We've gone through quite a few pods on film, especially film noir and police procedurals, but this might be the first that has a female protagonist as the main character! More's the pity! But what a protagonist! Perhaps to make up for the previous imbalance, we're going to encounter one of the fiercest, most insightful, and action-oriented characters yet, regardless of gender. She has high EQ, high IQ, empathy, but she's also tough as nails, and wise. Nicely enough, she was the character that introduced me deeply, as she did for so many people, to the wonder of the Coen Brothers and their cinematic universe. 4:[3:32] He would put anything on and he just.Track 1:[3:35] He was a massive pavement fan as well.Track 4:[3:37] So I would have heard him on his show. I had a great friend called Mark Porchani, who was, in those days, he was an avid cassette taper of all the radio shows.I believe that he still has his archive stretching back then.Track 1:[3:52] So he might be someone that would be good for you to speak to.Holy shit. Yeah, man. That would be cool to get digitized. Yeah.Track 4:[3:59] Man.Track 1:[3:59] Well I can we can speak after this but I'll I'll yeah I think he would be someone really fascinating for you to speak to as well anyway digressing so yeah so I would have heardPavement on John Peele but then I missed their I think it was 92 they toured Slatted and Enchanted, and they played at Strathclyde Uni Strathclyde University in Glasgow, but I missedthat gig by a couple of days oh man yeah man yep same thing happened to Nirvana when they played the QMU in Glasgow I missed it I bought the single, three days after they playedyeah, good luck but I mean I got to see Pavement on all the other tours after that you did?Yeah yeah so oh you're a turbo fan man awesome man yeah yeah so Crooked Rain, and then yeah Breaking the Corners and.Track 4:[4:55] Hi what do you think it is about the uk that that really um they adopted pavement in a way in a way that the rest of the world just didn't you know like they were popular in the us ofcourse and popular in canada but it seems like the uk and scotland like it's much bigger than that yeah it's it's exactly that it's almost the same way people are about like the rocky horrorshow.[5:25] So i'm i'm a huge fan of the rocky horror show huge fan of pavement and it's kind of like it for a long time like in the mid 90s it was certainly it was like a barometer you know touse of okay these people seem kind of cool do you like pavement yes awesome you know so it's like like not not being like you know cool and elitist but just kind of okay these are peoplewho are obviously switched on they're probably into the same kind of literature and other bands that we would like so then it's just i think they were just such a are they still are they're justan amazing stepping stone into so much other you know literature and and architecture and psychology just the the subject matter of the songs once you actually delve through the lyricsit's yeah it puts you on a lot of different nice paths i would say yeah but i think especially like so i'm from glasgow so as you've experienced a glasgow audience we're very vocal and wereally we really attach ourselves you know it's the cities you know there's a lot of uh emotion there a lot of it's centered towards football teams but it's also bands we really really love ourbands.I'd say the next kind of Samoan city is probably like Manchester or Liverpool where it's the same kind of vibe as Glasgow.Track 1:[6:45] Wow. I visited both on my UK tour when I followed Pavan.I didn't tour, but I followed their tour. I went to Manchester.I was only there for like 30 hours, so I didn't get to see much, but I saw a show.So that was cool. What was your favorite tour that you saw them on?Track 4:[7:06] It would have been Brighton and the Corners because they played at the Glasgow School of Art.Track 1:[7:16] Okay. So I was studying just around the corner from it at the time.Track 4:[7:20] So I went up to the art school and I knew the guy called Simon Fox.Track 1:[7:27] Who was the entertainment officer there.Track 4:[7:29] So he was the one responsible for booking all the bands for that year.And I said to him you know like I'm obviously a massive fan is it possible to maybe see the guys before the sound check you know just just to say hi and stuff and he was like well wecan't do that but because we knew each other as well so he was like, do you want to come to the after show and I was like yeah, so yeah so yeah so there was a bunch of us went and Ithink it was five of us that went there and then And watched an amazing gig, a really, really great gig.And then we went to the after show afterwards and got hung up with them.I had a chat with Malcolmus for about two hours and just such, such engaging people.Track 1:[8:16] You know.Track 4:[8:16] And like met the whole band. Yeah.Track 1:[8:20] I had on like an old.Track 4:[8:21] It was a t-shirt that it turns out that Mark Ibold designed it.Track 1:[8:29] So I got it on the I got it on the Crooked Rain tour so it's like this kind of cross stitch thing, and then at the gig at the art school like, I bowed I was like can I buy that t-shirt off youbecause we don't have any left and I was like nah I love this man but they were just I mean I think they spent, easily five six hours just chilling with the fans in the after show and justbeing just really really nice guys and, And you nailed Malcomus down for that long. Yeah, yeah, man.Nicely done. I think I really annoyed, I don't know if you know.Track 4:[9:06] There's a really kind of very important band from Glasgow called The Pastels.Track 1:[9:11] So they were both on Geographic Domino at the same time.Track 4:[9:17] Okay.Track 1:[9:17] Yeah, Domino. So Pastels were the support band.Track 4:[9:20] Oh, okay. For that gig. And then Stephen.Track 1:[9:24] The singer, so he was talking to Malcomus and I came down the stairs was in Spotted Malcomus and I think I kind of interrupted him being a bit of a fanboy and I think Stephen gota little bit annoyed at me Stephen Pastel got a little bit annoyed at me but you know I think I've, we've made up since then I'm sure so well I mean.Track 3:[9:45] Man yeah so what was it about that show other than meeting the band or was that was that why that was the the show is it because you met the man, no i mean i think i bumped intohim again after other gigs as well and like, i just think it was uh it was that i think that that was peak i think that was just it was like they were just completely riding the zeitgeist and yeahthey were they were on their absolute a game you know and just yeah i just i just felt like they could have you know i can, thrown out a can down a set of stairs and it would still theywould still have got something really musical from it and you know just create some wonderful piece of music so what's your record which which is your record the one that you cleave tothe most right, tough call right sophie's choice yeah totally man but the track that i always go back to is here, like yeah but actually that's just my go-to and i was actually i was playing i'vegot two kids i've got a nine-year-old and a seven-year-old and uh i was playing it and my daughter was like is that your band and i was like no no this is uncle steven and his band it's uhit's not us but we would maybe aspire to being a tenth of that or even 1% of that.Track 4:[11:09] I think that's I think it's the same for a lot of people who've got so much attachment to that track but it's the same as any song really I mean it's for me having grown up you knowthat was my kind of teenage formative years.[11:26] Late teenage formative years in the early 20s and, just so many memories memories on you know when i bought that record or you know any of the records and you knowremembering being at different friends at their house and sticking vinyl on and listening to it for the first time and people there was a a guy uh i think his name was dawson he was acomplete metalhead uh he was a friend of a friend and he was like what is this you know and i was like oh this is paving this is a new record and he was he was hooked you know fromfirst listen yeah man we we just put it on.Track 1:[12:01] I think it was.Track 4:[12:02] Um, it was crooked rain put on and just, you know, play that four or five times in a row.Track 1:[12:08] And he was, he was like, this is awesome stuff. And then from that, that's a fucking record.Track 4:[12:12] Yeah.Track 1:[12:12] Yeah. I mean, that's fine.Track 4:[12:14] It's that they are just such a good, great gateway band.Track 1:[12:17] You know?Track 4:[12:18] I think they're like now I would say, the band that I'm probably equally as passionate about after them would be the Super Furry Animals Oh cool.Track Florida Panthers (Puma concolor couguar) are a distinct population of pumas that live predominantly in southwest Florida. Pumas used to roam the entire United States, but habitat loss and overhunting in the 1800's and early 1900's caused them to disappear from much of their range. Every breeding population of pumas East of the Mississippi river vanished, except for one. South Florida, due to its wild and swampy nature, was able to thwart human development just enough for its inhabiting pumas to narrowly avoid oblivion. These surviving pumas, or Florida Panthers, represent the resiliency of wild south Florida. The decades since their near extinction have been filled with controversy, conservation, and politics. This iconic cat still has many challenges to face, and with only 200 or so remaining in the wild, their future is uncertain. Daniel was first