Our skin is an incredibly important, multifunctional organ. It has many different functions including helping us maintain our body temperature, it assists with the excretion of water and toxins, and most importantly, protective mechanism of acting as a barrier. But in newborns, especially preterm infants, there are developmental differences in the anatomy and physiology of their skin that place them at an increased risk for skin injury. Not only is the immaturity of an infant's skin problematic, but infants in the NICU are predisposed to skin trauma due to the life-saving monitors and equipment necessary for their care. Furthermore, infants with compromised skin integrity are at an increased risk for infection, excess water loss, absorption of toxic substances, and additional sequelae. On this episode, we review the basic anatomy of our skin and all of its important functions. We discuss how the skin of a term and preterm infants differs from adults and why the structural variations place infants at an increased risk for skin trauma and additional complications. The episode will be beneficial for parents of infants, especially preterm infants or those in the NICU to understand why their infant is at an increased risk for compromised skin integrity. NICU clinicians will also hear a great reminder of the anatomical and physiologic variations in infants that place them at an increased risk for skin injury. Skin 101 Your skin is the largest organ of your body. It has 3 jobs: Protection, Regulation, Sensation Natural complexions are being viewed as more beautiful than a "tan". 3 Layers Epidermis (above skin) - waterproof, gives you your color, the layer we see Dermis-Epidermis junction - a protein layer containing collagen and elastin to give skin flexibility and stretchability Dermis - where your glands, hair follicles, and nerves are Hypodermis - (below skin) - houses adipose tissue for insulation, and blood vessels The Epidermis is made up of several separate layers based on what is happening in the cell's lifetime. The basement membrane sits right on top of the Dermis-Epidermis junction, made up of fibrous proteins to be a solid foundation. Epidermis Layers Basal Layer - Keratinocytes (makes Keratin) and Melanocytes (makes Melanin) - stem cells. Spinous Layer - Cells are actively dividing and getting squished together Granular Layer - Cells start making the proteins (keratin or melanin) that they are coded to make and it fills up all the intracellular space; the organelles of the cells get crowded out Lucid Layer (Clear) - Keratinized cells are clear; Melanin-filled cells are colored. Cells are officially dead. Cells become coated in a hydrophobic (afraid of water) oil. Hard Layer - cells are tightly packed together and dry; the layer we can touch. These cells are being continuously produced and shed off and replenished because the skin takes a lot of abuse. Deeper Stuff UV light from sun or tanning beds (heaven forbid!) stimulates melanocytes to divide faster and create more melanin (because melanin is reflective and keeps UV rays away from the important cells). This is how a tan is created. But during times of huge exposure, like tanning beds, there's not enough melanin to keep all the UV rays out so those rays can wreak havoc on the collagen and elastin proteins. This is why over-tanned skin ages faster. Hydrating makes sure that the living and dividing cells are plump and as healthy as possible so when they move to the next layer, they are well nourished. Protection: keeps dirt and bacteria out; protections from UV radiation Regulation: releases sweat to cool the body's temp; subtly moves blood vessel closer or farther away from the surface to either cool the warm blood (like after exercise) or keep it warm (like in the winter). Sensations: allows you to feel things that touch you or come close to you. Die vorliegende Dissertation befasst sich mit den durch S-Lost-verursachten Symptomen in der Haut, die zunächst durch starke Blasenbildung und später durch eine verzögerte Wund-heilung charakterisiert sind. Bei S-Lost handelt es sich um einen chemischen Kampfstoff, der erstmals im ersten Weltkrieg zum Einsatz kam und bis heute in vielen internationalen Kon-flikten großen Schaden anrichtete, obwohl der Gebrauch schon 1925 durch die Genfer Konvention verboten wurde. Aktuell stellt S-Lost zudem eine Bedrohung durch terroristische Aktivitäten dar. Da für S-Lost-induzierte Verletzungen bislang keine spezifisch wirksamen Behandlungs-methoden verfügbar sind, besteht großes Interesse an der Aufklärung der dem Krankheitsbild zugrunde