This podcast features an interview with Laurie Sardo and Julia Bayadinova, experts on Cancer-associated Thrombosis, from Hamilton Ontario, authors of a series of publications in the Canadian Oncology Nursing Journal on Cancer-associated Thrombosis, called Spot the Clot.Laurie Sardo is a Nurse Practitioner in the Thrombosis Service, Department of Hematology at St. Joseph's Healthcare in Hamilton. She graduated with her NP from McMaster University in 2014, and her Doctorate in Nursing Practice from D'Youville University in 2022. Laurie's thesis work focused on patient VTE education in the high-risk cancer population.Her professional interests include patient and staff education and remains active on a number of educational interest groups including the Thrombosis Canada Patient & Family Education committee, the ISTH Nursing committee and the US-based Anticoagulation Forum.ANDJulia Bayadinova, is a nurse practitioner specializing in Thromboembolism, serving both inpatients and outpatients at St. Joseph's Healthcare Hamilton. In 2014, Julia graduated from Ryerson University with a combined Master of Nursing and Primary Care Nurse Practitioner Program, and has since found her passion in Thromboembolism. Her research interest lies in patient education.     Vidcast:  https://youtu.be/zk7Z0hbeK7I   Strokes don't occur often in young adults, but having Type A blood increases your risk of having a debilitating early onset stroke 16% while having type O blood reduces that risk by 12%.  Epidemiologists at the University of Maryland's School of Medicine draw these conclusions from their meta-analysis of 16,927 stroke victims who participated in 48 different multi-national studies.  The control group consisted of nearly 600,000 persons without a stroke history.   These blood group associations were stronger in younger patients. In this episode have a conversation with Dr Matthew Nicholson and Dr Noel Chan on their recent publication from Blood Review entitled No apparent association between mRNA COVID-19 vaccination and venous thromboembolism (https://www.sciencedirect.com/science/article/pii/S0268960X22000443) which was co-authored by Dr Hadi Goubran and Dr Deborah Siegal.Dr. Matthew Nicholson is a Clinical Hematologist at the Saskatoon Cancer Centre and an Assistant Professor in the College of Medicine at the University of Saskatchewan. He obtained his MD and Internal Medicine residency at the University of Saskatchewan in Saskatoon. Dr. Nicholson completed his Hematology fellowship training at the University of British Columbia in Vancouver and completed a clinical fellowship in thrombosis medicine through McMaster University, and is board certified in Internal Medicine and Hematology. Dr. Nicholson has acted as an advocate for improved training conditions for medical students and residents during his time with provincial health organizations and the resident doctors of Canada board.  Alok Khorana, MD, of the Cleveland Clinic joins Blood & Cancer host David H. Henry, MD, of Pennsylvania Hospital, Philadelphia, to break down the latest recommendations from the American Society of Clinical Oncology on venous thromboembolism (VTE) prophylaxis in cancer patients. Plus, in Clinical Correlation, Ilana Yurkiewicz, MD, of Stanford (Calif.) University, shares her answer to a frequent question from cancer patients: What should I eat? This Week in Oncology What is the role of thromboprophylaxis in patients with cancer in the outpatient setting?  Key change in ASCO recommendations: Thromboprophylaxis with apixaban, rivaroxaban, or low-molecular-weight heparin (LMWH) may be offered to select high-risk outpatients with cancer. Prophylactic anticoagulation should not be given to every patient with malignancy. Khorana score predicts the venous thromboembolism in patients with malignancy. Influenced by type of malignancy, hemoglobin, platelet count, leukocyte count, and BMI. High risk = Khorana score of 2 or higher may be offered prophylaxis. Patients with pancreatic cancer and gastric cancer are particularly coagulopathic. Does the presence of a CNS lesion(s) preclude anticoagulation for a DVT/PE? All CNS lesions have a risk of hemorrhage. A CNS lesion hemorrhage is not significantly greater when anticoagulated Among high-risk cancer patients who undergo surgery, is there a role for postoperative prophylaxis with LMWH? Venous thromboembolism (VTE) is one of the most preventable complications in hospitalised patients. Critically ill patients are at risk of VTE due to coexisting of multiple risk factors but, at the same time, often at risk of bleeding. Though not common, fatal pulmonary embolism (PE) continues to occur [1] – due to the alignment of failures (or ‘holes’) in each defensive layer according to the Swiss cheese model [2]. Tackling this is not easy because the pattern of the ‘holes’ in each layer of the cheese is different between patients and, to complicate the matter further, both the size and location of the ‘holes’ also change with time in each individual patient.  In brief, fatal PE occurs due to one of the three failures – failure to prevent, failure to diagnose and failure to treat (aggressively). It is well established that anticoagulants are very effective in reducing VTE. The golden rule to reduce the size of the ‘holes’ in prevention is to use a multimodal approach, with anticoagulants as a key player. The bottom line is that any anticoagulants, even at a reduced dose, is better than no anticoagulant. Judging bleeding risk to determine when anticoagulant prophylaxis can be safely initiated solely based on INR or aPTT is a last century practice. As for diagnosing PE in the critically ill, computed tomography pulmonary angiography (CTPA) is the practical gold standard. While contrast-induced-nephropathy (CIN) is real and critically ill patients are certainly at risk, the benefits of a CTPA will almost always outweigh the risk of CIN when intensivists suspect their patients may have PE (or when the pre-test probability is >10-15%)[3,4]. Immediate aggressive systemic anticoagulation is pivotal in confirmed PE. It is better to aim at a higher aPTT (80-100s) target than a lower one (e.g. 60-80s) as soon as possible to avoid clot propagation which may lead to requiring even higher risk therapies, such as thrombolysis, extracorporeal membrane oxygenation (ECMO) or surgical embolectomy. For those unfortunate few individuals who continue to deteriorate despite systemic anticoagulation, the options ‘to lyse, suck, use ECMO, or remove’ are endless; but in reality the choice is often limited by what expertise is most available at the time of crisis.    Finally, the controversial issue of using inferior vena cava filters as a primary VTE prophylaxis in patients with contraindications to anticoagulants will be discussed, including the results of our recently completed randomized controlled trial [5].    References:  [1] Ho KM, Burrell M, Rao S, Baker R.