introduced unofficially to this animal during his first trip to Everglades National Park in 2017. The Florida Panther and the swampy, remote areas in which they are found captivated Daniel's imagination. Every subsequent trip to the Everglades and southwest Florida had at least some time dedicated to searching for this cat, all to no avail. In January 2024, after spending months preparing and researching, Daniel embarked on a five-day solo trip in the backcountry of the Big Cypress National Preserve, with the sole objective of finally crossing paths with a panther.But what exactly IS a Florida Panther? Are they only found in Florida? Are they black? What is the difference between a Florida Panther and a mountain lion? And of course… do they eat people?! Luckily, Bill was able to meet Daniel in south Florida to record an episode about the Florida Panther and tackle the questions and misconceptions that leave this cat shrouded in mystery. This episode was recorded on January 8th, 2024 at Everglades National Park in Homestead, FL. Episode NotesWhen Daniel was talking about Florida Panther size, they were described as smaller than other pumas out west. It should also be noted that while they do fall to the bottom of the size and weight scale of pumas in North America, the populations of pumas closer to the equatorial rain forests in South America are even smaller. This reinforces the notion discussed in the podcast that pumas in colder climates or higher elevation tend to be larger than pumas in warmer climates and lower elevation. Also, Bill asked Daniel what their life span was, and Daniel was not sure. Bill guessed 20-30 years. According to Florida Fish and Wildlife Conservation Commission, “Panthers can live up to 20 years or more in the wild. Female kittens have a good chance of living 10 years or more. Males have a tougher time, but if they survive to five or six years old, they are likely to live even longer to 10 or more years.” Nicely done Bill!While discussing vehicular collisions as the number one cause of death of Florida Panther, Bill and Daniel discussed how in 2023, 13 Florida Panthers were killed by vehicular strikes. So far in 2024, there have been five. Bill and Daniel referred to the rule about animals being larger the farther you get from the equator, but they couldn't remember the name of the rule. It's Bergmann's Rule, and it's defined as: “one of the best-known generalizations in zoology. It is generally defined as a within-species tendency in homeothermic (warm-blooded) animals to have increasing body size with increasing latitude and decreasing ambient temperature. That is, Bergmann's rule states that among mammals and birds, individuals of a particular species in colder areas tend to have greater body mass than individuals in warmer areas. For instance, white-tailed deer are larger in Canada than in the Florida Keys, and the body size of wood rat populations are inversely correlated with ambient temperature. I'm your host, Anne Ganguzza, along with my amazing, awesome, most wonderful boss, co -host Lau Lapides. Hey, Lau, hi, hi, Annie. Happy Saturday, yes, happy Saturday, Lau. You know, Lau, it's a new year and I am full of promise and full of motivation and inspiration and we've just come off the season of. I like to call it the season of giving, because I love to give gifts and I love to donate to my favorite charities. And you know what? I also like to receive gifts, but we don't always talk about that. We talk about giving, but what about receiving? And sometimes Lau those gifts, they're not in a box. So let's talk about the gift of receiving, shall we?  01:14 - Lau (Host) It's so interesting to me. I love this topic and because there's so many cultural differences of how we receive. There's gender differences, there's age and generational differences and I don't hear anyone really talking about how do we get something, receive something, take something, and how do we behaviorally deal with that. Yeah, like something is simple. I'll tell you what's in my brain right now. When someone comes into my home or my studio, the first thing I do and this was in my training was to offer them something. I offer them a hot drink.  01:51 - Anne (Host) I offer them a warm beverage.  01:52 - Lau (Host) I offer them a warm beverage of their choice, right, and that is definitely generational, but it's also cultural because that's the background Eastern European background. We are gift givers. We give a lot to others, right, and there's different schools of thought business-wise as to how you receive that, and how I train my clients is take it, take it. You don't have to drink it, you don't have to want it, but take it because it's giving a message to the person who's giving it to you that you are cordially receiving something that they're offering to you.  02:29 That's part of their protocol and they're etiquette right.  02:32 - Anne (Host) Well, I'll tell you one thing that's always been hard for me to receive and I think a lot of maybe women, maybe a lot of people share. This is a compliment I'm just saying that is so difficult for me, without downplaying it, and honestly, the best thing you can do to receive that compliment is to receive it with gratitude even if you don't feel it right.  02:55 I mean, how many times do people send a compliment your way and you're like, oh no, it was nothing. Or, and especially, though, when it comes to our businesses too though, isn't it funny. When it comes to our businesses, I am more than happy to take a compliment from a client. Do you know what I mean? If a client's like, oh my God, great job, right, I'm very happy about that. Inside, I'm bursting with pride and joy and hopefully that's every boss out there when they get a compliment or good feedback from their client, and I think that you must receive that. And if you receive that, that really speaks volumes to professionalism, right and courtesy, and understanding what it means to the other person when they're giving you something such as a compliment doesn't have to have monetary value, of course.  03:37 Well, like to have gifts of gold but in addition to that, gift of a kind word is really something I think that is truly like giving of the other person, and if you reject it, then that's a little insulting and hurtful.  03:53 - Lau (Host) I think you hit the nail on the head too when you said, especially with women, how we're not always in the mood to receive that or expecting that or even deserving of that. In our mindset, it's a little bit of that imposter syndrome that comes in when someone gives you that compliment and that moment of like oh, how do I take that? Do I deserve that? Oftentimes we'll say, oh no, I just got this dumb outfit.  04:19 - Anne (Host) You know it was on sale. Anyway, you look terrific, you look great and I throw it.  04:22 - Lau (Host) This old thing, yeah, this old thing, it's been hanging in my closet Doesn't mean but you look terrific, you look amazing. We'll throw it right back at the other person, which they don't realize oftentimes. That is a passive aggressive non accepting value, because it's not just a compliment, it's showcasing your value.  04:43 - Anne (Host) It's almost like when you throw it back and it's not that you don't mean it, but sometimes when you throw it back immediately, it's almost like you don't accept what they're giving to you and you're like, oh no, you look great. And so then it becomes that kind of a battle of maybe, well, who's giving here and who's the gracious receiver right?  05:00 - Intro (Announcement) There's gracious giving and there's gracious receiving.  05:03 - Anne (Host) And I truly believe that it needs to be within our boss superpowers to be able to receive and something that's we're talking a little more on maybe a loftier level in terms of nothing that has monetary value, but definitely has value like a compliment right, an act of kindness, that sort of thing. What about money Lau? I mean, how hard is it sometimes for us to accept for a job Well done, I'm happy to accept money, but thinking about, am I worth the money, am I asking for enough money? And what if I get more money? And is that hard for me to receive? Because, again, we have some of these mental issues sometimes with accepting money and that really kind of, I think, goes back to our value or our feelings of self worth.  05:47 - Lau (Host) Yes, yes, yes, yes. And you know what that money may be in money form and it may go towards an actual gift right. And I recently received. One of the gifts in the mail I literally just got within a week ago was from a client of mine who sent me one of those beautiful boxes for a holiday for. Thanksgiving that was chock full of coffee, beans and caramel and just everything under the sun.  06:09 - Anne (Host) And.  06:09 - Lau (Host) I was so surprised when I opened it up. I was so amazed that someone was thinking of me in that way, but how I reacted to it now, on retrospect, when I look at that, what did I do immediately? I thanked her, I loved it, and then I immediately started to give it away. I immediately started to offering to my clients coming through that.  06:29 - Anne (Host) I see to my family.  06:30 - Lau (Host) Oh, here, let me share, let me, here, let me give this to you. Let me share this to you. And I oftentimes wonder is a gift really meant for you when it's given to you, or is it meant for you to do what you see fit with that gift? So, if you get the monetary cash and you want to give it to charity, how would that person, how would the giver actually feel about that if they really wanted to give it to you? That's an interesting question.  