Typhoid and cholera are high-burden infections in Pakistan, with typhoid being a year-round issue and cholera being seasonal. A holistic approach, involving various sectors and disciplines, is necessary in order to address the global AMR threat. Amir highlights the need for better communication and collaboration to bridge gaps and build trust between different organizations. Featured Quotes: I've been working at the National Institutes of Health for the last 7 years now. So, I've been engaged in the development and the implementation of the national action plan on AMR, and that gave me the opportunity to explore the work in the field of antimicrobial resistance. Reality of AMR in Pakistan [Pakistan] is an LMIC, and we have a huge disease burden of antimicrobial resistance in the country right now. A few years back, there was a situational analysis conducted, and that has shown that there is presence of a large number of resistant pathogens within the country. And National Institutes of Health, they have started a very standardized surveillance program based upon the global antimicrobial use and surveillance system back in 2017. And [those datasets have] generated good evidence about the basic statistics of AMR within the country. So, for example, if I talk about the extensively drug-resistant typhoid, typhoid is very much prevalent in the country. Our data shows that in 2017 there were 18% MDR typhoid cases through the surveillance data. And in 2021 it was like 60%. So that has shown that how the resistance has increased a lot. A number of challenges are associated with this kind of a thing, overcrowded hospitals, poor infection prevention and control (IPC) measures. So, there is AMR within the country—there's a huge burden—and we are trying to look for the better solutions. Local Factors Driving AMR Bacteria, they do not know the borders. We have a close connection with the other Asian countries, and we have a long border connected with the 2 big countries, which are Afghanistan and India and Bangladesh and China. So, we see that it's not limited to 1 area. It's not regional. It's also a history of travel. When the people travel from one area to the other, they carry the pathogen as a colonizer or as a carrier, and they can infect [other] people. So, it's really connected, and it's really alarming as well. You never know how the disease is transmitted, and we have the biggest example of COVID—how things have spread from 1 country to the other, and how it has resulted in a massive pandemic. AMR is similar. We have seen that it's not limited to 1 region. We are part of this global community, and we are contributing somehow to the problem. First, I'll talk about the health care infrastructure. We do have the capacities in the hospitals, but still, there's a huge population. Pakistan is a thickly populated country. It's a population of around 241 million. And with the increasing population, we see that the infrastructure has not developed this much. So now the existing hospitals are overcrowded, and this has led to poor infection control practices within the hospitals. The staff is not there. In fact, ID consultants are not available in all the hospitals. Infection control nurses are not available in all the hospitals. So, this is one of the main areas that we see, that there is a big challenge. The other thing that can contribute is the poor waste management practices. Some of the hospitals—private and public sectors—they are following the waste management guidelines—even the laboratories. But many of the hospitals are not following the guidelines. And you know that AMR is under one health. So, whatever waste comes from the hospital eventually goes to the environment, and then from there to the animal sector and to the human sector. [Another big] problem that we are seeing is the over-the-counter prescription of antimicrobials. There is no regulation available in the country right now to control the over-the-counter prescription of antibiotics. They are easily available. People are taking the antibiotics without a prescription from the doctors, and the pharmacist is giving the patients any kind of medicine. And either it is effective/not effective, it's a falsified, low-quality antibiotic for how long in duration antibiotic should be taken. So, there are multiple of things or reasons that we see behind this issue of AMR. Rational Use of Antimicrobials It is a complex process how we manage this thing, but what we are closely looking at in the country right now is that we promote the rational use of antimicrobials at all levels—not only at the tertiary care levels, but also at the general practitioner level. They are the first point of contact for the patients, with the doctors, with the clinicians. So, at this point, I think the empirical treatment needs to be defined, and they need to understand the importance of this, their local antibiograms, what are the local trends? What are the patterns? And they need to prescribe according to those patterns. And very recently, the AWaRE classification of WHO, that is a big, big support in identifying the rational use of antimicrobials—Access, Watch and Reserve