Working with pediatric/adolescent athletes is VERY different than working with professional athletes or the general population adults. Child and adolescent gymnasts train as much as professional athletes (20-30 hours/week) and yet their bodies, their physiology, their psyche is vastly different. Something that really troubles me is the lack of awareness around underfueling, poor growth, and injuries. If I were a sports medicine doctor or orthopedic doctor/surgeon, the VERY first thing I'd want to know when a gymnast sets foot in my office is1. Are they growing in a way that's right for them2. Are they getting ENOUGH nutrition to support normal growth/development and healing3. Are they getting ENOUGH micronutrients to signal all the vitamins/minerals where they need to go in the body. When I worked in Pediatric Endocrinology, the growth/hormone/metabolism specialists, one of the most important parts of the doctors workup was the dietitians nutrition valuation. Well, here we are—the final episode of Informatics in the Round (we think!). After five incredible years, 38 episodes, and thousands of listeners, we've decided it's time to wrap up this journey. But first, we're taking a minute (or maybe two hours) to reflect on this wild ride—and we've brought some people you might recognize! Robert H. Lustig is an American pediatric endocrinologist. He is professor emeritus of pediatrics in the division of endocrinology at the University of California, San Francisco (UCSF), where he specialized in neuroendocrinology and childhood obesity. He is also director of UCSF's WATCH program (Weight Assessment for Teen and Child Health), and president and co-founder of the non-profit Institute for Responsible Nutrition Lustig grew up in Brooklyn, New York, He obtained his bachelor's degree from the MIT in 1976 and an MD from Cornell University Medical College in 1980. He became a pediatrician in 1983 and subspecialist in Pediatric Endocrinology a year later. After that he started a post-doctoral in neuroendocrinology. Lustig has authored number of peer reviewed articles and reviews. He has held chair positions in several boards focusing on Obesity. He became known in 2009 for his stunning lecture “Sugar the bitter truth”, explaining how the toxic effect of fructose can cause NASH (non-alcohol steatohepatitis). He has been an advocator for real food and the importance of decreasing the intake of refined sugars and ultra processed food. He is the author of: Fat Chance, The Real Truth about sugar, The hacking of the American mind and lately Metabolical. He is an incredible lecturer that holds the capacity to intrigue the listeners with his brilliant mind and thinking. A family history, working with children, and a love of math motivated Radhika Muzumdar, MD, chief of Pediatric Endocrinology, to choose a career focused on diabetes. Her research and clinical care aim to meet the short- and long-term goals of treatment and management of diabetes. In this episode, Dr. Muzumdar covers her career beginning with her earlier work on growth hormones and IGF-1 (insulin-like growth factor 1), to her upcoming studies on the hypothalamus and the humanin peptide. Robert Lustig, MD, MSL is a Professor of Pediatric Endocrinology at the University of California, San Francisco. Dr. Lustig has become a leading public health authority on the impact sugar has on fueling the diabetes, obesity and metabolic syndrome epidemics, and on addressing changes in the food environment to reverse these chronic diseases. In his New York Times best selling book Fat Chance: Beating the Odds Against Sugar, Processes Food, Obesity, and Disease, Robert documents both the science and the politics that have led to the current pandemic of obesity and chronic disease. In the Fat Chance Cookbook, Robert provides practical examples for applying healthy eating principles with recipes by Cindy Gershen. Dr. Lustig is a neuroendocrinologist, with basic and clinical training relative to hypothalamic development, anatomy, and function. Prior to coming to San Francisco in 2001, he worked at St. Jude Children's Research Hospital in Memphis, TN. There, he was charged with the endocrine care of many children whose hypothalami had been damaged by brain tumors, or subsequent surgery, radiation, or chemotherapy. Many patients who survived became massively obese. Dr. Lustig theorized that hypothalamic damage led to the inability to sense the hormone leptin, which in turn, led to the starvation response. Since repairing the hypothalamus was not an