06:56 - Anne (Host) Yeah, that is an interesting question. Well, you know what I think? That if a gift is given, it is because or at least for me, the person that is giving it wants to extend a gift for the other person to experience joy, I would think, and appreciation, and I feel like for me, whatever they choose to do with it, I mean, unless I feel like they secretly hate it and they gave it away and they're like, oh gosh, this is like what is it called?  07:21 - Intro (Announcement) An elephant gift. An elephant gift yes exactly.  07:24 - Anne (Host) And so then I would be a little hurt if I knew. But I think my intentions are to provide a token of appreciation, a token of here's how I feel about you. And if the other person doesn't accept it for themselves graciously at first, and then I realize, oh, maybe they've immediately given it away or something, then I would be hurt. I would be hurt by that Feeling like they've rejected my gift.  07:45 - Lau (Host) But, annie, let's be honest, we've all done that. Haven't we all gotten that scarf? Or those gloves that are just the ugliest thing in the world, do you think? Oh, they're itchy, they don't really fit me. Well, let me, elephant, gift that to someone who will appreciate it. And if not, you notice people are starting to say if you don't want it, it's okay, pass it on. I'm like a donate.  08:05 - Anne (Host) The other thing would be like here's one things that people have made for me. Now, this could be homemade food. This could be homemade gifts. This could be my grandmother knitting me a sweater, or my mother knitted me.  08:18 I'm gonna get me all teary, but my mother, she knitted me afghans and blankets, and my aunt used to so close for me, and it reminds me of the bunny suit on a Christmas story with Ralphie, and so you accept that and you're grateful for it, because there was thought and love put into the gift and so receiving that means that you're receiving that love. You're receiving that gift.  08:41 - Lau (Host) You're receiving it, you're suck it up. You do you do. You're totally, because it's not the color of your bedroom and it just is not the feel that you wanna put on your body, but you love it. You put it on display somewhere. You just have to appreciate that, right. What about this one? What about from a philosophical point of view? As we live our lives every day around the holiday season, how are we viewing gifts that are given by others or by the universe? Are we recognizing any gifts, gifts?  09:09 - Anne (Host) from the universe Locke. Let's delve deeper into that. So what gifts from the universe? What would be some examples? I like that.  09:16 - Lau (Host) Well, I'm a New Englander, so on the New England track. I think all of us can relate that come from cold weather climates and when I have more, temperate weather or. I have a winter where we're not like snowed in and killing ourselves with the slush. To me, I recognize that as an immense gift.  09:35 - Anne (Host) Nice and Californians are very grateful for rain.  09:38 - Lau (Host) Yes, oh yes, yes, yes, yes. How about the gift of looking in the mirror and seeing yourself for what you actually see, not for what you want to see? So that is, as you age, seeing the wrinkles, seeing the imperfections, seeing the scars, but then feeling like, wow, there's a gift in there for me of understanding.  10:02 - Anne (Host) That represents my life, my heart. Here I have to tell you because my own personal experience. I have lost a little bit of weight and so I look at my body, which is now kind of a wrinkly because I got some skin, and I look at that and I'm like, ok, that could be considered by some to not be attractive, but for me it's almost like it's a battle scar. It's like you know what I'm proud of?  10:23 my body I am proud, I am grateful. I am actually so grateful for my body, for being resilient and for the things that my body has been through, and I think that we can apply this to our businesses as well right, our journey as our businesses. But, like my body has been through a journey, my mentality, my entrepreneurship has been through a journey, and I'm grateful for all of it because I've survived it and I've been able to come out the other side more joyful and educated and more motivated to do even more. And so I really try to take a look at that and be grateful for that gift from the universe right that I have received. And I love that we kind of brought the gifts from the universe into this discussion, because I think we have them every day and we don't always recognize them for what they are and we aren't always receiving them or acknowledging them with the amount of gratitude or thought that I think we should.  11:23 - Lau (Host) Right, or even just mindfulness, conscientiousness of seeing it and observing. We talk about it from an actor's point of view, but from a human point of view, are you really seeing what is in front of you? What are you uncovering? And since we're diving deep, I'll go one deeper, because you're not only surviving and you're thriving in the gratefulness and recognition, in the gift of that, and that's what makes you so beautiful, both aesthetically but also inside. There's a beauty and a light that happens in you. To me that's very recognizable and I'm going to dive even deeper. I'm going to say I have someone very close to my family suffering from a very deadly illness and eating it and I watch it every day go into remission and I watch it turn around and I watch it Now, from the outside, this person is somewhat suffering and all of that. From the inside, I see that little soul, that little spirit winning and I'm like a gift a gift, a gift, a gift every day.  12:28 - Anne (Host) It's so true.  12:29 - Lau (Host) If I lose the job, if I don't look good one day, whatever, this is the gift that I'm going to keep coming back to, of course, and I totally identify with that.  12:38 - Anne (Host) I mean it is, and we've talked about this before.  12:41 I mean, yeah, when someone that you love or someone you're close to is going through a challenge mental challenge, health challenge, whatever that might be I mean, in reality, it really points us towards knowing and understanding what those gifts are. And I've said it before, when I was going through my own health challenge, I mean, once I got back in the studio, I was like gosh, what was I ever worried about? You know, here I was thinking, oh, my voice doesn't sound in a particular way or I'd worry about those things, and I'm like my gosh, I have just been given this immense gift and I'm here to be able to experience and here to be able to just do what I love in this booth and just be, just be and be grateful. And well, I'll tell you what that comes through in our voices, doesn't it? It emanates from us and, as performers, I think that it behooves us to be good receivers, right Of the gifts that are being given to us, because it just absorbs into our energy and just radiates out from our performances.  13:39 - Lau (Host) It so does, and I mean the gift of knowledge of how to give as well, so like, for instance, when it comes to philanthropy and charity. I love that. I'm big in that. I'll give a quick plug to our 100 voices who care.  13:54 - Intro (Announcement) We love.  13:54 - Lau (Host) Claire and the gang we love so much. I'm a member and to me it's such a relief, it's such a relief gift to me to figure out how to give in a very safe and impactful and thought away. And as long as I make money, I'm going to give forever because I don't have to think about it. It's one less thing I have to think about because I know it's vetted, I know it's legit and I know the people come from their heart and soul. Wow, to me that's a gift.  14:23 - Anne (Host) Exactly, and you know, the cool thing about 100 Voices who Care is, honestly, it does not take a lot of money, and that's really what the big thing is, and it's literally $100, four times a year, and that's it. And ultimately that, combined with the community, the togetherness that is also in kind, donating that, that is $10,000 that can be given to people in need or organizations in need, and I think that that's absolutely wonderful and I think, really, whatever giving charity that you want, again giving that and then expecting that the people who are in need are receiving that and they're grateful for it right, that's what makes the world go round and that's what I think really giving is so easy to talk about, because I love to give myself and I mean I consider the VoBoss we're giving of ourselves joyfully, because it's something that we both love to do, we love to share, we love to be a resource, we love to help if we can, and that's what we're here for.  15:19 But I really really also want bosses out there to understand the importance of being able to see and acknowledge and receive in kind and be okay, it is for you, it doesn't have to be for anybody else, you don't have to feel compelled to turn around and give it somewhere else.  15:36 I fully believe that if a gift is meant for you and you love it and you keep it for yourself, you give in other ways, right? I mean it doesn't have to be that you have to turn around and feel like, oh, I either need to give that gift or give a portion of that gift, or you can keep it. I mean it's okay, it was meant for you and love, and that is something that I think is really wonderful. And, of course, I mean let's make it more from a business perspective, right, being able to ask for the money that you deserve for a job that has been well done is, first of all, when you do that yes, do that but when you get that actual money and you receive that money, then you'll notice that I feel like the cycle or the circle has been completed.  