For many, integrative medicine has become an unattainable luxury, and healthy diet and lifestyle the prerogative of the privileged. Today's guest, Dr. Ramona Wallace, is attempting to change that. Trained conventionally as a DO, she undertook additional training and certification via the Institute for Functional Medicine to broaden her clinical skills. She practices primary care in an underserved community in Kalamazoo, Michigan, where she incorporates diet and lifestyle recommendations to address her patients' chronic conditions. Careful nutritional assessment has revealed that many of her patients, while overtly overweight, are decidedly malnourished. She has documented a wide range of deficiencies—of B vitamins, vitamins A, C, and D, critical minerals like zinc and magnesium, even full-blown scurvy. These are precisely the patients who are most likely to benefit from nutritional support. Discovering obstacles to compliance is a key element to effect their health transformations. Dr. Wallace believes that individualized care is the key to reversing long-standing health conditions. She mentors medical students in the first-of-its-kind Functional Medicine program at a medical school. She has also co-authored a landmark paper arguing for wellness self-care by doctors, to stave off burnout and to help them be better role models for patients.

She is able to put the bones on the following basic philosophy of vitamin supplementation below: Vitamin supplements are not generally required for the average healthy person who maintains a balanced diet. A well-rounded diet that includes a variety of fruits, vegetables, whole grains, lean proteins, and healthy fats usually provides all the necessary vitamins and minerals the body needs to function properly. However, there are certain situations where vitamin supplements may be necessary or beneficial. These include: 1. Nutritional deficiencies: If blood tests or medical evaluations indicate a specific vitamin deficiency, supplements may be prescribed to address the deficiency and restore optimal levels. 2. Certain health conditions: Some medical conditions or factors such as pregnancy, lactation, malabsorption issues, or chronic diseases may require additional supplementation to meet the increased nutrient demands or compensate for poor absorption. 3. Restricted diets: Vegetarians, vegans, or individuals with limited food choices or restrictive diets may need to supplement certain vitamins such as vitamin B12, which is primarily found in animal-based products. 4. Age-related factors: Older adults may have reduced nutrient absorption or increased nutrient requirements due to age-related changes, making supplements beneficial in certain cases. 5. For this purpose use the contact details above Types of Bones (I use chicken carcass or beef bones) Knuckles Joints Feet Marrow bones For added flavour incorporate meaty bones like oxtail,shank, and short ribs. Chicken feet - contain loads of gelatine and are less expensive than other gelatine-rich bones. Pigs trotters are also full of gelatine and collagen. Vegetables I throw any in to the mix but generally have a base of carrots (including their tops) celery (including the leaf)leek, garlic and greens. Herbs/Spices/condiments I throw around an inch of turmeric root in, black pepper, Himalayan salt (more negative ions than usual salt), star anise, cinnamon stick, bay leaves and apple cider vinegar Step 1: Blanch the Bones Divide the bones between two large pots and cover with cold water. Bring to a boil over high heat and simmer for 10-15 minutes before draining and rinsing the bones with water. Do you have to blanch the bones? The answer is no.However, I recommend, especially if you're making brothfrom beef bones, that you do blanch them first. If you're using chicken bones or chicken feet blanching is less important, but beef bones really need this extra step,especially if you want a clear stock. What's the purpose of blanching? The purpose is to remove impurities, coagulated protein, and blood which ultimately results in a cleaner, better tasting broth. Step 2: Roast the Bones and Vegetables Now that your bones have been blanched, drained, and rinsed, it's time to preheat your oven to 180 C. Transfer the bones and vegetables to the roasting pans. . Roast for 30 minutes before gently tossing the bones and vegetables, and roasting for an additional 15-30 minutes more. Step 3: Transfer the Bones Back to the Stockpots & Bring to a Boil Wash the stockpots that were used to blanch the bones(this is super important) and divide the roasted bones and vegetables between the two pots. Scrape up any brown bits and juices remaining in the roasting pan using a metal spatula and a little water, if needed. With the bones and vegetables add the salt, bay leaves, black pepper, star anise, cinnamon sticks, turmeric and apple cider vinegar between the two pots. Pour enough water in to cover the bones by approximately an inch. Cover each pot with a tight-fitting lid and bring to a low boil. I use an instant pot to do this bit and basically put it onto a soup setting to cook as it is much quicker than the other way, but you can do it on the stove instead. Step 4: Simmer reduce heat to low and simmer, with the lids lightly ajar, skimming any foam or excess fat, as needed. Simmer for at least 8-12 hours Add more water if needed to make sure bones and vegetables remain fully submerged. Step 5: Strain the Bones, once the bones have simmered and your broth is ready, you will need to strain the broth through a strainer. For an extra clear broth, strain a second time through a food-grade muslin. I store in glass jars in the fridge and generally around 1KG of bones makes enough broth for me to use in 10 days.

