Dean's Chat hosts,  Dr. Jeffrey Jensen  and Dr. Johanna Richey, are  joined by Dr. Wayne Bakotic and Dr. Warren Joseph on today's show discussing their careers and their specialties that overlap in dematopathology and infectious disease.   Discussions range from PCR testing for both fungus and bacteria, to their careers, their influence in the podiatric community, the advancement of AI in medicine, and relationship among specialists. A wonderful discussion, you'll wonder where 45 minutes went! Dr. Wayne L. Bakotic began his medical training with early acceptance into Barry University School of Podiatric Medicine where he completed his biomedical sciences degree through his podiatric medical studies. Prior to graduation, Dr. Bakotic left his podiatry training to initiate studies in Osteopathic medicine at Nova Southeastern University College of Osteopathic Medicine. It was, however, his time at Barry University which set the stage for his interest in podiatric pathology. After graduating with honors and completing his Doctor of Osteopathy in 1998, Dr. Bakotic continued his journey at Emory University School of Medicine where he gained training in anatomic and clinical pathology. He remained at Emory University to complete fellowship training in surgical pathology, Cytopathology and Dermatopathology, gaining board certification in anatomic pathology, clinical pathology, and Dermatopathology. In 2008 Dr. Bakotic co-founded Bako Diagnostics in Alpharetta, GA with the goal of serving the podiatric medical community and where he continues in the role of Chief Medical Officer.  Dr. Joseph a leader in our profession and a member of the Podiatric Hall of Fame. Dr. Joseph is a specialist in the treatment of lower extremity infectious diseases. He received his podiatric degree from the Wm. Scholl College of Podiatric Medicine in Chicago. Dr Joseph completed his postgraduate training in podiatric medicine/surgery at St. Joseph's Hospital, and a fellowship in infectious diseases at Hahnemann University School of Medicine, both in Philadelphia. He is currently an Adjunct Clinical Professor at the Arizona College of Podiatric Medicine at Midwestern University in Glendale, AZ. Dr. Joseph is a fellow of the Infectious Diseases Society of America and is an author of the IDSA Diabetic Foot Infection Guidelines. He is also certified by the American Board of Podiatric Medicine and currently serves as Editor of the Journal of the American Podiatric Medical Association. Dr Joseph lectures extensively and his presentations have focused on the treatment of bacterial and fungal infections of the lower extremity. She is board certified in dermatology with a specialty certificate in dermatopathology. Most of her time is spent in the Division of Dermatopathology, where she renders diagnoses for skin biopsies, sent in to Yale from Yale Dermatology Associates and other dermatology practices in Connecticut. Dr. Ko currently specializes in Transplant Dermatology, providing care for patients with organ transplants who are generally on immunosuppressive medications and are at higher risk for skin cancer.  Dr. Ko received her AB at Princeton University and MD from New York University School of Medicine. She completed internship at UCLA, and her dermatology residency at University of California, Irvine. Her research focuses on skin cancer, particularly squamous cell carcinoma and keratoacanthoma, in collaboration with researchers in the Department of Genetics and the Division of Dermatologic Surgery. She's been at Yale for almost 20 years and has written academic books, papers for journals, and a nonfiction book on doctor-patient connection. She wrote a picture book with her son! I absolutely loved speaking with Dr. Alejandro Gru on burnout, improving, accepting and moving on from error (but also feeling appropiately bad about it), and the relationships that we should cherish. I learned so much! Dr. Alejandro Gru is the Leonard C Harber Professor of Dermatology and Director of Dermatopathology at Columbia University Irving Medical Center. He was previously at the University of Virginia, where he was Professor of Pathology and the Dermatopathology Section and Fellowship Program Director. He attended the Universidad de Buenos Aires, School of Medicine and did residency at Washington University in Saint Louis in Anatomic and Surgical Pathology. He has completed dermatopathology and hematopathology and surgical pathology fellowships. He is an expert in skin lymphomas and melanocytic pathology and currently serves as the President of the United States Cutaneous Lymphoma Consortium.

