I really appreciate you two and your no nonsense, but super informative podcast. It always hits home for me. I have gone back and forth between keto and carnivore since 2019. Felt great at first, but the last 18 months have had weight gain, bloating and constipation. Never really had a weight issue before. I'm 5'7” and have averaged 135 pounds pretty consistently. I was referred to a gastroenterologist, did the breath test, and tested very high for intestinal methanogenic overgrowth (IMO). The doctor gave me a two week dose of xifaxin and that was it. No other protocol or follow up. I just finished the antibiotic with no relief. I am an active 52 year old, weight train 3 to 4 days a week, take my dog for a hour walk almost every day, gets lots of Texas sun, drink only water with LMNT of course. My two part question to the both of you is how in the world was I blessed with this lovely methane bug and is there any way to rid myself of it? Any supplements? I am taking berberine. I also have hypothyroidism and take desicated thyroid for that. The more I research, the more confused and overwhelmed I get. It seems it's a little different from your standard SIBO. I've heard some doctors say to follow fodmap and others go all the way to carnivore. Any suggestions would be super appreciate, as I value your insight greatly. I feel like I have all the knowledge that I could ever need to optimize myself, but I'm still not implementing it in the right way to improve my body composition. I feel like I've been doing B+ work since 2021, and A+ work for the past 3 months but I haven't seen any results. I'm a type A personality but I've learned to forgive myself for not being perfect, and have added a little meditation in my life. I'll lose 5-10 lbs and then it always comes back. I'm 33, female, 5'4", and 175 lbs. I was 125 lbs when I was vegan in college at 20 but of course had no muscles and mild anemia, I figured that was still a healthy target weight for me. I don't even care what the scale says, I just want to look good naked! I slowly gained weight over the last decade due to burritos, pizza, beer, stress, and smoking. I really was a hot mess until 2020. I've been approaching my failure through the lens of Boyd Eaton's evolutionary discordance hypothesis. I'm wondering if it's appropriate to tackle body composition issues using the old evolutionary discordance hypothesis, and if there have been any recent updates to it? I figure I'm not losing weight because my nutrition/exercise regime is still too much of a departure from the ancestral model but I've been eating paleo since 2021 and feel like I have balanced my 4 pillars. I only drink 12-18 ounces of coffee day, so that doesn't seem like the culprit. What really annoys me and why I decided to write to you is because I am now exercising the most I ever had in my life and still not slimming down. I still just look puffy all over. I hike 5-7 miles a week with a 10-15 lb backpack, and one of those hikes is up a small east coast mountain. I get 5 miles of walking in most days a week, I run 3-4 miles a week 1-2 miles a day on top of the walking, and am now adding BJJ 3-4 hours a week, I don't lift weights and know this would be huge but I don't know how and cant afford a personal trainer yet. Hoping the BJJ does the trick, my training load is high but its enjoyable. I have used chronometer to track what I'm eating periodically, so I know I get 125+ grams of protein, 120 from animal sources (lots of steak, chicken, and pork) but I do eat 2300 calories easy. I'm doing 120-150 grams of carbs from mostly fruit, an ounce of maple syrup sometimes or a few tablespoons of honey also contribute. Diana Rogers suggests 1500 on her sustainavore course as a starting point. How many calories should I go for? That's got be my issue, I'm eating enough calories to maintain weight and I got to go into a comfortable non-lethargic deficit. Advice would be much appreciated! For optimal healing, a patient needs their symptoms, not their lab test results, to be treated. In this episode, I offer dietary and lifestyle solutions for people looking to improve their gut microbiome and overcome digestive and non-digestive symptoms caused by small intestinal bacterial overgrowth (SIBO) and gut dysbiosis. Watching out for non-GI symptoms of GI issues It's important to learn these because you might be chasing the wrong symptoms for the wrong issue. Food triggers, imbalances in the bacteria, colonies in the gut, or leaky gut can cause unexpected symptoms. Watch out for (non-exhaustive): skin problems, MCI, brain fog, anxiety, depression, joint pain, thyroid problems, small intestinal bacteria overgrowth, correlations to lipids, etc. Does everything start in the gut? Starting in the gut is important. Diet, Lifestyle and Gut Health Foundations Model. The testing doesn't really change a lot the way how you treat the person. The biomarkers in GI are theoretical for the most part but they don't necessarily tell you what diet or probiotics you need. The Ruscio Institute It's a multidisciplinary integrative clinic which promotes a more cost-effective model of functional medicine. We take what works from conventional medicine. Getting bias out of the way, questioning everything you're taught or told, and taking it on the merits of evidence, can be freeing and get you to really help people. Take the ideas on the basis of what support they have and not what camp they come from. Gut Health and Diet Paleo-like Diet is a rough proximity to the spirit of the paleolithic ancestor diet. Simply put, it means eating food that has not been or minimally processed. Pro-tip: Shop the isle of the grocery. Apply the 80-20 principle—if you can't hunt or forge it, don't eat it. But some people can't process too many fruits and vegetables. So better to resolve the dysbiosis first. Then you can reintroduce the grains or vegetables. As the gut heals, the lining of your intestines of villi that secrete enzymes repair, making people better able to digest food and develop tolerance. Choosing your pre/probiotics Both work. Probiotics have more evidence for most things. Prebiotics has more probability of flaring for people. Start with probiotics: more evidence, better evidence, fewer adverse events. People start to look at probiotics as drugs because of what companies are doing on specific strains and specific issues while they actually have tremendous overlap. It's not about using probiotics like a drug reductionistically but about how we can create a simple guide to using probiotics to heal the gut. Three main classifications of probiotics: Lactobacillus and Bifidobacterium. Blends, Saccharomyces boulardii, a beneficial yeast.Soil-Based Blends, usually Bacillus species. If we combine all three, they work better than having just one. How do you actually "heal the gut" We don't always need to quantify, at least in the early phase, pre and post-gut biomarkers. The individual symptoms are a much better way to stir the process than repeat biomarker assessments. Elemental diet has yielded great results for gut health reset and even some autoimmune issues. Use your own biofeedback to stir up how long you want to use it. Dr. Ruscio's Best Medicine: Fixing an overt problem in the gut. And sleep and exercise.

