Dean's Chat hosts, Dr. Jeffrey Jensen and Dr. Johanna Richey, are joined by Dr. Wayne Bakotic and Dr. Warren Joseph on today's show discussing their careers and their specialties that overlap in dematopathology and infectious disease. Discussions range from PCR testing for both fungus and bacteria, to their careers, their influence in the podiatric community, the advancement of AI in medicine, and relationship among specialists. A wonderful discussion, you'll wonder where 45 minutes went! Dr. Wayne L. Bakotic began his medical training with early acceptance into Barry University School of Podiatric Medicine where he completed his biomedical sciences degree through his podiatric medical studies. Prior to graduation, Dr. Bakotic left his podiatry training to initiate studies in Osteopathic medicine at Nova Southeastern University College of Osteopathic Medicine. It was, however, his time at Barry University which set the stage for his interest in podiatric pathology. After graduating with honors and completing his Doctor of Osteopathy in 1998, Dr. Bakotic continued his journey at Emory University School of Medicine where he gained training in anatomic and clinical pathology. He remained at Emory University to complete fellowship training in surgical pathology, Cytopathology and Dermatopathology, gaining board certification in anatomic pathology, clinical pathology, and Dermatopathology. In 2008 Dr. Bakotic co-founded Bako Diagnostics in Alpharetta, GA with the goal of serving the podiatric medical community and where he continues in the role of Chief Medical Officer. Dr. Joseph a leader in our profession and a member of the Podiatric Hall of Fame. Dr. Joseph is a specialist in the treatment of lower extremity infectious diseases. He received his podiatric degree from the Wm. Scholl College of Podiatric Medicine in Chicago. Dr Joseph completed his postgraduate training in podiatric medicine/surgery at St. Joseph's Hospital, and a fellowship in infectious diseases at Hahnemann University School of Medicine, both in Philadelphia. He is currently an Adjunct Clinical Professor at the Arizona College of Podiatric Medicine at Midwestern University in Glendale, AZ. Dr. Joseph is a fellow of the Infectious Diseases Society of America and is an author of the IDSA Diabetic Foot Infection Guidelines. He is also certified by the American Board of Podiatric Medicine and currently serves as Editor of the Journal of the American Podiatric Medical Association. Dr Joseph lectures extensively and his presentations have focused on the treatment of bacterial and fungal infections of the lower extremity. P08-Diagnostic Cytopathology Cell Block Preparation Methods- Anna Patterson FIBMS MSc CSci Diagnostic Cytology Scheme Coordinator Helen Naylor MSc Diagnostic Cytology Technical Specialist Cell blocks from Cytopathology samples have always had value in the diagnostic process as a complement to the traditional Cytopathology stains – Papanicolaou and Romanowsky. This has become more important to provide material for Immunocytochemistry to refine malignant diagnosis, and more recently, for the use of molecular testing to aid in the choice of tailored chemotherapy regimens. If this information can be obtained from Cytopathology samples, which are less invasive than biopsy samples, the patient will benefit. A variety of preparation methods are available for the preparation of cell blocks from cells from Diagnostic Cytopathology samples. The most popular methods will be discussed and how they can be used to optimise the quality of cell preservation if used correctly. Information garnered from the results of the recently introduced UK NEQAS CPT Diagnostic Cytopathology Cell block scheme will be presented and how this information can be circulated to laboratories experiencing difficulties with their preparation methods as an advisory service. This will include. • Understanding the clinical importance and diagnostic purpose of correct procedures in Diagnostic Cytopathology Cell Block preparation. • Identifying and determining factors affecting best practice and quality in Diagnostic Cytopathology Cell Block practice and how to resolve them. • Identifying and understanding the causes of artefacts experienced in Diagnostic Cytopathology Cell Block preparation methods and how to eliminate and prevent them. P11- Participating In A Digital Diagnostic Cytopathology Interpretive Proficiency Testing (iEQA) Scheme -Helen Naylor, Anna Patterson, Chantell Hodgson, Ashley Makela The iLabXCell Digital Diagnostic Cytopathology Interpretive Proficiency Testing Scheme or iEQA is facilitated by UK NEQAS Cellular Pathology Technique (CPT), and is designed to promote quality, excellence, and education for all involved in screening and reporting of Diagnostic Cytopathology. It is open to medical and non-medical staff, trainees, advanced practitioners, and staff associated with Cytopathology. iEQA provides superior, outstanding, and representative case examples for individuals to examine in a remote setting and submit an opinion via the user-friendly digital platform. Offering 2 circulations annually it provides • Easy access for registration • Ability to choose specimen types • Advanced slide viewing • Clinical details to assist diagnoses • Categorisation of images using benign or malignant Flexibility Remote learning allows participants to continue learning alongside their peers, in an environment accommodating their needs. It allows the flexibility of international participants to access the platform at any time, convenient to them. Remote learning via the iEQA platform, offers the flexibility for users to pick up learning where they left it – anywhere, anytime, from any location. Self-paced learning Traditional learning has long failed to acknowledge the individualised nature of learning, opting instead for a generalised approach that may not be optimised for everyone. One of the many benefits of this iEQA is providing participants more independence to: • Spend more time on cases/case types they find difficult • Revisit the images as often as they need Environmental benefits Digitising slides has led to a decrease in physical resources previously necessary for iEQA to function. Everything participants need is accessible on-line, eliminating the historical environmental impact and the time taken for circulations to be distributed and completed. Future developments An on-line educational image library, containing images of Diagnostic Cytopathology cases of urines, serous fluids, respiratory and head and neck cases used in previous circulations for educational and training purposes.
