Pelvic floor physical therapy (PFPT) is a crucial yet often overlooked component of gut health that can dramatically improve quality of life for people suffering from bowel issues, bladder problems, or pelvic pain. Dr. Alicia Jeffrey-Thomas joins us to demystify and normalize this specialized therapy and explain how proper pelvic floor function impacts everything from constipation to sexual health.• Understand that daily bowel movements don't necessarily mean you're not constipated• What happens during a PFPT session and how therapists create a safe + comfortable environment• The importance of proper pooping position• Why breathing techniques and sounds (like mooing!) can help relax your pelvic floor during bowel movements• Demystifying tools like pelvic wands and dilators for at-home maintenance • How dyssynergic defecation affects approximately 50% of people with constipation and how pelvic floor PT can help• Why all women can benefit from pelvic floor PT after childbirth (and even during pregnancy)•  Men face pelvic health issues too, like post-prostate surgery incontinence • The downside of "peeing just in case" and how it trains your bladder to signal fullness prematurelyIf you've experienced pelvic floor issues, don't accept them as normal - seek help from a pelvic floor physical therapist who can address these problems and improve your overall quality of life.References and Resources:Rao SS, Patcharatrakul T. Join us as we discuss effective non-diet focused methods for IBS relief. In this episode, Kate and Dr. Riehl review key factors on how gender can influence common symptoms of IBS.Hormones significantly influence IBS symptoms, with women being diagnosed 2.5 times more often than men, in part due to differences in gut function and pain perception. This episode features the following key discussion areas:• Women's fluctuating estrogen and progesterone levels affect gut motility and pain sensitivity throughout the menstrual cycle• Visceral hypersensitivity causes normal digestive sensations to be interpreted as pain - like a car alarm going off unnecessarily• The "microgenderome" refers to gender differences in gut microbiome that may contribute to women's higher rates of IBS and autoimmune conditions• Gut-directed hypnotherapy helps reprogram the subconscious mind to interpret gut sensations more accurately, with 70-80% improvement rates• Pelvic floor physical therapy can help address muscle tension that contributes to both constipation and diarrhea symptoms• Non-diet approaches including medications, supplements like enteric-coated peppermint, and lifestyle factors are effective IBS management tools• Regular exercise (especially in nature), adequate sleep, and stress management techniques complement other treatmentsFor more information on IBS-C treatments, check out our dedicated three-part series in episodes 14, 15, and 16.This podcast has been sponsored by Ardelyx and Nerva by Mindset Health.References:Mulak A, Taché Y, Larauche M. Is "leaky gut" real or just another wellness buzzword? In this episode of The Gut Health Podcast, we cut through the controversy with Dr. Alessio Fasano, a renowned gastroenterologist and microbiome expert from Mass General Brigham. Dr. Fasano explains what happens when the gut's protective barrier is compromised, resulting in increased intestinal permeability, and how this can affect not only gut health but also overall well-being. While some level of intestinal permeability is essential for health, in the presence of an altered gut microbiome, it can allow harmful molecules such as bacterial endotoxins and undigested food particles to pass through the intestinal lining. This can lead to inflammation, immune system activation, and may contribute to a variety of health issues, including autoimmune diseases, gastrointestinal disorders, heart disease, and more. We explore the molecular mechanisms that control intestinal permeability, examining how factors like epigenetic changes, diet, stress, and environmental factors can all impact the integrity of the gut barrier. Dr. Fasano breaks down the latest research on how intestinal permeability interacts with the immune system and other organ systems, highlighting the complex bidirectional relationship between gut health and overall wellness. Join us as we explore cutting-edge research on gut health, from breakthrough treatments to personalized diets and biomarkers for gut permeability. In this episode of the Legal Tech Startup Focus podcast, I welcome Damien Riehl, VP, Solutions Champion, at vLex and a leader at SALI (Standards Advancement for the Legal Industry). Damien provided an insightful overview of vLex, a legal data/legal data science company that boasts an impressive repository of over a billion legal documents from the United States and worldwide. He explained how vLex utilizes data science to tag and structure this vast amount of information, enabling users to run large language models (LLMs) across the data to answer complex legal questions. We also explored the significance of having well-structured data and how SALI contributes to this by providing a comprehensive taxonomy of legal concepts, dramatically enhancing legal data's usability. As we delved deeper, we discussed the retrieval-augmented generation (RAG) concept and how vLex employs a five-stage process to increase the likelihood that the legal answers provided by vLex's LLMs are grounded in reliable data. Damien elaborated on how this process not only retrieves relevant cases but also analyzes their validity and relevance, providing users with a robust legal analysis that would typically take hours of work for a lawyer. We also touched on the implications of generative AI for the legal profession, particularly how it could disrupt traditional billing models. Damien highlighted the advantages for plaintiff-side lawyers who can leverage VLex's technology to save time and increase profitability while addressing the potential challenges for defense-side lawyers relying on hourly billing. The conversation then shifted to the competitive landscape of legal tech, where we discussed the risks posed by large tech companies potentially absorbing legal tech innovations. Damien emphasized the importance of having unique data sets and value propositions to stand out in a crowded market, warning that companies that merely act as wrappers around existing LLMs without offering additional value may need help to survive. Towards the end of our discussion, we explored the future of AI in legal tech, particularly the integration of symbolic reasoning with neural networks. Damien shared his insights on how combining these two approaches could enhance AI's reasoning capabilities, making it more effective in the legal domain. We concluded with a discussion on the importance of free access to legal data and how it could foster innovation and competition in the legal tech space. This episode is packed with valuable insights for anyone interested in the intersection of law and technology, particularly those looking to understand how data science and AI are transforming the legal landscape.

