Is it just me, or did anyone else notice what a contrast it was watching Christopher Luxon cracking jokes with Anthony Albanese, compared to what it was like when Jacinda Ardern visited Australia? For all her kindness and communication when she was Prime Minister, she would use those trips to Australia to give then–Prime Minister Scott Morrison a tongue-lashing—usually over the 501 deportees, which was pointless because the Aussies weren't going to change their minds. This weekend, though, was a bit of a love-in. And that's despite the fact that we've done something that could genuinely have upset the Aussies. Because Nicola Willis has probably gone a bit too hard, having cracks at them for their capital gains tax changes in their budget—which they're very sensitive about, because they're copping huge blowback. And yet…it was no drama. Albanese wrote it off as cheekiness. And then, instead of yet another trans-Tasman drama, he was cracking jokes with Luxon about Kiwi immigrants. They were taking turns going first with the questions, and they were affirming each other—welcoming closer ties, strengthening shared resilience. It's turning into a bit of a cliché thing to say now, but Luxon is in his element overseas. He sounded every bit the statesman—someone who has thought deeply about the degrading state of international affairs and what New Zealand needs to do to weather the coming storm. And I thought, as I listened to him pitch how kick-ass Australia and New Zealand are going to be, that he was doing a better job of selling Australasia to the world than the Prime Minister of Australia was. He's a big-ideas guy—selling his country and his region and getting on with people is his party trick. Isn't that a better strategy, when you think about it, than always fighting with your only ally? We've got to start this week talking about that Auckland FC win on Saturday night. Did we not discuss on Friday's show the need for us in this country to be more ambitious for success? To have more confidence to back ourselves more and then a day later, just one day later, we have an example of exactly that. Now to be fair, obviously, this wasn't exactly us backing ourselves. It was an American billionaire Bill Foley backing us. But his attitude is the kind of attitude worth adopting. When he put his money into a football team in Auckland, he set them a goal of winning the A-League within three seasons. They did it in two. He did the same thing in Las Vegas with the team that he has there, he set them a goal of winning their competition in six seasons. They did it in six. Now of course, having heaps and heaps of money helps. If you're a football team, it helps to have the best of everything that money can buy. The best data, the best physio, the best premium accommodation, whatever it is. And we're going to find out just how much difference money really makes next year when the high salary cap that new teams get for the first two years disappears and they are on par with all the other clubs. But while I think money helps, I have a suspicion it's not the secret sauce that leads to Bill Foley constantly being successful. Because we've got myriad examples of money being thrown at things and them still failing. LIV Golf. The rugby league, the Rebel Rugby League, and that's just in the past year. I'd love to know what his secret sauce is. Is it getting the best coach? Is a coach more important than a team? Is there a formula to setting up a winning team? Is it the attitude of expecting success? Either way, when I stood there in that crowd on Saturday night in the minutes after the team won, actually just shocked, kind of speechless for a bit to think that they actually managed to pull it off. The thought occurred to me that this was good for Auckland. This is a city with high unemployment, grumpy about the past few years. Now a football team winning doesn't change that, right? But it is not a bad thing for a place like Auckland to score a win, to feel like it can be the best at something in Australasia. It's not bad to have all the free publicity in the Australian media market for our biggest city. It's not bad to get the kids really excited about a game and it is not bad to illustrate what can be done if you simply believe it can be done. In the second episode of this series, Dr. Justin Abbatemarco and Dr. Benjamin P. Trewin discuss what was found in non-steroidal maintenance therapies. Read more about this abstract on the AAN website. Show transcript: Dr. Justin Abbatemarco: Hello and welcome back. This is Justin Abbatemarco from the Cleveland Clinic. And we're joined by Ben Trewin on his abstract maintenance immunotherapy and MOGAD, early steroid benefit, dose thresholds and disability risk. Ben, in our first episode we really talked about corticosteroids, but your paper and abstract looked at other therapies. What did you find in