16:18 - Lau (Host) Yes, absolutely. And here's another one that is amazing but can be tricky in how it's received. You and I, I know I'm grateful and fortunate that we're nominated.  16:27 You and I and our companies for SOVA's nominations, which I'm thrilled I'm over the moon. So when I learned that now you've been nominated for years now that was my first time around being nominated when I learned that it challenged me a little I'll be honest with you. It challenged me. I had to sit there and go, well, okay, do I deserve that? Is that okay, do I deserve that? Was there anyone that deserves it more than me? Or, like it challenged me in a way, I was over the moon, thrilled. I love to be a part of it, the community be recognized. But at the end of the day, you have to sit alone with yourself and say I did this, we did this, I did this and I accept the good things coming my way, along with the challenging things as well.  17:14 It's like you have to have a little conversation with yourself about that and I'm very careful, like I don't want my head to be big. I don't want to be gloaty about it, I don't want to hold it over anyone else. I want to be very humane about the recognition that I slash we as the company get because I feel so grateful for it. I don't feel like, oh, of course I deserved it, I'm the best. I feel like that's going in a direction of morality that is not of high integrity. The highest integrity people are people who are humble and they're modest and they have humility about themselves and their work. Those are the people that I always have admiration for yeah, absolutely, I love that.  17:54 - Anne (Host) And receiving acknowledgement right Receiving acknowledgement is absolutely another important, very important, I think, factor in really having fulfilling and successful entrepreneurial journeys and businesses right.  18:08 I think a lot of times what happens? We get a lot of people coming into this industry that are so unhappy and there are jobs, because I always tell people in the corporate world all we really want is love. All we want is love and acknowledgement right Like validation. It helps to make money right so you can pay the bills. Absolutely, I mean that's first and foremost.  18:26 But I think a lot of times when people are dissatisfied with their work, it's because they've really put their heart and soul into it and nobody's acknowledged that and nobody has really recognized that or given them a compliment or credited them for that, and so that, I think, is one source of people really wanting to come into business for themselves. And so now when we're entrepreneurs feedback that we get and it's rare that we get feedback from clients right, unless we go out and ask for it, which I'm the biggest fan of, going out and getting testimonials right Get yourself that acknowledgement. But also, when it comes unexpectedly, it's something that I think we need to be prepared to just receive it and be grateful for it and really know that you're worth it, because there's something to be said about people who are constantly degrading or downplaying or shunning those compliments or acknowledgments or awards, and we did a podcast on awards. There's just there's lots of different opinions about awards. Do people need an award to do their business? Not necessarily.  19:26 But I'll tell you sometimes that recognition very similar to being in a corporate job or whatever it might be getting a little bit of recognition can really help emotionally. Mental I mean. I feel like everything in life it all stems mentally and then it kind of I'm going to say, pushes out physically, right. Like I feel like everything is in our head, like that's what is controlling the earth, right? Our thoughts, right, I feel it's our thoughts and how we feel about ourselves and how we feel about others and how we treat it. It just directly affects the physical everything and I think that it's so important.  19:59 - Lau (Host) I just rambled off on this kind of tangent, but I think it's all related, right, it's all related because if think of any relationship we have, like I love acknowledging, I love validating, I love giving compliments, I love that it doesn't only make me feel good, it's necessary, it's like, okay, we're going to be tough on someone, we're going to give honest feedback, we're going to say no to someone, we're going to be harsh at times, but it's really important to have those moments where you go you know what, I'm really proud of you. Or hey, you did it, Nicely done. I'm so excited for you.  20:34 Because otherwise I find people in general get really down on themselves very fast and then lose steam, lose energy, lose motivation, because they ultimately start to want to please or be liked by the casting, producer, coach, whatever, and it becomes personalized in that way. So it's like a fine line. I of course, want to have that no-like and trust factor. Of course I want to have that in the business world. But there's a fine line Like we don't want to be motivated to only be liked and only be validated, right.  21:11 Otherwise you're seeking to receive things all the time which are not earned they're not always well earned. They're just that little honor, that little star, for no reason. I want to live in a meritocracy. Still, I want to be rewarded when I'm earned.  21:27 - Anne (Host) Yeah, I love that you brought that up because you're right, Seeking to receive all the time is not good either. Being able to receive is one thing, but seeking to always have likes or stuff like that is also not healthy and doesn't create for a healthy business either. Right, when you're constantly seeking approval and I think that really kind of stems from other issues, like when you're seeking approval all the time, and that could be a whole other podcast Lau.  21:51 - Lau (Host) That is a whole other podcast approval. Yeah, how do we respond to all of the things that happen in our world that are not what we want or not what we're expecting to come back to us? How do we respond to those things Like not being nominated, not getting the award, not getting? How do we respond to that?  22:13 - Anne (Host) That always is a little pain, oh, always a pain, even when you know better, like myself, and we know we've been on both sides right when we want that acknowledgement, we want that award, we want that feedback, and sometimes it just doesn't come back to us the way that we want. And so how do we deal with that right? Again, that's not necessarily a rejection right of something that you're receiving, it's that you haven't received it. And so now, what? Now, what do you do with that longing, that desire, that hope? How do you get yourself back in balance?  22:44 - Lau (Host) That's right. That's right. Balance is really the key because the more balanced we are, the more we can easily give and offer and receive and take. And not do it from an ego point of view, not do it from this heavy ego centric, really do it from a place that's very balanced and very open and very like. It's like a give and take of Feng Shui. You know what I mean. Like you're cleansing Every time you give something away. I find when I Feng Shui my office or my studio and I give things away, if it's received well, I feel cleansed right, and if I'm receiving something, well, I feel cleansed.  23:19 There's like a cathartic thing that happens, a purification that happens if it's not coming from pure ego which. I love. I think it's fabulous.  23:27 - Anne (Host) I can talk Clear the clutter, clear the clutter, clear the clutter, physically clear the clutter in your brain. I mean it can really do a lot to propel you and your business forward. Absolutely, absolutely. What a really cool conversation Lau. A heated mountain getaway for Elizabeth and Kyle.By ElizabethSam. Listen to the Podcast at Steamy Stories.She was now 44 and officially un-hitched, Elizabeth adjusted to newly divorced life pretty seamlessly and easily. She was loving her freedom, her home, and her chance to create a new start for herself. Even not having her kid for days at a time wasn't as bad as she had originally anticipated. Yes, it sucked not having him around all the time, but having some time completely to herself - to be just Elizabeth and not Mom Elizabeth - was pretty awesome, she had to admit.One tough thing about not having her kid around everyday though was trying to fill all of her free time. She did her best to pack her schedule: meeting with friends, taking walks, working out, watching shows, shopping, and doing chores around the house, but even still, there were a lot of hours to fill. And with time on her hands, she found herself often browsing and window shopping online for all sorts of things.One such night, with nothing much to do, she found herself on VRBO looking at cabin rentals with hot tubs, mostly just daydreaming and creating a made up vacation in her mind. Until, that is, she stumbled on a cabin in a mountain town about an hour and a half away. With a private hot tub. With great reviews. Not too pricey. And with availability that coincided with her kid-free days. “Hmm,” she thought to herself. “Free cancellation too? What the hell!” as she whipped out her credit card and made the spontaneous reservation.A few days later, Elizabeth was down at Kyle's and they were sitting on his couch catching up. “So, you know how I have all this free time now?” she said sheepishly. “Well, I got bored the other night and did something impulsive,” she started, “….and I rented this cabin,” she blurted out as she pulled up the VRBO listing on her phone. “Check it out,” and she handed her phone to Kyle. “So, wanna go to New Hampshire with me next month? It's in the mountains, right near a state park” she explained, “….and it has a hot tub,” she added in a sing-songy voice.He chuckled. “Looks pretty awesome,” as he scrolled through the pictures. “What dates?”