Show Notes To celebrate Pet Dental Health Month, the Skin Flints team looked a bit further afield this month, exploring gum health and Canine Chronic Ulcerative Stomatitis with Hannah van Velzen. Chapter 1 – Understanding the Oral Mucosa and Inflammation (02:53) John welcomes Hannah, who introduces herself and her journey into veterinary dentistry, from her studies in the Netherlands to her current role leading the dentistry referral service at Fitzpatrick Referrals. She highlights the small but growing number of veterinary dentistry specialists in the UK. (05:46) Sue asks for a basic overview of the oral mucosa, as it plays a key role in CCUS. Hannah explains that gingiva surrounds and seals the teeth, preventing bacteria from entering the body, while mucosa covers the rest of the mouth. The mucogingival junction marks the boundary between the two and helps differentiate between gingivitis and mucositis. She describes the different types of mucosa, including lingual (tongue), palatal (roof of the mouth), alveolar (bone covering), vestibular (cheek and lip folds), buccal (cheeks), and labial (lips). These structures vary in thickness and function, with keratinized areas like the tongue and hard palate providing protection, while thinner, non-keratinized areas aid in saliva flow and bacterial clearance. (13:24) John then asks Hannah to define common inflammatory conditions affecting the mouth, including: Gingivitis – Inflammation limited to the gingiva, without mucosal involvement. Mucositis (stomatitis) – Inflammation affecting the mucosa, which is central to CCUS. Periodontitis – Inflammation of the structures supporting the tooth, which can lead to tooth loss. Hannah emphasises the importance of accurately defining oral lesions to guide diagnosis and treatment.    Chapter 2 – What is CCUS? How Can It Be Diagnosed? (18:43) John introduces Canine Chronic Ulcerative Stomatitis (CCUS), asking how it relates to previous terms like CUPS (Canine Ulcerative Paradental Stomatitis) or contact mucositis. Hannah explains that CCUS was formerly known as CUPS, but the name changed as research showed that 40% of lesions occurred in areas without teeth, making the term "paradental" inaccurate. The condition is chronic, meaning it develops gradually rather than suddenly. (23:22) Sue asks how a primary care vet should determine whether a dog with oral ulcerations has CCUS or another condition, such as pemphigus vulgaris, lupus, or uremic stomatitis. Hannah acknowledges that many inflammatory and autoimmune diseases look similar and that no single exam finding confirms CCUS. She advises vets to follow key diagnostic steps: Perform a thorough dental cleaning and radiographs to rule out periodontal disease. Differentiate gingivitis (gum inflammation) from mucositis (mucosal inflammation). Take a biopsy if mucosal inflammation is present, as periodontal disease should not cause mucositis. Look for "lymphoplasmacytic infiltrates" on biopsy, which strongly suggest CCUS. If the biopsy findings suggest CCUS, referral to a dentistry specialist is recommended. If results are inconclusive, a dermatologist may need to investigate potential autoimmune conditions. (27:33) Sue asks whether "kissing lesions" (ulcerative lesions where mucosa touches the teeth) strongly indicate CCUS. Hannah agrees that they are a key sign, but notes that plaque build-up can also cause similar inflammation. A dental clean should be performed first—if inflammation persists despite clean teeth, CCUS is more likely. (28:31) Sue then asks if certain breeds are predisposed to CCUS. Hannah confirms that small breeds and terriers are overrepresented, particularly: Cavaliers, Labradors, Maltese, and Greyhounds. Greyhounds are prone due to poor dental health and periodontal disease. Spaniels may also be affected, though this is not yet confirmed in literature. Some affected dogs have severe gingivitis and mucosal inflammation despite excellent dental hygiene, making CCUS harder to recognise. (31:33) John asks how easy biopsies in the mouth are Hannah stresses that biopsies should always be done under general anaesthesia for pain control and a thorough oral exam. She typically uses a punch biopsy, ensuring a portion of normal tissue is included to help distinguish inflammatory from autoimmune causes. She highlights the importance of sending clear photos and case details to assist pathologists in interpreting results. Additional tests like immunohistochemistry may sometimes be useful.   Chapter 3 – Treating CCUS: What Are the Options? (35:44) John asks how CCUS is treated and whether treatment varies by severity. Hannah explains that CCUS treatment is multi-step and includes: Dental Cleaning & Plaque Management: Full dental cleaning is the first step. Extractions are considered only for teeth that contribute to inflammation. In mild cases, cleaning + home care (brushing, antiseptics) may suffice. Home Management & Pain Control: Some owners can maintain oral hygiene, others cannot. Pain relief options include NSAIDs, paracetamol, gabapentin, or amitriptyline. Feeding tubes may be used in extreme cases for pain-free nutrition. Medical Management for Severe Cases: Two main protocols exist: Cyclosporine + Metronidazole (immune modulation & bacterial control). Doxycycline (low dose), Pentoxifylline (ulcer management), and Niacinamide (vitamin B3). The choice depends on vet preference and patient response. Long-Term Management & Research Gaps: Some dogs may eventually stop medication once inflammation is controlled. More research is needed to determine which cases respond best to which treatments. Avoiding full-mouth tooth extractions remains a key goal. (45:14) Sue highlights the lack of published research on CCUS and urges vets to seek specialist advice before extracting all teeth.

