Welcome to Wednesday Q&A, where you ask questions and we answer them!In this Wednesday Q&A, we answer your questions about right leg pain, gentle ways to build core strength, and osteomyelitis.Your questions:1. I have pain deep in my right glute that shoots down the back of my leg. It started back last July after I finished my first half marathon. A few weeks after the race, I developed horrible low back pain that shut down the right leg. The doctors wouldn't give me a firm diagonal diagnosis without an imaging test, but he prescribed muscle relaxants, anti-inflammatories, and told me to do piriformis stretches. The meds and rest eventually put it into remission and I stopped doing all seated forward fold postures and never stretched my hamstrings without bending my knees, trying to be very mindful of neutral pelvis and to protect my SI joint and lumbar spine. But in February or March this year, it came back. This time I had no pain in the low back, just the glute in the back of the leg. It's worse first thing in the morning and sometimes it wakes me up at night. I've been working with some functional movement practitioners for about a month now, trying to relieve and correct the issue. But it's truly a roller coaster. Some days I take two steps forward, I think it's getting better, and then the next I take three steps back and can hardly walk without medication. Sitting aggravates it the most, especially sitting in a car. I have a desk job and I've been using a standing desk exclusively since March. When doing windshield wipers with my knees, I find that internal rotation is much more accessible and pain-free on the affected side. Any insight is greatly appreciated.2. Having connective tissue disorders, there isn't a whole lot I can do without making matters worse for myself by way of inflammation. So I stick to movement, “gentle stretches,” and things like qigong. This has always worked well since I'm naturally long and lean, but I'll be 50 in August and I'm in menopause and my middle is getting icky. Is there a gentle way to tighten this area without hurting myself? I can't plank because of what it does to my neck and shoulders. Physically, I can do almost anything. I'm not weak, I'm just very flexible. But my fascia is apparently a mess. 3. I have a friend recovering from osteomyelitis of the lumbar spine. While bloodwork and scans show the infection is going away, maybe two more weeks left of antibiotics, and he is moving better. He still has pain with bed mobility, rolling supine-to-sit and sit-to-stand, especially first thing in the morning. Any thoughts on exercises to help his back feel better, less stiff while he is recovering? Welcome to Wednesday Q&A, where you ask questions and we answer them!In this Wednesday Q&A, we answer your questions about knee pain, the risks of spinal rotation, holding tension in the body from previous injuries, and how to move around holding neutral pelvis and spine.Your questions:* My question is about my partner who recently developed pain in the front inside of his knee. He has just started training for a half marathon and after just a couple of runs he noticed the pain while he was running. He cycles, plays squash and football. And I think quite importantly as a photographer, which means he often finds himself in awkward positions trying to get the shot. For example, he often has to lower his body into a kind of low lunge with the bottom knee hovering and then stays there for a while while he shoots. He has an inkling that some of these awkward positions might have something to do with the knee pain. I think it's also important to note that in early 2021, he fell playing football and the fatty tissue of his knee was swollen for five months. I thought it would be a good idea to ask, number one where you think the supposed pain might be coming from. And number two, any ideas for him to continue training for the half marathon without encouraging more pain or at worst, creating further injury?* In all LYT classes we do spinal (I mean, all segments of the spine) rotation, whether it's in the reset, in supine, or in sequences. Over the last year, I have heard from a few students that they're osteopath or physio or even integrative doctors are telling them to practice these rotational movements sparingly as they lead to build up sometimes more tension in the body than the benefit we usually credit them, not in LYT by the way. Have you heard of any similar recommendations or client feedback? * In some classes, yoga, meditation, etc. there's an invitation to observe where we feel that our body might hold tension. One of my students, 56, had rotator cuff surgery a few years back, and I notice that I keep reminding her a couple of times to relax her right shoulder. She said that she feels like her shoulder got stuck in a sort of defensive position that translates in this tensed-up way of holding as it was before the surgery. And also since then, she was inwardly thinking, “Oh, I don't want to hurt my shoulder,” “I should be careful of my shoulder,” etc. I feel like there might be something to it as it seems really unconscious. But I was also wondering if it's also related to suboptimal posture, habits, and brain wiring. * Can you talk about neutral pelvis and Triple S while walking, running, etc.? She's not as tight as I assume she would be, but her glutes are super weak. She also has some pretty intense scars from her most recent hip surgery and redo of her hip replacement. Welcome to Wednesday Q&A, where you ask questions and we answer them!In this Wednesday Q&A, we answer your questions about glute activation in movements, whether Bikram yoga is advisable when you have long thoracic nerve palsy, and the meaning of a whole-food diet. Your questions:From Conor Harris on Instagram: When I hear that squeezing your glutes will help them grow or activate more... Hold up! One of the biggest myths in the fitness industry is that more sensation equals more and better activation of the muscles, so what you should feel after Taco Bell and not every hip-dominant exercise. So the problem with actively squeezing your glutes throughout an exercise is that you are going to keep them in a shortened position throughout the entire movement. Take a hip thrust, for example… the bottom position of the hip thrust is one of hip internal rotation where the back of the pelvis opens up and the glutes stretch. The push shortens the glutes as they close off the back of the pelvis and create hip extension and external rotation. This stretch to contract is a huge driver of mechanical tension and