In the first episode of a special daily series during the 2025 ASCO Annual Meeting, Dr. John Sweetenham discusses the results of 2 studies on the treatment of advanced colorectal cancer plus an additional study exploring the association of Medicaid expansion with cancer survival outcomes. Transcript Dr. John Sweetenham: Hello, and welcome to our special coverage of the 2025 ASCO Annual Meeting on the ASCO Daily News Podcast. I'm your host, Dr. John Sweetenham, and I'll be bringing you brief analysis on selected abstracts from each day of the Meeting. My disclosures are available in the transcript of this episode.  Today, I'll be reviewing three abstracts, the first two of which address the treatment of advanced colorectal cancer. Today's first study is Abstract 3501. These data were presented by Dr. Heinz-Josef Lenz from the USC Norris Comprehensive Cancer Center and report on the expanded analysis of the CheckMate-8HW trial. This was a phase 3, international, multicenter trial in patients with MSI-high/MMR-deficient metastatic colorectal cancer, who were randomized between nivolumab (nivo) alone, nivolumab plus ipilumomab (ipi) or investigators' choice of chemotherapy (FOLFOX or FOLFIRI) with or without bevacizumab or cetuximab. The study showed that nivo plus ipi demonstrated superior progression-free survival compared with chemotherapy in the first-line setting and superior progression-free survival compared with nivo alone across all lines of therapy. These results led to the approval of nivo + ipi in the first-line setting in patients with MSI-H/dMMR mCRC in the U.S., the EU, and many other countries.  In today's presentation, Dr. Lenz reported on the expanded analyses of nivo plus ipi versus nivo across all lines of therapy and longer follow-up results for nivo and ipi versus chemo in the first-line setting. With longer follow up (the median is now at 47 months) nivo and ipi continued to show progression-free survival benefit compared with chemotherapy with a median PFS of 54.1 months versus 5.9 months, for a hazard ratio of 0.21.  Additionally, the analysis of the effects on PFS2, defined as the time from randomization to progression after subsequent systemic therapy, start of second subsequent systemic therapy, or death, showed that compared with chemotherapy, first-line nivo and ipi was associated with a 72% reduction in the risk of death or disease progression, despite the fact that 71% of those who progressed following chemotherapy crossed over to receive subsequent immunotherapy. The study also showed that across all lines, nivo and ipi demonstrated superior progression-free survival compared with nivo alone, the median not reached versus 39.3 months, for a hazard ratio of 0.62. No new toxicity signals emerged after further analysis. Most treatment-related adverse events with possible immune etiology were observed within the first six months of therapy. The results for PFS2 are particularly significant. Up to now, there has been some reluctance to use nivo and ipi as first-line therapy, partly because of its toxicity profile and based on the rationale that it would be active after other frontline therapies. The observation in this study that the beneficial effects of nivo and ipi are maintained downstream is compelling. The results suggest that delaying the use of this combination to the second line or later may compromise subsequent PFS and supports the use of nivo and ipi as a standard-of-care frontline option for MSI-H/dMMR metastatic colorectal cancer. Moving on, the next study I'm featuring today is Abstract 3503, presented by Dr. Jeanne Tie from the Peter MacCallum Cancer Centre and the Walter and Eliza Hall Medical Institute of Medical Research from Melbourne, Australia. This study reported the impact of circulating tumor DNA (ctDNA)-guided adjuvant chemotherapy escalation in stage III colon cancer, focused on the primary analysis of the ctDNA-positive cohort from the randomized DYNAMIC-III trial. As background, about 30% of patients with stage III colon cancer will recur following standard-of-care adjuvant therapy with oxaliplatin-based regimens. And current data show that for those patients with high-risk disease, 6 months of chemotherapy is associated with a lower recurrence rate than 3 months. Circulating tumor DNA following initial surgery has been shown to be a strong independent prognostic factor for these patients, but questions remain about how ctDNA can be used for adaptation of treatment. Questions regarding treatment adaptation were addressed in the DYNAMIC-III trials – specifically, does treatment escalation benefit those who are ctDNA positive following surgery, and can therapy be de-escalated for those who are ctDNA negative. The first of these 2 questions – treatment escalation in the positive group – is the subject of this report. One thousand and two patients were randomized in this study, between ctDNA-informed therapy (502) or standard management (500). Of those patients included in the intent to treat cohorts, 129 were ctDNA positive in the ctDNA-informed arm compared with 130 in the standard management arm. Various pre-planned treatment escalation protocols were used, depending on the choice of first-line therapy. With a median follow up of 42.2 months, there was no difference in 3-year relapse free survival between the ctDNA informed group (48%) and the standard management group (52%). There was, however, a highly significant difference in relapse-free survival for patients who cleared ctDNA by the end of treatment compared with those who didn't. The authors concluded that the recurrence risk for this group remains high, at about 50%, after adjuvant therapy and that it increases with higher ctDNA burden, but treatment escalation didn't appear to reduce the recurrence risk. Clearance of ctDNA was associated with a favorable outcome, suggesting that as more effective treatments are developed in the future for this group, ctDNA will likely prove to have major utility. Changing gears now, my final selection for today is Abstract 11006, presented by Dr. Elizabeth Shafer from the American Cancer Society. This study explored the association of Medicaid expansion with 5-year survival after a cancer diagnosis.  