Check out today's questions:    Lara: Hi, dr. C, I'm wondering if you've heard of fermented fruits that are used in TCM? It's done by an ancient recipe by the Tang family, they ferment prunes and some other fruits for 30 months and then they contain prebiotics, probiotics, digestive enzymes, antioxidants, vitamins, flavonoids & different essential & nonessential amino acids.. supposedly it's amazing for our digestive health and subsequently the immune system.. Other than that, I've been hearing a lot how we should eat fermented foods like sauerkraut, kimchi and others daily but I remember hearing from you once that they're not so good on a daily basis for everyone, can you please explain? Thank you   Scott: Reishi mushrooms! I purchased a supplement from a company i love and it was just reishi mushrooms. I took one around 7pm and by bedtime felt nice and sleepy. However at 11:45 pm i woke up suddenly and felt Like i had slept all night and it had only been 2 hours! Plus my heart felt as if it was pounding. I had a handful of almonds to calm it down, fell back asleep until 4:30. This time i woke up and felt like i was almost in a haze. It was strange but i did notice i felt less sore and stiff waking which was great. So I am a little scared of taking this again but i am also curious as to why this happened. Would you stick with it? Just wanting a little feedback…i appreciate your time and as always, your excellent content.   Lauren: Hello, I'm reaching out because I'm interested in a heavy metal detox, but I have seen Dr. Pompa say detoxing with Chlorella and cilantro can be dangerous and ineffective because the binders are too weak to move the metals out of the body, and they are actually redistributed/reabsorbed throughout the body. He recommends something called DMSA which requires a prescription I think, and I'm not sure what the risks might be with that. Have you seen these claims? Can you respond? What is the difference and can I take it for an extended period of time? I definitely notice a difference in recall when I take this supplement. Also, I'm considering an hBot chamber. Does a less expensive chamber give the same amount of oxygen and benefits as a more expensive chamber? I liken it to cars. In today's podcast we talk about acupuncture and Ayurvedic medicine to support fertility with Michelle Oravitz.    Michelle Oravitz began her career as an architect upon graduating from Philadelphia University. After many years of experiencing missed periods (which traditional doctors could not resolve) and enduring stress at her high demand job in NYC, she decided to go to an acupuncturist named Dr. Li who immediately regulated her cycle.   Not only did her symptoms resolve, but she also felt a renewed sense of well being and clarity. She was unable to recognize just how disharmonious she felt until she experienced a new sense of well being that arose from her treatments.    When moving to South Florida, she enrolled in an Ayurvedic school which gave her tremendous knowledge about this ancient and rich healing approach. She then enrolled into school to get her master's degree in Oriental Medicine.    Michelle believes that if she had not gone to a TCM doctor, she probably would have had a hard time getting pregnant with her two children. This has influenced her to develop a special interest in treating gynecological disorders and infertility.   When you start to become more balanced in your body you start to get more clarity in your mind, and you start to see what's good for you and what's not so good for you and then your body and mind become aligned. You start seeing everything differently.    It is sometimes difficult to do this because our society dictates that if you have chosen one specific path, like a corporate job for example, and have invested money and energy on that path, then you should stick to it even if it doesn't fulfill your life.    The mind is such an important aspect of our health. Sometimes we just get used to the way things are in our lives and we are reluctant to change and to follow our hearts. This is exactly what acupuncture did for Michelle. It gave her this openness to change and to follow her heart.    If your body/mind are telling you something, like “I am getting the feeling that food is really related to one's health, maybe I should eat healthier” then you should listen to it and start making the necessary changes towards that direction.    If you recognize that there's something about that job or that career or that routine that's harming your health, then you need to act on it and make the shift to a new environment that feels good for you, otherwise you will not be healthy and you will not be able to enjoy life to the fullest. There is so much research coming out related to women's health and fertility, new natural treatments, supplements, herbs, and nutrients to help women with fertility that there is always something new to learn and to try.   Talking about alignment we can see that if a person is in a stressful environment or if they are not aligned in the mind-body connection, maybe because they don't really like their job or their relationship, or are under so much constant stress, then sometimes that is part of why the pregnancy is not happening.   