Rodolfo asks about ADHD & Intrusive Thoughts Hello Dr. Burns, my name is Rudy. First and foremost, your writings and podcasts have been life changing for me. You're AMAZING! I was recently diagnosed with ADHD and I started experiencing intrusive thoughts around November of last year. I thought I had OCD, but apparently intrusive thoughts are a common companion in ADHD. What would be the best course of action in defeating them? How would I apply TEAM to them? David's Response In all my books, like Feeling Great, I outline a step by step approach to writing down and challenging thoughts. You can also listen to the podcast on the four models for treating anxiety. Can we use your great comment on a podcast, and add your question to our next Ask David podcast? Can you send me an example of the types of intrusive thoughts you're having? Sometimes writing them down and challenging can be helpful, but need to see what we're talking about. Best, david Rodolfo responds Thank you for the swift response! Ok, so writing down my intrusive thoughts, regardless of their nature, and challenging them. I don't, however, know which method/route to challenge them with. I have Feeling Great, so I will read through it again. I will also check out the podcasts. My intrusive thoughts have been disgusting violent acts involving my wife and son. ***I HAVE NO INTENTIONS OR DESIRE TO COMMIT ANY OF THEM***. I've had images of my wife getting shot, not necessarily by me. Sometimes I'll see my arms give out and my son will fall. I've seen my wife sitting down, and she gets hit in the back of the head. When they appear, I begin to freak out immensely because they are the complete opposite of who I am and what I want to do, which is protect them. I know I'm not doing something right when it comes to your methods because I still freak out. David Responds Thanks! Although disturbing, this is a very common and often easily treatable OCD type of problem. Best, david Rodolfo responds What a relief. I thought I was going insane here! I would be honored if you all covered it in a podcast. All I listen to in my car now is the Feeling Good podcast! David Responds Often, intrusive OCD thoughts reflect suppressed problems / feelings people have, especially when the person is exceptionally "nice," and used to sweeping feelings under the rug, so to speak. I cannot treat you in this medium, obviously, but I'm wondering if you have some negative feelings, like anger or frustration, toward your wife and child? When these are "squashed," they can come out indirectly, disguised as anxious thoughts and feelings. This is called the Hidden Emotion Technique, which you can look up using the search on my website, if curious, or read about it in my book, When Panic Attacks. Best, david 2. Brittany asks if it's okay to enjoy / prefer being alone. Hi Dr. Burns, I was listening to your podcast on self-acceptance, and it was really interesting hearing the results. I like how you said that just because we accept something about ourselves doesn't mean we aren't still working on it. I think people confuse that a lot. One thing I've accepted about myself is that I really do enjoy being alone. I think in the past I would try to find a buddy in uncomfortable situations like the first day of work or orientation or going to a baby shower alone etc. but now I find that I am most comfortable when I'm not included with everyone else. However, I think it can come across as off putting to some. Sometimes I get the feeling they see me excluding myself as rejection to them. That's not my intention and nobody's ever actually said that but it's a thought that pops up. Just wondered if you think this form of acceptance is good or bad? I mean it's been good for me. Just maybe it's bad for others? -Brittany David's response This is a cool question and nice comment, too, about our podcast. Can we read this on a podcast and use it for an Ask David? Personally, I am trying to say "no" more often when asked to do things with other people, and sometimes it's hard. But if I don't say no, I get way too much on my plate and can't keep up! Warmly, david Brittany's reply That makes perfect sense to me. I've kind of taken the position where I don't really want to make more friends because I feel like I'm already falling short with spending enough time with my cats and everyone else. Let alone time for myself. David's reply Exactly! Our culture has this myth that's it's somehow impossible or immoral to enjoy just being with oneself! David 3. John, from Ireland, asks, “Help! I listen to the podcasts regularly and you are both bringing such great benefits to the world! I would love to hear another podcast on should statements. I've been going through a pretty bad and extended period of anxiety and depression in recent months and have been really trying to challenge my thoughts without much success. I'm addicted to shoulds unfortunately and beat up on myself relentlessly. I've tried to do a cost benefit analysis and the motivation piece is a huge part of the benefits side that is keeping me stuck. I really want to give up my should statements, they are making my life a misery. But I'm finding I don't want to let go of the motivation piece. I have given a sample below. I'd love to know if there were further steps/techniques I could use aside from the CBA to address the benefits and the perceived motivation piece so I can fully let go of the shoulds! Thanks so much both, John from Ireland Negative Thought: “I should have plans on a Saturday.” 