“It's two nights. Go up on December second and come back on the fourth. Think you can go?”“I have to check with work. And I have to see if I have anything going on with the kids, but I think I should be free. Can I let you know next week?”“Yeah, of course. There's free cancellation for another 3 weeks, so yeah…” she trailed off, excited that this mountain getaway/hot-tub/sexy-time trip with Kyle might actually happen.Next week, he texted her and let her know the trip was a go and Elizabeth was thrilled!Fast forward a few weeks, Their trip was now just a day away. Elizabeth was excitedly packing and getting ready for their getaway:Cold weather clothes? Check.Snow boots? Check.Card game? Check.Toothbrush? Check.Sexy undies? Check.Toys? Lube? Check and check.Bathing suit? No need.She packed another bag full of snacks and drinks and was soon all ready to go.Kyle arrived at Elizabeth's later the next morning as she was finishing up work. She snuck out of her Zoom call to give him a long kiss. Her heart always skipped a beat when she saw him. At just over six feet tall, he was slim but strong, with a neatly trimmed beard and matching buzz cut, sprinkled with grays. She thought he was handsome, rugged, and sexy as hell.“This meeting was supposed to be over half an hour ago,” she said with an eye roll “; but it should be over soon and then we can get going.”“No problem…I'll just chill on the couch until you're done,” and he went to hang out in the living room until she finished up her call.A few minutes later, she signed off the Zoom, closed out her email and shut everything down.“All done!” she said as she joined him in the living room, plopping down onto him and bringing her lips to his. “Sorry, that took longer than I expected, but I told work not to bother me from here on out.”“No worries,” he replied with a smile and a kiss. “You all packed?”“Yep, all set. My bags are by the door. Just have to pee and then I think I'm ready to go.”Kyle packed her bags in his truck while she went to pee and shut off the lights and then she joined him in the truck.“Ready?” he asked as she climbed into his truck.“Ready!” she said enthusiastically and then leaned in for a little kiss.“Which way do I go?”“Super easy, just head over to Rt 104 and then we just stay on that for like an hour and a half and then we're practically there.”They headed off, stopping for lunch along the way. When they pulled into town, they popped into the grocery store to grab some food so they could cook some meals in the cabin. As they got back into this truck, Elizabeth got a text. “Oh! Cabin's ready early so we can check in now,” she read out loud.They arrived at the cabin a little after two in the afternoon and were impressed right off the bat. Kimberley: Welcome, everybody. This is a very exciting episode. I know I'm going to learn so much. Today, we have Caitlin Pinciotti and Shala Nicely, and we're talking about when OCD and PTSD collide and intertwine and how that plays out. This is actually a topic I think we need to talk about more. Welcome, Caitlin, and welcome, Shala. Caitlin: Thank you. Shala: Thanks. Kimberley: Okay. Let's first do a little introduction. Caitlin, would you like to go first introducing yourself? Caitlin: Sure thing. I'm Caitlin Pinciotti. I'm a licensed clinical psychologist and an assistant professor in the Department of Psychiatry and Behavioral Sciences at Baylor College of Medicine. I also serve as a co-chair for the IOCDF Trauma and PTSD and OCD SIG. If people are interested in that special interest group as well, that's something that's available and up and running now. Most of my research specifically focuses on OCD, trauma, and PTSD, and particularly the overlap of these things. That's been sort of my focus for the last several years. I'm excited to be here and talk more about this topic. Kimberley: Thank you. You're doing amazing work. I've loved being a part of just watching all of this great research that you're doing. Shala, would you like to introduce yourself? Shala: Yes. I'm Shala Nicely. I am a licensed professional counselor, and I specialize in the treatment of OCD and related disorders. I am the author of Is Fred in the Refrigerator?: Taming OCD and Reclaiming My Life, which is my story, and then co-author with Jon Hershfield of Everyday Mindfulness for OCD: Tips, Tricks, and Skills for Living Joyfully. I also produce the Shoulders Back! newsletter. It has tips and resources for taming OCD. Kimberley: Shoulders Back! was actually the inspiration for this episode. Shala, you recently wrote an article about post-traumatic OCD or how PTSD and OCD collide. Can you tell us about your story, particularly going back to, I think you mentioned, May 2020, and what brought you to write that article? Shala: Sure, and thank you very much for having Caitlin and me on today because I really appreciate the opportunity to talk about this and to get more information out in the world about this intertwined combination of PTSD and OCD. In May of 2020, I moved to a new house, the house that I'm in now. Of course, we had just started the pandemic, and so everybody was working at home, including me. The house that I moved into was in a brand new neighborhood. While the houses on this side of me were completed, the houses behind me and on that side were not completed. I didn't think anything of that when I moved in. But what I moved into was a situation where I was in a construction zone all the time. I was working at home, so there was no escape from it. One day I was walking behind my house, where most of the houses were in the process of being built and there were no sidewalks. As I was walking down the street, I saw, down at the end of the street, a big forklift come down the street where I was walking with my two little dogs backwards at a really high rate of speed, and the forklift driver seemed to be looking that way, and he was going that way. It happened so fast because he was going so quickly that all of a sudden I realized he was going to hit us, my dogs and me, and there was no place for us to go because we were on the road because there was nowhere else for us to be. I screamed bloody murder, and he heard me. I mean, that's how loud I screamed, and he stopped. That was not all that pleasant. I was upset. He was not happy. But we moved on. But my brain didn't move on.  After that incident, what I noticed was I was becoming really hypervigilant in my own house and finding the construction equipment. If I go outside, I tense up just knowing that construction equipment is there. Over time, my sleep started becoming disturbed. I started to have flashbacks and what I call flash-forwards, where I would think about all these horrible things that could happen to me that hadn't happened to me yet but could. I'd get lost in these violent fantasies of what might happen and what I need to do to prevent that.  I realized that I seemed to be developing symptoms of PTSD. This is where being a therapist was actually quite helpful because I pulled the DSM open one night and I started going through symptoms of PTSD. I'm like, “Oh my gosh, I think I have PTSD.”  I think what happened, because having a forklift driver almost hit you, doesn't seem like that could possibly cause PTSD. But if you look at my history, I think that created a link in my brain to an accident I was in when I was four where I did almost die, which is when my mom and I were standing on the side of a road, about to cross. We were going to go between two parked cars. My mom and I stepped between two parked cars, and there was a man driving down the road who was legally blind, and he mistook the line of parked cars where we were standing as moving traffic. He plowed into the end of all the parked cars, which of course made them accordion in, and my mom and I were in the middle of that. I was very seriously injured and probably almost died. My mom was, too. Several months in the hospital, all of that.  Of course, at that point—that was 1975—there was no PTSD, because I think— Caitlin, you can correct me—it didn't become a diagnosis until 1980. I have had symptoms—small, low-level symptoms of PTSD probably on and off most of my life, but so low-level, not diagnosable, and not really causing any sort of problems. But I think what happened in my head was that when that forklift almost hit me, it made my brain think, “Oh my gosh, we're in that situation again,” because the forklift was huge. It was the same scale to me as an adult as that car that I was crushed between was when I was four. I think my brain just got confused. Because I was stuck with this construction equipment all day long and I didn't get any break from it, it just made my brain think more and more and more, “Boy, we are really in danger.” Our lives are basically threatened all the time.  