liegenden molekularen Pathomechanismen, um daraus Rückschlüsse auf besser ge-eignete therapeutische Maßnahmen ziehen zu können. In unseren ersten Experimenten wurden als mögliche Auslöser der Blasenbildung die Expression und Sekretion ausgewählter Matrix-Metalloproteinasen (MMPs) und deren endogenen Inhibitoren, den tissue inhibitors of matrix metalloproteinases (TIMPs) in einem 3D-Haut-modell und in verschiedenen Zelltypen der Haut (Keratinozyten, Fibroblasten, mikrovaskuläre Endothelzellen, mesenchymale Stammzellen, monozytäre Zellen, PMN-Granulozyten) sowohl in Mono- als auch in Mischkultur untersucht. Unter Verwendung von molekularbiologischen und proteinbiochemischen Methoden wie qRT-PCR, Zymographie und Western Blot gelang der Nachweis, dass MMPs - insbesondere MMP-9 - nach Exposition der Zellen (v.a. Fibroblasten und monozytäre Zellen) mit S-Lost deutlich hochreguliert wurden. Zu Erhärtung der Annahme, dass MMP-9 durch Degradation der Basalmembran zwischen Epidermis und Dermis zur Blasenbildung beiträgt, konnte als Pathomechanismus nun erstmals eine parakrine Stimulation von Fibroblasten durch S-Lost-behandelte Keratinozyten identifiziert werden, als deren Folge eine vermehrte MMP-9-Sekretion resultierte. Darüber hinaus zeigte sich in weiteren Versuchen unter Verwendung des sog. Scratch-Assays und eines Transwell-basierten Invasionsassays, dass das Migrations- und Invasionsverhalten der Fibroblasten in Gegenwart des konditionierten Mediums der S-Lost-behandelten Keratinozyten positiv beeinflusst wurde. Aus klinischer Sicht sprechen diese Erkenntnisse für neue therapeutische Ansätze, die darauf beruhen sollten, die S-Lost-induzierte, auf proteolytischer Aktivität basierende Blasenbildung der Haut durch Applikation spezifischer MMP-Inhibitoren zu behandeln. In einem weiteren Projekt wurde die verzögerte Wundheilung als spätes Symptom der S-Lost-Vergiftung auf zellulärer Ebene untersucht, bei der eine eingeschränkte Re-Epithelialisierung der betroffenen Hautstellen beobachtet wird. Sowohl für den Prozess der Wundheilung als auch für die stetige Erneuerung der Haut werden epidermale Stammzellen benötigt, die für die Bildung von Keratinozyten verantwortlich sind. Diese unipotenten Progenitorzellen befinden sich in der basalen Schicht der Epidermis und sind in der Lage zu proliferieren und anschließend terminal zu differenzieren. Um eine Beeinflussung dieser Prozesse durch S-Lost zu untersuchen, wurden primäre unreife Keratinozyten (NHEK) verwendet und hinsichtlich ihres Differenzierungspotenzials untersucht. Dabei erwies sich S-Lost als potenter Induktor der Differenzierung von NHEK, was durch Bestimmung der Expression typischer Markerproteine wie Keratin-1, Involucrin und Loricrin gezeigt wurde. Die Induktion des Reifungsprozesses war sowohl von einem Rückgang der Proliferation als auch von einer verminderten Migrationsrate der Zellen begleitet. Die eingehende Analyse von mitogen-activated protein kinase (MAPK)-Signaltransduktionswegen führte zu der Erkenntnis, dass die Aktivitäten von p38 und ERK1/2 gegenteilige Rollen im Differenzierungsprozess einnehmen. Studien mit spezifischen Inhibitoren der MAPK be¬legten, dass p38 für den Reifungsvorgang in NHEK essentiell ist, während ERK1/2 diesem entgegen wirkt. So konnte durch Blockade von p38 die von S-Lost ausgelöste Differenzierung der Zellen verhindert werden. Ebenso war es durch diese Behandlung möglich, die von S-Lost stark beeinträchtige Migrationsfähigkeit der Keratinozyten wiederherzustellen, welche mit einer erhöhten MMP-1-Expression einherging. Davon abgeleitet erscheint es therapeutisch sinnvoll, selektive p38-Inhibitoren für die Behandlung von Wundheilungsstörungen nach Exposition der Haut mit S-Lost einzusetzen. Zusammenfassend erbrachten unsere Studien also den Nachweis, dass der S-Lost-induzierten Blasenbildung (als frühes Symptom) die spezifische Induktion der MMP-9 zugrunde liegt. Darüber hinaus konnte erstmals eine verfrühte Differenzierung in unreifen Keratinozyten der Haut (als mögliche Ursache für die verzögerte Wundheilung) nachgewiesen werden, wobei die MAPK p38 bei der Initiierung des Prozesses von entscheidender Bedeutung ist. Aufgrund dieser Resultate empfiehlt sich eine kombinierte Applikation von Inhibitoren der Aktivitäten von MMP-9 und p38, wobei der Einsatz jedoch zeitlich abgestimmt erfolgen sollte, um pathologische Effekte (Blasenbildung bzw. Differenzierungsinduktion in Keratinozyten) zu blockieren, ohne die positiven Auswirkungen von MMP-9 und p38 auf die Heilung (Migration von Immunzellen und Keratinozyten bzw. Reepithelialisierung) zu hemmen.