She is able to put the bones on the following basic philosophy of vitamin supplementation below: Vitamin supplements are not generally required for the average healthy person who maintains a balanced diet. A well-rounded diet that includes a variety of fruits, vegetables, whole grains, lean proteins, and healthy fats usually provides all the necessary vitamins and minerals the body needs to function properly.   However, there are certain situations where vitamin supplements may be necessary or beneficial. These include:   1. Nutritional deficiencies: If blood tests or medical evaluations indicate a specific vitamin deficiency, supplements may be prescribed to address the deficiency and restore optimal levels.   2. Certain health conditions: Some medical conditions or factors such as pregnancy, lactation, malabsorption issues, or chronic diseases may require additional supplementation to meet the increased nutrient demands or compensate for poor absorption.   3. Restricted diets: Vegetarians, vegans, or individuals with limited food choices or restrictive diets may need to supplement certain vitamins such as vitamin B12, which is primarily found in animal-based products.   4. Age-related factors: Older adults may have reduced nutrient absorption or increased nutrient requirements due to age-related changes, making supplements beneficial in certain cases.   5. Geographic or lifestyle factors: People living in regions with limited sun exposure might require vitamin D supplements to maintain adequate levels. Additionally, some individuals with specific lifestyles, such as athletes or those under high physical or mental stress, may benefit from targeted supplementation.   It's important to note that while some individuals may benefit from supplements, excessive intake of certain vitamins can be harmful. It's always best to consult with a healthcare professional or registered dietitian who can assess your specific needs and provide personalized recommendations based on your health status, lifestyle, and diet. For this purpose use the contact details above   Dr Rachel's Broth recipe: Types of Bones (I use chicken carcass or beef bones) Knuckles Joints Feet Marrow bones For added flavour incorporate meaty bones like oxtail,shank, and short ribs. Chicken feet - contain loads of gelatine and are less expensive than other gelatine-rich bones. Pigs trotters are also full of gelatine and collagen. Vegetables I throw any in to the mix but generally have a base of carrots (including their tops) celery (including the leaf)leek, garlic and greens. Herbs/Spices/condiments I throw around an inch of turmeric root in, black pepper, Himalayan salt (more negative ions than usual salt), star anise, cinnamon stick, bay leaves and apple cider vinegar Step 1: Blanch the Bones Divide the bones between two large pots and cover with cold water. Bring to a boil over high heat and simmer for10-15 minutes before draining and rinsing the bones with water.   Do you have to blanch the bones? The answer is no.However, I recommend, especially if you're making brothfrom beef bones, that you do blanch them first. If you're using chicken bones or chicken feet blanching is less important, but beef bones really need this extra step,especially if you want a clear stock. What's the purpose of blanching? The purpose is to remove impurities, coagulated protein, and blood which ultimately results in a cleaner, better tasting broth. Step 2: Roast the Bones and Vegetables Now that your bones have been blanched, drained, and rinsed, it's time to preheat your oven to 180 C. Transfer the bones and vegetables to the roasting pans. . Roast for 30 minutes before gently tossing the bones and vegetables, and roasting for an additional 15-30 minutes more. Step 3: Transfer the Bones Back to the Stockpots & Bring to a Boil Wash the stockpots that were used to blanch the bones(this is super important) and divide the roasted bones and vegetables between the two pots. Scrape up any brown bits and juices remaining in the roasting pan using a metal spatula and a little water, if needed.   With the bones and vegetables add the salt, bay leaves, black pepper, star anise, cinnamon sticks, turmeric and apple cider vinegar between the two pots. Pour enough water in to cover the bones by approximately an inch. Cover each pot with a tight-fitting lid and bring to a low boil. I use an instant pot to do this bit and basically put it onto a soup setting to cook as it is much quicker than the other way, but you can do it on the stove instead. Step 4: Simmer reduce heat to low and simmer, with the lids lightly ajar, skimming any foam or excess fat, as needed. Simmer for at least 8-12 hours Add more water if needed to make sure bones and vegetables remain fully submerged. Step 5: Strain the Bones, once the bones have simmered and your broth is ready, you will need to strain the broth through a strainer.   For an extra clear broth, strain a second time through a food-grade muslin. I store in glass jars in the fridge and generally around 1KG of bones makes enough broth for me to use in 10 days.

