Dr. Paul Wischmeyer, MD, EDIC, FCCM, FASPEN is a board certified anesthesiologist and Professor with Tenure at Duke University School of Medicine, currently serving as Associate Vice Chair for Clinical Research in the Department of Anesthesiology and Director of the Nutrition Team at Duke Hospital. His research interests include surgical and critical care nutrition and exercise rehabilitation therapy, parenteral nutrition and personalized nutrition trials, perioperative optimization, post-illness muscle mass and functional recovery, and role of probiotics/microbiome in illness, specifically COVID-19 prevention and treatment. Dr. Wischmeyer has received significant funding from the National Institutes of Health and US Department of Defense as well as numerous awards for his work from national and international societies, including the Jeffrey Silverstein Award and Memorial Lecture for Humanism in Medicine from the American Delirium Society, Fellow of the Society of Critical Care Medicine (FCCM), the John M. Kinney Award for the most significant contribution to the field of general nutrition, and the Stanley Dudrick Research Scholar Award provided by the American Society for Parenteral and Enteral Nutrition where he is also recognized as an honorary Fellow. He has over 200 publications in nutrition, critical care, and perioperative care, including publications in New England Journal of Medicine. He has been an invited speaker at numerous national and international medical meetings, delivering over 1,000 invited presentations in his career and is an advocate and lecturer for improving the patient experience and teaching providers to keep CARE as the focus of healthcare. I'm Stacey Simms and these are the top diabetes stories and headlines happening now XX In the news is brought to you by AG1. AG1 helps you build your health, foundation first. XX Our top story, Tandem's Mobi Pump is approved. The FDA cleared the tiny pump for people with diabetes ages six and up. Tandem says mobie is fully controllable from a mobile app and is the world's smallest durable AID system. It still has a button on the pump to dose and holds 200 units. You use the same infusion set as the current Tandem pumps and Mobi uses Control IQ. Limited release planned for late this year and fully available early 2024. https://www.drugdeliverybusiness.com/fda-clears-tandem-mobi-automated-insulin-pump/ XX Learning more about a newly approved therapy for type 1 diabetes called Lantidra. Two safety and efficacy studies found that 21 participants who took Lantidra did not need to administer themselves insulin for a year or more. Twelve of those participants did not need to take insulin for up to 5 years, and 9 did not need insulin for over 5 years. Lantidra is an allogeneic (donor) pancreatic islet cellular therapy. In other words, Lantidra uses cells taken — or isolated — from human organ donor pancreases. “In [a] subsequent process, the insulin-producing islets are purified from the rest of the pancreatic tissue using a density gradient. Once the islets are isolated, purified, and put for a short time in cell culture, the cell preparation is infused into the liver of the recipient,” he explained. https://www.medicalnewstoday.com/articles/fda-approves-first-cellular-therapy-for-type-1-diabetes-what-does-it-do#How-does-islet-cellular-therapy-work? XX Once weekly basal insulin for type 2 gets through another trial. After 26 weeks of treatment and five weeks of follow-up, patients on weekly insulin icodec had significantly larger improvements in their HbA1C than those using daily insulin. Both groups had an extremely low rate of adverse events. These researches say next step is FDA evaluation. https://www.utsouthwestern.edu/newsroom/articles/year-2023/july-weekly-insulin-found-safe.html XX That's the idea behind Fractyl Health's treatment for type 2 diabetes—which could also be used for weight control. The Lexington, Massachusetts-based biotech company is in the early stages of developing a one-time gene therapy intended to lower blood sugar and body weight using the same mechanism as semaglutide. “You have this problem where you need to stay on therapy for efficacy,” says Harith Rajagopalan, a cardiologist by training and CEO and cofounder of Fractyl Health. “That's the Achilles heel.” The company wants to deliver an artificial gene to the pancreas that continuously produces the GLP-1 hormone so there's no need for weekly injections. The approach, called gene therapy, uses inactivated viruses to carry a therapeutic gene to pancreatic cells. (Viruses are used because of their natural ability to deliver genetic material to cells.) The company is aiming to begin an initial human trial by the end of 2024 