list—that should be propagated and that should be understood by all the general practitioners. And again, I must say that it's all connected with the regulations. There should be close monitoring of all the antibiotic prescriptions, and that can help to control the issue of AMR. National Action Plan on AMR So, when I joined NIH, the National Election plan had already been developed. It was back in 2017, and we have a good senior hierarchy who has been working on it very closely for a long period of time. So, the Global Action Plan on AMR, that has been our guiding document for the development of the national action plan on AMR, and we are following the 5 strategic objectives proposed in the global action plan. The five areas included: The promotion of advocacy and awareness in the community and health care professionals. To generate evidence through the data, through the surveillance systems. Generation of support toward infection prevention and control services IPC. Promoting the use of antimicrobials both in the human sector and the animal sector, but under the concept of stewardship, antimicrobial consumption and utilization. Invest in the research and vaccine and development. So, these are some of the guiding principles for us to develop the National Action Plan, and it has already been developed. And it's a very comprehensive approach, I must say. And our institute has started working on it, basically towards recreating awareness and advocacy. And we have been successful in creating advocacy and awareness at a mass level. Surveillance We have a network of Sentinel surveillance laboratories engaged with us, and they are sharing the data with NIH on a regular basis, and this is helping NIH to understand the basic trends on AMR and what is happening. And eventually we plan to go towards this case-based surveillance as well, but this is definitely going to take some time because to make people understand the importance of surveillance, this is the first thing. And very recently, the Institute and country has started working towards the hospital acquired infection surveillance as well. So, this is a much-needed approach, because the lab and the hospital go hand in hand, like whatever is happening in the lab, they eventually reach the patients who are in the hospitals. Wastewater surveillance is the key. You are very right. Our institute has done some of the work toward typhoid and cholera wastewater surveillance, and we were trying to identify the sources where we are getting these kinds of pathogens. These are all enteric pathogens. They are the key source for the infection. And for the wastewater surveillance mechanism, we can say that we have to engage multiple stakeholders in this development process. It's not only the laboratory people at NIH, but we need to have a good epidemiologist. We need to have all the water agencies, like the public health engineering departments, the PCRWR, the environmental protection agencies who are working with all these wastewater sites. So, we need to connect with them to make a good platform and to make this program in a more robust fashion. Pathogens and Disease Burdon For cholera and typhoid within Pakistan, I must say these are the high burden infections or diseases that we are seeing. For typhoid, the burden is quite high. We have seen a transition from the multidrug-resistant pathogens to the extensively drug-resistant pathogens, which now we are left with only azithromycin and the carbapenems. So, the burden is high. And when we talk about cholera, it is present in the country, but many of the times it is seasonal. It comes in during the time of the small zone rains and during the time of floods. So, every year, during this time, there are certain outbreaks that we have seen in different areas of the country. So, both diseases are there, but typhoid is like all year long—we see number of cases coming up—and for cholera, it's mainly seasonal. Capacity Building and ASM's Global Public Health Programs Capacity building is a key to everything, I must say, [whether] you talk about the training or development of materials. I've been engaged with ASM for quite some time. I worked to develop a [One Health] poster in the local language to create awareness about zoonotic diseases. So, we have targeted the 6 zoonotic diseases, including the anthrax, including the Crimean-Congo hemorrhagic fever and influenza. And we have generated a very user-friendly kind of layout in the local language, trying to teach people about the source of transmission. What are the routes of transmission, if we talk about the CCHF? And then how this can be prevented. So, this was one approach. And then I was engaged with the development of the Learnamr.com. This is online platform with 15 different e-modules within it, and we have covered different aspects—talking about the basic bacteriology toward the advanced, standardized methods, and we have talked about the national and global strategies [to combat] AMR, One Health aspects of AMR, vaccines. So, it's a huge platform, and I'm really thankful to ASM for supporting the program for development. And it's an online module. I have seen