those non-steroidal maintenance therapies? Dr. Benjamin Trewin: In addition to looking at oral corticosteroid therapies, we also looked at B-cell depleting therapies, namely rituximab and ocrelizumab, and intravenous immunoglobulin and steroid-sparing therapies, namely azathioprine and mycophenolate predominantly, I suppose a couple on methotrexate. Now, what we found, it's important to note that we were able to tease apart the effects of all these drugs with our Cox proportional hazard model chops up, follow up into distinct intervals with different combinations and permutations of these medications and their different doses in a more granular way than is allowed by previous techniques like incident rate ratios when we compare pre and post annualized relapse rate, and we think this is a strength of the study. With this methodological strength, we were able to see that steroid-sparing therapies, despite 334 patient years of data, do not appear to have any independent benefit with respect to time to next relapse. The estimate of effect there was 1.06. And then for time to confirm sustained disability, there was also no confidence signal, the confidence interval being 0.15 to 1.4, that it actually prevented any disability despite a wealth of data, which I think is an important thing to note. And I think previous studies, particularly looking with incident rate ratios, have been a little more optimistic with that. And I think there might be misattributing some of the benefit of concomitant steroids to the steroid-sparers, but it's more complex than that, of course. And then with respect to B-cell depleting therapies, we did have 48 of 261 patients exposed, which is reasonable, but not quite enough to get the signal we're looking for. However, we found something quite interesting, because when we compared the Liverpool data to the Australasian data, the two big study groups involved, we saw that it wasn't quite as effective in Liverpool as it was in Australasia in this subgroup analysis. And so we dug a little deeper, as one should, and found that the dosing is actually different. And in Australasia, we have a tendency to just give two grams of rituximab up front, or 600 milligrams of ocrelizumab. And then six-monthly, you give a gram of rituximab without fail, without trying to watch the B cells or trying to muck around with doses in any way. And when we looked at that, the threshold dosing, as we termed it, as compared to below threshold dosing, there actually was weak evidence at a PVA of 0.08 that threshold dosing is superior to below threshold dosing. And that needs to be reproduced, but I think that was an important signal. And finally, I would say IVIG, of course, has some very strong data in this area. And I think it's important from this study at least to remain a little agnostic on that as we only had 31 patients on IVIG, and so I absolutely wouldn't say it's not effective. I would say unfortunately, we had insufficient data to make any big claims about that. Dr. Justin Abbatemarco: I think some really great data to help pick apart here and help inform practice. I think your point about looking at the previous literature and trying to tease apart these steroid-sparing agents, that corticosteroids they're not uniformly addressed, and so it's difficult to think about at those previous data points, so I appreciate that. And then this dose response to the B-cell therapies, there's been questions in the literature, because I think we've gotten a lot of mixed results on B-cell therapies. And so this to me is one of the larger studies that really help answer this question that maybe B-cell therapies are effective and maybe we need to be a little more sensitive to dose, which is the same theme we saw on IVIG. IVIG, maybe at higher doses, could be more effective for MOGAD. What do you think about that comparison? Dr. Benjamin Trewin: I like where you're going with that because we're quite interested in these dose responses as we introduce this 12.5 milligram per day oral corticosteroid dose or 0.16 milligrams per kilograms per day in kid. And so we're quite interested. And, of course, that work by Dr. Chen and Dr. Mariner has revealed that IVIG also has quite a sensitive dose threshold there at one gram every four weeks. And we followed that precinct because that research was so strong. So it's nice to feel like we're building on previous studies and then perhaps even detecting another dose threshold with respect to rituximab. And I must say, it was a little bit of a surprise, we came in and saw why is the Liverpool data moving that way and the other one moving this way? So it was a nice data-driven evolution of our multi-variable model. Dr. Justin Abbatemarco: So helpful. And I'll ask everyone to come back for the final episode, where we try to put this all together. I greet you in Jesus' precious name! It is