Dr. Phillip McKee is a renowned pathologist and dermatopathologist who needs no introduction in dermatology and pathology. At age 76, still very active in the dermatopathology community, he shares his thoughts with me on burnout, relationships, and error. Dr. Philip McKee is a Fellow of the Royal College of Pathologists and received his medical degree from Queens University in Belfast, Northern Ireland.He trained in pathology at the Royal Victoria Hospital in Belfast and completed a dermatopathology fellowship at St. John's Hospital for Diseases of the Skin in London. He has authored and edited many books, including the amazing Pathology of the Skin. And that edition that I fell in love with won several medical book awards. He was awarded the Elson B Helwig Lecture Award in 2003 by the American Society of Dermatopathology. He is married to a retired cytopathologist, who was also a Professor at Harvard Medical School. She is now a full-time artist. He also has four adult children with very varied interests.

After being educated in Sheffield which is north of London, he started practicing in London with “big city ways”. He has now been practicing in the same community for 25 years in Warwick, England. He is a dedicated teacher, locally, nationally, and internationally. He is married with two daughters.

He interned at the University of California, Irvine and then did his residency and fellowship at Thomas Jefferson University Hospitals. I find awareness difficult to cultivate, and I have always admired Dr. Earl Glusac's ability to be present, calm, and steady. Dr. Glusac has been meditating regularly since 2005 (as well as when he was younger), and he shares his view on meditation as well as tips for a productive career in academic medicine. Dr. Earl J. Glusac, MD is a distinguished dermatopathologist and esteemed professor at Yale University School of Medicine. He has actively shaped the field through his roles on various medical board. He has served the American Boards of Pathology and Dermatology and the editorial boards of the Journal of Cutaneous Pathology and the American Journal of Dermatopathology; he was program director and president of the American Society of Dermatopathology, and he is an author for the World Health Organization's Classification of Tumours of the Skin. He attended Michigan State University for his medical degree, spent time in California for his residency training as well as dermatopathology fellowship, and has spent the majority of his dermatopathology career at Yale. Much of his research relates to the problem of overdiagnosis, especially as related to benign lesions that mimic malignant melanoma.

He received his PhD in Cognitive Psychology from Florida State University in 2015, during which time he overlapped a little bit with Kyle. He has both an MA and BA degrees from Yonsei University in South Korea. His research interests include memory, cognitive aging, human factors, and the development of expertise. Dr. Kyle Harwell, received his MA from Florida State University and his PhD in Cognitive Psychology. His undergraduate degree is from the University of West Florida. Both doctors Yoon and Harwell are grounded in the study of expert performance, the essence of deliberate practice, and applying the ideas of deliberate practice and the expert performance approach to the study of human performance in various fields, including in healthcare.  Dr. David Caruso, PhD, is a management psychologist who develops and conducts emotional intelligence training around the world. He co-authored the Mayer, Salovey, Caruso Emotional Intelligence Test; he co-wrote several books including The Emotionally Intelligent Manager with Dr. Peter Salovey and A Leader's Guide to Solving Challenges with Emotional Intelligence with Lisa Rees.  He has been an executive coach to leaders around the world.

I was confused about "heuristics"! It is always so exciting when I learn something new that makes things clearer to me. According to Dr. Gigerenzer, heuristics are NOT the same as cognitive bias. Cognitive bias describes PAST behavior, often in situations of so-called "risk" (the economic term) (that actually means that all variables are known). Heuristics guide what can be DONE in the FUTURE and are helpful in situations of uncertainty. Dr. Gigerenzer is an international expert on judgments under uncertainty, and this is part 1 of my conversation with him on heuristics. Dr. Gerd Gigerenzer, PhD is Director of the Harding Center for Risk Literacy at the University of Potsdam, Faculty of Health Sciences Brandenburg and partner of Simply Rational - The Institute for Decisions. He is former Director of the Center for Adaptive Behavior and Cognition (ABC) at the Max Planck Institute for Human Development and at the Max Planck Institute for Psychological Research in Munich, Professor of Psychology at the University of Chicago and John M. Olin Distinguished Visiting Professor, School of Law at the University of Virginia. Awards for his work include the AAAS Prize for the best article in the behavioral sciences, the Association of American Publishers Prize for the best book in the social and behavioral sciences, the German Psychology Award, and the Communicator Award of the German Research Foundation. Dr. Irwin Braverman, MD is Professor Emeritus of Dermatology at Yale University. He received his undergraduate degree from Harvard College and his MD from Yale University in 1955 and trained as an intern, resident, and fellow at Yale as well as Yale-New Haven Hospital and Medical Center. He is board certified in dermatology as well as dermatopathology. He retired from being a full-time faculty member in the Department of Dermatology in 2010 after 48 years of service. His research focused on the cutaneous microcirculation, cutaneous T-cell lymphoma, and aging. He is the author of Skin Signs of Systemic Disease.