that there are around more than 500 subscribers to this program right now, and people are learning, and they are giving good feedback to the program as well. We keep on improving ourselves, but the good thing is that people are learning, and they are able to understand the basic concepts on AMR. The Furnace Of AfflictionIsaiah 48:10 “See, I have refined you, though not as silver; I have tested you in the furnace of affliction.”This is a verse that most of us probably don't like. No one wants to be tested in the furnace of affliction. No one wants to face affliction at all, and yet this is where we grow the most. Do we need to have affliction and trouble in our lives to grow? No, God could just give us all that we need without affliction. God can do whatever He wants. However, He knows if He gives it to us, then we won't appreciate it; we will take it for granted, and we will waste it.This is true with most things. If we are handed everything we ever need and want from our parents, then we won't appreciate what we have, and we probably won't appreciate all they do for us. We will take it for granted because we don't know anything else. How do I know that God knows this? First, He knows everything. Second, He has seen it in his people time and time again. Remember the Israelites that were saved from Egypt? God saved them in a miraculous way from the years of slavery that they had endured. He saved them, and then he provided food for them every single day. However, because it happened every single day, they started to take it for granted. They decided they would rather be in captivity and have more variety in food than continue to be free and eat this miraculous bread from heaven.Can you think of a time in your life when you didn't have to work for something, and you took it for granted? What about the opposite? You had to work really hard for something, and you appreciated it so much more. Another example that just came to mind was water. In America, everyone I know has more water than they know what to do with. Some pay for water, and some have a well. Either way, they don't work super hard for their water. Because of this, I have seen a lot of waste when it comes to water, especially in my family. We don't understand the value of water because we have never gone without water. It has always been right at our fingertips. We currently don't pay for water either because we have a well. I have seen my kids turn the water on to heat up and then not get in it for 10 minutes or more. Can you imagine how much water we are wasting, just letting it run down the drain?I know we are not the only people to do this, either. What about all the water bottles we start to do but then don't finish? I can just imagine what the people in Africa would say or think if they could see all the ways we waste water in America. Globally, more than 1 million people die each year from a lack of access to safe water and sanitation. Unsafe water is a leading risk factor for infectious diseases, including Cholera, Diarrhea, Hepatitis A, Typhoid, and Polio. I find it so hard to understand how there can still be this many people without safe water around the world. I think it can be so easy to think that everyone has the things that you have because you have always had them. You have never known anything different.I think if we lost power in our houses for a week, or if we lost water for a week, we would have a new appreciation for these things. If we had to grow our own fruits and vegetables instead of just buying them at the store, then we might appreciate them a bit more and not waste them. How many of us buy fresh vegetables with every intention of eating them and then wind up throwing them away because they went bad before we could eat them? I think if we had to do without them for a while, or if they were harder to get, we might appreciate them more.The same is true with the afflictions we encounter in our lives. Affliction means any pain or suffering we have had to endure. Today's verse tells us that we are refined through the furnace of these afflictions. We all want to be refined. We want to grow closer to the Lord. We want to grow more into who the Lord is calling us to be, and yet we don't want the afflictions. Who wants to endure pain and suffering? Who wants to be in a furnace? Can't we just be refined without all that? Yes, but this is how God has chosen to refine us. This is for our own good.I was just listening to a sermon by Steven Furtick from Elevation Church the other night while driving home. He said that there was a false article written about their church a few years ago saying that they fake Baptisms. He was not happy about the article, and it was a really stressful time for him and the church. However, now that they are through it, He said he is glad it was written. The reason he said this is because the church was refined through this adversity. When the article was written, they were doing Baptisms at their church once or twice a year. Since that article was written, they now do Baptisms every single week. Was the affliction they had to go through fun? No! But they did grow from it, and God brought a