In this episode Prof Brett McDermott is in conversation with A/Prof Yoram Barack about the Kioku project. This study is looking to test new ways to reduce loneliness. Loneliness has an incredibly negative impact on health, especially so for older adults. Rates of loneliness are creeping up in most high income and middle income countries, especially for women and men over the age of 70. Based on some research that was developed and tested in the early 2020s and this is the beginning of a journey of developing games in which we share personal true stories with each other and reduce loneliness. A/Prof Yoram Barak's research interests include multiple sclerosis, cancer, a wide range of psychiatric conditions (such as depression, schizophrenia, and obsessive–compulsive disorder), old-age psychiatry, and suicide. He is also interested in Alzheimer's Disease and has published a popular book called "Preventing Alzheimer's: A Personal Responsibility".Prof Brett McDermott is an Australian medical graduate who trained in Psychiatry, and Child and Adolescent Psychiatry in the UK and Sydney. Apart from being a Professor at the University of Tasmania, he holds other academic appointments: By-Fellow at Churchill College, Cambridge University; Adjunct Professor at JCU; Professorial Fellow at Mater Research and from 2006-2016 was a Board Director of beyondblue: the National Depression Initiative. I was honored that one of my readers asked for my assistance in evaluating the health claims made by products referred to as “Hormonal Teas”. The specific advertisement she referenced was from a company called “Avery Apothecary” and the product is “Goddess Tea Blend.” The product makes several claims on their website: “Ladies, enjoy a cup of this tea every day and the synergy of the plants help to restore and keep balance in your cycle. Regular health and hormone upkeep can help prevent issues in the future, can help to ease symptoms of PMS and any pain associated with your period. When you experience symptoms related to your cycle is not the time to fix the issues, an ounce of prevention is worth a pound of cure! Enjoy this blend daily to support your hormones all cycle long.” (link)The website does have a pretty large disclaimer on the page. Which on one hand, is reassuring that they are being transparent and honest about the limitations of the data supporting the claims. But on the other hand, doesn't demonstrate a high level of confidence that the claims are supported by research. Here is the disclaimer: “Disclaimer: These products have not been evaluated by the FDA. The information we provide is intended for educational purposes only and is not meant to represent or replace the advice of a medical professional or to diagnose, treat or cure any disease. It is the responsibility of the customer to test products for allergies before use of products. The ingredients are listed on each product and we do our best to eliminate cross contamination between products. Some people will have allergies to certain plants or botanicals so please test before use to ensure you do not have an allergy. Stop using immediately if an allergic reaction, redness or irritation occurs. We are not responsible for the misuse or reactions to our products. Usage is always at the buyer's own risk. Please reach out to a professional for any possible contraindications and/or interactions with medications and be sure to inform your doctor about all herbal remedies or supplements you are taking.”Let's take a look at each of the claims independently and evaluate the research. The claims are pretty impressive: * Helps to restore and maintain balance in the menstrual cycle.* Supports regular health and hormone upkeep to prevent future issues.* Aids in easing symptoms of PMS. Provides relief from any pain associated with menstruation.* Continuous hormonal support throughout the menstrual cycle.Let's look at each claim independently. * Helps to restore and maintain balance in the menstrual cycle:* Marjoram tea showed beneficial effects on the hormonal profile of women with polycystic ovary syndrome (PCOS) by improving insulin sensitivity and reducing levels of adrenal androgens (Haj-Husein, Tukan, & Alkazaleh, 2016).* However, this study was very small (25 individuals), it was only looking at patients with PCOS, and it was only a Pilot Study which requires further research. * Chinese herbal medicine was found to improve menstrual rates in women with PCOS, suggesting potential benefits in menstrual cycle regulation (Lai et al., 2017).