Pediheart Podcast #386: What Is The "Right" Starting Dose Of Adenosine For SVT In Children?

Pediheart: Pediatric Cardiology Today

This week we review a recent meta-analysis of studies assessing efficacy of adenosine in children to terminate SVT. Does the PALS recommendation of 100 mcg/kg as a starting dose for SVT management make scientific sense given what we know about the efficacy rates of this dose? Why might inadequate doses be potentially dangerous for children with acute SVT? Is there adequate data to consider changes to the recommended starting dose in this situation? Dose adenosine work well for all forms of tachycardia involving the AV node? Pediatric emergency physician and family medicine physician, Dr. Lais dos Santos of Mossoro', Brazil shares the results of a large scale meta-analysis that she performed and offers some answers to these and other questions. DOI: 10.1007/s00246-026-04281-5

Viņa darbs sākotnēji bija dokumentāls – viņš fotografēja nelielas Dienvidāfrikas pilsētiņas un to iedzīvotājus. Vēlāk viņa stils būtiski mainījās un kļuva daudz mākslinieciskāks un inscenētāks. Mākslinieka fotogrāfijās sastopami cilvēki, dzīvnieki, zīmējumi un tēli, kas vienlaikus šķiet gan pazīstami, gan sveši. Vairāk nekā piecdesmit gadus mākslinieks pētījis cilvēka zemapziņu. Tieši šim pasaules redzējumam veltīta viņa retrospekcija. Rotko muzeja vasaras sezonas izstādes apmeklētājus gaida līdz pat 30. augustam.

#266: The two levers left for app marketers with Shumel Lais, Founder of Day30

Performance marketers are running out of levers — and the one most teams haven't pulled yet is signal. In this episode, we speak with Shumel Lais , Founder of Day30, about signal engineering and why it may be the most important skill in performance marketing right now. Shumel explains what signal engineering actually is, how prediction models use behavioural data to identify high-value users before they ever convert, and why synthetic events — goals you engineer rather than observe — help ad platforms like Meta find better users faster. He also shares how subscription apps have achieved up to 50% reductions in CAC, and why, with everything else becoming automated, signal is now one of only two levers performance marketers still control. If you work in user acquisition or subscription growth, this is a must-listen. Today's topics include: What signal engineering is — and how it differs from simply tracking conversion events How prediction models use behavioural data to score users by conversion probability Synthetic events — engineering goals that don't yet exist to give ad platforms a sharper target Why subscription apps generate the behavioural data depth that makes signal engineering work The three components of an effective signal: volume, velocity, and precision Why performance marketers are down to just two levers — creative and signal Links and Resources: Shumel Lais on LinkedIn Day30 Business Of Apps - connecting the app industry Quotes from Shumel Lais “The concept of signal engineering is to see how we can manipulate that event to give the ad platforms a stronger correlation to the business value that you're after." "A synthetic event is, ultimately, when we're creating an event that doesn't actually exist. These are not things that have actually occurred — but based on the data we take in, we can build this from scratch." "When I think of performance marketing now, everything's become very algorithmic and very black box. There's less and less levers available for marketers to pull. I think there's only really two levers left — one is creative, and the second lever is signal." Host Business Of Apps - connecting the app industry since 2012

Together we dive into the deeper layers of decluttering and why the objects in our homes often hold powerful emotional stories.Heather shares how clutter is rarely just about “stuff.” Instead, it often reflects unfinished chapters, emotional attachments, past identities, and the transitions we are moving through in life.In this conversation we explore:• Why some objects feel almost impossible to let go of• How clutter can mirror what is happening in our inner world• The energetic imprint objects can carry• Why honoring the items we keep is essential• The connection between mind, body, soul, and home when decluttering• The emotional roots behind hoarding, minimalism, and clutter patternsHeather also shares fascinating stories from her own journey into spiritual awareness, including synchronicities with dimes, intuitive sensing of objects, and how her work helping others declutter evolved from running a cleaning business to supporting people through deep personal transformation. This episode beautifully bridges the energetic work of space clearing with the physical act of decluttering, showing how both are deeply connected when creating a harmonious home.If you have ever looked around your space and wondered why certain items still linger, or why letting go can feel so emotional, this conversation will offer both insight and compassion.00:00 Honor What You Keep00:21 Podcast Intro And Guest03:00 Teen Paranormal Signs06:38 Smells As Psychic Sense08:59 Dimes And Divine Nudges13:12 From Breakup To Business14:34 Why Organizing Fails16:21 Four Pillars Method19:17 Seniors And Scarcity Clutter22:12 Emotional Attachment To Stuff23:41 Declutter For Change Origins25:39 Minimalism Vs Hoarding26:59 Clutter Spectrum Balance27:48 Letting Go Old Identity29:54 Books Emotional Attachment32:58 Objects Carrying Energy37:58 Group Declutter Sessions39:21 Breakthrough Case Study41:44 Origins Of Organization44:57 Trends Cozy Beige Minimalism49:54 No Shame Closing AdviceHeather Clark knows your world because she lived it.On the outside, she was a high-achiever who had it all. It honours the pain of suicide loss while also opening the door to continued connection, love, and spiritual evolution. If you are grieving, may this conversation remind you that relationships do not end with death. If you are struggling, may this episode remind you that you are not alone. And if you are an empath who feels overwhelmed by the collective weight of the world, may you remember to love yourself as deeply as you love humanity.----------------------------------------------------------------------Caroline is an entrepreneur and blogger who lives in Chicago. How Lula pivoted from B2C to B2B after discovering landlords were 80%+ of users | Bo Lais