We want to balance our hormones and we want to balance our microbiome and neurotransmitters but if there is something not working on the mind-body connection then it is much more difficult to heal.   A very interesting aspect of Chinese medicine is that it works by acknowledging connection. Everything that happens around us (the universe, the sun, the stars, the moon, etc.) reflect in the rhythms of life and in our bodies. We are walking living ecosystems that thrive on balance.    When something in our bodies is out of balance, we start to see health problems. Our bodies could have too much heat or too much cold or not enough blood production in a day-to day basis. There are so many different things that can cause the same symptom. We must look at the pattern, we look at the underlying root cause and then we can solve the problem.    The human body is incredibly intelligent, but it is a living organism, and it requires attention and care, it is not a machine, and even machines require attention and care. If we do not give it what it needs to be in balance it is going to eventually give you some signs and signals and disharmonious feelings that something is not right.    Acupuncture is all about reaching that balance in your body and mind. It works on our energy as its flowing to feed all the different systems and get into different organs. And once you give the body what it needs then it removes the blockages in order to move that energy across all systems and creates that homeostasis state where it begins to heal.    In this modern world we are so used to have everything we think we need readily available for us, like fruits and vegetables that don't grow in our regions depending on the seasons, or artificial light disrupting our inner resting cycles and hormones, or not going out because it's too cold so we don't get sunlight, all these things make our bodies go out of balance and we suffer the consequences in our health.    We also forget that our Earth has all the necessary nutrients and herbs to keep us as healthy as we can be, and that there are alternative ways instead of the traditional western medicine. For example, there is a lot of research showing the benefits of Ashwaganda in reducing stress, which also causes fertility issues, or Phosphatidylserine to reset our Cortisol levels which help regulate our mood and sleep patterns.   In these alternative treatments it is important to know that there is no “one pill fits all” it has to be about the person, his unique body and his unique response to external factors. In this modern world we sometimes forget that excessive amounts of stress are probably one of the main root causes of serious health issues like heart attacks, cancer, dementia, infertility, etc.   We have to recognize our own patterns and our unique bodies' response to stress, so we can control it and not the other way around. X., Xu, T.-L. Abstract: Phospholipid-protein interaction is highly specialized at the membranous nanodomains and critical for membrane receptor signaling. Calcium-permeable acid-sensing ion channel isoform 1a (ASIC1a) is a major neuronal proton sensor that contributes to synaptic plasticity. The functional outcome of ASIC1a is dependent on its surface targeting in synaptic subdomains; however, the lipid environment for ASIC1a and its role in channel targeting remain poorly understood. Here, we report that anionic phosphatidylserine (PS) is enriched in dendritic spines during neurodevelopment and it directly binds to ASIC1a through an electrostatic interaction with a di-arginine motif at ASIC1a C-terminus. PS regulates the membrane targeting and function of ASIC1a, which are both strongly suppressed by inhibition of PS synthesis. In cortical neuron dendrites, both PS and ASIC1a are predominately localized to peri-synaptic sites of spine heads, surrounding instead of overlapping with postsynaptic markers, PSD-95 and GluN1. Uncoupling the interaction between PS and ASIC1a by changing the charges to neutral or acidic at the di-arginine PS-binding motif, or applying a membrane penetrating competing peptide, caused mistargeting of ASIC1a at the synaptic sites, an overall increase in internalization and/or cytoplasmic accumulation of ASIC1a, and a decrease in its channel function. Together, our results provide novel insights on lipid microenvironment that governs ASIC1a expression and function at the membrane surface, especially peri-synaptic regions of dendritic spines, through an electrostatic interaction with anionic phospholipids. The Case:  Amanda suffers with frequent colds and sinus infections (especially around her menstrual cycle). Her hands and feet are always cold and she gets tension headaches all the time.  Standard blood tests came back normal, leaving the doctors with no answers.   The Investigation I run more comprehensive labs that many traditional doctors I make sure to run them at a specific time in the menstrual cycle. My suspicions were that this was not related to her sex hormones but rather something upstream. I suspected that a combination of hormones were being affected and leading to tension and immune challenges. In other words, I suspected that Amanda had fallen into sympathetic overdrive.  One of the top experts in this field is Dr. Amir Rashidian so I was eager to discuss Amanda’s case with him.  