* However, this is a small study (40) and once again it was only looking at patients with an existing medical condition. This was also a pilot study which requires further research. * Supports regular health and hormone upkeep to prevent future issues.:* This claim is too vague to be evaluated accurately with randomized controlled trials. * Aids in easing symptoms of PMS. Provides relief from any pain associated with menstruation:* Chamomile tea was effective in relieving pain related to primary dysmenorrhea (cramps or painful period) and its consequent psycho-social problems (Jenabi & Ebrahimzadeh, 2010).* However, this was a very small (80) experimental study, that only asked participants to drink 2 cups of chamomile tea and report their PMS symptoms after 1 month. * Ginger tea significantly reduced heavy menstrual bleeding among high school girls, providing relief from dysmenorrhea (Kashefi et al., 2015).* This was probably the strongest of the studies that I found. It was well designed and it seemed to show a reduction in the menstrual bleeding of high school students. The participants were instructed to take 3 - 250mg tablets of ginger compared to the control group that was given a lactose pill. It did provide an improvement. * Concerns: The participants were not exactly given a “hormonal tea” they were given a high dose pill amounting to about 750mg of ginger each day. Would you be able to ingest 750 mg of ginger to match this amount by drinking this hormonal tea? I'm not really sure but I'm a bit skeptical on that. * Continuous hormonal support throughout the menstrual cycle:* I wasn't able to find any RCT' studies that examined this claim specifically. Conclusion & ResultsDo “Hormonal Teas” work? Maybe. There is some evidence that the individual ingredients contained in some hormonal teas could alleviate some of the pains, cramping, and heavy bleeding associated with PMS. However, the studies are small, many are pilot studies, and none of the studies evaluate the effectiveness of Hormonal Teas specifically. In most cases, they just look at the individual ingredients and they are often tested on individuals that already have a medical condition. In my layman's view, the jury is still out. If I was advising my wife or loved one on Hormonal Teas', I'd probably say: “There is some evidence that the individual ingredients contained in some “Hormonal Teas'“ may help alleviate some of the symptoms of PMS, however there aren't any studies that I could find that evaluate the claims of Hormonal Tea blends specifically. Since the products seem to be safe, if you enjoy drinking them, there doesn't appear to be any harm in consuming them. However, I would stay away from any high priced premium products that make grandiose medical claims and I would try to buy the highest quality ingredients directly like ginger and chamomile. These ingredients were the ones specifically tested and they are fairly generic, fairly cheap, and did show some effectiveness in these limited studies. In summary, I would only drink these if you find some personal enjoyment and benefit from them. Not solely as a medical product.” I was so incredibly honored that one of my readers trusted my research enough to ask me to evaluate these claims. This post summarises our work to date, what we plan to work on going forward, and clarifies areas where we're not focusing our attention. We have four broad workstreams: corporate engagement, farmer support, research, and raising issue salience. We believe our key achievements to date are: Corporate engagement: Our Humane Slaughter Initiative (commitments with large producers to purchase electrical stunners, such as MER Seafood, and Seajoy), ongoing conversations with UK retailers (including Marks & Spencer, who now have a published Decapod Welfare Policy), and contributing to the Aquaculture Stewardship Council's (ASC) Shrimp Welfare Technical Working Group. Humane Slaughter Initiative: This work in particular seems to be our most promising work so far, and we Guesstimate that our work to date will reduce the suffering of ~1B shrimps (in expectation per year) at a cost-effectiveness of ~1,300 shrimps per $ (in expectation per year). Farmer support: The launch of the Sustainable Shrimp Farmers of India (SSFI) program, Scoping Reports in India and Vietnam, a Pilot Study in India, and MoUs with prominent farmer-facing stakeholders (in Gujarat, and with ThinkAqua) Research: Working on a number of research projects to answer some of our key uncertainties, such as the Shrimp Welfare Report, an Alternative Shrimps report, a Supply & Demand economic analysis, a Consumer Research report, an Impact Roadmap, and coordinating academic research on the effectiveness of electrical stunning. Raising issue salience: Highlighting the issue of shrimp welfare through conferences/podcasts/articles in the shrimp industry, animal welfare, and Effective Altruism spaces. in addition to working in coalitions