She subsequently worked in North Carolina at Wake Forest University. Currently, she is a Distinguished Professor of Psychology at the University of Leuven, Belgium, where she studies the role of culture in emotions, and of emotions in culture and society. She directs the Center for Social and Cultural Psychology in Leuven. I know...dialectical behavior therapy sounds complicated - but it is DBT for short, and Dr. Osmond does a great job of summarizing a key concept - wise mind being a combo of rational mind and emotion mind. This is an important concept for us all, relevant to home life and any decision-making task, including diagnostic work for physicians. DBT (first brought up to me by David Caruso in a prior episode) can also give insights into burnout (consider listening in to Michi Shinohara, if you haven't yet!). Dr. Allison Osmond is an Assistant Professor of Pathology and Laboratory Medicine at the University of Saskatchewan. Dr. Osmond completed residency in Anatomic Pathology at Western University in London, Ontario Canada in 2015, followed by a Surgical Pathology Fellowship at Western, with a focus in gastrointestinal pathology. She then worked in a general Anatomical Pathology practice for one year and then completed Dermatopathology fellowship at the University of Toronto in 2018. Dr. HongYu (Henry) Yang, MD PhD is an experienced pathologist and dermatopathologist with a robust social media following. Dr. Yang was initially trained in China, finishing his MD/PhD (in Immunology) in the 1990s, did postdoctoral research in Rheumatology at Mayo Clinic for 5 years, then pursued residency/fellowship training in pathology/dermatopathology. His anatomic and clinical pathology fellowship was at the University of Missouri, Columbia; and his dermatopathology fellowship was at Mayo Clinic in 2004. He is currently a dermatopathologist and general pathologist in the Department of Pathology at St. Vincent's Medical Center in Evansville, Indiana. He has been a valued educator for many years and was awarded the James A Esterly Award for Resident Teaching at the University of Missouri-Columbia in 2003 and 2004. He is one of the founding members, in 2015, and has been president of the North American Chinese Dermatologists Association since 2020.

In dermatopathology, we love our diagnostic names! Is it a "nevus with architectural disorder" or "atypical nevus" or "dysplastic nevus" or "Clark nevus"...? Similarly, in emotions research, there are distinct definitions for affect, feeling, emotion, and mood. Dr. David Caruso helps explain the gut feeling in relation to these definitions and to emotional intelligence; he refers to an important paper by Dr. Sigal Barsade. He also touches on the social emotion of shame and how shame, like any other emotion, can be used as a signal in our lives to get to the best decisions we can, or the so-called "wise mind". Dr. David Caruso, PhD, is a management psychologist who develops and conducts emotional intelligence training around the world. He co-authored the Mayer, Salovey, Caruso Emotional Intelligence Test; he co-wrote several books including The Emotionally Intelligent Manager with Dr. Peter Salovey and A Leader's Guide to Solving Challenges with Emotional Intelligence with Lisa Rees. He is married with 3 children and 4 grandchildren; he was raised Catholic and converted to Judaism. He has been an executive coach to leaders around the world.

Dr. Philip LeBoit is Professor of Pathology and Dermatology as well as Division Chief of the Dermatopathology Service at the University of California, San Francisco. He founded the UCSF Dermatopathology and Oral Pathology Service in 1987. He trained in Anatomic Pathology at University of California, San Francisco and then in Dermatopathology at New York University under Dr. Bernard Ackerman and at New York Hospital-Cornell Medical Center under Dr. N. Scott McNutt. He has written several academic textbooks, including one of my favorites with Dr. Guido Massi, Histologic Diagnosis of Nevi and Melanoma. He is a past-president of the American Academy of Dermatology and the American Society of Dermatopathology, has served on the board of directors of the International Society of Dermatopathology and the American Board of Dermatology, and serves as the Editor of the Journal of the American Academy of Dermatology. Dr. Elston is a graduate of Jefferson Medical College, did his dermatology residency at Walter Reed Medical Center, and completed a dermatopathology fellowship at the Cleveland Clinic. He is the author of over 600 peer reviewed publications, is one of the authors of Andrews Diseases of the Skin, Associate Editor in Chief of eMedicine dermatology, and has a textbook of Dermatopathology with Dr. Tammie Ferringer that received the Highly Commended Award in the British Medical Association Medical Book Competition. He received the 2008 Walter Nickel Award for Excellence in Dermatopathology Education, the 2013 Founder's Award of the American Society of Dermatopathology, and the 2021 Gold Medal of the American Academy of Dermatology.