Deine Haut ist ein Donut – wusstest du das? Wie ein Donut geht deine Haut von äußerer Haut über in die innere Haut. Und die innere Haut nennt man Schleimhaut. Und es wird ja mal sowas von Zeit, dass wir uns bei Zauberhaut mal die innere Haut anschauen. Und wow, gibt es viele Schleimhäute in unserem Körper: Nasenschleimhaut, Mundschleimhaut, Magenschleimhaut, Harnblasenschleimhaut und viele mehr. Alle Schleimhäute zusammen haben eine Gesamtoberfläche von 600 Quadratmeter – das sind mehr als zwei Tennisfelder zusammen. Sie heißen Schleimhäute, weil sie Schleim produzieren, der dazu dient, die Oberflächen zu befeuchten und zu schützen. Vor Fremdkörpern, Krankheitserregern oder Reibung durch unsere Bewegung. Einige Schleimhäute nehmen aber auch Nährstoffe auf oder übertragen Sinnenreize. Jede Schleimhaut hat spezifische Funktionen – und auch Bedeutungen. Ich konzentriere mich in der Folge auf die Augenbindehaut und Konjunktivitis, die Mundschleimhaut und Entzündung dessen (Gingivitis) und die Nasenschleimhaut und Rhinitis. I never thought that I would get diagnosed with gingivitis + struggle with horribly painful, bleeding gums. But after months of intense stress, my mouth was really struggling at the end of 2022. After what was one of the most painful dental cleanings of my entire life, the dentist explained that I had pretty bad gingivitis with pockets forming around my teeth. Plus, a really sensitive spot on a tooth + weird burning spots on my gums that he couldn't explain. After this tear-filled experience, I promised my dentist to turn things around. Since many of you know from other episodes of the Healthy Skin Show that the health of your oral microbiome is equally important to that of your gut, I want to share what helped me reverse gingivitis + bleeding gums. Here's what I did AND the products that helped me! In this episode: How I developed bleeding gums + gingivitis What symptoms led to this diagnosis (+ what the dentist recommended) What steps I took to reverse bleeding gums The products that I used to treat my gingivitis What my dentist said at my six-month follow-up visit (amazing!) ORN Dr. Al Danenberg show notes 11/8/22 Author of Eat As If Your Life Depends On It, plus other books. Dr. Danenberg.com Gum disease (periodontal disease) is 2 infections. Gingivitis is infection is In the gum tissue. Is very reversible, no bone infection. Can advance to periodontitis, which is more aggressive. Mix of different types of bacteria, which get under the gum, causing damage to the jawbone, loosening and then loss of teeth. Infection in the mouth generally starts in the gut. Due to emotional distress, diet, damage to the garden of bacteria. Immune system becomes dysfunctional, infection spreads through circulatory system. Bacteria in the mouth become overgrown with pathogenic types which cause tooth decay. It's like a splinter in your finger that can become a systemic infection. Local irritants in mouth and gut issues lead to dental decay. The mouth and body professions don't talk on the same plane. Their interplay is not taught in medical or dental school. Root canals and mercury fillings still being taught. Sugar is main factor in causing tooth decay. But damage from it requires a pathological situation in the garden of bacteria. Sugar feeds the bacteria, causes acid release, leads to tooth decay. Immune system controls balance of bacteria. If overgrown, bacteria cause problems, but if not overgrown, they serve a purpose as a balance. Don't want to kill off the oral microbiome. Bacteria in dental plaque is thought of needing to be destroyed. Rather, it has a purpose. Are several hundred microbes that produce peroxide, which kill potentially pathogenic bacteria from getting under gum. Has buffers that control acidity to keep it more than pH of 5.5. Is a gatekeeper allowing minerals to penetrate the biofilm of dental plaque to get to tooth root and remineralize. Worse thing you can do is use antimicrobial toothpaste and mouthwashes. They destroy a healthy biofilm. Looking at skulls of archeological remains, find calcified dental plaque, but no dental decay. Calcified dental plaque is protective if its not overgrown with pathological bacteria. Is teeth cleaning needed? It's not medically necessary to remove excess tartar. Doesn't recommend hydrogen peroxide or silver for prophylactic use. Dr. Danenberg was diagnosed in 2018 with multiple myeloma, a bone marrow cancer. Believe it started by exposure to mercury and low dose ionizing radiation in his dental practice. Investigated lifestyle changes. Started with more strict animal based diet, mostly carnivore by Jan 2020. Treated at Intl Center for Medical Nutritional Intervention in Budapet, Hungary. No chemo. Eats a Paleoketogenic diet and is in ketosis most of the time. Liver produces ketones when there is not enough carb available. Ketosis is when level of ketones improves healing and antioxidant potential. What you're not eating is more important than what you're eating. Retired dentist calls in. Says Dr. Huggins found that sugar reverses flow in the tooth tubules from an outward direction to inward from the mouth. Listener asks: Can a tooth ever be healed or fixed if it's cracked? No, the tooth will not self-heal. Usually a crown or extraction is required. Zirconia in implants don't have the immune response that titanium does. Titanium implants like in the femur can create Inflammatory response around the screw that causes dissolving of the bone. Can't rebuild enamel but can remineralize it. Use baking soda paste made with salt water applied with a toothbrush. Will carnivore diet heal leaky gut? Absolutely. But it must be one that eliminates plants and includes organs. Is pate a good source of Vitamin A? Beef is good. Pork and chicken pate are fed products of soy or corn, which have high linoleic acid that oxidize when eaten in excess, which damages fat cells and mitochondria.
