CT is the best initial imaging modality, although it has low sensitivity. If there is high concern for a pancreas injury based on mechanism or associated injuries, further investigation is required. 3. Pancreas injuries are like crawfish: suck the head and eat the tail. 4. Injuries to the left of the SMV can generally be treated with distal pancreatectomy and splenectomy, whereas injuries to the right of the SMV are usually drained. 5. Its important to identify and address any concomitant injuries, with duodenal injuries being the most common in higher grade injuries. 6. In this podcast, Dr. Kristin Zimmermann (DE) and Dr. Felix Campos-Juanatey (ES) discuss a grade 4 renal trauma case in a young woman who was kicked by a horse. They talk through the decision-making process for her diagnosis, treatment and follow-up, as well as the complications encounted.Diagnostics included FAST (Focused Assessment Sonography in Trauma), and then the American Association of Surgery of Trauma classification (AAST) was applied for clinical decision-making and planning further management. The patient underwent a CT scan, which revealed a blunt grade 4 trauma with urinary extravasation. The conservative approach involved bed rest, thrombo-prophylaxis, and a long course of antibiotics due to the risk of infection, but the patient experienced a drop in haemoglobin levels. This prompted more investigations and a repeat CT scan, which revealed active bleeding. Subsequently, an embolisation procedure successfully stopped the bleeding. The patient recovered well, and after monitoring her condition, she was discharged with pain medication. Follow-up appointments and tests were planned to assess her long-term recovery.Dr. Zimmermann and Dr. Campos-Juanatey are members of the EAU Guidelines panel on Urological Trauma.

To operate or not to operate, to drain the urine leak or to not drain it, those are the questions. Join our Miami Trauma team including Drs. Urréchaga, Neeman, and Rattan- in their final episode together! - as they discuss how to navigate the ins and outs of renal trauma!  Learning Objectives:  ·       Define the different renal injury grades and how to manage each, ·       Identify when to take the renal trauma patient to the operating room, ·       Discuss when to involve IR for urinary drainage or embolization. ·       Explain when and how to perform a nephrectomy. ·       Debate the treatment of penetrating zone two injuries- to explore or not explore? Quick Hits: 1.     Most kidney injuries, the vast majority, can be non-operatively managed.  2.     For pretty much all AAST grade of injury, the choice to go to the OR immediately lies in whether the patient is stable or unstable.  3.     If there is a urinary leak seen on imaging, it can usually just be observed and followed with repeat imaging to determine the need for drainage, unless the injury is significant or if there is injury to the renal pelvis- then the patient will usually need a drainage procedure.  4.     Consider IR in any stable patient found to have active extravasation, fistula, or pseudoaneurysm. 5.     In the case of an unstable patient, except very rare circumstances, you should be going to the OR  6.     If there is another cause of instability, address that first. If you're opening Gerota's fascia, be prepared to commit to a nephrectomy.  7.     In penetrating injury, the formal teaching is mandatory exploration of a renal hematoma. An avid educator and recipient of several teaching awards, Dr. Kim is an Associate Professor of Clinical Surgery and the former Co-Chair of the College of Applied Anatomy at the David Geffen School of Medicine at UCLA. Dr. Kim is actively involved in and holds numerous leadership roles in several key national trauma organizations including EAST, AAST, and the American College of Surgeons Committee on Trauma. He has published over 140 peer-reviewed publications, 25 book chapters, and several Practice Management Guidelines focused on the optimal care of critically injured and ill patients. Dr. Kim hosts the very popular Trauma ICU Rounds Podcast and is active across several social media platforms. Dr. Kim joins us with his wife, Alexis, their four children, and dog. Dr. Burlew is an active surgical investigator, educator, and clinician; she has received multiple awards in each of these areas including the J. Cuthbert Owens Award, the DHMC Award for Academic Excellence, the Bartle Faculty Teaching Award, the Eiseman Medical Student Teaching Award, the Academy of Medical Educator's award for Excellence in Mentoring, and the 2017 American College of Surgeons Travelling Fellowship to Australia/New Zealand. She serves on the Committee on Trauma for the American College of Surgeons, the Board of Managers for the American Association for the Surgery of Trauma, and is a Past-President of the Southwestern Surgical Congress. She is on the Editorial Board of The Journal of Trauma and Acute Care Surgery, The World Journal of Emergency Surgery, and Trauma Surgery and Acute Care Open, and reviews for an additional 15 journals in an ad hoc capacity. She has authored over 200 peer-reviewed articles and 70 book chapters. She has given over 100 national lectures and scientific presentations.