Lancaster Farming Industrial Hemp Podcast

This week on the hemp podcast we saddle up and gallop out to Gordonville, Lancaster County, for Horse Progress Days, a two-day celebration of all things horsepower. Nearly 40,000 people were in attendance July 5-6, mostly Amish and Mennonite farmers and families. The weather was hot, the air was thick with humidity and the sounds of horses whinnying. What does Horse Progress have to with industrial hemp? Good question. Bear with me. In one of the vendor tents there was a cluster of businesses that specialize in working with hemp. Cameron Macintosh, hempcrete builder at Americhanvre, was there representing his company as well as the Pennsylvania Industrial Hemp Council. He said the horse power farming communities know about hemp, but because of the volatile CBD market, hemp does not have the best reputation among the horsepower set. “Many farmers in this community invested heavily in growing CBD back in 2018 and '19,” he said, “and then suffered through the subsequent crash of that market in 2020 and 2021.” So when you mention hemp in this community, that's what they think of. “That's why we're here,” he said, “to show the community that the fiber industrial side of the (hemp) plant has dramatically more promise, more opportunity for their community than CBD ever did.” On this episode, we will talk to several of the hemp folks in attendance at the event, including Heidi Custer from Tuscarora Mills in Bedford, Pennsylvania, and Kelly and Jarrett Burke from KifCure, a hemp company based in Northern Illinois that's developing regional infrastructure in the Great Plains. I also had the chance to chat briefly with Reuben Riehl from Lancaster County Marketing, who was involved in planning the event. Everyone I spoke to about hemp mentioned Reuben as the reason they were at Horse Progress in the first place. “It fits very well with our community,” Riehl said, mentioning the farming and building aspects, but he doesn't expect it to be an overnight change. “I think it it'll take some time for it to resonate here, but it will be okay,” Riehl said. Also on this episode, Lancaster Farming visits with Sam Connor at Free Flow Farm in Montgomery County, Pennsylvania, just north of Pottstown, where he's growing about 30 acres of industrial fiber hemp and raising grass-fed and finished beef and pastured chickens. “We have two different varieties that we planted here,” he said. “We have a Uma for variety, which is a Chinese variety, as well as Futura 83, which is a French variety.” Connor said he is growing for I-Hemp Katalyst, a company that is developing processing capacity in Pennsylvania. Connor is excited to be an early adopter of what he sees as an industry with vast potential and is proud that his crops will be used in innovative ways. “I believe some of it's going to be used for biodegradable hemp plastics, some of it's going to be used for hempcrete. And I think there's even a possibility of something to do with ceiling tiles and things like that. Vlex Pushes Legal Tech Boundaries with New Features and Top Talent

Our friend Damien Riehl stopped by to talk with Marlene Gebauer about two big happenings at vLex. Riehl unveiled exciting advancements in vLex's AI-powered legal research platform and shed light on vLex's commitment to streamlining legal workflows and reducing the need for extensive prompt engineering. One of the major developments is the enhanced document analysis feature. Users can now upload legal documents, such as complaints, and vLex's AI will automatically extract key information including claims, facts, parties involved, and potential legal defenses. This eliminates the tedious manual process of reviewing and analyzing documents, saving lawyers significant time and effort. Additionally, the platform suggests relevant legal research questions based on the document's content, further expediting the research process. vLex's advancements directly address the growing concerns surrounding prompt engineering in legal tech. By automating key analytical tasks, the platform empowers lawyers to focus on higher-level strategizing and client interactions, rather than spending hours crafting the perfect prompts for AI tools. Riehl echoes the sentiment of OpenAI's Sam Altman, believing that successful AI integration should render prompt engineering obsolete. He acknowledges that the option to fine-tune prompts remains, similar to Boolean search techniques, but emphasizes that vLex aims to make it a choice rather than a necessity. The potential impact on the legal industry is substantial. Irritable bowel syndrome (IBS) impacts more than 45 million Americans and about 1 in 10 globally. So, if you have abdominal pain, diarrhea and/or constipation on a regular basis...you are not alone. We are thrilled to have world-renowned gastroenterologist, Dr. William Chey, on this episode to discuss this diagnosis along with common IBS mimickers.  