In this episode of Digital Conversations, Jay Roszhart, President of the Ambulatory Group at Memorial Health, joined us for a deep discussion about how he envisions the future of healthcare — one that is predicated on building loyalty, trust, and long-term relationships. Jay is driving a digital transformation that is designed to make the patient experience as easy as using Google or Facebook. This podcast describes the findings from a population-based retrospective decedent cohort study which suggest that palliative care involvement may be associated with a decreased need for high intensity of care during the final weeks of life in adolescents and young adults with cancer. TRANSCRIPT This JCO Podcast provides observations and commentary on the JCO article “Impact of Palliative Care Involvement on End-of-Life Care Patterns Among Adolescents and Young Adults with Cancer: A Population-Based Cohort Study” by Kassam et al. My name is Erica Kaye, and I am Director of the Quality of Life and Palliative Care Research Division at St. Jude Children's Research Hospital in Memphis, Tennessee, USA. My oncologic specialty is pediatric palliative oncology. In this paper, the authors aim to evaluate the prevalence and predictors of high-intensity end-of-life care in adolescents and young adults with cancer in Ontario, Canada, as well as assess the impact of palliative care involvement on intensity of care received at end of life. This topic is timely and important. Growing evidence demonstrates that adolescents and young adults with cancer, defined by the National Cancer Institute as patients diagnosed between the ages of 15 and 39, face a unique set of circumstances that may adversely impact their end-of-life experiences. Several single-institution studies in the U.S. have suggested that adolescents and young adults with advanced cancer disproportionately receive high intensity medical care in their final month of life compared to children or older adults, and these findings recently have been replicated in population-level studies in California and New York. These observations raise concerns about exposure of a vulnerable population to toxicity from high intensity care at a time when such interventions are less likely to offer meaningful benefit. However, these studies had several notable limitations: some involved only insured patients; others presented data only from inpatient end-of-life care; and none evaluated the impact of palliative care provision on the intensity of end-of-life care for adolescents and young adults with cancer. Moreover, findings from U.S.-centric data sets may not be generalizable to other countries. In this study, the authors assembled a retrospective decedent cohort of adolescents and young adults with cancer who died between 2000 and 2017 in Ontario, Canada using a provincial registry linked to population-based health-care data. Predicated on previously published validated metrics, they used a primary composite measure for high-intensity end-of-life care that included receipt of intravenous chemotherapy less than 14 days from death; more than one emergency department visit; and more than one hospitalization or intensive care unit admission less than 30 days from death. Secondary outcomes included measures of the most intensive end-of-life care, such as intensive care unit admission within 30 days of death, mechanical ventilation within 14 days of death, and death in the ICU, as well as palliative care physician involvement. Predictors of outcomes were determined using regression models. In a cohort of more than 7,000 adolescents and young adults with cancer across Ontario, authors found that approximately 44% experienced high-intensity end-of-life care – double the rate for adults and slightly higher than the rate for children in the region. This percentage is lower than that seen in single-site and population-level studies in the U.S., however lack of consistently defined high-intensity end-of-life indicators for adolescents and young adults with cancer make it challenging to compare findings across studies. Importantly, adolescents and young adults with hematologic malignancies were at highest risk for receipt of high-intensity care at the end of life in Ontario, affirming findings seen across pediatric and adult populations in different countries. Some hypothesize that unique pathologies specific to hematologic malignancies may increase the risk of experiencing higher intensity end-of-life care. For example, a patient with refractory leukemia often requires frequent blood transfusions to prolong life and optimize quality time; this can be challenging to coordinate in the outpatient setting and may necessitate inpatient hospitalizations at the end of life. In both the U.S. and Canada, continuation of transfusion support often precludes hospice enrollment, further preventing patients from receipt of subspecialty palliative care in the home. In this context, legislation to support concurrent provision of palliative and hospice care with cancer-directed therapy may have the potential to mitigate high-intensity end-of-life experiences for this vulnerable patient