Category Visionaries

Play Episode Listen Later Feb 20, 2026 19:39

Lula rebuilt property maintenance from the ground up by solving a fundamental problem: property managers spend 40% of their time coordinating maintenance with zero visibility into work order status. After pivoting from a B2C app when they discovered landlords were their actual users, Bo Lais and his team made a critical insight—deep PMS integration wasn't a feature, it was the entire go-to-market strategy. Today, Lula's 9,000-contractor network processes 1,000 work orders daily across 50 markets, performing 30 HVAC replacements per day at scale that enables direct manufacturer relationships. Now they're commercializing their internal tech stack as Foresight, a standalone SaaS platform launching Q1. In this episode of BUILDERS, Bo breaks down the strategic decisions behind building integrations as distribution, using network density to create pricing advantages competitors can't match, and knowing when to productize your internal tools. Topics Discussed: Why the B2C to B2B pivot happened after discovering usage patterns, not market research How PMS integration eliminated "swivel chair" friction and became the primary distribution channel Strategic partnership depth over breadth: enabling co-selling with AppFolio, Buildium, Yardi rather than partner proliferation The 250-door threshold where maintenance coordination breaks and technology becomes necessary Network density economics: 30 daily HVAC replacements creating leverage for direct manufacturer negotiations and flat-rate service catalogs The decision framework for commercializing Foresight based on upstream customer advisory group feedback Maintaining discipline around ICP when sales teams naturally want to expand GTM Lessons For B2B Founders: System of record integration is your distribution strategy, not a feature: Lula's standalone app created adoption friction because property managers refused to work outside their PMS. Bo's realization: "They need everything to live in their system of record...They don't want swivel chair. And then providing that real time visibility throughout the entire life cycle of the work order was really valuable because prior to that they assign it to a vendor, and then they cross their fingers and hope that it gets done." The integration solved both adoption friction and delivered continuous visibility their workflow demanded. For B2B founders: if your users live in Salesforce, HubSpot, or vertical-specific platforms all day, your integration strategy IS your distribution strategy—build there first, not alongside. Strategic partnerships require enablement infrastructure, not just signed contracts: Bo's approach rejects partnership sprawl: "It's not about stacking on another 10 partnerships, it's about how do we go deeper and enable those partners to co-sell with us and talk about the value props that together we can provide." This means building co-selling toolkits, joint value propositions, and partner success metrics. For B2B founders: one partnership where the partner's sales team actively sells your solution beats ten partnerships where you're just listed in a marketplace. Invest in making partners successful sellers, not collecting logos. ICP discipline requires sales team enforcement mechanisms, not just definitions: Lula knew their ICP but struggled with execution. Bo learned "it's one thing when we understood who our ICP was, but then it's a whole nother thing to adhere to that and get the sales team to adhere to that ICP." The specificity matters: residential (not multifamily), single-family, 250+ doors (where coordination breaks), capped at several thousand doors (before enterprise needs diverge). For B2B founders: document your ICP, but also build the compensation structures, deal approval processes, and CRM workflows that prevent sales from chasing deals outside the sweet spot—even when quota pressure hits. Message outcomes customers measure, not the technology delivering them: Bo's AI framing: "They care about the outcomes, right? If we're able to move the needle on the outcomes and provide a better experience for residents by automating communication, automating the time to schedule, automating the time to get resolution...it's not the how, it's the result." Lula's AI eliminates truck rolls through upfront troubleshooting and improves one-trip resolution rates—that's what property managers track. For B2B founders: if your customer's boss asks "how's that new tool working," they answer with metrics they're held accountable for (resolution time, truck rolls, resident satisfaction), not "it uses AI." Lead with those metrics. Productize internal tools when customer advisory groups request them and you have defensible advantages: Lula commercialized Foresight after upstream customers specifically asked for their tech during advisory sessions. Bo's competitive moat thinking: "Everyone else thinks they're going to do it better with the AI and automation they have. But our competitive moat is that our on-demand network is built inside this AI work order management system. And because of the scale of our network and the buying power, we can provide instant quotes for a lot of services...our competitors that are just doing software don't have this network of contractors nationwide." For B2B founders expanding product lines: customer pull plus operational advantages competitors can't replicate (Lula's contractor density, manufacturer relationships, 1,000 daily work orders of training data) create viable new products. Without both, you're just building undifferentiated software. // Sponsors: Front Lines — We help B2B tech companies launch, manage, and grow podcasts that drive demand, awareness, and thought leadership. www.FrontLines.io The Global Talent Co. — We help tech startups find, vet, hire, pay, and retain amazing marketing talent that costs 50-70% less than the US & Europe. www.GlobalTalent.co // Don't Miss: New Podcast Series — How I Hire Senior GTM leaders share the tactical hiring frameworks they use to build winning revenue teams. Medication nonadherence in schizophrenia is often framed as defiance — but that narrative misses the truth and harms the very people it claims to help. Guest host Rachel Star Withers (who lives with schizophrenia) unpacks why up to 70% of people with schizophrenia have trouble taking medication as prescribed — and why the reasons are far more complex than “noncompliance.” From severe side effects and cognitive symptoms to stigma, trauma, access issues, and being dismissed by providers, this conversation exposes the real barriers standing in the way of effective treatment. Rachel shares deeply personal experiences of both intentional and unintentional nonadherence, including how side effects like extreme fatigue and tardive dyskinesia shaped her decisions — and how lack of education nearly cost her years of quality of life. The episode also explores how medication refusal can be weaponized, stripping people with schizophrenia of autonomy under the guise of “lack of insight.” Later, Rachel sits down with board certified toxicologist and psychopharmacologist Dr. Alberto Augsten, who brings clarity, hope, and science to the discussion. Together, they explore long-acting injectable antipsychotics (LAIs), new treatment advances, realistic expectations, and how education — not force — improves outcomes.