blessing from their church out of it.Think back to some of the hardest times in your life. Are you different now because of them? Are you stronger in some way? Can you see how God used that affliction to refine you? God most likely didn't cause the affliction, yet He allowed you to go through it because He knew He could use it to refine you. He knew He could use it to make you a better person, a better parent, spouse, partner, son, daughter, sibling or wherever else He wanted you to grow.I know adversity and affliction is hard. I know it is not something we look forward to. However, I wonder how differently we would handle affliction if we were able to shift our mindset about it? I wonder how different of an experience it would be if we went into curious and even excited about how God was going to use this to help us grow. If we were able to keep our eyes on Jesus while in the middle of the pain and the suffering and try to find where He is using it to help refine us. Things tend to be easier if we can find meaning in them. Maybe that meaning isn't coming from the situation, but how God is using the situation to refine us make us better?Dear Heavenly Father, I ask you to bless all those listening today. Lord, help us see how you are using our painful situations to refine us. Help us to look at our situation with new eyes. Help us to be curious and excited, not for the pain and suffering but for the new growth in us that came because of this suffering. Lord, help us to remember we are never alone, and you are always right there with us as we go through any adversity. We love you, Lord, and we ask all of this in accordance with your will and in Jesus's holy name, Amen!Thank you so much for joining me on this journey to walk boldly with Jesus. Matt refuses to do so, wins the fight and saves Nyla with the help of Ben Urich Meanwhile Bullseye continues to pretend to be Daredevil and ruin his reputation, but is confronted and defeated by Murdock who's disguised as Bullseye. After taking on Bullet for one last time, Matt Murdock reunites with his friend and law partner Foggy Nelson – who's been working on getting Murdock's law license reinstated. As Daredevil and the Punisher deal with Taskmaster and Tombstone causing trouble, we see Kingpin and Typhoid Mary enter a deal with a General Strang, an investor who agrees to fund Fisk's media enterprises but secretly hopes to infiltrate Fisk's finances from within. Halfway through the year we see the Hand returns to New York and we learn they were the ones behind setting lose Taskmaster and Tombstone on the city. What follows is a multi-book story arc involving Daredevil attempts to take down the Hand. This includes a cameo by the Ghost Rider and some of Stick's old students (including Stone) showing up to help Daredevil in the final showdown – which is our spotlight story this week. The year ends with the first 3 of 4 parts to the Last Rites story arc. In it, we see Daredevil causing division between Kingpin and Typhoid before having her committed. Then we see Nick Fury bring Matt Murdock into SHIELD and ask him for help taking down Kingpin and Hydra who's bankrolling his media enterprise. We then see the chess pieces put in place for a final showdown. The finale of this story is a giant double issue #300 we'll talk about in 1992. Episode 27 - Murdock and Marvel: 1988 This week we begin to see what happens when two massive Kaiju go to war in a region. Everything else either runs away or is destroyed. Such is the world of comics, circa 1988. The Year in Comics Notable and Newsworthy Industry Trends Awards Dan's Favorite The Year in Marvel Events & Happenings New Series/Limited Titles New Characters Series Ending Who's in the Bullpen ROOKIE OF THE YEAR: Bryan Hitch Dan's Favorite The Year in Daredevil Appearances: Daredevil #250-261, West Coast Avengers #28, Thor #392, Amazing Spider-Man Annual #22, Marvel Masterworks #5, Punisher War Journal #2 along with the Daredevil/Punisher Child's Play and Hawkeye graphic novels. Writing credits: Ann Nocenti (#250-261) Pencilers: John Romita Jr (#250-261) Inks: Al Williamson (#250-261) The year being with a new artist, John Romita Jr, and the introduction of a new villain (Bullet) who's been hired by Kelco as we see the continuation of the Tyrone Janson story from late last year. We also meet Bullet's son, Lance, who is afraid the world will end at any moment. Next Daredevil takes on another new villain in Ammo and the Wild Boys during a blackout caused by the Apocalypse's Horsemen – which is part of the Fall of the Mutants event. Daredevil stops him and he ends up in prison. After being robbed several times, Kingpin hires the robber by offering the dissociative identity disorder alter Typhoid $1 million dollars to make Murdock/Daredevil fall in love with her and then crush him – which she agrees. When then see Matt Murdock meeting the other alter, Mary. What follows is a brilliant story arch that lasts through the end of the year. Murdock does fall in love with Mary and ends up fighting Typhoid several times (but doesn't realize the connection). Other subplots going on throughout this arch: Tyrone gets his day in court