Saturday morning, the 14th of March, 2026, and this is your friend, Angus Buchan, with a thought for today. We go to the Gospel of John 20:15-16, where Mary Magdalene asks the man standing there, “Sir, if You have carried Him away, tell me where You have laid Him, and I will take Him away.” ”Jesus said to her, “Mary!” She turned and said to Him, “Rabboni!” (which is to say, Teacher).”She heard His voice and recognised Him immediately, but she didn't recognise Him when she saw Him standing there. She thought He was the gardener: “Sir, please tell me where you have put Him and I will take Him away', and the voice, it was the voice that she knew. I want to ask you a question this morning, and I am asking myself the same question, do we know His voice? Mary knew the voice of Jesus. Straight away she said, “Rabboni!” I have heard that many young Muslims in Iran at the moment are having dreams, and they are seeing a Man dressed in a white robe with eyes of fire, talking to them. Now I want you to read the Word of God and you will see in the Book of Revelation, that is the description of Jesus Christ. In 2003, I was in the game reserve in the north of Kwazulu-Natal, and the Lord spoke to me so clearly in my heart, “Go home, cancel all of your preaching appointments. I want you to mentor young men.” That was the foundation and the beginning of the Mighty Men phenomenon that is going right around the world as I speak to you today. This very year, we are having Mighty Men Conferences in Europe, in Australasia, in America, all over Africa - and we give the glory to God. Today, you need to really start to listen to the voice of God. “But” you say to me, “how will we know His voice?” Well, how do you know the voice of anyone? How do you know my voice? Because you listen to it and you hear it every single morning. Isn't that right? People meet me at the airport. They don't know what I look like but when I open my mouth they turn around and they say, “We know who that is” because they hear my voice.Now, when you spend time with Jesus Christ every single day, you will learn to hear His voice and He will direct you, and by the way, He says very clearly in Revelation 22:20, He who testifies to these things says, “Surely I am coming quickly.” Amen! Had a really cool discussion with golfing legend Peter Senior. We mainly chatted about the swing changes he made with his coach Gary Edwin and the drills he did to accomplish ten changes. In 1978, Senior turned professional. He joined the PGA Tour of Australasia shortly thereafter. During his career, he has won 21 tournaments on the tour, including the Australian PGA Championship in 1989, 2003 and 2010, the Australian Open in 1989 and 2012, and the Australian Masters in 1991, 1995, and 2015. Senior has won professional events on the main men's tour in five separate decades, a feat achieved by very few players previously anywhere in the world. He also has the distinction of winning the Australian PGA, Open and Masters tournaments after he turned 50. He also topped the tour's Order of Merit in 1987, 1989 and 1993. Amongst his many honours is the Clarke Silver Medal, the Royal Humane Society of Australasia's highest award. He is one of only 3 Victoria Police officers to receive the award in its 150-year history. After retirement, Trigger and his family endured the devastating Black Summer fires, followed by COVID-19 lockdowns that almost broke their hometown apart. Using his expertise, Trigger set up a recovery website that funnelled nearly one million dollars for the victims of the fires and earned him a Resilient Australia Award. He continues to live in remote coastal Victoria today. Mark is a born storyteller, and wrote his autobiography, Back Up Is 3 Hours Away, which details his incredible career & shares amusing anecdotes as well as incidents that changed his life forever. This is a raw account of what remote policing is all about. Mark, like all veterans and first responders, struggled with the transition from service. The BMW Australian PGA starts today at Royal Queensland and we sat down with the Australian PGA CEO Gavin Kirkman for a chat about the tournament and what he is working on currently to grow golf.We had to start with an update. Brisbane has been hit by some quite severe storms last week, and as recently as Monday this week, was the course damaged, and have the storms caused any issues with, or changes to, the structure of the tournament? Gavin gives us the latest information and looked at everything having just walked the course prior to our conversation.Nick and Mark are impressed with the calibre not only of the Australian and New Zealand players, but of the International contingent who have come to Brisbane for the event, and ask Gavin whether he feels there is further plans for co-sanctioning with the DPWT, following the excellent work with the Hotel Planner Tour.