Dr. Schafer began her presentation by providing some historical perspective on the impact of the Affordable Care Act on reducing the number of uninsured adults aged less than 65 years in the United States. She then reviewed some recent data on the impact of Medicaid expansion on cancer care, including improved screening rates, improved access to cancer surgery, and an increase in earlier cancer diagnosis. The current study builds on earlier data from the American Cancer Society which showed improved 2-year overall survival for patients with newly diagnosed cancer following Medicaid expansion. The new study reported by Dr. Schafer examined 5-year cause-specific survival in individuals with cancer since Medicaid expansion, analyzed according to cancer type and various demographic and social factors. Using data from more than 813,000 individuals from 26 states that expanded Medicaid compared with more than 610,000 from 12 states that did not, the authors reported that similar improvements in 5-year cause-specific survival were observed in the expansion and the non-expansion states, but when analyzed by other factors, differences in outcome emerged. For example, although similar improvements in survival between expansion and non-expansion states were seen in urban communities, there was a significant improvement of 2.55 percentage points in survival for individuals in rural communities in expansion states compared with those in non-expansion states. Similar trends were observed in high poverty areas, where improvements in survival were superior in expansion versus non-expansion states.  When examined by cancer type, the authors observed greater improvements in 5-year survival for those with pancreatic, lung, and colorectal cancer, possibly due to improvements in screening and early access to treatment.  The authors concluded that those residing in rural and high-poverty areas experienced the most improvement in cause-specific cancer survival following Medicaid expansion. In summary, it's encouraging to see an improving trend in cancer mortality overall, independent of Medicaid expansion, but it's also important to remember that this is yet another study which confirms how implementation of the ACA has improved cancer outcomes and begun to address some of the disparities in cancer care. In Episode 240 of The Business Development Podcast, we welcome back former FBI Special Agent and behavioral expert Dr. Jack Schafer for an eye-opening conversation on the power of influence, trust-building, and the subtle science of deception detection. Drawing from his groundbreaking work in counterintelligence and his bestselling books The Like Switch and The Truth Detector, Dr. Schafer unpacks the psychological techniques used to build rapport, uncover hidden truths, and ethically guide conversations in both business and life. Whether you're negotiating deals, leading teams, or building relationships, the insights shared here are nothing short of transformative.Kelly and Dr. Schafer explore real-world examples of elicitation—how subtle cues and well-placed questions can unlock critical information without confrontation. From reading non-verbal cues to crafting empathetic statements, you'll learn how to master the unspoken side of communication and gain an edge in high-stakes interactions. This episode is packed with practical takeaways that can immediately elevate your ability to connect, persuade, and lead with confidence.Key Takeaways:1. People have a powerful psychological need to correct others, making “presumptive statements” a highly effective elicitation tool.2. Friendship signals—like eyebrow flashes, head tilts, and genuine smiles—are subtle but vital cues for building trust and rapport.3. The principle of “If I make you feel good about you, you'll like me” underpins nearly every successful relationship-building strategy.4. Elicitation works best when it's undetectable; most people won't even realize they've revealed valuable information.5. Empathic statements keep the focus on the other person and can deepen trust faster than direct questioning ever could.6. Bracketing (giving a high and low estimate) invites correction and is a powerful way to learn sensitive information like salaries or margins.7. Direct flattery can raise defenses, but indirect praise through intermediaries or subtle validation builds influence.8. Effective listening means silencing your internal dialogue and responding to what's actually said, not what you plan to say next.9. Most people are already using elicitation techniques unknowingly—naming them allows you to master and defend against them.10. Practicing one technique at a time builds confidence and competence; start with presumptive and build from there.

