In this episode, we're going to continue our discussion with Dr. Andrea Spiker and focus on open surgical treatment for hip preservation, then wrap up with a discussion on post-op rehab and return to play.Our conversation picks back up with an article from the September 2017 issue of AJSM titled “Early Functional Outcomes of Periacetabular Osteotomy After Failed Hip Arthroscopic Surgery for Symptomatic Acetabular Dysplasia.” Bryan Kelly and his team at HSS investigated the effect of prior arthroscopic hip surgery on clinical outcomes after periacetabular osteotomy or “PAO”. They found that patients who previously underwent hip arthroscopy had inferior functional outcomes at 6 months and 12 months after PAO compared to patients with no prior hip surgery. The authors note that both groups significantly improved post-op and there was no difference in complications or reoperation between the groups.We finish up today with a surgical technique article authored by our guest, Dr. Andrea Spiker, from the August 2022 issue of Arthroscopy Techniques. The article is titled “Treatment of Coxa Profunda with Open Surgical Hip Dislocation, Rim Resection, Cam Resection, and Labral Reconstruction” and describes an open surgical approach to address many of the challenges posed by coxa profunda. For our listeners, coxa profunda is essentially a deep acetabular socket. This differs from acetabular protrusio, in which the femoral head and socket displace into the pelvis. The global acetabular over-coverage associated with coxa profunda makes arthroscopic management very challenging in terms of obtaining adequate hip distraction and concerns for traction injury. For these reasons, an open approach has been proposed.

Sales is all about solving problems. But are you going about solving problems the right way? In today's episode of The Sales Evangelist, Donald is joined by guest Bryan Kelly to discuss how we can ask more beautiful questions, using the book A More Beautiful Question by Warren Berger as a framework. Phase One: Why? When we ask ‘why,' it requires you to adjust the way you look at the world. There are three different ways to get into this mentality. First, step back and disengage. Taking a break will break up your routine and help inspiration strike. Next, challenge your assumptions. Whether they're your assumptions or the assumptions of others, asking questions about your current process will help you find new avenues to explore. Finally, question the questions. Reframe a question to challenge the expected answer. Remember the five why's. (Literally ask the question “why” five times in a row to get to the root understanding of the situation.) Phase Two: Wondering “What If?” Get everybody (including yourself) to avoid thinking about specifics. Similar to phase one, there are a few practices you can implement: Combinatorial thinking is one method where you look at combining existing practices and how that combination can lead to new possibilities. Next, you can live with the question. We often try to answer questions in the moment, but that's not usually necessary. Instead, take time to relax and distance yourself from a problem to view the situation with a fresh perspective. Finally, think of wrong ideas. It might seem counterproductive, but this process can help lead to the concept you need. Phase Three: Determining How We want to jump to the “how” of a solution, but there's a reason it's the last stage. If you start there, you're missing the critical time to go deeper into the question itself. Converge the ideas you've explored into one idea worth pursuing and share it for feedback. Give form to the ideas you've worked through. Before you go and ask others for feedback, bring the idea to life. In other industries, it might look like a prototype. In the sales space, write a summary, proposal, chart, or whatever would help people give the best feedback. Rapidly test and learn. If you have several potential solutions, run a quick test to see how the change affects your problem. Bryan Kelly, a native Texan, is an Intensive Care Unit nurse. After college, he went to Chiropractic School, along with his brother Scott, in 2003. Wanting something different after graduation, Bryan entered nursing school, graduated, and has been practicing since 2008. The majority of his work is in Critical Care in the ICU. Bryan now lives in Fort Worth, Texas with his wife Andrea and their two children. Fun fact: Bryan ran a marathon, which is how he met co-host Rand, with absolutely no training!

He has also pursued additional dedicated training with Dr. Bryan Kelly in arthroscopic hip surgery for young athletes. While in New York, he was an assistant team physician for the New Jersey Nets professional basketball and New York Mets professional baseball organizations with Dr. Riley Williams, Struan Coleman, and David Altchek. He was also an orthopaedic consultant for the U.S. Open Tennis Tournament in 2007 and 2008 with Dr. David Dines and an assistant team physician for Iona College Athletic Programs. Dr. Bedi has also been recognized with the Leonard Marmor Outstanding Orthopaedic Resident Award, and the Hospital for Special Surgery Philip D. Wilson Award for Excellence in both 2008 and 2009 for his efforts in orthopaedic research and tendon-bone healing studies with Dr. Scott Rodeo. He is the winner of the 2010 Neer Award of the American Shoulder and Elbow Surgeons (ASES) and a recipient of the 2010 Cabaud Award from the American Orthopaedic Society for Sports Medicine (AOSSM). He has authored over one hundred articles, chapters, and peer-reviewed publications on shoulder, elbow, knee, and hip injuries in athletes.

Bryan Kelly is the CEO of Prison Entrepreneurship Program (PEP) and Sam Rangel is a PEP entrepreneur. Prison Entrepreneurship Program is a business program that connects executives, entrepreneurs, and MBA students with convicted felons. PEP is a nine-month "mini MBA" course culminating in a shark-tank style competition. 100% of PEP's graduates land a job within 90 days of release and nearly 100% remain employed after 12 months. Surprisingly, this non-profit receives no tax dollars. They are funded entirely by our community, from every dollar to every volunteer (no volunteers has ever been victimized while serving the program). PEP is a tremendous success and they just started a program in a women's prison. They are looking to replicate the system in other prisons and we can help by going to www.pep.org and giving a few dollars or a few hours of our time.

Hey everybody, we're back. That's right, Spotty Translation is back, and it didn't take singing dinosaurs to make it happen. In fact, all it took was some simple social engineering to convince Whales are Whales co-founder Bryan Kelly to bring back the show, and he even agreed to come along for the ride! How exciting!