99% SHLD, ER, SB, MF, DP. Benefits Lets me know when I'm not performing to the standard I set for myself Shows me my passion/goal in life for doing fun exciting things Motivates me to move to make plans, scolds me until I do so. Keeps me vigilant that I'm aware of my shortcomings Reminds me of the value of being socially connected, meeting and having fun with friends. It's easy, it's familiar, it's such a habit. (process resistance) Costs Emotional Cost. Beats me up relentlessly, makes me feel depressed, anxious and lowers my self-esteem. Reinforces perfectionistic beliefs Counterproductive in terms of motivation, makes me feel so low, Huge interpersonal cost, feel isolated alone. Prevents intimate connections Not able to be present or flow in a conversation, quality of conversation/connection drops Self-fulfilling prophecy, if I'm telling myself that then I ignore other possibilities. David John continues the exchange Hi David, Thank you so much for taking the time to correspond today and thank you so much for all the work you're doing for free. The world would be a better place with more Rhonda's and Dr. Burns' in the world. One of the things that has brought me some peace in the last few weeks is going for a long walk in the evening after work listening to your podcasts! I have attached a DML that I've been working on to give a bit more context. I made some inroads on that original should statement, however, as you can see from the DML more shoulds keep popping up. I have identified the Self Defeating Beliefs that resonate with me at the bottom of the document. I'm working hard to find healthy acceptance, I know this is key for me. I'm not entirely sure a lot of the time what exactly I should be accepting. That I'm a flawed person with many defects, just like everyone else maybe? To give some further context and without overburdening you with too much detail: I feel my anxieties are one of the main parts of my problem and I would love to get over these. I feel defective that I'm not resilient enough for the challenges of life. I have fears mainly around people, fear of letting people down, fear of being judged or rejected and just feeling different to others I suppose. I've struggled with recurring anxiety and some depressive periods from when I was a young child (likely biological - my dad has the same issue). I have strong feelings of defectiveness and not being up to standard compared to my peers. I have a very busy corporate finance job where I've been working very long hours over an extended number of weeks and months. I don't' love my job, fell into it really. Could be a hidden emotion thing going on. I started working from home more and more in recent months as my workload increased and my anxiety and mood really started to dip as work took over my life. I started to get really burned out in April. A lot of why I enjoyed the job previously was having fun with my teammates but that has become impossible in recent times due to everyone's workload. I do CrossFit which is probably an extreme form of exercise and between that and work I fell into some kind of perfectionism where I couldn't really find joy out of anything, aiming for higher and higher achievements. This brought on tons of shoulds and all or nothing thinking and self-blame, I think. My dad, who had been doing well for a good few years fell into a deep depression and anxious state around April time too and has been in hospital for a number of weeks and our family is trying to support him through this. We're finding the standard of mental health care in Ireland really really poor unfortunately. I find his issues quite triggering for me a lot of the time and I'll believe I'm defective because he is defective and there is nothing I can do about it. Dad is entirely dependent on medication to get him out of his slump and has a bad case of do-nothingism. He is beating himself up relentlessly too and I gave him a copy of Feeling Good to read but he hasn't looked at it at all. I have just turned 40 and I'm still single, so I feel like I'm letting my family down for not being emotionally able for a relationship or kids or be a better brother/son. I know I'm probably being harsh here as they say the opposite, but I feel I could/should be doing more. I have probably fired way too much detail at you, so I'll stop there! Thanks so much again! (Adam@TheWorldview.com) By Adam McManus Nigerian Muslims shot two Christian men On August 4th, Islamic Fulani militants shot two Christian men in the Bokkos area of the Plateau State in Nigeria, reports International Christian Concern. The men, whose names are being withheld for their protection, are thankful to be alive. One of them said, “To the glory of God, we both survived.” David John, the medical director of the hospital where the men are currently receiving treatment, said the hospital's staff would provide them with the best medical care. Bokkos was one of three Plateau communities targeted by Muslim Fulani militants during the Christmas Eve massacre in 2023. In those attacks, more than 200 Christians