Episode 09:  Life in the ICU - Leslie Buller-Hayes, ICU Nurse, Death Doula, Stonehedge Transitions   Leslie Buller-Hayes is an ICU Nurse and end of life doula who is fueled by a commitment to fostering open, authentic conversations, providing compassionate support, and guiding others towards embracing the natural cycle of life with dignity and peace. She went to college in Chicago before traveling the world with MTV and then moving to Los Angeles to work as an actor. Kendal started getting sick in her early thirties and saw scores of doctors before getting diagnosed with Lyme disease, Babesia, Bartonella, and Ehlers-Danlos syndrome (EDS) through IGeneX. Kristi Kusske is a 37-year-old Registered Nurse (RN) and Functional Diagnostic Nutritional Practitioner from Lakeville, Minnesota. Briana Jamshidi is a 27-year-old Registered Nurse (RN) and certified personal trainer from Charleston, South Carolina. Ms. Jamshidi loved to lift weights, run races, dance, go to concerts, attend music festivals, and was a top performer on her National Guard physical fitness test. At the age of 21, Ms. Jamshidi suddenly fell ill while attending her annual training with the Army National Guard in Virginia. Shew was first diagnosed with Urinary Tract Infections (UTIs) and then doctors said it was Interstitial Cystitis (IC) because her urine cultures never grew any bacteria. Some of Ms. Jamshidi's early symptoms included severe burning when peeing like glass slicing her bladder, spasms of her bladder and pelvic floor, frequent and urgent urination, urinary incontinence, and painful sex. Her symptoms worsened and she developed severe lower back pain, swollen lymph nodes, neck pain, hives, muscle spasms, hand tremors, extreme fatigue, cognitive decline, constipation, food sensitivities, and the feeling of bugs crawling all over her at night. Åsa Rydmark Kersley is a 48-year-old woman from Sweden who has traveled all around the world. She's worked in the medical field as a Registered Nurse (RN), research nurse at a gynecological ward, and for the last two years as a research coordinator at Linköping University. Ms. Kersley had many tick bites throughout her life, got a bullseye rash when she was 14, and Lyme came out and wreaked havoc when she was 39 due to mold exposure and environmental toxins weakening her immune system while performing home renovations. At first, she thought her symptoms were due to “sugar addiction” after reading an article in the newspaper about it. Removing sugar and gluten from her diet helped her feel a little bit better. Ms. Kersley quickly realized there was more to the picture and saw many doctors, all of which told her she was fine and her tests all came back normal. She was finally diagnosed with Lyme disease at the age of 46. Ms. Kersley was also diagnosed with Acrodermatitis Chronica Atrophicans (ACA), a late and chronic manifestation of European Lyme borreliosis (Borrelia afzelii), which is characterized by red or bluish-red lesions and leads to extensive flaccid atrophy of the skin. Ms. Kersley was treated with Doxycycline for a month, but her doctor wouldn't do any more for her and she only made minimal health improvements. She started researching and came across the Tick Boot Camp Podcast, Dr. Rawls' work on chronic Lyme, and Dr. Horowitz's Multiple Systemic Infectious Disease Syndrome (MSIDS) all of which solidified her gut feeling that she was still sick with Lyme disease and needed more help. Jeni Quante is a 29-year-old Registered Nurse (RN) and chronic illness patient advocate from San Antonio, Texas. She has earned high regard from the Lyme disease community for producing creative educational content on social media platforms including TikTok and Instagram. Ms. Quante was born with Congenital Lyme disease, but the undiagnosed illness did not become debilitating until the age of 14. Her disability activated her medically educated and trained parents who dedicated all their resources to advocating for their daughter. In total, the family visited “well over 100” doctors before she was diagnosed by a primary care physician. Her diagnosis took more than 12 years. Despite debilitating symptoms, Ms. Quante graduated from college and nursing school, earning a BS, BSN, OCN, and RN. She also dedicated time to “engaging others in the chronic illness community and found some wonderful lymies who kept her going through the hell”. This far, her 3-year treatment plan has included the use of several western and eastern treatment modalities, including, hyperthermia, SOT, herbs/tinctures, detox work, disulfiram, and biofilm busters. Lisseth is a Registered Nurse (RN) and has been a nurse for over 10 years with most of her career working in emergency departments and Level I trauma centers. In recent years she has also worked in a children's hospital working in vascular access, and notably, in the past year and a half, Lisseth had worked in Queens, NY working at the well known Level I trauma Center - Elmhurst - standing strong and in unity alongside other healthcare heroes on the frontlines of the pandemic. Her current full time role is within the ER unit at Northern Memorial Hospital in Minneapolis. Where you're based out of and what got you started in the influencer marketing space? So right now I am in central Florida. We just moved here a couple of months ago. Awesome.  Yeah, I'm loving it.  Where did you move from?  Well, we were living in a tiny house RV, so we were kind of moving around constantly before COVID and everything. But we had like a little main base in North Carolina. So we would always like go back to North Carolina.  