This month, you might have noticed that we are breaking into a new season on the podcast, Season 8, where we are focusing on brain health and learning with a look at how an understanding of our brain can have an immediate improvement on our life, as well as our future generations. If you have followed our past few episodes, you'll recall me mentioning our guest for today, Ashok Gupta, who is a well-known Neuroplasticity "brain retraining" expert who has spent the last 25 years researching the effects of the brain and the mind on illness. He suffered from a condition called ME, or Chronic Fatigue Syndrome, around 25 years ago when he was studying at Cambridge University. Through neurological research that he conducted, he managed to get himself 100% better. He then set up a clinic to treat others, and published the well- known neuroplasticity “brain retraining” recovery program he created, known as the Gupta Program[i] in 2007. He has published several medical papers, interviews experts in the field himself, like his recent interview with Dr. Joe Mather, medical director of a well-known functional medicine clinic in England called the Ruscio (ru-show) Institute[ii], and is continually researching these chronic conditions. Recently, a randomized controlled trial was published showing the Gupta Program (that Ashok created himself) was highly effective compared to a control. Welcome Ashok, thank you for coming on the podcast to share the years of research you have done in the field of chronic pain and illness. What part of the UK have we reached you today? INTRO Q: To begin, I want to start from the beginning of your story because it really is incredible to see what you have built. I have watched some of your recent podcasts that people can access in the show notes[iii] and everything you share in all your interviews I find fascinating, and even ground-breaking. Can you share where this all started for you beginning with your own illness? Q1: Ashok, the whole reason we launched this podcast, was to connect the brain to learning, and I heard you say it yourself that “most medicine is based on what's measureable, and what is measureable is in the body. But the brain is a black box.” Without looking at the brain with fMRI scans, we just don't know what's happening (in medicine and with learning/education). How did you first think to look at the brain with chronic illness that led you to your hypothesis that I will let you share and explain? How did you come to discover or hypothesize that in the case of chronic pain or illness that “maybe the brain is creating an artificial environment in the body because it's overprotecting the system to ensure its survival” explaining why we feel pain or discomfort that doctors say isn't there? 1B: We've all had an injury or something where the pain just never seems to go away or neck pain I've always had, if I came to see you about it, would you say that it's a conditioned, or learned response? Perhaps tied to my posture at my desk, by not sitting up straight—what's happening in this case? Why would the pain from an injury a long time ago still remain in my neck years later? How does my brain create the inflammation/irritation I feel and how does it create a viscous loop building up more inflammation and pain in the body? 1C: What about hypnotherapy for pain reduction? With everything you have learned studying the brain and pain, why do some people respond really well to this type of therapy, while visits to the chiropractor seem to have very little impact on long-term pain relief? 1D: Has modern medicine caught up with brain retraining as a viable solution for chronic pain or disease? Q2: When you discovered how the brain, specifically the amygdala and insula were connected to chronic pain and disease, what happened next? You obviously knew how it helped you with Chronic Fatigue Syndrome, but what about Long-Haul Covid, Fibromyalgia, Chemical or Mold Sensitivities, Gut Issues, Anxiety, Lyme Disease? Even with re-use at front of mind, things have to get thrown out eventually. Trove is looking to sustainable production processes for consumable, like foamcore derived from mushrooms, etc. Reducing the lifecycle of items in the studio is another key sustainability effort. Trove allows them to take control of the lifecycle of their products by receiving returns and trade ins, cleaning and repairing, and then enabling the resale channel. Condition grading adds a new dimension to sample management for creative production. It's not enough to have a photo of a sample, Trove needs to have images that accurately reflect the condition of a specific item. Trove not only accepts returns and trade-ins, but unsellable returns go into a robust recycling program, to avoid sending items to a landfill. Post-production for Trove needs to accurately depict condition. Where traditional retouching strives to make an item look perfect (removing dust, reflections, styling issues, etc). Trove needs customers to see the flaws clearly. Trove has learned a bit about who buys used and what they like to see, and has adjusted creative direction accordingly.Re-using images when possible is another way that Trove works to minimize energy use. If they don't have to reshoot something, that saves the time and energy of the process. Across all of Troves partners, they estimate that they've saved over 1 million kg of C02.