The seemingly outsized strength of cold-formed steel is not well-known. In this episode, earthquake engineer Ben Schafer, Johns Hopkins University, describes a research-industry collaboration with the automotive industry resulting in code changes for high-strength sheet-steel. Sheet steel has also been successfully tested in flooring systems. The upcoming CFS10 shake table test at UC San Diego is the high-rise building test for cold-formed steel. Schafer addresses misconceptions that structural engineers have regarding CFS: Basically: cold-formed steel looks too thin to be strong. However, with high-strength sheet steel, deformations do not correlate to lack of strength, which is something that automotive and aircraft engineers have long understood.

Meet Johns Hopkins University engineer Ben Schafer, authority on cold-formed steel (CFS), also known as sheet steel or thin steel. Schafer explains that CFS is both strong and ductile – and therefore a remarkably high-performance structural framing material. Builders use CFS in a variety of ways – including as building-frame members, much like timber. Schafer's research centers on CFS as structural framing to resist wind and earthquake loading. Thin and lightweight, CFS members comprise relatively little material; in the US, all cold-formed steel is made from recycled materials.

Struggling with Networking? You're Not Alone.  Let's be real – networking can feel cringy. The pressure to be perfect and the fear of rejection hold many people back. But the truth is, it doesn't have to be awkward or salesy! Here are the top takeaways from my recent conversation with Michelle about overcoming networking struggles: Shift your mindset: Networking isn't about pushing sales. It's about having genuine conversations. Prepare in advance: Know who you want to talk to, what you want to learn, and how you'll talk about yourself. Focus on curiosity: Approach networking with a mindset of learning, not just pitching. Be authentic: People appreciate real, meaningful conversations – that's how connections happen. I'm embracing these principles myself in both my career and business growth. Running the Light: A Novel by Sam Tallent Amazon.com Samtallent.com A bona fide “instant classic” (Doug Stanhope) novel that tells the story of a road comic crashing and burning by acclaimed comedian Sam Tallent Billy Ray Schafer stepped off the plane in Amarillo, Texas, with twenty-six hundred dollars tucked down the leg of his black ostrich-skin cowboy boot. He walked to baggage claim slowly, jelly-legged and nearing lucidity, coming out from under the Xanax he snorted before the flight. Debauched, divorced, and courting death, Billy Ray Schafer is a comedian who has forgotten how to laugh. Over the course of seven spun-out days across the American Southwest, he travels from hell gig to hell gig in search of a reason to keep living in this bleak and violent glimpse into the psyche of a thoroughly ruined man. Ex-inmate, ex-husband, ex-father—comedian is the only title Schafer has left. Trapped in the wreckage of his wasted career, Billy Ray knows the answer to the question: What happens when opportunity doesn't come—or worse—it comes and goes? “In vivid, electric sentences that read like cinematic tracking shots,” (Denver Post) Tallent hurls you into an absolute mess of a man's life as we search for the mercy he does not want. About the author Sam Tallent is a comedian. For the last decade he has performed more than 45 weekends per year across America, Canada, Europe, Japan and Australia. His writing has appeared in Birdy Magazine and on VICE.com. Running the Light, his first novel, is soon to be a major motion picture. He lives in Colorado and cooks dinner for his wife every night.