He is a recent President of the European Society for Dermatological Research, serves on the editorial board of 9 international journals, and is the section editor for genetics for the Journal of Investigative Dermatology. He has published over 400 manuscripts, including reports of mutations in Nature Genetics. About 50% of doctors have burnout, and burnout is increasing among dermatologists. Pathologists have been reported to have the least amount of burnout. In this episode, Dr. Michi Shinohara shares how you can recognize burnout, some steps to take to combat it, and the importance of letting go. Dr. Michi Shinohara is Associate Professor, Associate Chief of Dermatology, and Director of Dermatopathology in the Division of Dermatology at the University of Washington. She attended Reed College in Oregon, starting as an art major before majoring in chemistry. She got her medical degree from the University of Washington School of Medicine. She has completed both an internal medicine and dermatology residency, and her dermatopathology fellowship was completed at the University of Pennsylvania. Dr. Shinohara runs the Cutaneous Lymphoma Clinic at the Fred Hutchinson Cancer Center. Her research interests are quality of life for patients with cutaneous lymphoma, and mentorship and sponsorship of women and those who are historically excluded in medicine. He is internationally renowned for his diagnostic skills that span the breadth of dermatopathology. Recently, Dr. McCalmont has moved into a new role as a dermatopathology consultant in the private sector at Golden State Dermatology Associates.

Dr. Antley is a dermatopathologist and talks to High Truth about whether marijuana products can help acne or harmful to the skin. Catherine Antley, M.D. Catherine Antley, M.D. graduated from the University of North Carolina School of Medicine, completed her residency in anatomic and clinical pathology at Duke University Medical Center and fellowship in dermatopathology at the University of Arkansas for Medical Sciences.  She is board certified in Anatomic and Clinical Pathology and in Dermatopathology and is a Fellow of the American Society of Dermatopathology. For 20 years she has served as laboratory director of Vermont Dermatopathology, a woman owned, independent dermatopathology lab serving VT, NY, and NH.  She has a strong interest in public health and prevention and exploring the impact of policy on addiction and health care costs. She has co-authored a number of Vermont Medical Society resolutions on cannabis and recently organized and led a delegation of 10 Vermonters to Iceland to learn about their 20 year history bringing down the country’s high addiction rates using their successful, science based prevention model. She recently contributed a chapter “Cannabis in Dermatology” to the textbook “Cannabis in Medicine, an Evidence Based Approach” edited by Dr. Ken Finn and published by Springer Nature. In today's episode, we discuss wrinkles, a topic not too many men talk about but one that a lot of men think about.  How do we know this? A recent study was conducted about the use of Botox, one of the most common treatments for wrinkles.  That study found that in 2019, men underwent nearly half a million botox injections in the US alone  That was nearly a 400% increase as compared to the year 2000.  So while men may not be openly talking about their wrinkles, they certainly appear to be thinking about them.  Joining us today to help us better understand the causes and management of wrinkles is Dr Jasmine Yun,  Dr. Yun is a board-certified and licensed dermatologist who earned her undergraduate degree from UCLA. She then received a joint MD/MBA from the UCLA School of Medicine and the Anderson School of Management at UCLA. She proceeded to complete her dermatology resident at MLK-Harbor and Harbor-UCLA and then completed a dermatopathology fellowship at the Ackerman Academy of Dermatopathology in New York. Dr. Yun serves as a Clinical Associate Professor of Dermatology at USC Keck School of Medicine where she has devoted much time educating resident physicians and also has spent time participating in medical missions in developing countries.  She also has a private dermatology practice in Los Angeles.SHOW LESS

Despite being integrally involved in making diagnoses and treatment plans, pathologists remain fairly invisible to most patients. According to pathologist Greg Osmond, some of his colleagues don't mind staying behind the scenes, but out of concern that the profession in undervalued and at risk for automation, he's sees an opening for greater relevance in having pathologists provide a coherent picture to the wider care team of the many diagnostic and prognostic test results any given patient may have. In addition to considering that new role, the profession is also facing a deluge of digital tools and techniques that are coming online. Osmond, despite co-founding a digital pathology company, shares with host Dr. Rishi Desai that doctors really need to understand the limits of AI and other emerging modalities that are sure to change the practice of pathology in the coming decade.