https://www.wired.com/story/a-one-time-shot-for-type-2-diabetes-a-biotech-company-is-on-it/ XX Gov. Phil Murphy signed three bills into law Monday aimed at cutting the high cost of prescription drugs in New Jersey. The new laws will cap how much residents will pay for some popular medications such as insulin, prevent pharmacy benefit managers from engaging in practices that make drugs more expensive, and create a panel that will monitor prices set by pharmaceutical companies. One measure (S1614) would cap the cost of insulin, epinephrine auto-injector devices and asthma inhalers for state and local public workers enrolled in the State Health Benefits Program, the School Employees' Health Benefits Program and other state-regulated plans. For a 30-day supply, insulin will cost $35, EpiPens $25 and asthma inhalers would be capped at $50, according to the bill. The agency began its review on July 3 after Iceland's health regulator flagged the reports of patients thinking about suicide and one case of thoughts of self harm after use of Novo's drugs. https://www.reuters.com/business/healthcare-pharmaceuticals/eu-extends-ozempic-probe-include-other-drugs-2023-07-11/ XX We often hear that diabetes can lead to problems with your teeth.. but one researcher says it might be the other way around. A University at Buffalo researchers says there is a notable correlation between chewing functionality and blood glucose levels in people with type 2. Specifically, he found that patients with T2D who maintain a full chewing ability exhibit significantly lower blood glucose levels compared to those with compromised chewing function. The thinking here is that chewing stimulates the body in all sorts of ways, including releasing hormones that help you feel fuller and help in digestion. More studies are expected. The original randomized DCCT clinical trial results, published September 30, 1993, in the New England Journal of Medicine, proved that early intensive glycemic control was the key to preventing or slowing the progression of long-term eye, kidney, and nerve complications of type 1 diabetes. Subsequently, EDIC has yielded many more major findings including that early tight glycemic control also reduces cardiovascular risk and prolongs survival in type 1 diabetes. subsequent EDIC data also have suggested that it is never too late to initiate intensive glycemic control Together, DCCT and EDIC — both funded by the National Institutes of Health at 27 sites in the United States and Canada — have changed the standard of care for people with type 1 diabetes Prior to the DCCT, between 1930 and 1970, about a third of people with type 1 diabetes developed vision loss and one in five experienced kidney failure and/or myocardial infarction. Stroke and amputation were also common, DCCT/EDIC chair David M. Nathan, MD, said while introducing the symposium. "All of the advances in care of type 1 diabetes have developed because this study demonstrated that it was important — continuous glucose monitoring (CGM), new insulins, better [insulin] pumps…I think the most profound finding is that mortality in our intensively treated cohort is the same as in the general population. That says it all," Nathan told Medscape Medical News. https://www.medscape.com/viewarticle/994221 XX Commercial – AG1 XX Today, JDRF, the leading global type 1 diabetes (T1D) research and advocacy organization, urged members of the Senate Appropriations Committee to renew the Special Diabetes Program (SDP) and support policies to ensure insulin is available at an affordable and predictable price. Opened by U.S. Senator Patty Murray (D-WA) and led by U.S. Senators Susan Collins (R-ME) and Jeanne Shaheen (D-NH), the hearing took place during the 2023 JDRF Children's Congress, a biennial event that brings over 160 kids and teens living with T1D face-to-face with lawmakers and top decision-makers. The youth delegates traveled to Washington, D.C. from across the country and JDRF's international affiliates in the United Kingdom, Australia, the Netherlands, and Canada. The SDP has accelerated the pace of T1D research through a long-term investment of funding at the National Institutes of Health. The program has led to significant scientific breakthroughs, including Tzield, the first disease-modifying treatment for T1D, which can delay the onset of the disease by over two years. Without Congressional action, the SDP will expire at the end of September. "The Special Diabetes Program has fundamentally changed what it means to live with diabetes, put new life-changing therapies in our hands, and brought us closer to cures," said Aaron Kowalski, Ph.D., JDRF CEO. "We must keep this momentum going so we can capitalize on the progress to date and realize cures. However, until