Biography: Dr. John Mullen, DPT, CSCS is a World renowned expert and speaker in sports training and rehabilitation. He is the owner of COR in which Elite level adult fitness, sports performance, and physical therapy are offered, by world renowned experts, designed to help people get the most out of themselves. He is also the CEO of Swimming Science which is a website for those wanting to learn more about swimming, improve their swimming, or make the swim team, but don't know where to start. He received his Doctorate in Physical Therapy at USC in 2011, as well as the Josette Antonelli Division Service Scholarship, Order of the Golden Cane, and the Order of Areté. At USC, he also performed research on swimming biomechanics and lung adaptations in swimming training. Dr. John has worked with multiple professional and Olympic athletes, helping them earn Olympic medals. His dedication to research and individualization spurred him to open COR in 2011. Since 2011, Dr. John has been featured in Gizmodo, Motherboard, Stack Magazine, Swimming World Magazine, Swimmer Magazine, USA Swimming, USA Triathlon, Swimming Science, and many more.  He has worked with the numerous colleges and teams regarding rehab and performance. Before his Doctoral program, Dr. John swam on an athletic scholarship at Purdue University. At Purdue, Dr. John was an Academic Honorable Mention All-American and was awarded the Red Mackey Award and R. O. Papenguh Award. He also won the Purdue Undergraduate business plan and elevator pitch competition, as well as 1st prize with the Indiana Soy Bean Alliance. Dr. John was born in Centerville, Ohio and was a 24-time high school All-American Swimmer. The Paul Gough Audio Experience: Business Lessons for Physical Therapists

In this episode, I reveal how I was able to spot a bad habit that had crept into my business - just by keeping a closer eye on the clinics KPI'S. I noticed a drop in the average visit and the amount each patient was spending and I was able to track it back to the physio's NOT doing a great job in the initial evaluation… In this episode watch and learn as I talk to my staff about getting the first session right - and how if you do (get it right), it sets the scene for a very healthy relationship for both you and the patient. We should all re-name that initial evaluation as the “re-setting expectations” session because that is really what it is required. Do not take for granted - ever - that they know what is happening when they arrive. Patients come with so many false beliefs about what we do that unless we do, indeed, re-set their expectations from the get-go then there could be a less than smooth transition to a complete plan of care. Want to raise your rates? You do that by changing the way they see the proposition you are offering. Want to increase the number of sessions they agree to? You also do that by changing the way they see the value of what you do. There are so many things that are affected by a solid initial consultation and in this episode, I talk to the staff about how all I want them to do in that first session is re-set their expectations to bring them in line with what we really do to help them. I know that if I can get the patients to see what it is that we really do - there'll be no issues and they'll leave very happy at the end. So on this weeks show we are talking “KPI's” - and more specifically, how I can get my staff more aware of what I want to us to achieve - and how each one of the KPI's I have set, is actually effected… It is one thing to know your numbers - it is another to know what INFLUENCES how they appear. Basic cause and effect. The NUMBER you look at, that is the EFFECT. But the magic is understanding what CAUSES the number to appear as it does. As in, what is the driver or the activity that has occurred in the business that month to give you the number or percentage you are looking at. Most of the success in hitting your numbers is simply down to consistently and repeatedly raising your staffs AWARENESS of them. That's why at the Paul Gough Physio Rooms I have recently made each member off staff responsible for one of our KPIS… It doesn't mean it is their fault if we don't hit the target - it just means it is their job to bring everyones attention to it each Monday morning at 10am. I spend so much of my time away from the business these days that I am looking more towards protecting what I have got - than constantly thinking about growth. Better said, I am focused more on “optimising to protect from a down-turn” - than I am constantly “optimising for growth and scale”. Much of that mindset is covered on this episode! The Paul Gough Audio Experience: Business Lessons for Physical Therapists

So this episode comes on the back of an “issue” in my clinic, caused by a lack of communication in my office, about what the difference is between a “sports massage” and actually “getting physiotherapy”… We have had a few patients lately asking for one but not the other and we've realized that they're not always sure what the difference is. The reason it is important? Because the PRICE is different - but there perception of what they're getting can be that it all just FEELS the same. Let me explain: What patients are often looking for is someone to just “get on with it”… Meaning, if you let them, they will value the “DOING” more than the “THINKING” that goes into the diagnosis and creating a plan of care. We live in a world where instant gratification rules… …people want you to do just “do something” - “anything” - to justify the dollar