That began my journey of figuring out what was going on with me and then also trying to understand why my OCD seemed to be getting worse and jumping in to help because I seemed to get all these compulsions that were designed to keep me safe from this construction equipment. It created a process where I was trying to figure out, "What is this? I've got both PTSD now, I've got OCD flaring up, how do I deal with this? What do I do?" The reason why I wanted to write the article for Shoulders Back! and why I asked Caitlin to write it with me was because there just isn't a lot of information out there about this combination where people have PTSD or some sort of trauma, and then the OCD jumps in to help. Now you've got a combination of disorders where you've got trauma or PTSD and OCD, and they're merging together to try to protect you. That's what they think they're doing. They're trying to help you stay safe, but really, what they're doing is they're making your life smaller and smaller and smaller.  I wanted to write this article for Shoulders Back! to let people know about my experience so that other people going through this aren't alone. I wanted to ask Caitlin to write it with me because I wanted an expert in this to talk about what it is, how we treat it, what hope do we have for people who are experiencing this going forward. THE DIFFERENCE BETWEEN OCD AND PTSD (AND POST-TRAUMATIC OCD)  Kimberley: Thank you for sharing that. I do encourage people; I'll link in the show notes if they want to go and read the article as well. Caitlin, from a clinical perspective, what was going on for Shala? Can you break down the differences between OCD and PTSD and what's happening to her? Caitlin: Sure. First, I want to start by thanking Shala again for sharing that story. I know you and I talked about this one-on-one, but I think really sharing personal stories like that obviously involves a lot of courage and vulnerability. It's just so helpful for people to hear examples and to really resonate with, “Wow, maybe I'm not so different or so alone. I thought I was the only one who had experiences like this.” I just want to publicly thank you again for writing that blog and being willing to share these really horrible experiences that you had.  In terms of how we would look at this clinically, it's not uncommon for people to, like Shala described, experience trauma and have these low-level symptoms for a while that don't really emerge or don't really reach the threshold of being diagnosable. This can happen, for example, with veterans who return home from war, and it might not be until decades later that they have some sort of significant life event or change. Maybe they've retired, or they're experiencing more stress, or maybe, like Shala, they're experiencing another trauma, and it just brings everything up. This kind of delayed onset of PTSD is, for sure, not abnormal.  In this case, it sounds like, just like Shala described, that her OCD really latched onto the trauma, that she had these experiences that reinforced each other. Right now, I've had two experiences where being around moving vehicles has been really dangerous for me. Just like you said, I think you did such a beautiful job of saying that the OCD and PTSD colluded in a way to keep you “safe.” That's the function of it. But of course, we know that those things go to the extreme and can make our lives very small and very distressing.  What Shala described about using these compulsions to try to prevent future trauma is something that we see a lot in people who have comorbid OCD and PTSD. We're doing some research now on the different ways that OCD and trauma can intersect. And that's something that keeps coming up as people say, “I engaged in these compulsions as a way to try to prevent the trauma from happening to me again or happening to someone else. Or maybe my compulsions gave me a sense of control, predictability, or certainty about something related to the trauma.” This kind of presentation of OCD sort of functioning as protection against trauma or coping with past trauma as well is really common. STATISTICS OF OCD AND PTSD Kimberley: Would you share a little bit about the statistics between OCD and PTSD and the overlap? Caitlin: Absolutely. I'm excited to share this too, because so much of this work is so recent, and I'm hopeful that it's really going to transform the way that we see the relationships between OCD and PTSD. We know that around 60% of people who have comorbid OCD and PTSD tend to have an experience where PTSD comes first or at the same time, and the OCD comes later. This is sort of that post-traumatic OCD presentation that we're talking about and that Shala talked about in her article.  For folks who have this presentation where the PTSD comes first and then the OCD comes along afterwards, unfortunately, we see that those folks tend to have more severe obsessions, more severe compulsions. They're more likely to struggle with suicidality or to have comorbid agoraphobia or panic disorders. Generally speaking, we see a more severe presentation when the OCD comes after the PTSD and trauma, which is likely indicative of what we're discussing, which is that when the OCD develops as a way to cope with trauma, it takes on a mind of its own and can be really severe because it's serving multiple functions in that way. What we've been finding in our recent research—and if folks want to participate, the study will still be active for the next month; we're going to end it at the end of the year, the OCD and Trauma Overlap Study—what we're finding is that of the folks who've participated in the study, 85% of them feel like there's some sort of overlap between their OCD and trauma. Of course, there are lots of different ways that OCD and trauma can overlap.  I published a paper previously where we found that about 45% of people with severe OCD in a residential program felt that a traumatic or stressful event was the direct cause of their OCD on setting. But beyond that, we know that OCD and trauma can intersect in terms of the content of obsessions, the function of compulsions, as we've been talking about here, core fears. Some folks describe this, and Shala described this to this, like cyclical relationship where when one thing gets triggered, the other thing gets triggered too.  This is really where a lot of the research is focusing on now, is how do these things intersect, how often do they intersect, and what does that really look like for people? Kimberley: Thanks. I found in my practice, for people who have had a traumatic event, as exactly what happened to Shala, and I actually would love for both of you maybe to give some other examples of how this looks for people and how it may be experienced, is let's say the person that was involved in the traumatic event or that place that the traumatic event was recent that recently was revisited just like Shala. Some of them go to doing safety behaviors around that person, place, or event, or they might just notice an uptick in their compulsions that may have completely nothing to do with that. Shala, can you explain a little bit about how you differentiated between what are PTSD symptoms versus OCD, or do you consider them very, very similar? Can you give some insight into that? SYMPTOMS OF OCD & PTSD  Shala: Sure. I'll give some examples of the symptoms of OCD that developed after this PTSD developed, but it's all post-traumatic OCDs. I consider it to be different from PTSD, but it is merged with PTSD because it's only there because the PTSD is there.  For instance, I developed a lot of checking behaviors around the doors to my house—staring, touching, not able to just look once before I go to bed, had to be positively sure the doors were locked, which, as somebody who does this for a living, who helps people stop doing these compulsions, created a decent amount of shame for me too, as I'm doing these compulsions and saying, “Why am I not taking my own advice here? Why am I getting stuck doing this?”  But my OCD thought that the construction equipment was outside; we're inside. We need to make sure it stays outside. The only way we do that is to make sure the door stays locked, which is ridiculous. It's not as if a forklift is going to drive through my front door. As typical with OCD, the compulsions don't make a lot of sense, but there's a loose link there. Another compulsion that I realized after a time was probably linked with PTSD is my people-pleasing, which I've always struggled with. In fact, Kimberley, you and I have done another podcast about people-pleasing, something I've worked really hard on over the years, but it really accelerated after this. I eventually figured out that that was a compulsion to keep people liking me so that they wouldn't attack me. That can be an OCD compulsion all by itself, but it was functioning to help the PTSD.  Those would be two examples of compulsions that could be OCD compulsions on their own, but they would not have been there had the PTSD not been there. Kimberley: Caitlin, do you want to add anything about that from symptoms or how it might look and be experienced? Caitlin: Sure, yeah. I think it's spot on that there's this element of separation that we can piece apart. This feels a little bit more like OCD; this feels a little bit more like PTSD, but ultimately they're the same thing, or it's the same behavior.  In my work, I usually try to, where I can, piece things apart clinically so that we can figure out what we should do with this particular response that you're having. When it comes to differentiating compulsions, OCD compulsions and PTSD safety behaviors, we can look towards both the presentation of the behavior as well as the function of it.  In terms of presentation, I mean, we all know what compulsions can look like. They can be very rigid. There can be a set of rules that they have to be completed with. They're often characterized by a lot of doubting, like in Shala's case, the checking that, “Well, okay, I checked, but I'm not actually sure, so let me check one more time.” Whereas in PTSD, although it's possible for that to happen, those safety behaviors, usually, it's a little bit easier to disengage from. Once I feel like I've established a sense of safety, then I feel like I can disengage from that. There doesn't tend to be kind of that like rigidity and a set of rules or magical thinking that comes along with an OCD compulsion.  