we have cures, people need access to affordable insulin. JDRF urges Congress to pass the bipartisan INSULIN Act of 2023, which will establish a $35 per month insulin copay cap for people with commercial insurance and includes other provisions that would make insulin more affordable for everyone, regardless of insurance status." Dr. Kowalski was joined at the hearing by Dr. Griffin Rodgers, Director of the National Institute of Diabetes and Digestive and Kidney Diseases at the National Institutes of Health, who provided testimony on the scientific value of the SDP, and Jimmy Jam, award-winning producer, songwriter, musician, member of the Rock & Roll Hall of Fame, and T1D parent. "My son Max, who is now in his twenties, was just two years old when he was first diagnosed with type 1 diabetes. Late nights working on music became late nights working on music and waking up my son to test and manage his blood sugar levels," said Jimmy Jam. "Type 1 diabetes should be one of those things we can all agree on. Diabetes doesn't care if you're a Democrat or a Republican. We can all agree that there should be research to improve the lives of those with diabetes. We can all agree that insulin should be affordable for all who need it." Two 2023 JDRF Children's Congress delegates shared their T1D experience with the committee: Maria Muayad: 10-year-old Maria is from Maine and is a member of her school's civil rights club and math challenge group. Every November, Maria, and her mother, Golsin, give blue ribbons to the staff at her school in honor of National Diabetes Awareness Month. Elise Cataldo: 15-year-old Elise lives in New Hampshire and is passionate about educating those around her about life with T1D and tries to use things like pump site changes and blood sugar checks as opportunities to help others learn. Dougal Glaisher beat the previous record by 27 days. Glaisher was diagnosed with type 1 diabetes at the age of 19 shortly after winning the Oban Sea Kayak race in Scotland, when he had blurred vision and struggled to recover his energy. The expedition was to raise money for the UK-based charity Action4Diabetes and Glaisher livestreamed his blood sugar On Tuesday he was well stocked with a bunch of bananas and a stash of cereal bars lashed to his boat. But in remote areas it was more challenging. Around the islands off Scotland he ran out of freeze-dried meals and survived on porridge for several days. He also sourced drinking water from streams. . Michigan State University Interim President Teresa K. Woodruff elaborates on some of the topics she covers in her May 2023 Spartan Community Letter, which you can read by clicking on the communications tab at president.msu.edu. Could you start by reflecting a bit on commencement? And it was cool to see you with the tiny mic going around Breslin Center talking to some of our grads.“Oh, it was so exciting, and I'd never known that tiny mic was a thing. I got to talk to a lot of students, and the really neat thing was to just see their spirit and their enthusiasm. They were excited for that beautiful graduation day. One of the things I did, Russ, is to ask them about their favorite places on campus, and they ranged from the front of Cowles House to the frog pond south of the tracks. I had a couple of people who told me about the frog pond, which I quite love as well, to places for meditation and places that they had been for their classes. It was a wonderful representation of the students love for this campus and this place.”Placemaking is the theme in this month's community letter. How do you define placemaking?“I'm reading William Beal's book on the founding of MSU, and it really is about place and about that place where people could come and learn. This is a place that remains a natural wonder, and we really want to protect the learning, living environment that is the campus. People gain wholeness and in fact in this last semester, of course, healing out of this campus and our campus places. So, the key for us is to really think about those spaces and places and how the new buildings are situated within that larger ethos of a place that is restorative but continues to evolve, and that's what our campus is doing right now. It has the ability to accommodate the new evolution of the way in which we're thinking and learning and doing while really holding fast and true to that beauty and that living, learning environment where we gather when we walk between those places and spaces.”Talk about the freestanding multicultural center that we touched on last month, and what are some of the other facilities on the horizon?