What inspired you to start out as an influencer? What are your two channels?I would say it all started in 2015. It was just a really, really rough year for me and my therapist suggested I start a journal or a blog just cause I express myself so much better writing. And so I did, and I wrote about everything I was passionate about and what stage of life I was in all of that. I literally just dumped it and That was the Meg for it brand. I don't know where that name came from. It just like came to me one day. I thought it was funny and kind of cheeky. And that was the first blog that I started, the first brand that I started. And then I just started to learn SEO and best practices in the industry and started following other big bloggers and saw what they were doing. And it, it grew and exploded. And then about two years ago is when I really did start the nurse Meg. Brand. I just wanted to reach more nurses. I was working as a night nurse for the last 13 years. Oh my God. We just wanted to reach more nurses. I had so much knowledge and so many successful little resources that I saw my little baby nurses use and I just wanted to reach more people. So I just decided to copy paste, start a blog and do all the same things that I did with the first brand. And now I'm here doing two different brands. So I have so many questions. I don't even know where to start. Let's start with the fact that you actually started with a blog because so many people now don't. Do you think having blog content has helped you? It is a lot more to maintain, but it is yours and. In a second, an entire platform can disappear. Like we saw, I think it was in 2018 where Facebook and Instagram were down for a whole 12 hours platforms will come and go. But your blog is your blog. And I think even if you just want to be an Instagram influencer, I really think you need to own a space and have a blog and maintain it. If you really want to be a true influencer and transition from a hobby to a business. So how did you kind of start learning about SEO and all this stuff? I'm a big believer in YouTube university. There's some, I mean, I even learned how to drive, stick shift on YouTube. Like there's so many free resources and if you want to support, you know, a creator and buy their course and, you know, kind of get their story along with it. That's super valuable, but if you're just starting out and you don't have a budget to like invest in, in big courses, watch as much YouTube as you can, like replace any like entertainment, Netflix, Hulu, whatever, with learning on YouTube. It's so, so great. There's so many great resources of people that are ready to share their knowledge. It's so funny. Cause you forget, like I'll be in the midst of something. And then I forget, I could just go to YouTube and I'm like, how do I do this? And then it's like, oh. Let me just go to cute. Even if it's like...

In our conversation, we go over essential qualities that are necessary for a person interested in becoming an RN, the duties and responsibilities associated with this profession, the different patient groups with whom RNs may work, the educational path to follow to earn a BSN (Bachelor of Science in Nursing) degree. Additionally, we explore the components of the typical Nursing Program pre-requisites and explore those of the undergraduate nursing program at Loma Linda University. We also discuss the NCLEX (National Council Licensure Examination and other licensing requirements. The episode ends with a look at statistical data related to pay and employment prospects for registered nurses, and the five reasons why this profession is expected to grow at almost twice the percentage average of other professions. We hope this episode allows all high school students in our audience to gain an appreciation for this noble, stable, and much-needed occupation. Heather Ehle, founder, and chief executive officer of Project Sanctuary, is a Registered Nurse (RN) with more than 25 years of experience in the health care industry as a nurse, manager, volunteer, and entrepreneur. She has extensive experience in the nonprofit health care system. A proud single mother of three, Heather adopted a sibling pair through foster care. She is driven by a strong desire to give back and do more. After reading that the rates of child abuse and neglect triple when a parent is deployed, Heather was moved to do something and founded Project Sanctuary in 2007. Being a civilian without direct ties to military families, she reached out to the community for support and education. Led by veterans and her board, she carefully listened to what the military families needed and then structured Project Sanctuary's programs to meet those needs, pulling from her background as an RN to establish an evidence-based program that encompasses physical and emotional healing. In 2010, Heather was a finalist for People Magazine'sAll Stars Among Us. The Role of Social Workers in Nursing Homes “The social isolation caused by COVID-19 requires an expansive workforce considering not all nursing homes have trained social workers.”— Lynn Friss Feinberg, MSW Nursing homes and COVID-19 are today's hot topics. With mounting cases of COVID-19,  nursing homes have faced many challenges, especially with providing social services. On top of helping older adults adjust to their new life in a supportive care setting and advocating for their rights and needs, social workers are faced with ethical dilemmas, stress, and fears for resident safety. What can we do about it?  