with other orgs in this space (i.e. EuroGroup for Animals, and the Aquatic Animal Alliance). As we are moving into our Exploit phase, we plan to focus our work on the following key projects: Humane Slaughter Initiative: Significantly accelerating the adoption of electrical stunning prior to slaughter in the farmed shrimp industry is a key goal of ours. We do this by purchasing the first stunner for a few different medium-large producers in different countries/contexts and in different farming systems in order to remove barriers to uptake. We believe we can realistically absorb ~$2,000,000 in funding over the next couple of years for our Humane Slaughter Initiative, at a cost-effectiveness of 1,500+ shrimps per $ per year, depending on producer volume and demand from their buyers.. Sustainable Shrimp Farmers of India: Our farmer support project is still somewhat exploratory, but we are excited by the tractability of interventions we have tested, such as offering free welfare-focused technical advice to farmers via WhatsApp, and promoting additional pond preparation (such as sludge removal) and lower stocking densities. Shrimp Welfare Index: Building on the Shrimp Welfare Report, and our experience trying to standardise a set of Asks across all the shrimp production systems, we wanted to clearly define what "higher welfare" looks like across different contexts. The Index offers an assessment of current practices and provides clear, actionable processes for improving shrimp welfare depending on the issues present in each pond. Abstract: Background: Individuals with schizophrenia are at elevated genetic risks for comorbid cannabis use, and often experience exacerbations of cognitive and psychotic symptoms when exposed to cannabis. These findings have led a number of investigators to examine cannabinoid CB1 receptor (CB1R) alterations in schizophrenia, though with conflicting results. We recently demonstrated the presence of CB1R in both excitatory and inhibitory boutons in the human prefrontal cortex, with differential levels of the receptor between bouton types. We hypothesized that the differential enrichment of CB1R between bouton types - a factor previously unaccounted for when examining CB1R changes in schizophrenia - may resolve prior discrepant reports and increase our insight into the effects of CB1R alterations on the pathophysiology of schizophrenia. Methods: Using co-labeling immunohistochemistry and fluorescent microscopy, we examined total CB1R levels and CB1R levels within excitatory (vGlut1-positive) and inhibitory (vGAT-positive) boutons of prefrontal cortex samples from ten pairs of individuals diagnosed with schizophrenia and non-psychiatric comparisons. Results: Significantly higher total CB1R levels were found within samples from individuals with schizophrenia. Terminal type-specific analyses identified significantly higher CB1R levels within excitatory boutons in samples from individuals with schizophrenia relative to comparisons. In contrast, CB1R levels within the subset of inhibitory boutons that normally express high CB1R levels (presumptive cholecystokinin neuron boutons) were lower in samples from individuals with schizophrenia relative to comparison samples. Conclusion: Given CB1Rs role in suppressing neurotransmission upon activation, these results suggest an overall shift in excitatory and inhibitory balance regulation toward a net reduction of excitatory activity in schizophrenia. Summary Subjective wellbeing (SWB) data, such as answers to life satisfaction questions, are important for decision-making by philanthropists and governments. Such data are currently used with two important assumptions: Reports are comparable between persons (e.g., my 6/10 means the same as your 6/10) Reports are linear in the underlying feelings (e.g., going from 4/10 to 5/10 represents the same size change as going from 8/10 to 9/10). Fortunately, these two assumptions are sufficient for analyses that only involve the quality of people's lives. However, if we want to perform analyses that involve trade-offs between improving quality and quantity of life, we also need knowledge of the neutral point, the point on a wellbeing scale that is equivalent to non-existence. Unfortunately, evidence on all three questions is critically scarce. We propose to collect additional surveys to fill this gap. Our aim with this report is two-fold. First, we give an outline of the questions we plan to field and the underlying reasoning that led to them. Second, we present results from an initial pilot study (n = 128): Unfortunately, this small sample size does not allow us to provide clear estimates of the comparability of wellbeing reports. However, across several question modalities, we do find