In the 19th Century there was an increasing numbers of Black Tumours reported in medical literature - described as Melanose in 1806 when Rene Laennec published that the Blackness of these tumours was not related to black carbon deposits found in the lungs of patients upon autopsy. By mid 20th Century, a more evidence based approach was taken. Today, through molecular biology advances, we know genetic mutations are important in the genesis of tumours such as melanoma. She has extensive experience in private practice and managed to make the transition from private to academics is now the Program Director where she trained. She has a particular interest in immunodermatology and the impact of nutrition and supplementation on dermatologic disease. Over the next 500 years, the medical attempts to understand and treat this new disease -- syphilis -- would mold and shape medicine in surprising ways. He then interned in 1969-70 at the Univ. of Southern CA. & completed his residency @ the Univ. of Lou’s School of Medicine from 1972-1975-becoming Chief Resident in Dermatology in 1975. Today he holds Bd. Certifications from the American Bds. of Dermatology, Dermatopathology, of Hair Restoration Surgery. Parsley holds his ACLA certification as well and belongs to MANY medical professional organizations such as: Being a fellow with American Academy of Dermatology The American Society of Dermatologic Surgery Medical Association- Dermatologic Society Jefferson Co. Session 01 Welcome to the first episode of the Specialist Stories podcast, sharing with you stories of specialists from every field to give you the information you need to make sure you make the most informed decision possible when it comes to choosing your specialty. This podcast is hosted by Dr. Ryan Gray where he will interview different physicians from various specialties to help medical students and premedical students get different perspectives on what led them to their career path. The Specialist Stories podcast is part of the MedEd Media Network where you will find all of our other shows. In this week's episode, Ryan talks with Dr. Michelle Hure, a dermatopathologist who has her own solo practice in her community. A brief look into Ryan's background: Ryan went to medical school wanting to be an orthopedic surgeon and he came out of medical school (through an HPSP scholarship from the Air Force) knowing that orthopedic surgery was right for him. Unfortunately the Air Force had different plans for him. So he ultimately did not practice orthopedics and went on as a flight surgeon. Here are the highlights of the conversation with Dr. Hure: When Michelle knew she wanted to be a dermatologist: From an interest in trauma surgery to dermatopathology Realizing the need for work-life balance Coming to a point of not wanting to do until her 4th years during rotation What she likes about her specialty: Changing people's lives and curing cancer Getting to do surgery Being able to get home at 5 Making use of her brain everyday What a dermatopathologist does: Two routes: Dermatology residency Pathology residency As a pathologist, it involves diagnosing conditions or interpreting biopsies that is key to a patient's treatment plan. You are the doctor's doctor Can do both clinical and pathology A day in the life of Michelle: Reading slides of biopsies she has taken personally or those from other doctors Seeing patients at 10 am Traits that lead to being a good dermatopathologist: Open mindedness: Being able to think of different possibilities and looking at slides without any biases Knowledge of clinical history and clinical medicine Curiosity Openness to different differential diagnosis A lot of thinking and investigation What makes a competitive applicant to dermatology and dermatopathology: Dermatopathology is very tough to get into since there aren't many programs so programs available are highly competitive. Be always in your game. Walk the extra mile. Do rotations in a place you're really interested in doing your residency as well as your fellowships. Be willing to take initiative. What residency was like for her: Collaboration as an important piece Pick a residency at the particular institution where that fellowship is to have a higher chance of getting in. What she wished she knew going into dermatology/dermatopathology: It's possible to have a family early on. Family comes first, residency and fellowship come second What she wished primary care providers knew more about dermatopathology: Training in dermatology and pathology What Michelle likes most about being a dermatopathologist: Intellectual stimulation Patient interaction Surgery Being able to cure cancer What she likes the least about her practice: Dealing with insurance companies If she had to do it all over again, would she choose another specialty? No, not at all. What is the future of dermatopathology? The pressure of being more noticeable to people so that biopsies must be done by experts in the field and not just "general" pathologists - It's not about money, it's about patient care! The saturation of the field Some pieces of advice to those wanting to be a dermatopathologist: Look for work-life balance. You have to be happy with the specialty you pick. In dermatology or pathology, you will do well money-wise, but you're also going to have a good work-life balance, which is one of the most important things you need to consider in going to a particular field.