were killed, including 148 people in Bokkos. According to Open Doors, Nigeria is the sixth most dangerous country worldwide in which to be a Christian. Pray for God's justice in Nigeria. Proverbs 21:15 says, “When justice is done, it brings joy to the righteous but terror to evildoers.” Ukrainian navy takes out Russian gas rig Ukraine blasted a Russian gas rig in the Black Sea near the Dnipro River, killing 40 Russian soldiers, reports The U.S. Sun. Dramatic footage shows a giant fireball erupting from the Russian headquarters after it was targeted by the Ukrainian Navy. The decimated rig was packed with “reconnaissance equipment” to help with Russia's war effort. Ukrainian officials claimed the gas rig used to be controlled by their military, but had become occupied by Russian President Vladimir Putin's forces in recent years. Harris leads Trump in 3 battleground states Vice President Kamala Harris leads former President Donald Trump in three crucial battleground states, according to new surveys by The New York Times and Siena College. This is the latest indication of a dramatic reversal in standing for Democrats after President Biden's departure from the presidential race. Harris is ahead of Trump by four percentage points in Wisconsin, Pennsylvania and Michigan, 50 percent to 46 percent among likely voters in each state. The surveys were conducted from Aug. 5 to 9. Trump: Kamala Harris not smart enough to hold news conference Despite her rise in the polls, Donald Trump does not think Kamala Harris is smart enough to hold a news conference. TRUMP: “She can't do a news conference. She doesn't know how to do a news conference. She's not smart enough to do a news conference. I'm sorry. We need smart people to lead this country because our country has never been in this danger before, both economically and from an outside perspective.” Kamala Harris: “Everybody needs to be woke!” A video of Vice President Kamala Harris declaring that “everybody needs to be woke” in 2017 has resurfaced and gone viral on social media, reports Media Research Center. Appearing with philanthropist Laurene Powell Jobs, the widow of Apple Founder Steve Jobs, Harris spoke at the 2017 Recode Code Conference. Listen. HARRIS: "We have to stay woke. Like everybody needs to be woke. You can talk about if you're the woke-est or woke-er, but just stay more woke than less woke." Send letters to 6 imprisoned pro-lifers Pro-life rescuers either in or awaiting prison have made – and are making – gigantic sacrifices for practicing non-violent civil disobedience at abortion mills on behalf of their pre-born brothers and sisters. By giving up their emotional support networks of family, friends, and colleagues, these pro-life heroes are in danger of a loneliness that can be felt even – or especially – in a crowded prison cafeteria or yard. Some of them have been handed years-long sentences, reports LifeSiteNews.com. MacGyver-minded officer saves toddler And finally, from Kansas comes the story of a quick-thinking rescue that hauled a 14-month-old toddler from a would-be tomb at the bottom of a drain pipe, reports Good News Network. The terrifying ordeal was caught on camera and saw a Moundridge Police Department officer build on the spot a makeshift catchpole typically used for pulling varmints out of hard-to-reach places. Secured under his shoulders, the loop of rope at the end of the catchpole was able to lift the boy, named Bently, out from where he was trapped 10 feet down a section of 12-inch-wide PVC pipe that had been buried vertically. The parents said they called 9-1-1 just before 2:00 p.m. when they realized Bently had fallen into the PVC hole. Blake, the father's boy, described his emotions before the rescue to the local news station. BLAKE: “Looking down at him as he was screaming, he wanted out of there, he wanted help and you can't do anything. It's complete helplessness. It's horrifying, it's haunting, to feel so helpless, knowing that your child is in serious need of help.” When police, fire department, and EMS arrived at the family's home about 40 miles northwest of Wichita, Officer Ronnie Wagner had an idea. WAGNER: “I threaded some rope through a PVC pipe, tied a knot at the end of it so that it would slip, and we used it to wrap around under his shoulders here and lift him out of the hole.” Watch a video of the dramatic rescue through a special link in our transcript today at www.TheWorldview.com. Dr. David John is a member of the American Academy of Family Physicians and has demonstrated leadership and inspiration to his medical colleagues throughout his career. His passion for providing the best medical care possible has led him to assist in the implementation of population health programs, including mental health and substance abuse screenings. After arriving in New York in 1980, Dr. John completed his residency at Catholic Medical Center and has been rated an average of 5.0 stars by his patients. He obtained his Bachelor of Science from the City University of New York and earned his Medical Degree from Weill Medical College of Cornell University. He then completed an Internal Medicine Internship in Northshore University Long Island and a Family Medicine Residency in Brooklyn. Dr. John is board-certified in Family Medicine by the American