In terms of the function, and this is where it gets a little bit murky with post-traumatic OCD, broadly speaking, the function of PTSD safety behaviors is to try to prevent trauma from occurring again in the future. Whereas OCD compulsions, generally speaking, are a way to obtain certainty about something or prevent some sort of feared catastrophe related to someone's obsession. But of course, when the OCD is functioning along with the PTSD to cope with trauma, to prevent future trauma, that gets a little bit murkier.  In my work, like I said, I try to piece apart, are there elements of this that we can try to resist from more of an ERP OCD standpoint? If there's a set of rules or a specific way that you're checking the door, maybe we can work on reducing some of that while still having that PTSD perspective of being a little bit more lenient about weaning off safety behaviors over time. TREATMENT FOR OCD AND PTSD Kimberley: It's a perfect segue into us talking about the treatment here. Caitlin, could you maybe share the treatment options for these conditions, specifically post-traumatic OCD, but maybe in general, all three? Caitlin: Absolutely. The APA, a few years back, reviewed all the available literature on PTSD treatments, and they created this hierarchy of the treatments that have the most evidence base and went down from there. From their review of all the research that's been done, there were four treatments that emerged as being the most effective for PTSD. That would be broadly cognitive behavioral therapy and cognitive therapy. But then there are two treatments that have been specifically created to target PTSD, and that would be prolonged exposure or PE, and cognitive processing therapy or CBT. These all fall under the umbrella of CBT treatments, but they're just a little bit more specific in their approach. And then, of course, we know of ACT and EMDR and these other treatments that folks use as well. Those fall in the second tier, where there's a lot of evidence that those work for folks as well, but that top tier has the most evidence.  These treatments can be used in combination with OCD treatments like ERP. There are different ways that folks can combine them. They can do full protocols of both. They could borrow aspects of some treatments, or they could choose to focus really on if there's a very clear primary diagnosis to treat that one first before moving on to the secondary diagnosis. TREATMENT EXAMPLES FOR POST-TRAUMATIC OCD Kimberley: Amazing. Shala, if you're comfortable, can you give some examples of what treatment looked like for you and what that was like for you both having OCD and PTOCD?  Shala: Yes, and I think to set the ground for why the combined treatment working on the PTSD and the OCD together can be so important, a couple of features of how all this was presenting for me was the shift in the focus of the uncertainty. With OCD, it's all about an intolerance of uncertainty and not knowing whether these what-ifs that OCD is getting stuck on are true or going to happen. But what I noticed when I developed PTSD and then the OCD came in to help was that the focus of the uncertainty shifted to it's not what if it's going to happen. The only what-if is when it was going to happen because something bad happening became a given.  The uncertainty shifted to only when and where that bad thing was going to happen, which meant that I had lower insight. I've always had pretty good insight into my OCD, even before I got treatment. Many people with OCD too, we know what we're doing doesn't make any sense; we just can't stop doing it. With this combined presentation, there was a part of me that was saying, “Yeah, I really do need to be staring at the door. This is really important to make sure I keep that construction equipment out.” That lowered insight is a feature of this combined presentation that I think makes the type of treatment that we do more important, because we want to address both of the drivers, both the PTSD and the OCD. The treatment that I did was in a staged process. First, I had to find a treatment provider, and Caitlin has a wonderful list of evidence-based treatment providers who can provide treatment for both on her website, which is great. I found somebody actually who ended up being on Caitlin's list and worked with that person, and she wanted to start out doing prolonged exposure, which I pushed back on a little bit. Sometimes when you're a therapist and you're being the client, it's hard not to get in the other person's chair. But I pushed back on that because I said, “Well, I don't think I need to do prolonged exposure on the original accident,” because that's what she was suggesting we do, the accident when I was four. I said, “Because I wrote a book, Is Fred in the Refrigerator? and the very first chapter is the accident,” and I talked all about the accident. She explained, “That's a little bit different than the way we would do it in prolonged exposure.” What's telling, I think, is that when I worked on the audiobook version of Fred—I was doing the narration, I was in a studio, and I had an engineer and a director; they were on one side of the glass, I'm on the other side of the glass—I had a really hard time getting through that first chapter of the book because I kept breaking down. They'd have to stop everything, and I had to get myself together, and we had to start again, and that happened over and over and over again.  Even though I had relived, so to speak, this story on paper, I guess that was the problem. I was still reliving it. That's probably the right word. Prolonged exposure is what I needed to do because I needed to be able to be in the presence of that story and have it be a story in the past and not something that I was experiencing right then.  I started with prolonged exposure. After I did that, I moved on to cognitive processing therapy because I had a lot of distorted beliefs around life and the trauma that we call “stuck points” in cognitive processing therapy that I needed to work through. There were a good 20 or so stuck-point beliefs. “If I don't treat people perfectly nicely, they're going to attack me somehow.” Things that could be related directly to the compulsions, but also just things like, “The world is dangerous. If I'm not vigilant all the time, something bad is going to happen to me.” I had to work on reframing all of those because I was living my life based on those beliefs, which was keeping the trauma going.  I recreated a new set of beliefs and then brought exposure in to work on doing exposures that helped me act as if those new beliefs were the right way to live. If my stuck point is I need to be hypervigilant because of the way something bad is going to happen to me, and I'm walking around like this, which was not an exaggeration of really how I was living my life when this was all happening—if I'm living like that, if I'm acting in a hypervigilant way, I am reinforcing these beliefs. I need to go do exposures where I can walk by a dump truck without all the hypervigilance to let all that tension go, walk by it, realize what I've learned, and walk by it again.  It was a combination of all these and making sure that I was doing these exposures, both to stop the compulsions I was doing, like the door checking, but also to start living in a different way so that I wasn't in my approach to life, reinforcing the fact that my PTSD thought the world was dangerous.  I also incorporated some DBT (dialectical behavior therapy) because what I found with this combination was I was experiencing a lot more intense emotions than I'd really ever experienced in having OCD by itself. With OCD, it was mostly just out-of-this-world anxiety, but with the combination of PTSD and OCD, there were a lot more emotional swings of all sorts of different kinds that I needed to learn and had to deal with. Part of that too was just learning how to be in the presence of these PTSD symptoms, which are very physiological. Not like OCD symptoms aren't, but they tend to be somewhat more extreme, almost panicky-like feelings. When you're in the flashbacks or flash forwards, you can feel dissociated, and you're numbing out and all of that. I'm learning to be in the presence of those symptoms without reacting negatively to them, because if I'm having some sort of feelings of hypervigilance that are coming because I'm near a piece of construction equipment and I haven't practiced my ERP (Expsoure & Response Prevention) for a while, if I react negatively and say, “Oh my gosh, I shouldn't be having these symptoms. I've done my therapy. I shouldn't be having these feelings right now,” it's just going to make it worse. Really, a lot of this work on the emotional side was learning how to just be with the feelings. If I have symptoms, because they happen every now and then—if I have symptoms, then I'm accepting them. I'm not making them worse by a negative reaction to the reaction my PTSD is having.  That was a lot of the tail end of the work, was learning how to be okay with the fact that sometimes you're going to have some PTSD symptoms, and that's okay. But overreacting to them is going to make it worse.  