tentative evidence in favour of approximate linearity. With respect to neutrality, we assess at what point on a 0-10 scale respondents say that they are 'neither satisfied nor dissatisfied' (mean response is 5.3/10). We also probe at what point on a life satisfaction scale respondents report to be indifferent between being alive and being dead (mean response is 1.3/10). Implications and limitations of these findings concerning neutrality are discussed in Section 6.2. In general, the findings from our pilot study should only be seen as being indicative of the general feasibility of this project. They do not provide definitive answers. In the hopes of fielding an improved version of our survey with a much larger sample and a pre-registered analysis plan, we welcome feedback and suggestions on our current survey design. Here are some key questions that we hope to receive feedback on: Are there missing questions that could be included in this survey (or an additional survey) that would inform important topics in SWB research? Are there any questions or proposed analyses you find redundant? Do you see any critical flaws in the analyses we propose? Are there additional analyses we should be considering? Would these data and analyses actually reassure you about the comparability, linearity, and neutrality of subjective wellbeing data? If not, what sorts of data and analyses would reassure you? What are some good places for us to look for funding for this research? Of course, any other feedback that goes beyond these questions is welcome, too. Abstract: Objectives: Mapping the neurobiology of meditation using 3 Tesla functional MRI (fMRI) has burgeoned recently. However, limitations in signal quality and neuroanatomical resolution have impacted reliability and precision of extant findings. Although ultra-high strength 7 Tesla MRI overcomes these limitations, investigation of meditation using 7 Tesla fMRI is still in its infancy. Methods: In this feasibility study, we scanned 10 individuals who were beginner meditators using 7 Tesla fMRI while they performed focused attention meditation and non-focused rest. We also measured and adjusted the fMRI signal for key physiological differences between meditation and rest. Finally, we explored the 2-week impact of the single fMRI meditation session on mindfulness, anxiety and focused attention attributes. Results: Group-level task fMRI analyses revealed significant reductions in activity during meditation relative to rest in Default-mode network hubs, i.e., antero-medial prefrontal and posterior cingulate cortices, precuneus, as well as visual and thalamic regions. These findings survived stringent statistical corrections for fluctuations in physiological responses which demonstrated significant differences (p less than 0.05/n, Bonferroni controlled) between meditation and rest. Compared to baseline, State Mindfulness Scale (SMS) scores were significantly elevated (F = 8.16, p less than 0.05/n, Bonferroni controlled) following the fMRI meditation session, and were closely maintained at 2-week follow up. Conclusions: This pilot study establishes the feasibility and utility of investigating focused attention meditation using ultra-high strength (7 Tesla) fMRI, by supporting widespread evidence that focused attention meditation attenuates Default-mode activity responsible for self-referential processing. Future functional neuroimaging studies of meditation should control for physiological confounds and include behavioural assessments. Copy rights belong to original authors. INTRO: To start off, Rohan, I had a blast researching your work, and am so glad I listened to you on The Abstract Doctors Podcast[ii] because I tend to more on the serious side, and could lighten up my interviews to make them more fun/less stressful and as I was learning more about you, I was dying laughing with these two doctors covering your Lief Device. They were asking why it's called Lief, and they were guessing “it's about turning over a new leaf?” Why is it called Lief? Then I watched your clip of the Today Show[iii] with Hoda and Savanna trying out your device, and I thought, wow, I take learning and this podcast a bit too seriously because they were having such a good time investing the high stress parts of their day.  