“The groundbreaking for that multicultural center was so exciting with standing room only and a lot of folks pitching in to throw that first scoop full of sand to get that process going. And boy, they're going fast over there. In addition, we have the dairy and greenhouses. Those are really teaching and learning facilities that we must have to enable the very best teaching within agriculture, which is our founding and necessary for the state of Michigan. We also are working on our digital innovation center, or EDIC, and this is a place where we'll rebuild what it means to do engineering and digital innovation, scholarship and learning, and bring six colleges together. It's inverting the model of having individual colleges. We're bringing colleges together in this new model.“Our new greenhouses are housing National Academy of Science members working on the scientific discoveries that could really sustain us and sustain this population into the future. We have the Student Recreation and Wellness Center that's going to replace the IM West facility, and folks are really excited about that on this campus.”MSU's annual research and development expenditures grew to almost $760 million in fiscal year 2022. How are some of those funds going towards placemaking?“These new buildings and facilities are going to house new faculty and students, and it's in those facilities that we'll be able to continue our upward trend and upward reach to the heights of research expenditure across not only the Big Ten, but literally in the nation.”Our placemaking extends beyond the East Lansing campus to places like Flint, where you recently visited.“I had the greatest time in Flint, and I really enjoyed that Norm Beauchamp and Aron Sousa were with me when I visited our Charles Stewart Mott Foundation work that's happening there with Mona Hanna-Attisha. We are building a new community partners and research facility there that's really exciting. I met some of the kids who are in that Flint downtown area and they are really excited about Michigan State University. I think I admitted a couple of six-year-olds or maybe even one that's about four years old. So, just full disclosure, Russ, we've got some Flint kids that are coming to MSU in 2042, and I'm already excited to welcome them.”We have the very first philanthropically named department in MSU's history with the Charles Stewart Mott Foundation Department of Public Health.“Public health is so critical as we saw through the COVID context and will continue to have an important way of enlivening the research we do. And particularly in Flint, we're really thoughtful about public health not just being about what MSU researchers think we ought to do and then go do. This is a completely different model where during my visit we had all our community partners who are active participants in the work, folks who then say, ‘Well, this is what we need to know.' In many ways, what we're doing in Flint in health is what we've done in Extension in agriculture. We've really asked for and then developed the partnership that enables the best kind of thinking and therefore the best kind of work, and it's that positive productive cycle that is so exciting in Flint and across all of Michigan.”You also recently had some opportunities to recognize and celebrate the achievements of our amazing staff and faculty.“The key for me is that Michigan State has been celebrating its employees for nearly half a century. This is not a new one off. This is a place that really values every single individual who is part of this whole ecosystem, and I don't think there's anyone who thinks they themselves are above and beyond each other. Whatever part we play within this orchestra that is Michigan State University, we play our parts as well as we can. And in the end, it's a beautiful symphony.”Ensuring the safety of university community members is another vital element of MSU's placemaking for wellbeing. You were recently pleased to welcome a new vice president for civil rights and Title IX education and compliance, Laura Rugless. Tell us about Laura and her important role.“Laura comes from Cornell. She's a veteran and brings an incredible sense of the ways in which we need to focus on prevention and the ways we respond to actual discrimination and sexual violence and misconduct. The folks that I talked with think of her as an ethical people-driven leader. I shared with her this morning that on my computer is a little yellow sticky that says, ‘Today is my favorite day.' I offered to her that every day at MSU is my favorite day, no matter what comes. And I offered that hopefulness and that positivity to her. She embraces MSU, and I'm excited for what her leadership will bring to campus.”You have a bit of a travel advisory for Spartans and others visiting camping this summer.