Kimberley: Thank you so much for sharing that. I just want to maybe clarify for those who are listening. You talked about CPT, you talked about DBT, and you also talked about prolonged exposure. In the prolonged exposure, you were exposing yourself to the dump truck? Is that correct? Shala: In the prolonged exposure, I was doing two different things. One is the story of the accident that I was in. Going back to that accident that I thought I had fully habituated to through writing my book and doing all that, I had to learn how to be in the presence of that story without reliving it while seeing it as something that happened to me, but it's not happening to me right now. That was the imaginal part of the prolonged exposure.  This is where the overlap between the disorders and the treatment can get confusing of what is part of what. You can do the in vivo exposure part of prolonged exposure. Those can also look a lot like just ERP for OCD, where we're going and we're standing beside a dump truck and dropping the hypervigilant safety behaviors because we need to be able to do that to prove to our brain we can tolerate being in this environment. It isn't a dangerous environment to stand by a jump truck. It's not what happened when I was four. Those are the two parts that we're looking at there—the imaginal exposure, which is the story, and then we've got the in vivo exposures, which are going back and being in the presence of triggers, and also from an OCD perspective without compulsive safety behaviors. Kimberley: Amazing. What I would clarify, but please any of you jump in just for the listeners, if this is all new to you, what we're not saying is, let's say if there was someone who was abusive to you as a child, that you would then expose yourself to them for the sake of getting better from your PTSD. I think the decisions you made on what to expose yourself were done with a therapist, Shala? They helped you make those decisions based on what was helpful and effective for you? Do either of you want to speak to what we do and what we don't expose ourselves to in prolonged exposure? Caitlin: Yeah. I'm glad that you're clarifying that too, because this is a big part of PE that is actually a little bit different from ERP. When somebody has experienced trauma, when they have PTSD, their internal alarm system just goes haywire. Just like in Shala's example, anything that serves as a reminder or a trigger of the trauma, the brain just automatically interprets as this thing is dangerous; I have to get away from it.  In PE, a lot of what we're doing is helping people to recalibrate that internal alarm system so that they can better learn or relearn safe versus actual threat. When you're developing a hierarchy with someone in PE, you might have very explicit conversations about how safe is this exposure really, because we never want to put someone in a situation where they would be unsafe, such as, like you described, interacting with an abuser.  In ERP, we'd probably be less likely to go through the exposures and say, “This one's actually safe; I want you to do it,” because so much of the treatment is about tolerating uncertainty about feared outcomes. But in PE, we might have these explicit conversations. “Do other people you know do this activity or go to this place in town?” There are probably construction sites that wouldn't be safe for Shala to go to. They'd be objectively dangerous, and we'd never have her go and do things that would put her in harm's way. Kimberley: Thank you. I just wanted to clarify on that, particularly for folks who are hearing this for the first time. I'm so grateful that we're having this conversation again. I think it's going to be so eye-opening for people. Caitlin, can you share any final words for the listeners? What resources would you encourage them to listen to? Is there anything that you feel we missed in our conversation today for the listeners? Caitlin: I think, generally, I like to always leave on a note of hope. Again, I'm so grateful that Shala is here and gets to describe her experience with such vulnerability because it gives hope that you can hear about someone who was at their worst, and maybe things felt hopeless in that moment. But she was able to access the help that she needed and use the tools that she had from her own training too, which helped, and really move through this.  There isn't sort of a final point where it's like, “Okay, cool, I'm done. The trauma is never going to bother me again.” But it doesn't have to have that grip on your life any longer, and you don't need to rely on OCD to keep you safe from trauma.  There are treatments out there that work. Shala, can you share any final words about your experience or what you want the listeners to hear? Shala: One thing I'd like to share is a mistake that I made as part of my recovery that I would love for other people not to make. I'd like to talk a little bit about that, because I think it could be helpful. The mistake that I made in trying to be a good client, a good therapy client, is I was micro-monitoring my recovery. “How many PTSD symptoms am I having? Well, I'm still having symptoms.” I woke up in the middle of the night in a panic, or I had a bad dream, or I had a flash forward. “Why am I having this? I must not be doing things right.” And then I took it a step further and said, “It would be great if I could track the physiological markers of my PTSD so I can make sure I'm keeping them under control.” I got a piece of tracking technology that enabled me to track heart rate and heart rate variability and sleep and all this stuff. At first, it was okay, but then the technology that I was using changed their algorithm, and all of a sudden my stats weren't good anymore, and I started freaking out. “Oh my gosh, my sleep is bad. My atrophy is going down. This is bad. What am I doing?” I was trying with the best of intentions to quantify, make sure I'm doing things right, focus on recovery. But what I was doing was focusing on the remaining symptoms that were there, and I was making them worse.  What I have learned is that eventually, things got so bad—in fact, with my sleep—that I got so frustrated with the tracking technology. I said, “I'm not wearing it anymore.” That's one of the things that helped me realize what I was doing. When I stopped tracking my sleep, when I let go of all of this and said, “You know what? I'm going to have symptoms,” things got better.  I would encourage people not to overthink their recovery, not to be in their heads and wake up in the morning and ask, “How much PTSD am I having? How much OCD am I having? If I could just get rid of these last little symptoms, life would be great,” because that's just going to keep everything going.  I'll say this year, two has been a challenging one for me. I've been involved in three car accidents this year; none of them my fault. One of my neighbors, whom I don't know, called the police on me, thinking I was breaking into my own house, which meant that a whole army of police officers ended up at my house at nine o'clock at night. That's four pretty hard trauma triggers for me in 2023.  Those kinds of things are going to happen to all of us every now and then. I had a lot of symptoms. I had a lot of PTSD symptoms and a lot of OCD symptoms in the wake of those events, and that's okay. It's not that I want them to be there, but that's just my brain reacting. That's my brain trying to come to terms with what happened and how safe we are and trying to get back to a level playing field.  I think it's really important for anybody else out there who's suffering from one or the other, or both of these disorders to recognize we're going to have symptoms sometimes. Just like with OCD, you're going to have symptoms sometimes. It's okay. It's the pushing away. It's the rejecting of the symptoms. It's the shaming yourself for having the symptoms that causes the symptoms to get worse.  Really, there is an element of self-compassion for OCD here. I like having bracelets to remind me. This is the self-compassion bracelet that I've had for years that I wear. By the way, this is not the tracking technology. I'm not using tracking technology anymore. But remembering self-compassion and telling yourself, “I'm having symptoms right now, and this is really hard. I'm anxious; I feel a little bit hypervigilant, but this is part of recovery from PTOCD. Most people with PTOCD experience this at some point. So I'm going to give myself a break, give myself permission to feel what I'm feeling, recognize how much progress I've made, and, when I feel ready, do some of my therapy homework to help me move past this, but in a nonhypervigilant, nonmicro monitoring way.” As I have dropped down into acceptance of these symptoms, my symptoms have gotten a lot better. I think that's a really important takeaway. Yes, we want to work hard in our therapy, yes, we want to do the homework, but we also want to work on accepting because, in the acceptance, we learn that having these symptoms sometimes is just a part of life, and it's okay.  I would echo what Caitlin said in that you can have a ton of hope if you have these disorders, in that we have good treatment. Sometimes it takes a little bit longer than working on either one or the other, but that makes sense because you're working on two. But we have good treatment, and you can get back to living a joyful life.  Always have hope and don't give up, because sometimes it can be a long road, especially when you have a combined presentation. But you can tame both of these disorders and reclaim your life. Kimberle: You guys are so good. I'm so grateful we got to do this. I feel like it's such an important conversation, and both of you bring such wonderful expertise and lived experience. I'm so grateful.