On Friday, actress and model Hunter Schafer posted an eight-and-a-half minute video on TikTok in which she revealed that the replacement passport she had just received had the gender marker changed from female to male.Ms. Schafer is a trans woman, widely beloved among Gen-Z for her acting—particularly her performance in “Euphoria”—and broadly considered one of the most beautiful women in the world. Over the years, she's modeled for Prada, Vera Wang, Marc Jacobs, Alexander McQueen, and many other houses. She came out as a teen girl and has renewed previously with the correct gender marker, with no issues. That means the State Department is either intentionally flagging all American passports with record of a gender marker change and/or they're intentionally cross-referencing passports with birth certificates, some of which are issued by states that ban gender marker changes. Not to mention: the ridiculous complexity for intersex Americans.Florida, Kansas, Montana, Oklahoma, Tennessee, and Texas all prohibit gender marker amendments on birth certificates. Their number will very likely grow by the end of this year. Last week, the Arizona House approved a bill banning residents from amending their birth certificates to reflect their authentic gender identity.I'm worried for all the obvious reasons: the government tracking, the likely surveillance of trans people, the grave risk of an incorrect, “outed” passport for any trans person who needs to travel internationally, the potential for confusion and obstruction over government documents for things like security clearances, etc. That same day, the Bureau of Prisons announced that trans people incarcerated in federal prisons would be moved to facilities that align with their sex assigned at birth, as early as next week. There are nearly 2,300 trans people incarcerated in federal prisons at the moment.Trans women transferred to men's prisons will obviously be at substantially greater risk of violence. The same day—yes, all on Friday—Iowa Republicans announced a bill that, if successful, would remove ‘gender identity' as a protected class in the Iowa Civil Rights Act, thus eliminating anti-discrimination protections for all transgender Iowans. It would make Iowa the first state to remove a protected class from civil rights law.Meanwhile, at the White House—yes, same exact day—Trump was speaking to a large, bipartisan group of governors and went after Maine Gov. Janet Mills from behind his podium.“Is Maine Here? The governor of Maine?”“Yeah, I'm here,” she said from her seat in the audience.“Are you not going to comply with that?” Trump demanded of her, referencing the executive order he signed on Feb. 5th that bans trans girls and young women from competing on girls' and women's sports teams.“I'm complying with the state and federal laws,” she responded.“We are the federal law,” snapped Trump. “You better do it… You better comply , you better comply, because otherwise you're not getting any federal funding.”“See you in court,” she retorted from her seat.“Good,” said Trump. “I'll see you in court. I look forward to that. That should be a real easy one, and enjoy your life after governor, because I don't think you'll be in elected politics.”It was yet another moment Trump attempted to intimidate elected officials across the political spectrum over trans issues. On Saturday, Trump spoke to the 2025 Conservative Political Action Conference (CPAC), where he took a victory lap for ending “transgender insanity.”“It's all out now — critical race theory and transgender insanity. It's all gone from our schools and from our military and I believe it's gone, too… I believe that it's all gone.”Today, Bill Maher appeared on the “Pod Saves America” podcast and argued with co-host Jon Lovett about trans issues, urging him that Democrats would “lose every election” if they don't give up more on trans equality.Side note here: anti-trans punditry that claims to be offering the “reasonable middle” argument always seems to intensely focus on what they don't want. There is never a good faith framing of what they believe should be legal. There are reasons for that: either they don't actually believe in minimum rights for trans people or they fear being targeted by extremists for offering support for even a modicum of trans equality. The relentless pace of anti-trans actions and rhetoric have become overwhelming. That's how I'm doing.Charlotte's Web Thoughts is a reader-supported publication. This week, on episode 495, I'm talking with Will Schafer, founder of Altisnooze. The First-of-its-kind sleep aid for high-altitude. After struggling for years with sleeping during alpine adventures, Schafer came to learn from a sleep doctor that altitude insomnia is a thing. He connected with formulation experts to develop a natural solution, ALTISNOOZE. Facebook   Twitter   Instagram Love the show? Subscribe,  rate, review, and share! Sign up for my Newsletter  HERE I'd love to hear your feedback about the show! You can contact me here:  rick@theoutdoorbizpodcast.com What Happened: For years, I chalked up my awful sleep in the mountains to bad