Today, we are joined by Nancy Kusmaul, PhD, MSW, and Lynn Friss Feinberg, MSW, to discuss social workers' role in nursing homes. Part One of ‘The Role Of Social Workers In Nursing Homes’ Nancy Kusmaul, PhD, MSW, is an Associate Professor in the Baccalaureate Social Work program at UMBC. She received her PhD from the University at Buffalo School of Social Work and her MSW from the University of Michigan. Dr. Kusmaul worked in nursing homes and hospitals for more than a decade.  Her research focuses on organizational culture, trauma-informed care, and the impact of trauma experienced on the workforce. She is interested in direct care workers' experience in organizations, particularly Certified Nursing Assistants in nursing homes. She is a 2019-2020 Health and Aging Policy Fellow with Senator Ron Wyden and the Centers for Medicare and Medicaid Services. She is a member of the Baltimore County Elder Abuse Coalition and the National Association of Social Workers (NASW). She co-chairs the Aging Committee for the Maryland Chapter of NASW, and she is a fellow of the Gerontological Society of America. Lynn Friss Feinberg is a Senior Strategic Policy Advisor at the AARP Public Policy Institute, responsible for family caregiving and long-term care issues.  She came to AARP from the National Partnership for Women & Families, serving as the first Director of the Campaign for Better Care. Previously, she served as the Deputy Director of the National Center on Caregiving at the San Francisco-based Family Caregiver Alliance (FCA). At FCA, she directed the National Consensus Project for Caregiver Assessment and led the first 50-state study on publicly-funded caregiving programs in the U.S. She has published and lectured widely on family care policy and practice. She has held leadership positions on numerous advisory boards and committees, including the American Bar Association's Commission on Law and Aging, and the National Academies of Sciences, Engineering, and Medicine's Committee on Family Caregiving for Older Adults. Ms. Feinberg is a past Chair of the American Society on Aging (ASA) Board of Directors and a past Chair of the Public Policy Committee at the Gerontological Society of America (GSA).  In 2007-2008, Ms. Feinberg served as the John Heinz Senate Fellow in Aging in the office of former U.S. Senator Barbara Boxer. She received the ASA Leadership Award in 2006, and the Paul Nathanson Distinguished Advocate Award in 2015 from Justice in Aging for her career work on family care issues.  Ms. Feinberg holds a master's degree in social welfare and gerontology from the University of California at Berkeley. “Any facility with more than one hundred and twenty beds must employ a qualified social worker on a full-time basis.” — Nancy Kusmaul, PhD, MSW COVID-19 spreads easily in nursing homes and older adults living in these facilities (residents) are at the greatest risk of death if there is an outbreak. This is why a lot of attention has been focused on keeping residents safe. Not only the nursing homes experiencing a shortage of PPE, but the implementation of visitation restrictions made it challenging for them to communicate with the resident's family members. We didn't have a national strategy to deal with a pandemic, and because of all of these things, nursing homes were hit hard by COVID-19.  While it's critically important to have adequate nursing certified nursing assistants, we would all agree with that if we want to improve the quality of care or people residing in nursing homes and other long-term care facilities, COVID-19 has pointed out that we need to have more social support, more emotional support, and connection to the outside world. Family and friends are critically important, particularly at the end of life. That is a key component of both health care and long-term care, particularly since long term care is someone’s home! Social workers are uniquely trained to recognize the importance of family support and the social aspects of care. We need trained social workers who are willing to get involved in national conversations to ensure that we have high-quality social work that includes emotional support at the community level.  Part Two of ‘The Role Of Social Workers In Nursing Homes’ Any facility with more than one hundred and twenty beds must employ a qualified social worker on a full-time basis. However, a qualified social worker is an individual with a minimum of a bachelor's degree in social work or a bachelor's degree in a human services field, including but not limited to sociology, gerontology, special education, rehab, counseling, and psychology, and one year of supervised social work experience in a health care setting, which means that the person in the social work role may not have social work training.  The broad qualifications for social workers could be because of the same struggles we have nurse staffing requirements. Nursing homes are only required to have one Registered Nurse (RN) in the building for 8 hours a day, and that could be a RN with an associate's degree, a baccalaureate degree or higher. The majority of care delivered in nursing homes, however, is delivered by Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs). “Assisted livings are uniquely challenged in the pandemic because they are not federally regulated.” —  Nancy Kusmaul, PhD, MSW Unlike skilled nursing homes, assisted living facilities are uniquely challenged in the pandemic because they are not federally regulated. There are no federal statutes that cover assisted living, and they're not required to have a social worker. So, they have faced some of the same challenges as nursing homes with less guidance and no social work support for residents or their families. COVID-19 resulted in millions of older adults living in both of these environments to be denied visitation by family and/or friends. This has been devastating to both the older adults and their family members - who are considered to be “visitors”.  