population. This study also found that palliative care physician involvement substantially reduced the odds of adolescents and young adults with cancer receiving high-intensity end-of-life care, including the most intensive experiences such as mechanical ventilation and dying in the intensive care unit. Palliative care physicians may lessen intensity of end-of-life care by promoting conversations about goals of care and advance care planning, coordinating complex medical care in the community, and providing real-time symptom management to decrease emergency visits or hospitalizations. However, further research is needed to explore these hypotheses. Ultimately, this study provides compelling evidence that palliative care involvement may lessen the risk of high-intensity end-of-life care in adolescents and adults with cancer. Unfortunately, universal access to subspecialty palliative care remains challenging, particularly in rural regions. Disparities in access to hospice and palliative care services for racialized communities remain deeply problematic as well. This study supports the need for institutional and regional programs and policies to provide equitable access to palliative care for adolescents and adults with cancer. Strengths of this study include its population-level design, large health services dataset, and use of relatively standardized indicators for defining high-intensity end-of-life care. Study limitations include its retrospective design, which inherently precludes causal inference. Additionally, involvement of a palliative care physician was used as a proxy for integration of subspecialty palliative care; however, subspecialty palliative care is an interdisciplinary field in which care may be provided by non-physicians, and the study did not account for this phenomenon. Lastly, the study is predicated on certain assumptions about the negative ramifications of high-intensity end-of-life care. Notably, certain experiences categorized as high-intensity care, such as more than one hospitalization during the final month of life, may not be experienced as high-intensity or burdensome by patients or families. An adolescent patient may wish to be hospitalized on multiple occasions to optimize comfort at the end of life. Or a family may request hospitalization for respite, or to protect siblings from seeing suffering at end of life. Although technically high-intensity, these interventions also may represent goal-concordant comfort care, and further research is needed to determine best practices for optimizing care that aligns with the goals of adolescents and young adults at end of life. The majority of people know deep down that their reality isn't what they thought life would be. The reality for the majority of the masses is full of calamity, depression and constant stress. The truth is that the reality that we're all living is a Babylonian facade. Predicated off of deception and manipulation. The eduction, financial, government, healthcare and religious indoctrination systems have brainwashed the vast majority of people into a reality full of stress. A reality that is lived in the rat race and the hamster wheel. Ecclesiastes 1: [18]. The truth is, the vast majority of people are unaware that they are truly enslaved by these indoctrination systems and the overall Babylonian, Nephilim beast system. Systems that were created to give us false realities and take away our quality of life. Systems that were created to drain our energy, life force and time here in the creation of The Most High. Clarity is the Key to All Success All success, whether it is in business, relationships, or health comes from clarity. Imagine that we could rewind to the beginning of the new year and examine two different men. One of them sets his new year’s resolutions with a simple statement “Get healthy”. The other, clearly writes out his goals for his health, stating “I will go to the gym at least twice a week, working through a comprehensive weight lifting program with the goal of losing 10 lbs. of fat and adding 50 lbs. to all of my major lifts” Which one do you think will be successful? You see, without a clear roadmap for who we are and what we want to achieve, we will never find true success. You must know exactly what you want, when you want it, and why you want it. Without this clarity, you will find yourself wandering aimlessly for years, or even decades of your life and looking back, wishing that you’d had more direction. 