muscle damage needed for hypertrophy. Because the glutes wrap around the pelvis and attach on the femur, opening the outlet for the stretch is key. This applies to many different exercises such as squats, deadlifts, and even more basic glute bridges. So what he's saying is muscles should activate and contract because they are moving to create a necessary joint position. Keeping the muscle in a shortened position will not only throw off the mechanics of the joint, but limit your gains around it. So he's going to say that's why you shouldn't keep a band around your legs while you're going it like holding it there while doing bridges. You shouldn't be doing the sidestepping, keeping that tension because you're keeping the muscles in a shortened position. What are your last thoughts about this?Do you think Bikram is good to do when I have a long thoracic nerve palsy? I've had it many, many, many years ago and it seems like it just acted up again. Do you have an explanation of what it means to eat a whole-food diet? I think a lot of people don't understand what that means and why it makes a difference in your body. Welcome to Wednesday Q&A, where you ask questions and we answer them! In this Wednesday Q&A, we answer your questions about weight bearing in the hands, practicing LYT Yoga for decreased anxiety, and advice for persistent anterior knee pain. Your questions:Can we talk about the hand? I've heard of toe yoga and I use it in my PT practice and self-practice. Is there hand yoga? One thing my local LYT instructor says in class is to press the base of my index finger into the mat during any weight-bearing through the arm. I struggle with this. How can I improve and can you go into the physiology behind it?Can LYT help my anxiety? Sometimes I've felt increased anxiety in other flow classes.My question pertains to the stigma of knee pain. It seems as though the cottage industry of relief for knee pain has created an environment whereby if you experience ongoing knee pain, you must simply be doing something wrong. Despite my ongoing LYT practice and other attempts to remedy said knee pain, it still persists. Specifically, I experience this pain when walking downhill, downstairs, or doing things like weighted lunges or squat pulses from high crescent. In approximate terms, it generally radiates from below the patella on either side, which makes me think meniscus. I would characterize the discomfort as acute in that in more than a few cases, it may decrease when I'm more warmed up, but it never fully goes away. While I would not expect a diagnosis to be made from anecdotes in an email, it is unreasonable to think that some knee pain may not be entirely correctable through “better movement patterns,” strengthening the support structures. And if so, what metrics/means can be used to make that conclusive determination? In this Wednesday Q&A, we answer your questions about handstand drills to get away from the wall, myofascial release, our favorite things about being physical therapists, and how LYT Yoga has grown over the years. Your questions:My question today is about handstand drills. I have a client similar age to me, in her fifties, very strong and fit as a fiddle, solo mother of four, and full-time nurse. She keeps herself strong with my LYT yoga classes at the gym, and weight training, and has PT once a month. She is in the gym five days a week doing something, averages about two LYT classes a week with me. She would like to progress more with her handstand journey. So I have just started doing some more short privates like 30 minutes after class. I have some drills on the wall that we are working on as she is not confident enough to come away from the wall. She gets up just fine, but I noticed she has the small beginnings of a dowagers hump, so an outward curve of the thoracic spine, more pronounced when weight bearing in the hands than just standing. Would appreciate some exercises to help with this and enable her to feel more confident so we can get off the wall. As always, so appreciate your advice.What do you think about myofascial release? Der LYT Yoga Stil basiert auf Vinyasa Yoga, hat also dieselbe Lebendigkeit und Kreativität, beinhaltet aber eine Symbiose aus physiotherapeutischen und neurologischen Ansätzen, die anders aufgebaut sind als im Vinyasa Yoga. Ein großer Unterschied ist vor allem, dass es beim LYT vor der Praxis ein “Reset” gibt - einen Fokus auf die Körperhaltung und das Gleichgewicht, bevor es in das Komplexe, Lebendige geht. Dabei werden die richtigen Muskelgruppen aktiviert und aufgewärmt für die Sequenzen danach. LYT beinhaltet also sehr fundiertes Anatomiewissen. Man kann sich das vorstellen wie bei Menschen, die in der Reha erst wieder die Bewegungen neu erlernen, und Muskelgruppen aufbauen müssen. Jennifer hat über die letzten Jahre selbst sehr viele Verletzungen mitgenommen: Ein Sehnenriss im Knie, den Yoga Butt, Entzündungen in der Schulter, im Knie, am Handgelenk und ein blockiertes Kreuzbein, sowie Blockaden und Probleme mit der Halswirbelsäule, Brustwirbelsäule und dem Kiefer. Diese Probleme nahm sie oft, teils unbewusst, in die Yogapraxis mit, was diese Beschwerden schlimmer machte oder neue Verletzungen verursachte. Mit der LYT Yoga Ausbildung fühlte sich Jennifer “erleuchtet”: Sie durfte sich selbst kennenlernen, und ihre Verletzungen und Ursachen waren auf einmal verständlich. Sie begann dann auch ihre vorherige Praxis zu hinterfragen, denn schließlich unterrichtete sie nach gutem Gewissen das, was sie in ihrer Ausbildung gelernt hatte. Jetzt weiß Jennifer, daß Anatomie ein wichtiges Fundament in der Yogapraxis sein muss, was leider in vielen Ausbildungen vernachlässigt oder nur angedeutet wird. Grundlegende Mechaniken müssten runter gebrochen werden, damit zukünftige Yogalehrer:innen den Schüler:innen in der Zukunft im Unterricht besser assistieren können. The investigation of the internal workings of our body, A big pillar is eduction - we aren't taught about our bodies. 16:00 Typical vinyasa sequences are lacking intelligence because we are carrying natural imbalances. I do a reset at the start of class - neuro-development patterns,. 12:30 Most people don't use their glutes enough. Retrain neutral pelvis and work the glutes. Stabilise the skeleton with proximal muscles - those closest to your skeleton. 14:00 Working with rowers. We use large muscle groups repetitively. If the underlying stabilisers aren't as strong there will be an imbalance. 20:00 Rowers learn with mimicry. The instruction is so clear you can inhabit and embody it. 21:00 How to be better?