Joe DuBose is a graduate of the Virginia Military Institute and the University of Virginia School of Medicine. He performed surgical training at Keesler Air Force Base Medical Center and the University of Virginia from 2001-2006. Additional fellowship training in surgical critical care and trauma surgery was completed at Los Angeles County + University of Southern California Hospital from 2006-2008. Lt Col DuBose has been a staff trauma surgeon and surgical intensivist at Wilford Hall Medical Center and as an assistant professor of surgery at R Adams Cowley Shock Trauma Center / University of Maryland since 2008. During that period he has deployed as a trauma director once to the Air Force Theater Hospital at Balad Air Base in Iraq (2009) and twice in support of Operation Enduring Freedom in Afghanistan (Kandahar-2010; Bagram Air Base 2011-2012). He presently holds the title of Clinical Professor at the University of Maryland Medical Center. Grace Rozycki is a professor of surgery. Primarily practicing at Johns Hopkins Bayview Medical Center, Dr. Rozycki specializes in trauma surgery and surgical critical care. Don Jenkins, Joseph Dubose and Eileen Bulger explain NTI, NTRR and CNTR – and how these organizations can take your research to the next level!  The National Trauma Institute (NTI) is a nonprofit, national coordinating center for trauma-related research and a strong advocate for an increased federal investment in trauma research. Projects under NTI’s management have engaged more than 70 investigators across the U.S. and drawn nearly $90 million in federal and state funding since its start in 2006. NTI is the organizing force behind the Coalition for National Trauma Research (CNTR), which includes EAST, AAST, ACS-COT and WTA.  Dr. Donald Jenkins is a founding board member of NTI and Principal Investigator on the National Trauma Research Repository (NTRR) project, a comprehensive web platform for sharing research data among trauma investigators. Dr. Joseph Dubose is the Principal Investigator on the Prospective Observational Vascular Injury Trial (PROOVIT), a study managed by NTI. Dr. Eileen Bulger is an NTI and CNTR board member and the Principal Investigator on the National Trauma Research Action Plan (NTRAP) project, managed by CNTR. NTRAP is a large-scale effort to establish a national research agenda that spans the continuum of care. The NTRR launched in 2018 and is now accepting clinical trials data. De filmstreamingdienst Amazon Prime Video zal vermoedelijk op korte termijn in Nederland verschijnen, aldus bronnen in de tv-wereld. Daarmee krijgen Netflix en Videoland er een nieuwe concurrent bij op de Nederlandse markt. De komst van Prime Video zou voor Amazon opnieuw een kleine stap zijn richting volwaardige aanwezigheid in Nederland. Maar voor fysieke producten moeten Nederlanders nog altijd bij de Duitse versie van de webwinkel zijn. Hoe serieus neemt Amazon Nederland? Overnames Twitter en SoundCloud Twitter is op zoek naar een koper - en grote techbedrijven lijken interesse te hebben. Volgens bronnen denken onder andere Google, Microsoft en Disney er over na op Twitter te kopen. Tegelijkertijd zou muziekdienst SoundCloud te koop staan en heeft Spotify hier interesse in. Met deze potentiële overnames kunnen techbedrijven groeien, maar wat zijn ze precies met die aanwinsten van plan? UberEats Thuisbezorgd heeft er in Nederland steeds meer concurrenten bij. Na Deliveroo en Foodora is nu ook UberEats gestart. De dienst levert eten bij klanten thuis af, zelfs als het restaurant geen bezorgservice heeft. UberEats belooft bezorging een stuk makkelijker te maken, maar heeft dus te maken met flinke concurrentie. Het bezorglandschap lijkt steeds meer op een Wild Westen.