Dr. Amir Rashidian is the founder of the Mid-Atlantic Chiropractic Center where he focuses on high-tech diagnostics to detect and correct disturbances in the nervous system. Like me, he is a believer in drugless health solutions and eliminating the causes of disease (not suppressing the symptoms).    What is Sympathetic Overdrive? We’ve talked a lot on this show about how our nervous system reacts to stress. We have a primitive response to danger that triggers our nervous system to ready the body for fight, flight, or freeze.  Dr. Amir explains that this unconscious response to our environment can often go undetected (because we don’t have that obvious ‘sabertooth tiger’ threat).  So, it might be traffic, work stress, our boss being demanding, our spouse being upset, or our busy lifestyle that the nervous system is perceiving as danger. And, when you are in this state, day in and day out, your nervous system never gets the chance to calm down. This leads to the sympathetic nervous system moving into overdrive.    Chronic Stress vs. Sympathetic Overdrive Everyone handles stress differently so it’s difficult to say at what point chronic stress turns into sympathetic overdrive. The body does have the ability to adapt to stress so in some cases, stress can be present without it triggering sympathetic overdrive. Think of a weightlifter - they stress their muscles and cause them to adapt (by becoming bigger) without any health concerns. Stress is the same, you can become more resilient to stress once you understand your stress threshold. However, it may be difficult to determine how much stress the body can safely handle, so you have to watch out for the symptoms of sympathetic overdrive.    Symptoms of Sympathetic Overdrive There are a variety of symptoms that suggest your nervous system may be in sympathetic overdrive like chronic sinus infections and insomnia. Additionally, someone may experience dry eyes, dry mouth, dry skin, cold hands, cold feet, constipation and indigestion. However, not everyone will have these symptoms and having these symptoms doesn’t always mean you are in sympathetic overdrive.    Three Stages of Sympathetic Overdrive Dr. Amir explains that there are three stages of sympathetic overdrive. They are: Stage 1 - Wired. In this stage, you’ve got a lot of energy, your senses are heightened so you might be looking around a lot and feeling hyper-aware of your surroundings. You will likely crave sweets because your body is burning a lot of energy and will feel a need to replace it.  Stage 2 - Wired & Tired. In this stage, you are still feeling wired but at the same time, you are tired. You are likely struggling to fall asleep or stay asleep regardless of how tired you feel. You will likely continue to crave sweets but also fatty foods (so, ice cream, donuts, etc) because your body is over producing cholesterol in response.  Stage 3 - Exhausted. You can’t seem to stay awake. You fall asleep anywhere regardless of what stimulates (like caffeine or loud music) you have been exposed to. Your adrenal glands are shutting down because they can’t produce any more adrenalin. Your cravings will switch to salty foods because your metabolism is looking for fuel.    How Sympathetic Overdrive Affects the Immune System In Amanda’s case, we saw a shift in her symptoms during her menstrual cycle. I asked Dr. Amir about why this was happening. He explained that there is a direct connection with the immune system and sympathetic overdrive because the immune system relies on energy produced by the adrenals. When the body is in sympathetic overdrive, the adrenals can’t produce adequate energy to support other systems in the body like the immune system.  Another option is to do a cortisol test to determine autonomic tone. This is a saliva test    Diagnosing Sympathetic Overdrive The first step in diagnosing sympathetic overdrive is to look at the collection of symptoms. To confirm suspicions of sympathetic overdrive, Dr. Amir uses a heart rate variability test. This is a process of determining the heart’s ability to vary its own rate. It’s measured on a scale between zero and 100. If someone scores 100, they are fully capable of handling all of the stress in their life. Trainers in the UFC (mixed martial arts fighting) use this test to determine if a fighter is in a good state to train or if they need a day off.  done at different times of the day to see if cortisol is following the natural fluctuations as it should.    Three Keys to Treating Sympathetic Overdrive Dr. Amir shared three critical ways to get the body out of sympathetic overdrive (and to keep it from slipping back). Here are the three keys to reversing sympathetic overdrive:  1. Quality Sleep.  Sleep is so critical to repairing sympathetic overdrive but Dr. Amir says that how well you sleep is more important than how much you sleep. Getting quality sleep at night (by reducing stimulation and controlling your evening cortisol levels) results in a release of melatonin which calms the nervous system and takes it out of parasympathetic state.   2. Controlled Breathing.  