patients with persistent atrial fibrillation with tachycardia induced cardiomyopathy, timely restoration of normal rhythm is important. In patients not on established oral anticoagulation one option is to wait 3 weeks on oral anticoagulation prior to considering cardioversion. Another option is to pursue TEE prior to cardioversion as TEE is currently the gold standard imaging modality to exclude a LAA thrombus. Following cardioversion (chemical or electrical), anticoagulation must not be interrupted for 4 weeks due to atrial stunning. This is especially true for patients who have been in atrial fibrillation for an extended period of time. Individualizing assessment of stroke and bleeding risk is imperative when determining perioperative anticoagulation (AC) management. ACC has a helpful app (ManageAnticoag App) to make this easier. When considering AC in triggered atrial fibrillation (e.g., pneumonia, sepsis), it is important to consider the substrate that made the patient susceptible to developing atrial fibrillation. AC is favored in patients with high CHA2DS2-VAsC score and many traditional risk factors for atrial fibrillation as they are at high risk for future development of atrial fibrillation. Atrial fibrillation is a marker of poor outcomes in patients who have undergone coronary artery bypass graft (CABG) surgery. It is unclear if patients should be started on long-term AC for new onset atrial fibrillation after CABG regardless of risk factors. This is currently being investigated in the PACES trial. Notes - Atrial Fibrillation: Situational Assessment of Stroke and Bleeding Risk How do we choose an imaging modality for excluding LAA thrombus exclusion prior to cardioversion? TEE is the gold standard. It also provides other information that is important for management of atrial fibrillation (e.g. LA size/volume, presence/degree of mitral regurgitation/stenosis, ejection fraction). Gated cardiac CTA may have a growing role for evaluation of LAA thrombus. What is the data behind the recommendation for uninterrupted AC following cardioversion and what is atrial stunning? The following question refers to Section 9.5 of the 2022 AHA/ACC/HFSA Guideline for the Management of Heart Failure.  The question is asked by Keck School of Medicine USC medical student & CardioNerds Intern Hirsh Elhence, answered first by Duke University cardiology fellow and CardioNerds FIT Ambassador Dr. Aman Kansal, and then by expert faculty Dr. Javed Butler. Dr. Butler is an advanced heart failure and transplant cardiologist, President of the Baylor Scott and White Research Institute, Senior Vice President for the Baylor Scott and White Health, and Distinguished Professor of Medicine at the University of Mississippi. The Decipher the Guidelines: 2022 AHA / ACC / HFSA Guideline for The Management of Heart Failure series was developed by the CardioNerds and created in collaboration with the American Heart Association and the Heart Failure Society of America. It was created by 30 trainees spanning college through advanced fellowship under the leadership of CardioNerds Cofounders Dr. Amit Goyal and Dr. Dan Ambinder, with mentorship from Dr. Anu Lala, Dr. Robert Mentz, and Dr. Nancy Sweitzer. Question #14 Mrs. Hart is a 70-year-old woman hospitalized for a 2-week course of progressive exertional dyspnea, increasing peripheral edema, and mental status changes. She has a history of coronary artery disease, hypertension, and heart failure for which she takes aspirin, furosemide, carvedilol, lisinopril, and spironolactone. On physical exam, the patient is afebrile, BP is 80/60 mmHg, heart rate is 120 bpm, and respiratory rate is 28 breaths/min with O2 saturation of 92% breathing room air. She is sitting upright and is confused. Jugular venous pulsations are elevated. Cardiac exam reveals an S3 gallop. There is ascites and significant flank edema on abdominal exam. Her lower extremities have 2+ pitting edema to her knees and are cool to touch. Her labs are significant for an elevated serum Creatinine of 3.0 from a baseline of 1.0 mg/dL, lactate of 3.0 mmol/L, and liver enzyme elevation in the 300s U/L. Which of the following is the most appropriate initial treatment? A Increase carvedilol B Start dobutamine C Increase lisinopril D Start nitroprusside Answer #14 Explanation The Correct answer is B – start dobutamine. This patient with progressive congestive symptoms, mental status changes, and signs of hypoperfusion and end-organ dysfunction meets the clinical criteria of cardiogenic shock. The Class 1 recommendation is that in patients with cardiogenic shock, intravenous inotropic support should be used to maintain systemic perfusion and maintain end-organ performance (LOE B-NR). Their broad availability, ease of administration, and clinician familiarity favor such agents as first line when signs of hypoperfusion persist. Interestingly, despite their ubiquitous use for management of cardiogenic shock, there is a lack of robust evidence to suggest the clear benefit of one agent over another.  Therefore, the choice of a specific agent is guided by additional factors including vital signs, concurrent arrhythmias, and availability. For this patient, dobutamine is the only inotrope listed. Although she is tachycardic, her lack of arrhythmia makes dobutamine relatively lower risk and does not outweigh the potential benefits. Choice A – Increase carvedilol – is not correct. Beta-blockers should be continued in HF hospitalization whenever possible; however, in a patient with low cardiac output and signs of shock, beta-blockers should be discontinued due to their negative inotropic effects. Choice C – Increase lisinopril – is not correct. Afterload reduction is reasonable to decrease myocardial oxygen demand. However, given the hypotension and renal dysfunction, increasing lisinopril could be potentially dangerous by fur...

Amy's House is a Medical Lodging Facility in Temple Texas, just minutes away from Baylor Scott and White Health Center. Transplant recipients are able to stay at Amy's House free of charge.Phyllis graduated from the University of Central Texas with a graduate degree in Counseling in December of 1997. She has served in many roles over the course of her career to include 17 years as a director at Baylor Scott and White Health, but it was an accident over a July 4th weekend in 2013 that actually led her to discover her passion for the transplant world and the people on both sides of that process. She lost her 13-year-old granddaughter, Monika, in a car accident when a distracted driver ran up on her daughter's vehicle as they were sitting stopped in traffic. In this episode of the TA Leaders podcast, we have a great discussion with Lisa Starin, Director of Talent Acquisition and Talent Management at Baylor Scott & White Health. Lisa is currently serving in a dual role. She leads the talent acquisition team of 22 members. She also serves as the interim talent management director in charge of a new performance management framework and platform for 50,000+ employees. Interestingly, Lisa started her career in a hybrid role as office support and an associate recruiter. But she took the opportunity to become a full-fledged recruiter as soon as it became available. She shares the “old school” recruiting method she mainly used at the beginning of her career, and why she wouldn't change anything about her early days. Lisa spent about 17 years at UnitedHealth Group as a recruiter, recruiting manager, and a few other roles. She's spent most of her career in what she calls “the business of people” and “being able to match careers with unique talent.” In her current role, Lisa and her team hire approximately 12,000 – 13,000 people per year. She shares how they've had interesting challenges in the healthcare industry in the middle of the pandemic. Lisa also talks about some of the biggest challenges and initiatives they're working on moving forward, including some significant changes in workflows for specific roles and what they're doing to help with internal mobility. Finally, Lisa shares what she sees for the future of talent acquisition.
