Neste episódio, conversamos com Fabíola Sucasas, promotora de Justiça do MPSP, sobre violência de gênero e a Lei n. 14.245/21. Em novembro de 2021 entrou em vigor a Lei Mariana Ferrer, cujo objetivo é coibir a prática de atos atentatórios à dignidade da vítima e de testemunhas de crimes. As imagens da audiência de instrução demonstram como a vítima passou por cenas de violência psicológica durante o ato processual, motivando as normativas estabelecidas neste conjunto legislativo. Enquanto juiz, promotor e defensor público se omitiam, o advogado de defesa do réu ofendeu por diversas vezes a honra da vítima, tentando desqualificá-la, apresentando fatos e provas alheias aos autos. Apesar do juiz repreender o advogado por algumas vezes, ele permitiu que as ofensas continuassem. As imagens levaram o CNJ a instaurar procedimento para investigar a conduta do magistrado. Por sua vez, a OAB absolveu o advogado do réu na seara administrativa disciplinar. Tais eventos nos fazem refletir se o Poder Judiciário está de fato preparado para cuidar dessas mulheres vítimas de violências sexuais. A Justiça deve ser um local de acolhimento para a mulher e a vítima tem que se sentir segura ao buscar ajuda. Os dados do Anuário Brasileiro de Segurança Pública registraram quase 75.000 estupros em 2022, o maior número da história. Vamos refletir se o sistema de Justiça está plenamente estruturado para ser efetivamente local de acolhimento e não de revitimização. Em como evitar que esses casos de humilhação desestimulem as vítimas a denunciar seus agressores. A advogada Daniela Teixeira (foto) foi indicada nesta terça (29), pelo presidente Lula, para uma das vagas abertas no STJ (Superior Tribunal de Justiça). Giriş Yaşamsal bulgular acil serviste veya yoğun bakımdaki kritik hastaları belirlemede en önemli göstergelerdir ve vücut kompansasyon mekanizmaları yetersiz kaldığı zaman değişmeye başlar. Bu mekanizmaların devreye girmesinden önce bozulmaya başlayan doku perfüzyonunun durumunu hızlı, non- invaziv bir şekilde belirlemeyi amaçlayan yöntemler (brakial indeks, torasik impedans gibi) üzerinde çalışmalar yapılmaktadır. Bu nedenle doku perfüzyon durumunun hızlıca değerlendirilmesi; acil ve kritik hastaları öngörmede yön göstericidir. Günümüzde yüksek enerjili travma, şok, sepsis gibi mortalitesi yüksek durumların prognoz tahmininde “Şok İndeksi (Şİ)” kullanılmaktadır. Şİ'nin prognoz tahmininde yalnızca sistolik kan basıncı (SKB) ya da yalnızca nabız ölçümüne üstün bulunduğu birçok çalışmada gösterilmiştir​1​. Şİ hesaplarken hastanın diyastolik kan basıncının (DKB) hesaba katılmaması bir noksanlık olarak değerlendirilmiş ve “Modifiye Şİ” geliştirilmiştir. Daha sonra yaşlı hastaların komorbiditeleri ve kullandığı ilaçlar nedeniyle nabız ve kan basıncı değerlerini etkilediği öne sürülerek “Yaş Şİ” tanımlanmıştır. Girişte bahsettiğimiz bu şok indekslerini genel bir bakış olarak ele alacağız. Şok İndeksi(Şİ) Şİ, kalp atım hızının SKB'ye bölünmesiyle elde edilir. Sağlıklı erişkin bireylerde normal değerleri 0,5-0,7'dir ve hemodinamik stabiliteyi gösterir​2​. Travma, sepsis, kanama gibi sol ventrikül (LV) atım hacminin azaldığı durumlarda artmaktadır​3​. Tedaviye rağmen Şİ'nin yüksek seyretmesi LV fonksiyonunun kötü olduğunu göstermektedir. LV iş yükü, LV preload, afterload, diyastolik kompliyans ve sistolik kontraktilitesinden etkilenmekte olup bu faktörlerden herhangi birinde ya da birkaçında kötüleşme olması Şİ'nin artmasına katkıda bulunmaktadır​4​. 