“We've got some traffic detours. As you traverse the campus, you'll have to make some detours, but it's really wonderful. I really like the terrestrial cranes. There are a lot of cranes that are building some of our new buildings, but you'll also see cranes going overhead as we did over graduation; they were headed north. Cranes in the air and cranes on the land really represent the natural and physical beauty of this campus. I invite people to come in this summer. Come to Summer Circle Theatre. Come to just walk in the Beal Garden. Come to see the frogs in the frog pond on the south side of the tracks. There are just so many great places to be and to be present within this great and storied institution.”Results of research you and your husband Tom O'Halloran worked on were recently reported in the Journal of Biological Chemistry. The research looked at the role of zinc in follicle development and how scientists use X-ray beams to determine the role of zinc in development of ovarian follicles. “It represents some profound biology. Before Tom and I collaborated - he was an inorganic chemist, and I'm a reproductive scientist - we collaborated on an area no one had ever thought to look at. We found that the egg, just before fertilization, takes up 20 billion zinc atoms. If it doesn't take up that amount of zinc atoms over a 12-hour period of time, it can't progress to fertilization and then onto an embryo. At the time of fertilization, the zinc is exported from the egg in this big explosion we call the zinc spark. “That zinc spark means that the subordinate sperm cannot actually then come in and have what's called polyspermy. Nobody knew any of this before that work. Tom and I with my last graduate student, Alison Tange, did some work to look at the very earliest stages of follicle development. We used one of the most sophisticated microscopes in the world at Argonne National Labs that Tom actually helped build in order to really look at the zinc and all the other metals in these early follicles. It really was the perfect bridging of new technology and this biology. Again, nobody would've looked for these signatures of life except for that interaction, and it really is just some of the most exciting discoveries that basically happens to all of us at the first moment of conception. I'm just really excited that it has been published.”What are your final thoughts as we really dive into the summer.“I'm excited for everyone to come back to this great campus. The Red Cedar is beautiful, and the trees are in full bloom. During graduation, I think every tree on campus was in peak. MSU is a wonderful place to live and work. This video honors Paul Wischmeyer, MD, EDIC, FCCM, FASPEN, Professor of Anesthesiology and Surgery, in the division of Critical Care Medicine at Duke University in Durham, NC. Dr. Wischmeyer's nutrition journey began in high school when he was diagnosed with inflammatory bowel disease and was on parenteral nutrition for a year. Many of the medicine he was given had severe side effects. It motivated him to get into medicine and do research on alternative treatments. Dr. Wischmeyer's first grant was from the ASPEN Rhoads Research Foundation; it set him up for his NIH K Award. Dr. Wischmeyer provides tips for finding mentors in clinical nutrition, talks about an online nutrition fellowship at Duke, and shares what he thinks are the pressing questions in clinical nutrition. Big issue for a popular app – users of Sugarmate have been told as of this week – November 4th to be precise – customers in the US will lose connection. Those outside the US have seen issues since October. This is because of a change Dexcom made to its servers. Sugarmate says Dexcom is working with them to fix the issue – basically they're going to join the Dexcom Partner API – we've told you about that, it's how third party apps can talk with Dexcom.. In the meantime, Nightscout is probably the best alternative if you use Sugarmate. Quick note: Tandem acquired Sugarmate last year. And you'll recall that Dexcom does own a small piece of Tandem. Researchers at the University of Pennsylvania created a timing mismatch by altering the function of a molecule within the brains of mice.. shortened their circadian rhythms from 24 to 21 hours. These mice gained more weight, had higher blood sugar, and fattier livers. This all corrected when the researchers changed their environment – sleep and meals – to match that shorter, 21 hour day. This is the POGO … with 10-test cartridge technology. The strips and lancers are loaded inside already, so you don't carry anything separate. You just put your finger down and press the button. They're calling this automatic blood glucose monitoring or ABGM. On the inside it's still a basic finger stick and blood collection. But