Our breath is very connected to our nervous system. The intake of breath is parasympathetic (most noticeable when we gasp) and the exhale is sympathetic. Doing deep breathing exercises takes us out of automatic breathing (innate) and voluntary breathing (intentionally controlled) which helps us reset the nervous system. Breathing is so much more powerful than most of us realize. In fact, Dr. Amir shared an interesting study about the role breathing plays in fat loss and weight loss.  3. Intentional Eating.  Like sleeping, Dr. Amir believes that the secret to resetting the parasympathetic nervous system is not in what you eat but how you eat it. He says we need to remove stress from the eating ritual. So many of us eat distracted and in a hurry which keeps us in a stressed state. This makes it very hard to digest the food. Instead, he suggests that eating is a perfect time to give the nervous system a break and allow it to calm down by eating slowly and intentionally.    Nutrition for Balancing the Nervous System Dr. Amir mentioned eating foods that are powerful antioxidants including those rich in minerals like sea kelp. With Amanda, we focused on making sure she had balanced meals and a regular routine that included mindful eating practices.   Supplement Support for Sympathetic Overdrive While eating a diet that intentionally balances the nervous system is powerful, there are a few supplements that can also help. Dr. Amir suggested ashwagandha and I love this option. With sympathetic dominance, I also recommend getting the system back in line with the help of Phosphatidylserine and then supporting the adrenal glands with adaptogenic herbs (like those found in Orthomolecular’s AdreneVive   Chiropractic Treatments for Sympathetic Overdrive Most people think that going to a chiropractor only happens when you have a kinked neck or you need your back cracked. Dr. Amir explains that while a chiropractic adjustment can help in those cases, there is much more to it. When it comes to Sympathetic Overdrive, you want everything to be aligned because it ensures that none of the pathways to the brain are blocked. The nervous system relies on signals from the brain and if the body is out of alignment, it can cause challenges for an already stressed nervous system.    Mystery Solved It would have been easy to assume that the issues Amanda was facing had to do with her sex hormones since her menstrual cycle seemed to trigger illnesses. What we discovered was that the issue was further upstream with her adrenals. She was suffering from sympathetic dominance.  Her monthly cycle simply added that little bit of extra demand that made her stress bucket overflow.    Conclusion The first thing we worked on was balancing out her eating. As Dr. Amir noted, it was less about what she was eating and more about how she was eating so we focused on making sure she was eating regularly and not skipping meals (to balance her blood sugar). Her meals were balanced with a lean protein, vegetables, good fats, and healthy grains (like quinoa, brown rice, and millet). She also started practicing mindful eating.  Speaking of mindfulness, I taught Amanda the box breathing technique which is breathing in, holding, exhaling and holding all for equal counts (for example 5 in, 5 hold, 5 out, 5 hold). We also introduced mindful walking and sitting quietly.  For supplements, Amanda started taking Phosphatidylserine (I recommended PS 150 from Designs For Health). She took two tablets at dinnertime to lower her cortisol for six weeks. After that, she switched to a blend of adaptogenic herbs called AdreneVive by OrthoMolecular.    Happy Ending This course of action really helped to rebalance her adrenals and she noticed that she felt better, her energy was back, and her hands and feet weren’t cold anymore. Within four months, she was no longer getting colds during her period and her headaches subsided too. [Ask Inna] Your Thyroid and Hashimoto’s Questions Answered

I recently opened the door to questions about thyroid conditions and Hashimoto’s on my Instagram page. And, wow, was I flooded with really great questions. In this episode, I answer as many as I can.    The Question - How do you Shrink Your Thyroid Nodules? This was the question I got the most from my Instagram followers. Before I answer, I want to clarify a few things. There are many different types of growths on the thyroid. Many people confuse enlargement of the thyroid with nodules or inflammation. If the thyroid is enlarged in one spot, that is a nodule. However, if the whole thyroid is enlarged it’s either a goiter or thyroiditis. A goiter is caused by iodine deficiency (uncommon in the US), consumption of too many goitrogenic foods, or thyroiditis. This swelling is often painless (pain may indicate that inflammation is from a virus).  Goiter growth is fed by a high TSH (Thyroid Stimulating Hormone) and also by high hCG (human chorionic gonadotropin) which is present during and after pregnancy. Eventually the hCG will go back down but if the TSH level is not treated, it can continue to feed the goiter and it will continue to grow.  The first step in reducing a swollen thyroid is to get TSH levels in balance and to reduce goitrogenic foods.  