Die Differentialdiagnose entzündlicher Hauterkrankungen setzt seitens des Dermatologen ein hohes Maß an klinischer Erfahrung voraus. Labortechnische und histologische Untersuchungsverfahren sind dabei nur eingeschränkt hilfreich. Ausgehend von der Annahme, dass erstens das atopische Ekzem ein krankheitsspezifisches Mikromilieu in der Epidermis und Dermis aufweist und dass sich zweitens dieses spezifische Mikromilieu im Phänotyp der immunkompetenten CD1a positiven epidermalen dendritischen Zellpopulation widerspiegelt, wurde seit 1995 ein Verfahren zur Immunphänotypisierung von Langerhans Zellen und IDEC entwickelt. Es konnte gezeigt werden, dass das atopische Ekzem durch eine hohe Expression des hochaffinen IgE-Rezeptors (FceRI) bei niedriger Expression des IgG-Rezeptors FcgRII auf den dentritischen Zellen der Epidermis hoch sensitiv und spezifisch von anderen Dermatosen abgegrenzt werden kann. Ziel der vorgelegten Arbeit war es, die standardisierte Phänotypisierung CD1a positiver epidermaler Zellen so zu erweitern, dass neben dem atopischen Ekzem auch die Diagnose anderer entzündlicher Hauterkrankungen möglich wird. Es wurden 296 Patienten mittels standardisierter Phänotypisierung epidermaler dendritischer Zellen untersucht. Bei 192 Patienten konnte nach Abschluss aller klinisch diagnostischen Untersuchungen eine eindeutige Diagnose gestellt werden, so dass die an diesen Patienten erhobenen Phänotypisierungsdaten für die Analyse krankheitsspezifischer Veränderungen im Oberflächenmarkerprofil von Langerhans Zellen und IDEC herangezogen wurden. Die Patienten verteilten sich auf 8 Gruppen: Normale Haut (n=31); Atopisches Ekzem (n=59); Psoriasis (n=41); Allergisches Kontaktekzem (n=23); Kontaktekzem bei atopischer Diathese (n=14); Lichen ruber (n=6); Mycosis fungoides (n=10) und Nummuläres Ekzem (n=8). Es konnte gezeigt werden, dass der prozentuale Anteil epidermaler dendritischer Zellen in Abhängigkeit von der Diagnose variiert und dass die bei allen entzündlichen Dermatosen nachweisbare Vermehrung epidermaler dendritischer Zellen vorrangig auf eine de novo Einwanderung von IDEC in die Epidermis zurückzuführen ist. Das wesentliche Ergebnis der vorgelegten Arbeit ist die Erweiterung der diagnostischen Phänotypisierung auf die Psoriasis und das allergische Kontaktekzem. So konnte die Psoriasis durch eine Ratio von CD64/CD11b auf IDEC sinnvoll abgegrenzt werden. Das Kontaktekzem war mittels einer Ratio aus E-Cadherin/CD86 auf Langerhans Zellen zu identifizieren. Die Gruppe der Patienten mit Kontaktekzem bei atopischer Diathese wurde durch die Ratio E-Cadherin/CD80 auf IDEC erkannt. Die bereits für das atopische Ekzem publizierte Ratio FcRI/CD32 auf allen CD1a positiven Zellen wurde mit der gleichen Ratio, ermittelt nur für IDEC, verglichen. Eine Verbesserung der Sensitivität und Spezifität für die Diagnosestellung des atopischen Ekzems konnte hierdurch nicht erreicht werden. Die starke Expression von FceRI auf LC und IDEC kennzeichnet das atopische Ekzem. Typisch für das atopische Ekzem ist weiterhin die Expression von CD36 auf LC und IDEC. Die Psoriasis ist durch eine besonders starke Expression der Fcg-Rezeptoren auf IDEC gekennzeichnet. Darüber hinaus findet sich eine erhöhte passive Anlagerung von sCD23 an IDEC bei Psoriasis. Gegenüber anderen Diagnosen ist die Expression von CD11b und CD11c auf IDEC vermindert. Für das Kontaktekzem ist die im Vergleich mit anderen entzündlichen Dermatosen niedrige CD49d Expression auf LC kennzeichnend. Die Einführung neuer diagnostischer Algorithmen in die Phänotypisierung epidermaler dendritischer Zellen konnte das diagnostische Spektrum dieser Methode deutlich erweitern. Neben atopischem Ekzem und normaler Haut wurden auch Algorithmen für die Psoriasis und das allergische Kontaktekzem beschrieben. Die systematische Analyse weiterer Dermatosen mit epidermaler Beteiligung dürfte auch für diese Erkrankungen diagnostische Algorithmen aufzeigen, die das Mikromilieu der entsprechenden Krankheiten widerspiegeln. Die Phänotypisierung epidermaler dendritischer Zellen dürfte insbesondere im Rahmen klinisch-therapeutischer Studien vermehrt zur Anwendung kommen, da mit dieser Methode eine objektive, quantitative Untersuchung einzelner behandelter oder unbehandelter Hautareale im zeitlichen Verlauf möglich ist.