1994'de, Rady ve arkadaşları, Şİ ≥ 0,9 olan hastaları triyajda kritik hasta olarak değerlendirmişler. Bu grupta hastane yatış oranlarının ve yoğun bakım gereksinimlerinin daha yüksek olduğunu göstermiştir​4​. Acil servise başvuran kritik hastalıkların erken tanınması ve değerlendirilmesinde, Şİ'nin önemli bir belirteç olabileceği gösterilmiştir. Ayrıca klinik izlem esnasında, resüsitasyon sırasında ve sonrasında yol gösterici olabileceğini göstermektedir​4​. Modifiye Şok İndeksi Şİ hesaplanırken sadece SKB kullanılır, ancak hastanın klinik ciddiyetini belirlerlemede DKB da yadsınamaz bir önemi vardır. Liu ve arkadaşları, her hastalık grubundan hastaların AS'ye başvurduğunu belirterek SKB'ye ek olarak DKB'nin de kullanılmasının, bu hastaların ciddiyetinin tahmininde hayati önemi olduğunu bildirdiler. Bu nedenle DKB'nin de dahil edildiği ve kalp atım hızının, ortalama arteriyel basınca (OAB) bölünmesiyle elde edilen modifiye şok indeksi (MŞİ) geliştirmişlerdir​5​. OAB= ((DKB×2) + SKB)/3 MŞİ=    Nabız / OAB OAB değeri doku perfüzyonunun en iyi göstergelerinden biri olup, MŞİ'yi hesaplarken OAB'nin kullanılması, MŞİ'yi daha anlamlı kılmakta ve mortalite tahmininde daha etkili bir belirteç yapmaktadır. Son yıllarda yapılan pek çok çalışmada, MŞİ'nin Şİ'ye kıyasla AS şartlarında kullanılmasının prognoz tayininde daha uygun olduğu bildirilmiştir. MŞİ, acil servis hastalarının kan basıncı ve nabız hızına göre mortalitelerinin erken dönemde fark edilmesinde çok daha kullanışlı bir belirteçtir. Singh ve arkadaşlarının yaptığı retrospektif bir çalışmada, DKB'nin de hastaların klinik durum ciddiyetinin bir göstergesi olarak göze alınması gerektiğini ve MŞİ'nin 1,3 olması durumunda daha yüksek mortalite oranlarına sahip olduğunu göstermişlerdir ​6​. Yaş Şok İndeksi Yaş ile beraber şoka karşı verilen sistemik yanıt değişebilmektedir. Özellikle geriatrik populasyonda kullanılan çoklu ilaçlar, sistemik fizyolojik yanıtın değişmesi ve sahip olunan ek hastalıklar şok yanıtını etkileyebilmekte ve Şİ'nin yanlış hesaplanmasına neden olabilmektedir. Conservative measures include pelvic floor exercises, diet, and lifestyle changes. If patients fail conservative measures, urologists can prescribe anticholinergics. However,beta agonists are preferable because they do not cross the blood-brain barrier. Then. the doctors briefly chat about the use of these agents in Alzheimer disease patients as well, which may be dependent on insurance approval. Next, Dr. Sutherland summarizes surgical devices to treat OAB and the clinical trials supporting each of them. Sacral nerve neuromodulation is a device that has been on the market for years, but newer models that include fixed and rechargeable batteries are starting to become more available. Additionally, the Neuspera is a sacral nerve neuromodulation that operates wirelessly through Bluetooth. The doctors also compare the two existing models of sacral neuromodulation (Medtronic vs. Axonics). One newer surgical treatment is tibial nerve stimulation, and Dr. Sutherland summarizes her placement technique, voltage settings, intraoperative testing, and the importance of a healthy ankle evaluation prior to placing the device. promising results, in-office, sedation, healthy ankle evaluation. Born in Rio de Janeiro, Rafaela Garreta knew early on that she would pursue a career in law. She came from a Christian family and received an education that was based on the values of justice and equality. She was raised in a highly educated environment and attended top-notch schools, including the Military-Fundamental School and the Technical Course in Accounting and Administration, where she received the support she needed to become an excellent professional. There will always be a family working to develop the profession behind a person of brilliance. Rafaela Garreta made the decision to pursue a career in law and started her academic studies at Estácio de Sá in Rio de Janeiro in 2001. She is successful in obtaining her title granted by the OAB in 2006, which gives her permission to practice law and launch her own legal company in RJ. Rafaela Garreta established her legal profession in Brazil. She started her studies in the US in 2007 and graduated from Northeastern University Law School with a master's degree in American and international law.