The lead article is the presidential address by Dr. Richard Falcone from the Cincinnati Children’s Hospital. Dr. Falcone’s central theme was the critical role of teamwork and team communication in caring for the injured child. The ensuing paper is the keynote lecture by Dr. Joe Tepas from the University of Florida at Jacksonville. Dr. Tepas provides an informative overview of the past, present, and future of pediatric trauma surgery. The next paper is by Dr. Sheila Hanson and colleagues from multiple pediatric surgical programs who address the ongoing debate of the appropriate prophylaxis for venous thromboembolism in the pediatric trauma patient. In a related paper, Dr. Ioannis Liras and colleagues from the University of Texas in Houston evaluated age related hypercoagulability in their rapid TEG database of greater than 7000 trauma activations from 2010-2013. I will conclude by suggesting careful reading of the AAST multi-institutional of hemorrhage from severe pelvic fractures authored by Dr. Todd Constanti and AAST members. Transcript

The lead paper is by Dr. Kenji Inaba and colleagues from the AAST who performed a multicenter review of temporary intravascular shunts in civilian trauma centers from 2005-2013. Next, we have Dr. Rachel Russo and colleagues from the Air Force conducted studies in a swine model of controlled hemorrhage that demonstrated the concept of partial REBOA inflation in Zone I for extending the golden hour. Joining Drs. Inaba and Russo, we have Dr. Jamie Coleman and associates from the trauma services at Indiana University describe a series of traumatic abdominal wall hernias from 2002-2014. Dr. Daisy Chou and associates from Cedars-Sinai in Los Angeles analyze the impact of field intubation in patients with hemorrhagic shock. Finally, I would like to highlight the systematic review of REBOA in the management of hemorrhagic shock written by Dr. Jonathan Morrison from the Glasgow Royal Infirmary and colleagues from around the world. Dr. Josh Brown and colleagues from the University of Pittsburgh detail their AAST presentation on the impact of geographic distribution of trauma centers and their relationship to injury-related mortality. Dr. Brandon Chapman et al at Denver Health/University of Colorado described a chest CT based scoring system of rib fractures that are associated with pneumonia, mechanical ventilation, and need for a tracheostomy. The lead paper presented at the AAST, is authored by Dr. Laura Moore from UT Houston along with colleagues from the Cowley Shock Trauma Center in Baltimore. In a related paper, presented at EAST, Dr. Sundeep Guliani et al from Virginia Commonwealth University demonstrated the reliability of ultrasound guided central aortic wire placement, thus, theoretically avoiding, the need for fluoroscopy for REBOA placement. Dr. David Notrica and colleagues from ATOMAC (which apparently is an abbreviation for Arizona-Texas-Oklahoma-Memphis-Arkansas Consortium), a group of ACS verified Level I pediatric trauma centers, reviewed the relevant literature with respective grading their recommendations for organ injuries ATOMAC proposed practice management guideline based on hemodynamic status rather than organ injury score formally employed in the American Pediatric Surgery Association guidelines that have been considered the standard for over two decades. Dr. Carl Wahlgren and Dr. Bjorn Kagsterman from the Karolinski Institute in Stockholm provide a review of pediatric vascular injuries managed in Sweden over the past 25 years. Transcript

The next paper, also an AAST presentation, is by Dr. Laura Godat and colleagues from the University of California at San Diego who queried the California Statewide Hospital Discharge Database to determine the time course of VTE in high-risk trauma patients. In a related paper, Dr. Casey Allen and associates from the University of Miami review their experience with major venous injuries following penetrating trauma, 74% of these were gunshot wounds, to determine the relative risk of pulmonary embolus with repair versus ligation. The final paper that generated considerable interest was a large animal study by Dr. Mohamad Tiba and colleagues from the University of Michigan who introduced a modified pelvic binder for hemostasis. Marci Williams is currently a Center Director, Wellness Coach, and a Management Consultant of a large recreation and fitness facility in which she oversees the day-to-day administration, operations, and programming. She has over 28 years of experience in the fitness and recreation industries both in the private and public sector. She holds multiple certifications along with a degree in Psychology.