Drug resistance can be delayed and treatment outcomes predicted in patients with ovarian cancer with the help of relatively low-cost molecular precision management techniques using liquid biopsies. These are being developed by a team at the University of California in Los Angeles (UCLA) led by Jian Yu Rao, MD, Vice Chair of Diagnostic Technology Innovation at UCLA, where he is also Chief of Cytopathology and Director of International Telepathology. At the 2024 Annual Meeting of the Chinese Society of Clinical Oncology (CSCO) held in Xiamen, China, Rao gave Oncology Times reporter Peter Goodwin details of the molecular methods he had just outlined to the conference.

Ron Balassanian is a Professor of Pathology at the University of California San Francisco (UCSF) with an appointment in UCSF Helen Diller Family Comprehensive Cancer Center. His clinical and research work focuses on fine needle aspiration (FNA) biopsy and general breast pathology, as well as patient communication and global health. Ron has led workshops on training pathologists to perform and interpret ultrasound guided FNA for the College of American Pathologists, the American Society of Cytopathology and the United States and Canadian Academy of Pathology. In collaboration with PATH, a Seattle based NGO, he helped develop the Community Program for Breast Health in Trujillo Peru, using FNA as a cost-effective tool for diagnosis and triage. Through the UCSF Global Cancer Program, he helped develop an US-FNA course in Dar es Salaam Tanzania as part of an ongoing educational collaboration between Muhimbili University and the UCSF Global Cancer Program. At UCSF he developed a patient education program called “Ask Your Pathologist” inviting patients to review their breast cancer slides with a Pathologist to better understand their pathology report. She received her Doctor of Medicine at the Medical University of South Carolina in Charleston. She completed her pathology residency training at University of South Florida, as well as a surgical oncologic pathology fellowship and cytopathology fellowship at Moffitt Cancer Center. As a clinical educator she is passionate and constantly involved in medical education of students, residents, fellows and clinicians. Her research efforts focus on the pathogenesis of soft tissue sarcomas and the identification of diagnostic, prognostic and predictive biomarkers. She has over 40 peer-reviewed publications and five book chapters. In the 19th Century there was an increasing numbers of Black Tumours reported in medical literature - described as Melanose in 1806 when Rene Laennec published that the Blackness of these tumours was not related to black carbon deposits found in the lungs of patients upon autopsy. By mid 20th Century, a more evidence based approach was taken. Today, through molecular biology advances, we know genetic mutations are important in the genesis of tumours such as melanoma. Bryan McIver, MD, PhD Dr. McIver contributes to Moffitt Cancer Center almost 20 years of clinical experience in the care of patients with endocrine diseases, specializing in the evaluation of patients with thyroid nodules and thyroid cancer. He has a particular interest in the management of patients with advanced and aggressive forms of cancer and the role of genetic and molecular techniques to improve the accuracy of diagnosis; to tailor appropriate treatment to a patientdisease. Dr. McIver has a long-standing basic research interest in the genetic regulation of growth, invasion and spread of thyroid tumors of all types. His primary research focus is the use of molecular and genetic information to more accurately diagnose thyroid cancer and to predict outcomes in the disease. Dr. McIver received his MB ChB degree from the University of Edinburgh Medical School in Scotland. He completed an Internal Medicine residency at the Royal Infirmary of Edinburgh, followed by a clinical fellowship and clinical investigator fellowship in Endocrinology at the School of Graduate Medical Education at Mayo Clinic in Rochester, MN. Prior to joining Moffitt, he was employed as Professor and Consultant at the Mayo Clinic and Foundation in the Division of Endocrinology & Metabolism. Amongst his most proud accomplishments, Dr. McIver counts his two commitment to education of medical students, residents and fellows; his involvement as a founding member of the World Congress on Thyroid Cancer, an international conference held every four years; and his appointment as a member of the Endowed and Master Clinician Program at the Mayo Clinic, recognizing excellence in patient care.