Our third Saturday of each month we host a segment called Physician's Third Saturday. We invite Tim Ryan, M.D. of The Physician's Practice to join in health related discussions. Today's episode begins with a discussion of "good" bacteria v. "bad" bacteria in your intestinal tract. We dive into a discussion of probiotics and pre-biotics and if there is any real benefit. We then move toward certain foods that fall into the family of nightshades. This leads to a discussion of the protein lectin. As we wrap up the episode, we then discuss vitamin supplements and are they really worth taking? Perhaps some are better and others don't stay in your body long enough to be helpful.  Dr. Ryan is a family practitioner who currently runs The Physician's Practice in NW Georgia and loves what he does. Growing up, he always thought being a doctor would be the most rewarding career. In 1995, after finishing his Family Practice Residency, he moved to Cartersville because the Olympics were coming to Atlanta and because he liked the community. Many years ago, impeccable research in reputable scientific journals confirmed that every one of us benefits from taking the right kind of vitamin supplements. One excellent study in The Journal of the American Medical Association found that women who take over-the-counter multivitamins in the first months of pregnancy dramatically reduce their risk of giving birth to a neurologically damaged child. Neural tube defects are amongst the most horrible and most common birth defects. They cause everything from death to paralysis in the child, and they affect between 1 and 2 babies in every 1000 births. The study was carried out under the direction of Aubrey Milunsky, director of Boston University's prestigious Center for Human Genetics. It involved 23,000 women, half of whom took multivitamin pills containing folic acid—a B-group vitamin in particularly high demand during pregnancy. The results were astonishing. The incidence of neural tube defects in babies born to the women who took the supplements was less than a quarter of what it was amongst those women who took no supplements. So remarkable were these results that they shook to the core FDA officials, who had insisted for generations that taking nutritional supplements is not only unnecessary but, can cause damage our health. Nevertheless, the FDA and CODEX continue to propagandize the ridiculous idea that “nutrition has no significance for health”. The notion that if vitamins and minerals are allowed on the market, they should only be permitted in ultra-low doses. As a result, month-by-month, clinically effective nutrients are being treated as “toxins”. The levels of nutrients they contain are being reduced. Many are being removed from public access in one country after another. There's a wonderful way around all this which I strongly urge you to explore. Shun chemically made vitamins—which over 90% on the market are—and take food-state nutrients instead. Most vitamins you buy in shops or online are made from synthetic chemicals. They've been manufactured in a laboratory in a futile effort to match the molecular structure of naturally occurring vitamins and minerals found in our foods. As a result, they are chock full of chemically-made nutrients. Your body has been designed to absorb vitamins and minerals from the foods you eat. Manmade synthetics can never match natural molecular structures. In no way are they the same. Our bodies do not handle synthetics well. You can be taking a high-potency, man-made synthetic that somebody's recommended, yet not effectively absorbing the vitamins and minerals it contains. This is a major reason why I am passionate about whole-food natural vitamins and minerals. Good manufacturers of whole-food vitamins usually grow most botanicals themselves that go into their food-state formulas. Using state-of-the-art scientific instrumentation, they test, validate and document the potencies of the raw materials that make up their products with high performance liquid chromatography for testing potency.
