Lisa Egeli paints the people and places we care about most.  Her ability to bring out the character of her portrait subjects has led to repeated commissions by many clients. Lisa has completed multiple commissions each for Johns Hopkins Hospital, Elon University and Washington College. Other portrait clients include but are not limited to the State of Maryland, University of California Davis Medical School, the University of Maryland Medical School, the George Washington University Law School and the Maryland National Guard. Lisa's land and seascapes portray the uniqueness of a place in a moment of time, capturing the light and its seemingly magical effects on the scene. Her work is inspired by her love of nature, and a belief in the need to highlight the vulnerability of our natural world.  Her penchant for travel has allowed her to build a body of work including views from around the world, working both en plein air and in the studio, most commonly in oil and often in pastel or watercolor. In this weeks episode of "Pushing the Limits" I interview one of my idols in the world of health optimisation medicine, hyperbaric oxygen therapy and cellular health Dr Scott Sherr, a repeat guest to the show. This is a fascinating conversation that reveals so much about what we know so far and what we have yet to learn in this unique microbiome.  Some background on Dr. Jacques Ravel, he is a professor in the Department of Microbiology and Immunology, and the Associate Director for Genomics at The Institute for Genome Sciences, University of Maryland, School of Medicine in Baltimore Maryland. Over the past 15 years, he has developed a research program focused on applying modern genomic technologies and ecological principles to characterize the role and dynamics of the microbial communities inhabiting the human body in health and disease and better define the interactions between the host, the microbes and the environment that drives these ecological systems. Specifically, he is interested in the role of the vaginal microbiome in women's health in order to develop improved strategies to manage gynecological and obstetrics conditions. He has published over 225 peer-reviewed publications and has attracted grant income of more than $30 million. His work is currently being translated by developing novel live microbiome-based biotherapeutic formulations targeted to restore vaginal health, treat conditions such as bacterial vaginosis, urinary tract infections and improved fertility. His work earned him to be elected to the fellowship of the American Academy of Microbiology, and in 2015, he was awarded the Blaise Pascal international research chair. He is the founder and editor-in-chief of the journal Microbiome. He joined the Institute for Genome Sciences at The University of Maryland Medical School in 2007 as an associate professor. The vaginal microbiome is just like the gut, it's a collection of bacteria that live together to perform a function that is beneficial. If it lives in the body, it's often beneficial to the host. When it comes to the vaginal microbiota, there are five types of women, four of these types are dominated by lactobacillus, one of the four species (lactobacillus crispatus, lactobacillus iners, lactobacillus gasseri, lactobacillus jensenii). And one of them is not and it's represented, it's more like the gut, this type is very diverse. So you can see it's diverse and very even, a lot more even than what you see in the others. Those bacteria, they acidify the vagina, the pH of the vagina is about 3.6-4, so it's very acidic Those that have an acidified environment are those that you would call very well-protected 25-30% of women that don't have lactobacillus How does the composition of the vaginal microbiome impact a woman's overall health? Without a healthy vaginal microbiome, women are subject to many different risks Sexually transmitted infections A woman being able to get pregnant - the implantation of the egg Chronic yeast infections Vaginal dryness which can make intercourse very painful Can lubricants affect the vaginal microbiome negatively? They are very hypertonic, that's a big problem because what happens is it dries the cells Most lubricants, especially in medicine, contain antimicrobial agents, so it kills even the good bacteria Not every woman is affected by the application of lubricant What are some of the things that people are doing in day to day life that seem to be hurting the vaginal microbiome? Douching - you're basically washing your vagina with chemicals Smoking has a major impact on the vaginal microbiota Poor diet A really heavy dose of azithromycin has a really detrimental effect on the vaginal microbiota What can women start doing right now to help support a healthy vaginal microbiome? Dr. Offit was a member of the Advisory Committee on Immunization Practices to the Centers for Disease Control and Prevention, a founding advisory board member of the Autism Science Foundation and the Foundation for Vaccine Research, a member of the Institute of Medicine, and co-editor of the foremost vaccine text, Vaccines. Dr. Offit is a recipient of many awards including the J. Edmund Bradley Prize for Excellence in Pediatrics from the University of Maryland Medical School, the Young Investigator Award in Vaccine Development from the Infectious Disease Society of America, a Research Career Development Award from the National Institutes of Health, and the Sabin Vaccine Institute Gold Medal. Dr. Offit has published more than 150 papers in medical and scientific journals in the areas of rotavirus-specific immune responses and vaccine safety. He is also the co-inventor of a landmark rotavirus vaccine recommended for universal use in infants by the CDC. For this achievement, Dr. Offit received the Luigi Mastroianni and William Osler Awards from the University of Pennsylvania School of Medicine, the Charles Mérieux Award from the National Foundation for Infectious Diseases, and was honored by Bill and Melinda Gates during the launch of their Foundation's Living Proof Project for global health. In addition, he has received numerous other awards and honors for his groundbreaking work. Today we are fortunate to welcome Sam Hsu, MD, RDMS, Assistant Professor for the University of Maryland Medical School. Dr. Hsu is the acting Emergency Department Ultrasound Director at one of the UMMC satellite centers her in Baltimore, Mercy Medical Center. He also takes on command of medical student emergency ultrasound education for the medical school. I guess you could say, he lives and breaths ED ultrasound! Today he takes us through his algorithm on how to approach ultrasonography of the lungs. I have attending many lectures on the topic and have even performed lung US research, but this lecture is the best presentation I have ever seen on the topic. You NEED to watch this lecture.

This week on IAQ Radio we are Hung K. Cheung, MD, MPH, FACOEM, President of Dr. Cheung/ OEM Advisor, LLC. in Baltimore, Maryland. Dr. Cheung was formerly with Building Health Sciences and was also the Maryland State Medical Director. He is a graduate of Loyola College in Baltimore and received his Medical degree from University of Maryland Medical School. His first residency and board certification was in Primary Care/ Internal Medicine from University of Maryland Hospital. He later received his Masterâ??s in Public Health degree from Johns Hopkins University in Environmental Science. He returned for further Postgraduate training (and received his second board certification) in Preventive Medicine specializing in Occupational Medicine from Thomas Jefferson University in Philadelphia. For the last sixteen years Dr. Cheung has been working full time in Occupational and Environmental Medicine, and Public Health consultation. In June 2000, he was designated as the State Medical Director for the State of Maryland agencies and State employees and in that capacity has worked closely with the State of Maryland (and local municipalities and many large corporations) in building a safe, healthful and productive workplace. Dr Cheungâ??s subspecialties are Respiratory Environmental Medicine, Indoor Environmental Quality investigations, Risk communication and Medical Advisory services. Dr. Cheung will also be the keynote speaker at the IAQ Training Institute/IAQ Radio Healthy Building Professional Summit August 21st at Seven Springs, PA. His presentation title is Learning to resolve rather than to merely inspect or sample. The hope is that tomorrow's physicians will take this perspective with them into practice, asking better diagnostic questions and being more sensitive to the patient's needs after diagnosis. During this week's show, I'll share what I said to the class, their questions and comments, and the take-aways from the effort to educate medical students in a new and different way.
