A new research paper was published in Oncotarget's Volume 14 on March 11, 2023, entitled, “Genomic landscape of metastatic breast cancer (MBC) patients with methylthioadenosine phosphorylase (MTAP) loss.” Homozygous deletion of methylthioadenosine phosphorylase (MTAP) upregulates de novo synthesis of purine (DNSP) and increases the proliferation of neoplastic cells. This increases the sensitivity of breast cancer cells to DNSP inhibitors such as methotrexate, L-alanosine and pemetrexed. In their recent study, Maroun Bou Zerdan, Prashanth Ashok Kumar, Elio Haroun, Nimisha Srivastava, Jeffrey Ross, and Abirami Sivapiragasam from SUNY Upstate Medical University and Foundation Medicine, Inc. analyzed 7,301 metastatic breast cancer (MBC) patients that underwent hybrid-capture based comprehensive genomic profiling (CGP). “We provide one of the first large analyses of the spectrum of GA [genomic alterations] occurring in MTAP deleted MBC with the hope that this would enable identifying potential therapeutic agents in the future.” Tumor mutational burden (TMB) was determined on up to 1.1 Mb of sequenced DNA and microsatellite instability (MSI) was determined on 114 loci. Tumor cell PD-L1 expression was determined by IHC (Dako 22C3). 208 (2.84%) of MBC featured MTAP loss. MTAP loss patients were younger (p = 0.002) and were more frequently ER− (30% vs. 50%; p < 0.0001), triple negative (TNBC) (47% vs. 27%; p < 0.0001) and less frequently HER2+ (2% vs. 8%; p = 0.0001) than MTAP intact MBC. Lobular histology and CDH1 mutations were more frequent in MTAP intact (14%) than MTAP loss MBC (p < 0.0001). CDKN2A (100%) and CDKN2B (97%) loss (9p21 co-deletion) were significantly associated with MTAP loss (p < 0.0001). Likely associated with the increased TNBC cases, BRCA1 mutation was also more frequent in MTAP loss MBC (10% vs. 4%; p < 0.0001). As for immune checkpoint inhibitors biomarkers, higher TMB >20 mut/Mb levels in the MTAP intact MBC (p < 0.0001) and higher PD-L1 low expression (1–49% TPS) in the MTAP loss MTAP (p = 0.002) were observed. “MTAP loss in MBC has distinct clinical features with genomic alterations (GA) affecting both targeted and immunotherapies. In this episode, we discuss a relatively recent paper from Jeff’s lab that illustrates the pros and cons of planning authorship lists in advance. We have an honest discussion about publishing in “high impact” journals (please note the ironic air quotes) as a pre-tenure professor and how his lab members made deliberate trade-offs in work-life balance to get experiments done quickly. Sesamoiditis - pain under the big toe joint, especially in runners and kickers. Morton's neuroma - pain and numbness between the third and fourth toes caused by a nerve entrapment. Metatarsalgia - pain under the ball of the foot. Arch Injuries Plantar fascia strain - pain along the inside of the arch. Stress fractures - frequently appearing in athletes and those with C-curved feet. Heel Injuries Heel spur - irregular bone formation resulting from arthritic change. Tendonitis is also common throughout the foot from overuse. Shoes are the base of support for your entire body. Stilettos don't have the infrastructure to support you. Stilettos also shorten the calf muscle and Achilles tendon. Ballet flats can also be troublesome because the support isn't there. It's best to wear a running shoe for exercise, even if you're just walking. Running shoes are more supportive. You might need insoles to provide extra support. Stretch your feet by sitting with your legs in front of use. Use a towel or band to gently pull the top of your feet toward you. Lie back, bend the knee and gently pull your foot back again. Extend the leg and use the band with each foot to stretch your leg muscles. Stand with your back to the wall and lift your toes a few times. Turn around and raise your heels for a few repetitions. Veteran saxophone player, Paul Sawtelle, has been paying his dues as a musician for over 30 years and he’s enjoyed every minute of it. As a top-shelf arranger, big band leader, funk, rock and blues band sideman, in-demand session player and featured soloist on scores of studio projects and live shows. He’s played for President Clinton, worked with Heart, opened for artists from Savoy Brown to Jonny Lang, appeared on national television shows and, in 2008, won a prestigious Spelleman Award, the Norwegian equivalent of The Grammy, for his horn arrangements and playing on Sony Music’s extraordinary blues band, Orbo and the Longshots album. When asked about the first music that inspired him as a player, he didn’t hesitate. “The first Chicago Transit Authority album had a tremendous impact on me. The mixture of rock and horns was inspiring and fresh. Seeing that band for the first time live was magical.” He also couldn’t get enough of other groove and horn based bands and players like Tom Scott, Tower of Power, Janis Joplin, Moby Grape, Blood, Sweat & Tears, David Sanborn, Lenny Pickett, Grover Washington, Jr. & Stanley Turrentine. The CD was recorded during the fall of 2014, at the legendary London Bridge Studio in Seattle with producer/engineer Geoff Ott. Fortunately they work well together. Jeffrey likes vintage records and knows every store that sells vinyl in a 50 mile radius. He has also figured out 5 different ways to sleep on an airplane and is considering writing a travel book about that hard won knowledge.

In the modern era of roasting, made popular by the Comedy Central Roasts, comedian Jeffrey Ross has taken on the symbolic title of Roastmaster General and run with it at a blazing speed. Never afraid of going too far (as long as the targets are in on it, of course), Ross has made a living roasting the likes of Hugh Hefner, William Shatner, Pamela Anderson, and Charlie Sheen. Besides the television appearances, however, he's performed extensively for U.S. troops overseas and appeared on many TV shows (CSI, Children's Hospital, The Sarah Silverman Program). Of course, a stand-up comic's real work is hitting the road, and Ross has been doing just that with his Jeff Ross Roasts America tour beginning last year.