luck. Every time I went to Summit County, Colorado, my nights were a mess—staring at the ceiling, waking up feeling like I'd been run over by a snowcat, and then trying to enjoy the outdoors while running on fumes. I blamed it on travel, excitement, bad pillows—you name it. But then I started noticing something weird. Friends on ski trips and camping adventures were having the same problem. Some of them, tough-as-nails hikers and backcountry warriors, were walking around like zombies after a single night at altitude. That's when I met Dr. Michael Breus, a legit sleep doctor, who hit me with a truth bomb: altitude insomnia is real. And worse? It affects up to 75% of people. Your body's struggling with low oxygen, which wrecks your sleep cycle, making it nearly impossible to rest. So, I set out on a mission to fix it. After a deep dive into research, testing a ridiculous amount of natural ingredients, and countless sleepless nights (for science), I created Altisnooze—a sleep aid designed specifically for high-altitude adventures, without the grogginess of melatonin. Principle: Bad sleep = bad adventure. Period. You can have the best gear, the most epic trail planned, and a perfect sunrise waiting for you—but if you don't sleep well, your trip is going to suck. Most people don't even realize altitude is the issue. They just assume they're bad sleepers, or that their body needs to “adjust.” But waiting around for a week to acclimate? Not ideal when your trip is only a few days long. Good sleep isn't just about comfort—it's about performance, recovery, and actually enjoying the adventure you planned. Transition: A lot of outdoor lovers deal with this and don't even realize what's holding them back. They push through exhaustion, drink too much caffeine, or try to knock themselves out with melatonin—only to wake up feeling worse. That's exactly why I created Altasnooze. Because when you finally fix your sleep at altitude, everything changes—your energy, your performance, your mood, and most importantly, how much fun you have. That's Why: …we want to introduce you to Altisnooze and just dropped an episode of the Outdoor Adventure Lifestyle Podcast with Will Schaefer, founder of Altisnooze. He went from being a sleep-deprived mess in the mountains to developing a game-changing solution for adventurers who want to feel fabulous at altitude. In this episode, we dive into the science of altitude insomnia, how it messes with your body, and what you can do to fix it—so your next high-altitude trip doesn't turn into a sleepless disaster. Call to Action: If you've ever struggled to sleep in the mountains, this episode is your wake-up call. (Pun intended.) Don't let altitude ruin your adventure—get the inside scoop on fixing your sleep at high elevation and feeling your best. #Outdoor Adventure Lifestyle Podcast, #altitude insomnia, #AltiSnooze, #high altitude sleep aid, #altitude sickness, #sleep doctor, #Michael Breus, #adventure travel, #melatonin-free, #sleep disturbance, #alpinists, #electrolytes, #high altitude adventures, #natural supplements, #altitude symptoms

Hildegard Westerkamp: A Life in Soundscape Composition

Today we speak to Hildegard Westerkamp, the pioneering composer, radio artist and sound ecologist. The centerpiece of all of her work is a close attention to the sonic environment and its relation to culture. We will listen to excerpts of six soundscape compositions made between 1975 and 2005, all of which reward the close listener–conceptually and aesthetically–with a deeper relationship to the sonic environment.  Mack Hagood interviewed Westerkamp shortly after the death of R. Murray Schafer in late 2021. Westerkamp worked closely with Schafer in the early 1970s and she graciously agreed to talk about him despite the grief being fresh. They also discussed her own amazing career and that's the part of the tape we are sharing in this episode. They talk about her formative years as a 20-something working with Schafer and his World Soundscape Project and then we jump into a number of her compositions, ending with the piece “Breaking News” from 2012.  Incredibly, she said Mack was the first person to ever ask her about that piece, even though it is one of her favorites. And sure enough, not long after this interview she released a retrospective album on Earsay Music called Breaking News, which features that piece and a number of others created between 1988 and 2012.  For our Patreon members we have the full, unedited interview for those who want to hear all her thoughts on R. Murray Schafer and her career. Join at Patreon.com/phantompower.  And a quick correction: Hildegard wanted me to clarify that the sentence “When there is no sound, hearing is most alert,” which she uses in “Whisper Study,” is a quote from the Indian mystic Kirphal Singh in his book Naam (or Word). Pieces featured in this episode:  “Gently Penetrating beneath the Sounding Surfaces of Another Place” (1997) “Whisper Study” (1975) “Fantasie for Horns” (1978) “A Walk through the City” (1981) “Für Dich – For You” (2005) “Breaking News” (2002) Today's show was written and edited by Mack Hagood.