As for nodules on the thyroid, these could be solid or cysts filled with fluid. About 50% of people have these and many are benign. Someone with Hashimoto’s may experience nodules during a flare up. However, it’s important to have them checked because they could lead to thyroid cancer. The good news is if the nodules are due to Hashimoto’s, addressing the root cause of Hashimoto’s can help shrink the nodules as well. For more on this, refer to past episodes 32 - Solving the Autoimmunity Mystery, and 68 - Ask Inna: Hashimoto’s Questions.   The Question - Do people with Hashimoto’s have a compromised immune system? This is a question that has popped up a lot more since the start of the pandemic. People want to know if having Hashimoto’s means your immune system is weakened.  Inna’s Response  This might seem logical but it’s actually the opposite.  Hashimoto’s makes the immune system hyper-vigilant. The problem is that the immune system is confused so it’s extra-active. But this extra activity means that it attacks your own tissue - in this case, your thyroid. So, it’s not weak in that sense but because it’s so hyper-vigilant and attacking your tissue, it can actually weaken its response to bacteria and viruses.    The Question - My antibodies are still high. Is there something else I should look at? This question came with quite a bit of backstory. She explained that she had switched from taking just Synthroid (a prescription synthetic thyroid hormone replacement) to adding Cytomel. However her hashimoto antibodies continue to be high despite adhering to the autoimmune protocol (AIP) diet (with the exception of eggs). She wondered if the eggs could really cause that much of an issue. An ultrasound also revealed that she has two nodules on her thyroid. So, the real question is, what’s the root cause and what else can she do? Inna’s Response  Finding the underlying cause can be tricky. It seems like she’s already looked at the food angle by doing a mostly AIP diet. It is possible that the eggs are a trigger because every person is different but it’s hard to say. A really good food sensitivity test (like the comprehensive panel test done at Vibrant America), and find out conclusively if eggs are an issue for you.  After that, you want to make sure you look for other underlying causes like stress, infections, and toxins. Keep in mind that you want to give new medication combinations a couple of months to shift your antibody response.    The Question - My TSH levels are very erratic as is my weight gain. What are the best diet plans, supplements and essential tests? Inna’s Response  In this case it sounds like it may be Hashimoto’s and the swings are caused by flare ups. TSH can go up and down erratically with Hashimoto’s.  For tests, you want to make sure to get a TSH a total T for the total T3 free, a T4, free T3, reverse T3 and (very importantly) both thyroid antibodies for Hashimoto’s - thyroglobulin and thyroid peroxidase.  As for the diet plan, the best one to start with is the AIP plan where you take out grains, dairy, nuts, eggs and processed foods. If this feels too restrictive at first, you can start with removing gluten and dairy. You should also look at underlying causes like infections, toxins and stress.   The Question - I’ve been diagnosed with Hashimoto’s but my tests are now normal and I don’t have any symptoms. Do I still have Hashi’s? This Instagram follower has seen her test results move into normal range lately with TSH at 1.5 and antibodies consistently below the threshold of 35 (from 17 to 34). And, without any mood or energy symptoms it has her feeling like she’s in a gray zone.  Inna’s Response  The answer here depends on when and how the diagnosis was made. Look back at your blood work and see that original test that led to a diagnosis. Were your antibodies actually high at the time? Or, did your diagnosis come from an ultrasound? Some practitioners assume Hashimoto’s when they see nodules which is not always an indication of Hashimoto’s. This can lead to a misdiagnosis.  If the diagnosis was correct, antibodies in the normal range does not mean that your Hashimoto’s is cured per say because technically once the autoimmune is there, it will be there, but it would mean that it’s in remission and what you are doing is working so keep up the good work.   The Question - What could cause T4 to convert to reverse T3?  This question is a bit complicated for anyone not familiar with how the thyroid works, but essentially T4 normally converts to T3 but in this case, it’s converting to reverse T3.  Inna’s Response  The biggest thing here is stress and inflammation. Stress can come from emotional stress (our thoughts). It can also be physical, for example if you have any type of underlying infection (like Epstein-Barr Virus, Candida, or a parasite). Another possible cause might be food or toxins.  You want to find that root cause but while you’re investigating, I do find taking Phosphatidylserine is helpful. I use PS 150 from Designs for Health which is a soy-free formula. It helps bring down cortisol and lowers reverse T3. I usually suggest people take two capsules around bedtime.    