Hospice Nurse Julie McFadden grew up in a normal 80s childhood, but obsessive thoughts plagued her from a young age. The people around her constantly asked “why can't you relax?” And she immediately internalized the idea that there was something wrong with her.At 13, she found alcohol and the thoughts would go away. It became her first tool to manage her obsessive compulsive anxiety. Now she knew how to keep things together, but she needed alcohol.In college she kept the facade going even when she'd evolved to a daily heroin user. As things got darker and darker she told herself she wasn't any worse off than anyone else. But in the back of her mind she knew there was something wrong.She was able to get off heroin and told herself getting off that single substance would solve her problems. Her problems continued as she drank and used drugs through nursing school and as she began her career. Even with all the partying she could always keep the wheels on.Then things became more and more problematic and she felt like she needed to change. She moved to California with a vow to change her life. Hospice Nurse Julie delves into the details of dying, so that we can all learn more ahead of time and be better prepared when someone we love is at the end of their life. “I know, as a hospice nurse who's used to seeing a body dying, that this is very normal: their face is going to look different; their eyes are often open, not blinking; they are not making eye contact; mouths are usually open, as all our muscles are relaxing; breathing changes; they are usually not eating or drinking; and they are probably unconscious for the most part." All of this is normal – we just don't know it, so we can find it unsettling at best and terrifying at worst. Julie and I have a mutual goal: to normalize talking about death.

Julie's mission is to educate people about death and help them feel more comfortable about it since it's the one thing we all do. Listen in to hear how Julie got into hospice nursing (it wasn't her first or even second pick), what active death is and what it's like, some of the phenomena people often experience close to death, and what life is really all about. Topics We Discuss: [2:56] Julie's experience creating a cult-like following by talking about death and dying on social media. [4:03] Julie's shift from ICU nursing to hospice nursing. Death happens a lot in the ICU but isn't discussed. Julie knew that needed to change. She went into hospice nursing because she knew there had to be a better way to die. [10:22] Hospice vs palliative care vs death doulas. [15:29] What is actively dying like? It usually lasts a few hours to a few days. Our body's natural dying processes kick in, similar to the way our bodies instinctively birth. We all pretty much look the same when we're actively dying, no matter what disease we're dying from. The process may seem graphic if you haven't seen it before, but it's all normal. [22:12] “Rallies” sometimes occur right before someone actively dies. The person can seem almost like they've made a full recovery for a day before suddenly dying. “Visioning”, or a dying individual seeing their dead loved ones, typically occurs in the month to weeks before death while the individual is still perfectly lucid. [26:03] Julie knows that visioning is not psychosis or drug-induced delirium or other conditions commonly attributed as the cause because she has seen those things in a clinical setting and says it's completely different. Many people find it very comforting. [29:54] Julie's shared death experience. See William Peter's book "At Heaven's Door" (affiliate) for more anecdotal stories of people's shared death experiences. [36:35] Death and dying and spirituality are not mutually exclusive. Julie has always been spiritual, and her work as a hospice nurse has only solidified her belief in something more. Dying and birth feel like we're going to or coming from a place we all know. [40:33] Hospice has made Julie realize that life is about connecting with people. Deaths of Julie's loved ones have given her the understanding to not take life for granted, to not hold grudges, and to be easy on herself and others. Connecting with a higher power and serving others brings contentment and connection. [44:42] Medical Aid In Dying (MAID) or “assisted suicide”. Julie is supportive of this practice. Some states in the US allow this. It can be a beautiful, intentional experience. [51:26] Psychedelics and death care. There is some promising research behind psychedelics, and Julie knows of healthcare practitioners who have witnessed it and are in favor of it. I am editing this episode as I sit next to my Grandpa's bedside. He was put on hospice a few days ago. I feel so incredibly grateful to have the opportunity to come be with him during his last days on earth. It is absolutely beautiful as I look at his wristband boasting "97 yrs" and think back on how much life he has experienced. It's been emotional but mostly comforting being here with him. He is ready to see Jesus and many loved ones who are waiting for him. When I interviewed Julie I had no idea that within a few months I would be experiencing so many things she talked about. Hospice Nurse Julie is our guest, talking about why her hospice videos have gone viral, and what she finds meaningful about working in end-of-life care. RN Julie McFadden started making the videos, never dreaming that four days later so many people would connect with her content. As a hospice nurse, Julie has heard all the questions from patients and families dealing with serious illness. Topics of her videos include the dying process, end of life medications, visioning, and what a natural death looks like. The average age of a TikTok user is 16-24 years old, not the group you'd think would be interested in conversations about death.