with Kelco Chemicals – whom is represented by Foggy Nelson – and wins thanks to some ghost lawyering by Murdock Karen Page realizes she's losing Murdock as he's sneaking around to meet with Mary Typhoid grows concerned that Mary's love for Matt is allowing her to grow stronger and could eventually take over. Kingpin falls in love with Typhoid and eventually gets jealous of the Murdock/Mary relationship. After ordering Typhoid to take out Murdock/Daredevil, She assembles a team of Bullet, Ammo, the Wild Boys and Bushwacker to soften him up before she finishes the job. This climatic story is our spotlight story for the week. As the year ends, Daredevil is missing and Johnny Storm (aka the Human Torch) has been enlisted to help a desperate Karen find him. Meanwhile Kingpin isn't exactly happy how Typhoid carried out his order, but sees it as an opportunity to expand in Hell's Kitchen. He is now running on a novel platform: "Trump 2024: EFF AMERICA!" He wants to crash the economy, leave the borders open so (he says) terrorists can pour in. Yesterday a resolution was floated to the RNC to effectively cancel the remaining 48 Republican primaries and declare Trump the nominee NOW. Even though the GOP rules require 1,215 delegates for nomination and he has exactly 32. Moreover, one of the underappreciated ratfinks of the fascist push in America for a quarter century, David Bossie of the Clinton Hunt and Citizens United, is the Committeeman who has actually introduced a resolution subverting what passes for democracy within the GOP, to the Republican National Committee. Last night Trump made a big deal of saying he doesn't want this, that he wants "to do it the old fashioned way" and guess what? I think he's lying. He wants it - he just doesn't want to be seen as being BEHIND it. So if it happens, does Nikki Haley then sue? She certainly hasn't shrunk away from challenging Trump. He threatened to blacklist her donors? She tweeted the news and attached a link TO donate to her campaign! She's clearly aware that unlike almost any other Republican since 2016, she has destabilized him, and threatened him back. How far will she go? Especially since Trump's desire to blow-up the border deal may have created an anti-Trump bloc among Republican Senators who might serve as a Haley base of support. The first is a focus on the Next Generation with the launch of a Pathways Learning Academy in our Parsippany building. This daycare will serve children from the ages of 0-5 Monday through Thursday. Our goal is to provide safe, high quality childcare based on the STEAM curriculum (Science, Technology, Engineering, Arts and Mathematics). In addition we have a passion to introduce our littlest ones to a Bible-based, Christian worldview in a fath-friendly environment. One of our second goals, also focused on the Next Generation, is to create a Summer Camp for kids, which we call Camp Rock. Our dream is to provide a world-class Creative Arts Camp for children aged 6-14 that is Christ-centered, Bible-based, and leverages the arts (music, theater, voice, dance, graphic design) to reach the hearts and minds of the next generation. Our third initiative will be Marketplace Mentoring, where students from the ages of 15-21 will be paired up with a mentor who will connect your Monday-Friday marketplace success to ministry significance, and become a spiritual father or mother to the Next Gen. And, as always, we are committed to global outreach. We will be breaking ground in a brand new country this year to bring fresh, clean drinking water to the people of Zambia, where we will bring God's Word to children with special needs. The water crisis in Africa disproportionately affects women and children who are responsible for fetching water from contaminated sources. Each day 2,300 children die from drinking dirty water and contracting Cholera, Dysentery and Typhoid. We also want to build two churches in Zambia where God's Word can be taught. Immunizations are a critical component of the well child management in pediatric patients, as they are responsible for the prevention of numerous serious diseases with devastating consequences even if adequately treated. While many providers are well versed in the standard immunization schedule for children, the care and management of under-immunized children may present itself a challenge. Join Dr. Shreeti Kapoor, a general pediatrician for over 25 years, and recent Pediatric Resident graduate, Dr. Monique Bailey as they discuss caring for under-immunized children. Specifically, they will: Review the recommended vaccination schedule for all children, with a focus on those aged 0-4 years. Discuss the CDC catch-up immunization schedule with examples from three different age groups. Consider the special case of catch-up vaccination for international adoptees as well as interpreting records containing other immunizations (e.g. BCG vaccine) from a few examples of international endemic areas. Review common causes of febrile illnesses in young, vaccinated and under-immunized children. Examine the treatment algorithm and recommended prophylactic antibiotic options for unvaccinated or under-immunized children.