The Question - How can I lower my TPO (Thyroid peroxidase)? There was a lot more to this question, so I wanted to share exactly what she wrote, “I have Hashi's. My naturopath has patiently and skillfully supported my thyroid, and other hormones, over the past two years to the point where I haven't needed any T4 or T3 for months (steady labs)! Yet, my system is so reactive, daily. I feel like I'm on the edge of hypo/hyper symptoms. And my TPO is still 75-100...? Is there anything I can do to help lower it? I'm on a dialed-in, wholefoods diet (lots of intolerances). Inna’s Response Looking at diet is definitely key here. You mentioned that your naturopath supported your thyroid and your hormones but have you supported the rest of your system? Dealing with food intolerances isn’t just about removing those foods from your diet. It’s good that you are but have you looked into the underlying cause of these sensitivities? So often, I see people take out foods that come up on their food sensitivity test and then they run more tests and they have more intolerances. So, they just continue to take more and more food out of their diet. They might even end up with only 5 or 6 foods left!  Instead, you might want to look at what’s going on in your gut. Typically, if you have a lot of food intolerances it stems from something in the digestive system. It could be either that you’re not digesting things properly. Or, it could be due to dysbiosis (an imbalance of bacteria or yeast in the gut that causes inflammation and leaky gut). If it’s the latter, any food would be inflammatory for you.  Healing the gut may result in food intolerances going away. Abstract: Glia contribute to synapse elimination through phagocytosis in the central nervous system. Despite important roles during development and neurological disorders, the eat-me signal that initiates glia-mediated phagocytosis of synapses remains largely elusive. Here, by generating inducible conditional knockout mice of Cdc50a, we induced stable exposure of phosphatidylserine in the neuronal outer membrane. Surprisingly, acute Cdc50a deletion in neurons causes specific loss of inhibitory post-synapses without affecting other synapses, thereby generating excessive excitability with appearance of seizure. Ablating microglia or deleting microglial Mertk rescues the loss of inhibitory post-synapses, indicating that microglial phagocytosis is responsible for inhibitory post-synapse elimination. Moreover, inhibitory post-synapses in normal juvenile brains also use phosphatidylserine for synapse pruning by microglia, suggesting that phosphatidylserine may serve as a general eat-me signal for inhibitory post-synapse elimination. Abstract: Developmental synaptic remodeling is important for the formation of precise neural circuitry and its disruption has been linked to neurodevelopmental disorders such as autism and schizophrenia. Microglia prune synapses, but integration of this synapse pruning with overlapping and concurrent neurodevelopmental processes remains elusive. Adhesion G protein-coupled receptor ADGRG1/GPR56 controls multiple aspects of brain development in a cell type-specific manner: in neural progenitor cells, GPR56 regulates cortical lamination, whereas in oligodendrocyte progenitor cells, GPR56 controls developmental myelination and myelin repair. Here, we show that microglial GPR56 maintains appropriate synaptic numbers in several brain regions in a time- and circuit-dependent fashion. Phosphatidylserine (PS) on pre-synaptic elements binds GPR56 in a domain-specific manner, and microglia-specific deletion of Gpr56 leads to increased synapses as a result of reduced microglial engulfment of PS+ pre-synaptic inputs. Remarkably, a particular alternatively spliced isoform of GPR56 is selectively required for microglia-mediated synaptic pruning. Our present data provide a ligand- and isoform-specific mechanism underlying microglial GPR56-mediated synapse pruning in the context of complex neurodevelopmental processes. Abstract: Neuronal circuits assembly requires the fine equilibrium between synapse formation and elimination. Microglia, through the elimination of supernumerary synapses, have an established role in this process. While the microglial receptor TREM2 and the soluble complement proteins C1q and C3 are recognized key players in this process, the neuronal molecular components that tag synapses to be eliminated are still undefined. Here we show that exposed phosphatidylserine (PS) represents a neuronal "eat-me" signal enabling microglial-mediated synapse pruning. In hippocampal neuron and microglia co-cultures, synapse elimination can be prevented by blocking accessibility of exposed PS using Annexin V or through microglial loss of TREM2. In vivo, exposed PS is detectable at both hippocampal and retinogeniculate synapses, where exposure coincides with the onset of synapse elimination and increased PS engulfment by microglia. Mice deficient in C1q, which fail to properly refine retinogeniculate connections, display elevated exposed PS and reduced PS engulfment by microglia. These data provide mechanistic insight into microglial-mediated synapse pruning and identify a novel role of developmentally regulated PS exposure that is common among developing brain structures. The dramatic hormone shifts in perimenopause into the menopausal transition make women more vulnerable to Alzheimer's disease.   We discuss the concepts of bioenergetics, glucose "hypometabolism" and mitochondrial changes in pre, peri, and postmenopausal changes. The premenopausal state is protective against brain changes leading to Alzheimer's.    