Since you are an expert at helping people relieve stress, I wonder what tips you would have for me or anyone else these days who spends most of their day sitting at a desk? What are most of us missing that could help us handle life's pressures/stresses better? Q1: I know there has to be a story here of how you came to be the founder of Lief Therapeutics that is focused on helping people to overcome stress and worry from the inside out. Where did this begin for you? Q2: Where did you come across HRV, and is there really a story like Steve Jobs where you started this whole idea in your garage? Q3: Can you give me YOUR definition of HRV for the lay person who is tuning in, and has no idea what it is? (We have talked about it often on this podcast but I recently reviewed it when I became stuck explaining it to someone else). Q4: Tracking HRV in real time is such an incredible idea. I track with a device that gives me a daily score and I wake up dying to see my score, hoping for improvements as I'm working on certain strategies. Why would someone need to know their HRV levels throughout certain parts of their day? How does this help them? I think I heard you say this knowledge is like our “superpower!” What do you mean by that? Q4B: Is the strap for athletes who might need to measure their HRV during play? Q5: So I wrote your questions BEFORE trying out the device and wanted to add a snapshot of my work day, to see what you see with my bar graph. I had an important LIVE presentation on Monday morning that I practiced all weekend for, and wore the Lief device thinking it would show how calm I was under pressure. Then the unexpected happened and I had 2 major unforeseen tech issues that really threw me off. This is what I see: I started out calm (green) which is the 60th-80th percentile and then as the time for the presentation came closer, I went into yellow, lowering my HRV to 20-40th percentile, most of the time presenting was yellow, with some RED (o-20th percentile) that happened about the time of the tech issue, and I ended up back to GREEN. While I was hoping to show you all GREEN with my morning, I can see that I had some moments of stress, but when something goes wrong, and we need to think and troubleshoot our way out of problems, I think this was me at yellow, thinking and not shut down in RED.  What do you see with these colors?     Q6: Now that we've talked about Lief measures, I wonder How exactly does the device work? What is this idea of biofeedback, and why are the sensors so important? 6B: What should we be looking for? We have already talked about the bar graph at the top of the dashboard that shows our HRV in real time. I put my WEEK 1 and WEEK 2 of using the device in the show notes to look at. Just glancing at my week 1 vs week 2, I can see my range improved from 46-55 (week 1) to 52-61 week 2, going from 65% success to 72% success. What should everyone be looking for in their first few weeks of using Lief to know that we are in fact showing an improvement? Q7: Have you ever done experiments like I've seen people do with glucose monitors, and look to see what are a few of the most common ways to increase your stress and decrease HRV? Q7B: I've been tracking my HRV and RHR with a different device since I heard Dr. Andrew Huberman's podcast on the effects of alcohol on our brain and body[iv], and after I heard how bad it is for us, I don't think I'll ever drink alcohol again. After stopping this toxin, I'm watching my HRV skyrocket up. After 2 weeks, my HRV went from 76 to 84 and after a month it's jumped up to the 90s.  I know it's clear that alcohol impacts our HRV, giving us less capacity to handle stress throughout our day, but what else have you noticed affects us in this drastically? (sleep, diet, hydration I'm sure…anything surprising you've seen)?   Q8: What's involved for individuals who want to try it out, as well as health care providers? The aim of this study was to design andconduct an Objective Structured Clinical Examination (OSCE) based on the Occupational Therapy Practice Framework (OTPF) for occupational therapy students on Level II fieldwork in Iran. A seven-station OSCE was designed and conducted with 13 students.

Anger, resentment, fear, frustration, these all are common emotions during withdrawal and expressing them can feel good — in the short-term. But the truth is, they don't help you heal. Having kindness for yourself, and for those around you trying to help you, is key to success during this difficult time. 5) ACCEPTANCE One of the best things that happened to me during withdrawal was when I found acceptance. I stopped struggling and fighting my symptoms and instead accepted my current condition. It sounds easy, but it's a difficult transition and it usually only comes with time. Most people experiencing benzo withdrawal see their recovery as a struggle. A fight. A battle of epic proportions. And it can be. They want to do everything they can to make it disappear. And if they can't make it go away, then make it as easy as possible. But the symptoms of benzo withdrawal are actually symptoms of healing. The drugs are gone, or at least reduced. All that's left is your body trying to right itself.