Board of Family Physicians and is also a certified physician in his home country with the Medical Board of Trinidad and Tobago. After working in New York for a few years, Dr. John decided to return to Trinidad in 1994 to contribute to his homeland. During his earlier years in Trinidad, his involvement as a faculty member in the Lutheran US-Caribbean HIV/AIDS Twinning Initiative, contributed to the success of this program. Dr. John volunteered as a medical professional at the Cyril Ross Nursery, a home and treatment center for HIV-infected babies who were neglected and abandoned by their parents. His focus at the Nursery was pain control and other comfort care, and he worked alongside fellow Trinidadian and Ms. Universe of 1998, Wendy Fitzwilliam, to bring anti-retroviral treatment to the children. Dr. John's dedication to providing care to HIV-infected children led him to become a board member of the Caribbean Women's Health Association, in 2009, working tirelessly to advocate, address: comprehensive education, healthcare disparities, improve healthcare access and promote better resources for women's health, in the Caribbean region, while continuing to assist patients with HIV screening, housing, and other social services. Dr. David John is Associate Professor of Medicine and the Associate Dean for Alumni and Community Initiatives at the University of Missouri Kansas City, School of Medicine. He has also served as a "Docent" mentor within the SOM. He served as the Chair of the state of Hawaii Physicians' Health Committee while also serving on the board for over two decades. He is heavily involved in the recovery community in Hawaii and Kansas City. He is a father to two wonderful daughter and has served as a confidante and mentor for many students, residents and colleague over the years. In this episode, Dr. John shares how important honesty with oneself is to be able to live a truthful and honest life. He shares his story of addiction to alcohol and the moment of clarity when he realized he had a real problem. This led to him packing his bags immediately and checking himself into a rehabilitation center. He talks about the power alcohol had over him and how he has kept his disease in front of him as a reminder of where he does not ever want to go back. He believes physicians struggling with these issues need to be reminded that taking the first step and acknowledging the problem is the hardest but things do work out. Going through hardships is something that pushes us to change. Having a support group where one can be vulnerable and not feel judged is important. Having role models who have made it and been successful and who can guide others through this tumultuous journey is important. One has to remember that the risk of losing everything is inevitable if help and treatment is not sought. But if we take the time to be honest and really focus on recovery long term, there is much to be mended and gained. He states that GRATITUDE is important. According to Alcoholics Anonymous, you can be sober and not grateful BUT you can't stay sober! David's enriched life started with his sobriety birthday. He strongly believes that the only way for an addict to live is to get clean, work towards recovery and rebuild their life. He is grateful for his second life and feels blessed to have a chance to love others wholeheartedly. He also serves as deputy director of the Retirement Security Project at the Brookings Institution. RSP focuses on improving retirement savings in the United States, especially among moderate and low-income workers. Before joining AARP, David was a senior research fellow at the Heritage Foundation. He also has extensive public policy experience working for a money center bank, a law firm, a credit union trade association, and four members of the House of Representatives. David has written and spoken extensively about the importance of reforming the nation's retirement programs. He is co-author with J. Professor David J. Helfand, a faculty member at Columbia University for forty-five years, served nearly half of that time as Chair of the Department of Astronomy, a position he has again resumed. He also recently completed a four-year term as President of the American Astronomical Society, the professional society for astronomers, astrophysicists, planetary scientists and solar physicists in North America. He is the author of nearly 200 scientific publications and has mentored 22 PhD students, but most of his pedagogical efforts have been aimed at teaching science to non-science majors. He instituted the first change in Columbia's Core Curriculum in 50 years by introducing science to all first-year students. In 2005, he became involved with an effort to create Canada's first independent, non-profit, secular university, Quest University Canada. He served as a Visiting Tutor in the University's inaugural semester in the Fall of 2007 and was appointed President & Vice-Chancellor the following year to lead this innovative experiment in higher education. For six years in a row, Quest has been ranked #1 in North America in the National Survey of Student Engagement. He completed his term as President of Quest in the fall of 2015 and returned to Columbia where he now heads the Committee on Innovative Teaching and Learning.