The HPA axis, or the control system for stress and hormone system, becomes dysregulated. Dr Sara describes how symptoms such as anxiety and depression may in fact may not be the diseases themselves, but harbingers of Alzheimer's and cognitive impairment.    Dr Sara's Basics for Estrogen and Hormone Balance: Food choices: basics of water, increased fiber, magnesium Assessing nutrient gaps (organic acid panels, essential fatty acid, oxidative stress panel) Assessment of microbiome (looking for infection, dysbiosis, beta-glucuronidase, etc) with Functional stool testing. Looking at Estrogen metabolism (estrone, estradiol, estrone) this can be done with dried urine testing and other testing to determine how your body is breaking down estrogen and if it is getting rid of it efficiently stool testing with beta-glucuronidase An approach to identifying and actively managing stress and to develop stress resilience Using a the Perceived Stress Scale to assess Measure blood cortisol, dried urine testing Core supplementation: magnesium, DIM (di-indolylmethane). Advanced maneuvers (of course, in the context of overall lifestyle changes) Phosphatidylserine 300-400mg - in divided doses or all at once or at bedtime Lavela (pill form of Lavender) for anxiousness - can get on Amazon Chasteberry - helps perimenopausal women who still get a menstrual cycle. What is Adrenal Fatigue? "Adrenal fatigue" doesn't technically exist despite what many people have read on the internet.  Adrenal fatigue is a term used by many alternative healthcare practitioners but there is no evidence that it is actually a condition.  I'll use the term "adrenal fatigue" however throughout this article to simply describe a hypothalamic-pituitary-adrenal axis that is out of balance. Adrenal fatigue basically means that your adrenal glands are out of balance with the brain and aren't producing healthy levels of the hormones cortisol, DHEA and aldosterone.   Additionally, your adrenals may be over-producing or under-producing adrenaline (norepinephrine & epinephrine).  When you first become stressed the adrenals will pump out a lot of cortisol and adrenaline but over time they become out of balance from prolonged stress. What causes Adrenal Fatigue? Anything that creates stress to the human body can result in adrenal fatigue.  When your body can no longer keep up with the daily stresses of your life it becomes too overwhelming for the adrenal glands and they finally become out of balance.  Single moms working two jobs, financial anxiety, divorce, death of a loved one, environmental toxins, infections, junk food, the shock of a loved one diagnosed with cancer or heart disease, electromagnetic radiation, food allergies, leaky gut, excessive caffeine intake, sugar, too much alcohol, staying up late and a society that is losing good manners and consideration for those around us is really wreaking havoc and causing stress to our bodies.  Your body can only handle so much stress and when it reaches it's breaking point, adrenal fatigue sets in. Adrenal Gland Basics The adrenal glands sit on top of the kidneys and weigh approximately 5 grams each.  Per gram of tissue the adrenals have one of the highest rates of blood flow and the highest concentration of vitamin C.  There are actually two parts to the adrenal gland.  The cortex produces the steroid hormones cortisol, DHEA and aldosterone while the medulla (inner portion) produces epinephrine and norepinephrine. The adrenals actually function based on a circadian rhythm with the greatest amount of hormones produced early in the day and then decrease as the day progresses.  An abnormal adrenal rhythm can wreak havoc on many functions of the body. What are the symptoms of Adrenal Fatigue? Fatigue Slow-starter in the morning Frequent urination Muscle and joint pain Low blood sugar Migraine headaches Osteoporosis Sleep disturbances Poor memory Low sex drive Low body temperature Weak immune system Abnormal thyroid function Sugar or Salt cravings Difficulty recovering from exercise Weight gain and difficulty losing weight Rapid muscle loss even within a few days of working out Dizziness when standing up from a seated or lying position What is the best test for adrenal fatigue? You may be surprised to learn the salivary hormone testing is not an accurate way to measure adrenal function.  Salivary hormone testing has become extremely popular but unfortunately is only gives you a small part of the picture. At the end of cytokinesis, the microtubule-rich midbody connecting the daughter cells is either shed into the extracellular space or retained by one of the daughters. Chai et al. find that the midbodies released from C. elegans Q neuroblasts are cleared via a pathway that closely mimics the removal of apoptotic cell corpses. Is this safe to take with caffeine know an empty stomach and are its effects on women different than men? Matthew asks: I have been looking into this machine called a versa climber to help improve my running and cycling. Do you have an opinion about it? Should I just stick to running and cycling? Joseph asks: My feet are nasty. my wife is on my case to get a pedicure, which i have refused. my latest argument is that the calluses on my feet are protective and if they sand my feet down i will get