2. Real Change is Predicated on Accountability Anytime we set out to make real changes in our lives, whether it is overcoming an addiction, repairing a marriage, starting a business, or achieving our dream body, we need accountability. Whenever you “go it alone” you are the only person who you can disappoint. If you miss a workout, spend too much money on going out, or relapse into old habits of drugs, alcohol, and porn, then you are the only person there to catch you when you fall. However, when you build a structure of accountability into your life and have other men who are willing to show up around you and hold you to a higher standard and help you up when you fall. When you have men who you are accountable to, you have a support system that will not allow you to fail, a support system that will call you on your crap, and encourage you whenever you are doing it right. No man is an island and the only true way to succeed is with a team of like-minded individuals around you. 3. Failure is Not an Option… It’s a Given So often, you hear the phrase “Success is my only option, failure is not.” As nice as that sentiment is, it is also wildly inaccurate. Not only is failure an option, it is a given! There is no way that you will succeed without failing on some level, it is simply a part of life. But what defines a man and the legacy that he creates is not his failures, what defines a man is the way that he responds to failures. Are you going to lie down and allow life to kick the motivation, joy, and passion out of you? Or are you going to get up, look failure in the eyes, smile, and be grateful for the lessons that failure has taught you? 4. Life is a Marathon All too often, men, and especially young men, feel the need to go, go go, pushing 60, 80, or even 100 hour weeks. They work themselves bare to the bone, keeping their nose to the grindstone trying to “sprint” their way to success. But you need to remember, life is a marathon. You cannot just “sprint” your way to a balanced and fulfilling life. A life of joy and happiness comes from years of doing the small things with excellence over and over and over again. Working yourself into oblivion is your gateway to an early grave. Pacing yourself and prioritizing the people and relationships in your life, on the other hand, is your gateway to success and fulfillment. 5. Remember 168 One of the only things in life that is truly fair is time. Every single person has the same 168 hours in a week. Bill Gates, Oprah, Elon Musk, all of these people have the same 168 hours in a week as you and me, the only difference is what they are doing with them. Remember that each week you are given a gift, you are given 168 hours of precious time that you will never get back. What are you going to do with those hours? Are you going to invest them wisely ot build a legacy and create a life of love and joy? Or are you going to squander them on television, video games, porn, and other traps? The choice is yours.

Abstract: Evolving diversity in globally circulating HIV-1 subtypes presents formidable challenge in defining and developing neutralizing antibodies for prevention and treatment. HIV-1 subtype C is responsible for majority of global HIV-1 infections. Broadly neutralizing antibodies (bnAbs) capable of neutralizing distinct HIV-1 subtypes by targeting conserved vulnerable epitopes on viral envelope protein (Env) are being considered as promising antiviral agents for prevention and treatment. In the present study, we examined the diversity in genetic signatures and attributes that differentiate region-specific global HIV-1 subtype C gp120 sequences associated with virus neutralization outcomes to key bnAbs having distinct epitope specificities. A total of 1814 full length HIV-1 subtype C gp120 sequence from 37 countries were retrieved from Los Alamos National Laboratory HIV database (www.hiv.lanl.gov). The amino acid sequences were assessed for their phylogenetic association, variable loop lengths and prevalence of potential N-linked glycosylation sites (pNLGS). Responses of these sequences to bnAbs were predicted with a machine learning algorithm bNAb-ReP and compared with those reported in the CATNAP database. Phylogenetically, sequences from Asian countries including India clustered together however differed significantly when compared with pan African subtype C sequences. Variable loop lengths within Indian and African clusters were distinct from each other, specifically V1, V2 and V4 loops. Furthermore, V1V2 and V2 alone sequences were also found to vary significantly in their charges. Pairwise analyses at each of the 25 pNLG sites indicated distinct country specific profiles. Ask Yourself “What Do I Really Want Out of This?” One of the greatest tragedies of modern society is that most people are vigorously pursuing goals that aren’t congruent with who they are and what they really want. Society, our friends, and our families all encourage us to earn more money, pack on more muscle, get married (or not), buy a house, and do what “good” little boys do. Unfortunately, most people never actually take the time to ask themselves what they really want. They never set aside the time to think purposefully about their life, their vision, and their goals. As a result, more and more people arrive at their deathbeds wishing that they could live their lives over again in a different way. I want to encourage you to take a step back and ask yourself “What do I really want from this?” In every situation, whether it’s in the bedroom, the boardroom, or the gym, set aside the time to think deeply about what you really want and how you can attain it. 2. Your Habits are Predicated on Queues, Actions, and Rewards While the old saying, “Knowing is half the battle” might be a little bit hyperbolic, it’s origins are found