option, he looked downstream, and noted that these patients had increased activity of the vagus nerve (a manifestation of starvation) which increased insulin secretion. By administering the insulin suppressive agent octreotide, he was able to get them to lose weight; but more remarkably, they started to exercise spontaneously. He then demonstrated the same phenomenon in obese adults without CNS lesions. The universality of these findings has enabled Dr. Lustig to weave these threads together into a novel unifying hypothesis regarding the etiology, prevention, and treatment of the current obesity epidemic, and the role of our environment in the biochemical changes that promote weight gain. This has led him to explore the specific role of fructose (half of sucrose and high-fructose corn syrup) as a specific mediator of both chronic disease, and continued caloric consumption. His acclaimed YouTube video, “Sugar: The Bitter Truth” continues its popularity with the lay public. A native of Brooklyn, New York, Dr. Lustig went to Stuyvesant High School in Manhattan, graduated from Massachusetts Institute of Technology in 1976, and received his M.D. from Cornell University Medical College in 1980. He completed his pediatric residency at St. Louis Children's Hospital in 1983, and his clinical fellowship at UCSF in 1984. From there, he spent six years as a post-doctoral fellow and research associate in neuroendocrinology at The Rockefeller University. He has also been a faculty member at the University of Wisconsin-Madison, and the University of Tennessee, Memphis. In 2013, Dr. Lustig received his Masters in the study of Law from University of California, Hastings to enable him to impact the food industry through policy change. Dr. Lustig has authored 125 peer-reviewed articles and 73 reviews. He has mentored 20 pediatric endocrine fellows, and trained numerous other allied health professionals. He provides endocrinologic support to several protocols of the Children's Oncology Group. He is the former Chairman of the Ad hoc Obesity Task Force of the Lawson Wilkins Pediatric Endocrine Society, a member of the Pediatric Obesity Practice Guidelines Subcommittee of The Endocrine Society, a member of the Obesity Task Force of the Endocrine Society, a member of the Pediatric Obesity Devices Committee of the U.S. Food and Drug Administration, a member of the Bay Area Board of Directors of the American Heart Association, and a member of the Steering Committee of Health Foods, Healthy Kids of the Culinary Institute of America. He also consults for several childhood obesity advocacy groups. Dr. Lustig lives in San Francisco with his wife Julie and two daughters. This month, we are featuring X-linked hypophosphatemia (XLH). XLH is a genetic disease caused by mutations in the PHEX gene. These mutations cause bone cells to secrete increased concentrations of a hormone responsible for inducing phosphate excretion in the kidneys, ultimately resulting in a deficiency of phosphate in the body. According to the 1964 Mary Poppins children's film, "just a spoonful of sugar" is said to make the medicine go down. But what if we say that those daily spoons of sweetness may actually result in the need for kids to take medication? According to the American Diabetes Association, approximately 30 million people in the United States alone in 2021 were diagnosed with diabetes mellitus, Types I and II combined, with 352,000 of these being under the age of 20. Diabetes is a chronic condition influenced by the body's inability to either produce the insulin hormone or effectively utilize it. Truly, hormones and their balance influence the optimal functioning of the human body, especially in the youth. According to the American Board of Pediatrics, if left untreated, diabetes and other leading chronic diseases influenced by hormones, such as obesity, can lead to detrimental health concerns, such as cardiovascular disease and mortality. Endocrinology is the complex field of hormones and their equilibrium, and there is no space for sugarcoating in this domain.We are joined in this episode by Dr. Sheila Pérez-Colón, a board-certified pediatric endocrinologist based in Puerto Rico. Yaa's research focuses on studying communication and documentation in healthcare and developing strategies to improve workflow and patient care delivery. Yaa works in the Innovations Portfolio at Vanderbilt HealthIT on the development of Voice Assistant Technologies to improve the usability of the EHR through natural language communication.