Many family members of people in nursing homes took care of them at home for years before they got to the point where they needed a nursing home. These caregivers know their family member's needs, moods, and changes in condition better than anyone else. They are the eyes and ears. They are the ones who can communicate with staff and advocate for their residents.  They are care partners. And yet, when they are placed in the category of visitors, it sounds more like a luxury than an integral and essential member of the care team. They should be allowed to be in the nursing home as a care partner - not as a visitor.  These are some examples of policies that impact social work practice and nursing home residents, and social workers are well positioned to advocate for changes to both.  Real change happens one step at a time.  We must remember that shaping good public policies is a cornerstone of social work. SHOW NOTES   Guido Van Ryssegem is a badass. If you haven’t heard of him before, you may have heard of some of his contributions to the healthcare-fitness world. Just to name a few, Nick Winkelman studied under him and has become wildly popular. The sleeper stretch for throwers was a Guido invention. I was fortunate to attend Guido Attractor State and Dynamic Systems Theory Workshop and I can say with confidence that it changed the way I treat certain cases dramatically. The easiest way to think of Guido’s work in reducing Attractor States, is figuring out what the body is “asking for” and then giving it proper dosage of the proper “exercise medication.” Or maybe simplest way is to just hear the conversation :) Using the tactics Guido shares, I’ve been able to reduce many of my patient’s symptoms with a single corrective exercise within a matter of seconds. Neck pain, back pain, knee pain, hip impingement and so much more are treatable with the CORRECT corrective exercises. Leslie is the Vice President of Operations and the chapter Patient Navigator for the Lupus Foundation of America, Greater Ohio Chapter. She has served over seventeen years in understanding the disease process of lupus and other illnesses. Leslie is a Registered Nurse (RN) and is sought out to provide education for patients and the public about various health conditions. This past year, organizations like the Ohio Department of Health, Kent State University, Northern Ohio Academy of Pharmacy, Cuyahoga County Jobs Education & Training Department, Akron Children’s Hospital, Columbus Nation Wide Children’s Hospital, local businesses, local churches, and patient family members have requested Leslie for speaking engagements and information. To best serve the community their requests were delivered to engage with lupus awareness. While most nurses care for one patient at a time, Leslie cares for an entire population. By working with the whole community, she is able to educate the public about health issues, improve community health and safety, and increase access to care while assisting lupus patients to better self-manage their disease.

This week our hosts sit down with Guido Van Ryssegem. Guido has been in clinical practice for 29 years and has been working in the strength and conditioning arena for 34 years. He is co-owner and co-founder of Safe Recovery, LLC. Guido is a practitioner and faculty at Oregon State University, and has published in the Asian Journal of Martial Arts. Guido is also invited faculty for the University of Oregon, Western University, Willamette University, local Community Colleges, and private schools. He has practiced for the Kansas City Royals, Texas Rangers and the Baltimore Orioles. His expertise extends to his own Kinetic Integrations Exercise Specialist Certification. He is a board certified Athletic Trainer (ATC), Registered Nurse (RN) and Strength and Conditioning Coach (CSCS) while also certified by the National Board of Fitness Examiners (NBFE). Guido serves as Northwest regional coordinator of the NSCA and is an NATA COR and Occupational Task Force Member. He is on the AQX Sports Incorporated and the Elevation Fitness advisory board. Additionally, he is an Athletes Performance Institute mentorship program presenter. Guido developed the Kinetic Integrations Exercise certification program that includes a corrective exercise approach to resolve movement dysfunctions after injury, a sedentary lifestyle, repetitive movements and incorrect movement. He presents nationally and internationally on this methodology. Guido has also published in the International Journal Of Sports Rehabilitation.

We'll also answer questions from our listeners.  Mr. Wolski has been a Registered Nurse (RN) since 1976, licensed to practice in New Jersey and Pennsylvania. He currently volunteers full time as Executive Director of the Coalition for Medical Marijuana—New Jersey, Inc. (CMMNJ). He co-founded this non-profit, 501(c)(3)educational organization in 2003. He volunteered part-time at CMMNJ until his retirement from the State of New Jersey in August 2006. Prior to 2006, he was employed full time with the State of New Jersey, Department of Human Services and Department of Corrections (DOC) for 25 years. At the State, he was a Graduate Nurse, Head Nurse, Supervisor of Nurses, Infection Control Officer, Quality Assurance Specialist, Quality Assurance Coordinator, and Health Services Manager. At the DOC, he was chairman of the statewide Policy Committee and statewide Telemedicine Committee for several years.