in reality. If you don’t truly understand a system, process, or job, you will never be able to accomplish it successfully and nowhere is this truer than in the realm of productivity, habits, and performance. Most people simply assume that they are lazy or a procrastinator or just plain unlucky. But the reality is that most people have never been taught how to properly build the habits of success into their lives. Luckily, building a habit… Any habit is very simple (notice I did not say easy). Habits can be broken into three parts: The trigger, the action, and the reward. If you’ve ever been addicted to cigarettes then I have no doubt that you are uncomfortably familiar with this process. You see a cigarette or experience stress (the trigger), you pull out a smoke and light it up (the action/habit), and then you experience a buzz from the nicotine and a sense of relaxation (the reward). But the same pattern can be found in positive habits. For example, you might see your running shoes and gym clothes (the trigger), decide to go workout (the action), and then experience a rush of endorphins and endocannabinoids that give you a feeling of elation and euphoria (the reward). Every habit can be broken down into these three simple steps and when you understand this framework you will be able to manipulate your actions and outputs at will. Pay close attention to the “Reward” element of this framework and you will be able to change your habits quicker and easier than you ever believed possible. 3. The Small Daily Decisions Matter It doesn’t matter what you had for lunch today, if you went to the gym or not, how many drinks you had, or how much you slept. But what does matter is what you consistently have for lunch, how often you go to the gym, how frequently you drink, and how serious you are about the quality of your sleep. For most people, there is no one action that will make or break their life and success. Instead, their lives are the direct result of thousands of tiny actions and decisions made overtime. When you realize this, you will begin to see the small decisions in a different light. Because even though one action might not make a difference today, when that action is repeated and compounded over years and decades, it will literally determine the quality and capacity of your life. 4. Set S.M.A.R.T. Goals With the new year right around the corner, many of you are undoubtedly setting New Year’s resolutions. Although I think resolutions are a terrible idea (which I will be discussing in an upcoming article), if you are going to set resolutions, Charles shared a few simple tips to help you actually stick to them this year. For starters, you want to set two major goals, a stretch goal and an easy goal. The stretch goal should scare the crap out of you and be something that you aren’t 100% certain you can achieve. It shouldn’t be unrealistic, just difficult. Your second major goal should be an important habit or result that you know you can achieve if you are simply willing to put in the work. Once you have your goals, you are going to break them down using the SMART formula. Specific: What do I want to achieve? Who is required to achieve it? What resources will I need? Measurable: How much? How many? How will I know my goal is achieved? Achievable: How realistic is this goal based on my current restraints? Relevant: Is this goal worthwhile? Is the timing right? Is it applicable in my current environment? Time Bound: When will my goal be achieved? What benchmarks and checkpoints should I set for myself? When you break down a goal in this manner, you clear all of the clutter from your mind and you’re left with nothing but clear, actionable steps that will help you get from where you are to where you want to be. 5. Assume That You Will Achieve Success One of the biggest roadblocks on your path to achieving your goals are your own limiting beliefs and b.s. internal stories. But I’ll let you in on a little secret. You are already doing better than you think you are and you’re much more likely to achieve success than you probably believe. So why not act in a way that is reflective of this reality? Assume that you will be successful at every endeavor you pursue. Assume that if you are willing to stick it out and persevere, you will achieve your goals and dreams. Act as if you are already successful, put in the work, and you will achieve the goals you’ve set for yourself.

I was surrounded by the best of the best at our craft and pressed everyday to be better than my best. Somehow somewhere an unspoken something something developed in the way we loved and respected each other. Predicated on experiences and values and a way of professionalism that was never intentionally designed. It was incredible and I look back at those guys and times with tremendous fondness.The Madness Challenge and manners fo undertaking it are my attempt to recreate some sort of that and try to share it in a certain way with others. At a minimum, it helps me attempt to hold myself accountable to those same energies that drove me at my best then; in the absence of having each other at one's side today.Semper Fi