Dr. Chey is the Chief of Gastroenterology at the University of Michigan and a true physician champion for the importance of a multidisciplinary approach to the management of IBS. He discusses how this complex condition, once perceived purely as a psychosomatic disorder, has evolved into a multifaceted syndrome with new and improved treatment methods. Discover the transformative potential of dietary strategies like the low FODMAP diet, the integration of behavioral therapy along with complementary and alternative medicine (like acupuncture) into patient care, as we explore the nuances of treating IBS and its masqueraders. Dr. Chey sheds light on the enlightening findings from a recent study on disaccharidase deficiencies, particularly the surprising prevalence of sucrase-isomaltase deficiency in IBS patients with diarrhea predominance. Gain insight into the challenges of current testing methods and the role of bile acid malabsorption, which opens the door to a more precise and personalized approach to managing unruly gut symptoms. The conversation steers toward the promise and limitations of breath tests and the significance of a Dream Team approach, blending expertise from various specialists to address the unique needs of each patient. We also delve into the intricate relationship between small intestinal bacterial overgrowth (SIBO) and bile acid diarrhea, and the innovative work of researchers like Dr. Mark Pimentel. Dr. Chey emphasizes the growing importance of precision medicine in the field of gastroenterology and gut health, highlighting the complexities of diagnosing and treating conditions such as SIBO. While we share a wealth of knowledge that promises to enlighten both healthcare professionals and individuals navigating the intricacies of IBS and it's common mimickers, you won't want to miss the introduction to a very special guest! On a special “on location” episode of The Geek in Review, Greg Lambert sits down with vLex's Damien Riehl for a hands-on demonstration of the new generative AI tool called Vincent AI. While at the Ark KM Conference, Riehl explains that vLex has amassed a huge legal dataset over its 35 year history which allows them to now run their own large language models (LLM). The recent merger between vLex and Fastcase has combined their datasets to create an even more robust training corpus. Riehl demonstrates how Vincent AI works by having it research a question on trade secret law and employee theft of customer lists. It retrieves relevant cases, statutes, regulations, and secondary sources, highlighting the most relevant passages. It summarizes each source and provides a confidence rating on how well each excerpt answers the initial question. Vincent AI then generates a legal memorandum summarizing the relevant law. Riehl explains how this is more trustworthy than a general chatbot like ChatGPT because it is grounded in real legal sources. Riehl shows how Vincent AI can compare legal jurisdictions by generating memorandums on the same question for California, New York, the UK, and Spain. It can even handle foreign language sources, translating them into English. This allows for efficient multi-jurisdictional analysis. Riehl emphasizes Vincent AI's focus on asking straightforward questions in natural language rather than requiring complex prompts. Looking ahead, Riehl sees potential for Vincent AI to leverage external LLMs like Anthropic's Claude model as well as their massive dataset of briefs and motions to generate tailored legal arguments statistically likely to persuade specific judges on particular issues. He explains this requires highly accurate tagging of documents which they can achieve through symbolic AI. Riehl aims to continue expanding features without requiring lawyers to become AI prompt engineers. On access to justice, Riehl believes AI can help legal aid and pro bono attorneys handle more matters more efficiently. He also sees potential for AI assistance to pro se litigants to promote fairer outcomes. For judges, AI could help manage pro se cases and expedite decision-making. Overall, Riehl is optimistic about AI augmenting legal work over the next two years through ongoing improvements. Riehl discusses vLex's new Vincent AI system and its ability to efficiently research legal issues across jurisdictions and across languages. He provides insight into the technology's development and potential while emphasizing understandable user interaction. The conversation highlights AI's emerging role in legal services to increase productivity, insight, and access to justice.