you don't see any of that on the outside. Of course, there's a Bluetooth connected app for you and your healthcare team to use. The IDF says more than 10-percent of adults worldwide live with diabetes.. by 2045 that number will be one in eight. The report also says that one in two people with diabetes across the world who need insulin cannot access or afford it. The theme of World Diabetes Day this November 14th is Access to Diabetes Care. https://www.prnewswire.com/news-releases/diabetes-is-a-pandemic-of-unprecedented-magnitude-now-affecting-one-in-10-adults-worldwide-reveals-the-international-diabetes-federation-301413238.html XX Good news for people with type 1.. when more intensive glucose management starts early, it greatly reduces the future risk of heart and kidney issues. This info comes from a look back at the DCCT and EDIC trials – which are 100% worth looking into if you aren't familiar with them. By the way, in these trials “intensive” glucose control was pegged at an A1C of 7 and the riskier group had an A1C of 9 or above. It's hard to say but a new study from Scotland suggests it's more common than we might think. These University of Edinburgh scientists say in Scotland, it's one in 20. They looked at everyone in the country over the age of 30 with type 2, based on A1C levels -that's about 160-thousand people. Then they said during the study year, 77-hundred people went into remission, which means their A1Cs dropped to 6.5 without medication. Those people were older, had lost weight since their diagnosis, had no history of glucose lowering therapy or bariatric surgery, and generally had healthier blood readings at the time of their diagnosis. https://www.sciencealert.com/reversing-type-2-diabetes-seems-to-be-more-common-than-scientists-realized XX College scholarship contest to tell you about. Amanda me contou esses dias que o pai dela tem uma horta. A tradição começou com o avô que, todo fim de domingo, levava os filhos para pegar pé de alface, de couve e um punhado de mandioca. A horta é um dos jeitos do pai dela se conectar com quem foi o avô e com o que ele deixou. Ela estava me contando isso, quando me falou de um imprevisto grande que aconteceu numa plantação de alface que o pai dela fez e isso que era só sobre algo muito distante da minha realidade, virou um aprendizado fundo sobre controle, ansiedade e esperança. Poucos aspectos da história norte-americana são tão opacos quanto a história do Partido dos Panteras Negras. Sua incrível trajetória de ascensão e queda, e o encerramento definitivo das atividades, em 1982, de forma praticamente anônima, vem sendo disputada e fixada por duas narrativas principais: a primeira narrativa, sustentada pelo FBI e repercutida pela cobertura jornalística, minimiza os Panteras Negras a uma ameaça à segurança nacional.•Já a segunda narrativa é sustentada por todos aqueles associados aos estudos afro-americanos, e vincula os Panteras Negros a um projeto de busca da unidade e do orgulho racial. •As duas narrativas são redutoras da complexidade sobre as contribuições, convicções e lutas dos Panteras Negras, a organização militante de poder negro mais influente da sua época. Embora haja tantas referências icônicas aos Panteras, que o diga a saudação de punhos erguidos e o imperativo de Black is Beautiful, ninguém mantém ou deve reivindicar o seu espólio, principalmente os promotores da chamada 'thug life', tão sedentos em atrelar o legado dos Panteras Negras a um sistema comercial e capitalista, tão distante da agenda e crenças do partido. •O Partido dos Panteras Negras lançou mais de 35 Programas de Sobrevivência e forneceu ajuda à comunidade, como educação, teste de tuberculose, abrindo clínicas de saúde gratuitas em uma dúzia de cidades atendendo milhares de pessoas que não podiam pagar, além de assistência jurídica, assistência de transporte, serviço de ambulância e a fabricação e distribuição de calçados gratuitos para pessoas pobres.•É fácil relacionar o poder da organização ao vertiginoso crescimento de uma estrutura que se mostrou tão organizada quanto o Partido dos Panteras Negras. Essa vai ser a nossa última etapa percorrendo as várias faces da Antropologia como ciência, explorando aquilo que alguns autores chamam de “seus sub campos”. Depois de termos refletido sobre a Antropologia Física (ou Biológica), a Antropologia Arqueológica e a Antropologia Linguística, chegamos ao cerne do tipo de Antropologia que é mais popular e conhecido: antropologia social ou antropologia cultural. (Ou, Como prefere uma parte considerável dos antropólogos, antropologia sócio-cultural.) Esta seria uma quarta identidade da Antropologia, que pensa a produção humana a partir de duas realidades que são irmãs-gêmeas: a sociedade e a cultura.