blisters. even i assume this argument has no merit. on the other hand i see no substantive reason to get one either. are there any good argument for or against getting pedicures (aside from being a man/looking pretty)? Tony Maldonado asks: Ben, I am an experienced ultra marathoner. At least I like to think I am. I've run a 100 miler, a couple of 100k and seven 50k events. Along with a few marathons. I am a middle of the packer. I am currently training for my first 50 miler on a very familiar very hilly course. I also want to run a flat 20 miler that takes place 1 week before the 50 mile event. I usually have a low mileage run the weekend before an ultra but really want to run this 20 miler and feel I can pull it off. What are you're suggestions or thoughts on this? In my response to Tony, I reference my article about Lightning Speed Recovery Secrets. Chris asks: What is your opinion about Phosphatidylserine and its use with endurance athletes? In my response to Chris, I mention Jarrow phosphatidylserine. Kathleen asks: Goat's Colostrum seems to be such a good supplement and I want to take it. But, I've had breast cancer and the info. talks about a growth hormone. It is important that I do not raise my estrogen levels. Is this growth hormone something I should stay away from? I am not on any anti-cancer drugs because of their side effects. Also, it says goat's colostrum can raise low blood sugar. Should people with diabetes not take this supplement? Tifani asks: In one of your podcasts, you mentioned that you were writing, or had written a book about eating in accordance with your body type. I have realized recently that I am a skinny fat person and as a vegetarian that despises weight and resistance training, I am trying to figure out what the right balance of protein/carbs/fat is for me. Thank you for any insight you can give me! Tomaz asks: TUM (Techniche Universitat Munich, Germany) has recently published research of positive impact of beer on endurance sport (emphasizing alkoholfree beer). What is your thought on that? Tom asks: At a running expo before a local run, a salesperson gave me a sample of a dietary supplement called "Limu Original", the "premium fucoidan blend". What is this? Jack asks: I recently competed at the IM70.3 UK and was in a good position going onto the run but after about 1km of the run I began to feel bloated and sick. This GI distress continued over the whole run and so I was unable to take on any more fuel and most importantly was unable to put together the sub 1h25m half I was looking for. Instead I was forced to slow down and stop to be sick regularly causing me to finish severely disappointed with a 1h42m half. I had similar issues last season at both IM70.3 and IM UK so this year I have adopted a paleo diet similar to that outlined in Joe Friel's book to not only improve my diet but to help reduce GI distress. I thought the reduced gluten leading up to the race would help but this was not the case. During the race I had a gel just before the swim start and then consumed 500ml of Gatorade with a gel per hour of the bike. My thoughts are that the other causes could be the energy gels consumed on the bike or inadvertently swallowing the lake water during the swim. I would appreciate your thoughts and ideas to help me discover the cause and prevent it from ruining yet another race. Justin asks: I just finished my 2nd half ironman this past Sunday, I followed one of your training plans on training peaks an took 23 minutes off my first 1/2 iron time. I wasn't sure how it would go as I missed a TON of workouts, but I guess between absorbing as much knowledge from your podcast and doing the workouts I did it came together for me on race day. My question is I have another 1/2 iron race in just under 4 weeks now. Knowing that I have the endurance to get thru the event I want to focus on getting faster now if I can. Should I be doing large ring hill repeats and running hills to boost my strength and speed? Or why is the best way to add some speed over the next few weeks? Sarah asks: I started doing crossfit Dec 1st. My background in fitness is a more traditional bodybuilding type workout with your typical cardio (running, elliptical etc). At first I had a hard time with crossfit since it's powerlifting type stuff, it was like I had to be trained all over again. Since then until now, I've improved greatly, but I lack in the intensity and endurance. Many times I'll stop midway to catch my breath and regroup, but I see the others pressing forth and pushing past. I'd really like to get better at it. I've heard that doing two WODs a day may help in my endurance and intensity, but isn't that overuse? I crossfit 5 days a week at 5:30 am, and my coach is stellar. What recommendations do you have for a crossfitter to get faster, stronger, and to gain endurance and stamina? In my response to Sarah, I recommend the Elite Performance Profile from Bioletics. Philip asks: I recently started taking Lipton Iced Green Tea for beneficial purpose, as i could not take the plain green tea. After bringing some 36 cans at one stretch, i realized that it would provide approx. 80 Cal for a full 330 ml can (every 100 ml is reported to produce 28 Cal). After this i have reduced the intake to 1 can a day with some exercise.