amount they are paying you. This is why people - who should be opting to go see a PT like you or I - may book an appointment with a massage therapist or a personal trainer because they THINK that something will get done right off the bat. In our world, we have to take the time to diagnose, communicate clearly what we find, then set up a plan for them to agree to and then if we have time at the end, maybe get going with some treatment. Why? Because Prescription without diagnosis is Malpractice. We know all of that pre-work is valuable - but do they see it that way? Not always. What I do know is that if you don't help them see the value of what you do - they won't. In this episode you can watch and learn as I spend time talking to my front desk team about how to articulate the difference between all our therapists and services and without talking about credentials or experience. The moment you resort to “qualifications” or “credentials” to justify your fees or services benefits, you are losing. Listen in right to the end as I move into the physiotherapy team meeting and talk to the them about the importance of changing the way that the patient “shows up”. Better said - if you are not educating the patient on how you want to be perceived BEFORE they arrive - they will show up with their own preconceived ideas of your value and most often, it's not as high as you have of yourself. And THAT is the primary reason for so much “push-back” to co-pay, deductibles and cash-pay fees in the PT profession right now. A reason, I believe, that so many Physical Therapist's are running into resistance from patients not-so-keen to pay out of pocket (or sign up for a full plan-of-care) is often because the ‘words' they are using DO NOT mean enough to the patient they are trying to motivate into giving up their time and money… Remember - business is about SOLVING PROBLEMS and we will be rewarded as business owners for our ability to motivate more people to give us their time and money than the other PT clinics available to them! Customers do not just magically walk through the door or any establishment and hand over their time and money - even when they are in pain or have lost function! However, if we are able to reposition our proposal as MEANING something MORE than they currently believe they are getting - then we will get a more favourable outcome for both parties… “Change the meaning, change the outcome” is something that I have drilled relentlessly into my staff if they are ever getting push back or resistance to our fees and in this episode I recall the story of the blind man who made $1000's begging - simply by changing a few words on the sign next to his begging bucket. You have to hear it to believe it. This week, on the Paul Gough Physio Rooms Show we're talking about “the right service for the right clients”… Or to put it another way, firing hassle clients! It's that fine-line between giving great customer service to the RIGHT client - and recognizing when it's time to say “bye-bye” to the wrong clients… It's easy to think that you have to give a “great service” to everyone… You don't. You should be respectful to everyone - as human beings - but when you identify the WRONG type of client for your practice, the energy and the love needs to be saved for the people who value what you do. If you're having issues with clients who “no-show” or are rude or disrespectful - even aggressive or demanding of your staff - then it's time to step up and show them the door! Here's the thing: At that point, you'll be accused of “bad service” - or even hear yourself question the service that you're providing - but the reality is that they're just a wrong fit for your service. They want a “Nokia” - you sell “iPhones”. Why pander to someone who wants something that you can't deliver on? This episode was inspired by some “hassle” clients that we have shown the door to recently - and the debate starts with a client who “no-showed” a “free” session, and then get this, later walked back in to ask if they could have another. Here's the thing: The decision to “no-show” happens days and maybe weeks before when the person was ALLOWED to perceive your service as “not all that important”. After all if they said NO to you - it means they said YES to do something else that they deemed more important. It's not about the “script”, it's not even about “respect” - and it's definitely not about them… …it's about you - and how what you let them believe... influenced the way that they think about what you do. It's why it's so important to understand that your MARKETING is about MORE than just getting patients… If your Marketing is done right - you can change their perception of you… If your incoming phone call is RIGHT - you can change their perception of what you do…. If your customer experience is right - you can change their perception of you - and the value you bring etc etc… And if you do all of that, you are much more likely to get LESS “no-shows”. Welcome to Episode 15 of the Podcast. In this Episode I share with you insights from my recent 3-Day Annual Planning Event. It's January, and I'm fresh from spending 3 entire days with my Team - Planning our success in 2017. I'm a big believer that if you want to be successful, you've got to PLAN for it. Too many people are hoping and wishing - some even praying - but there's not much strategic planning going on and no clear path to the goals. Of course, you need to EXECUTE on what you create during your Planning Meeting, but if you don't have the PLAN in the first place, then you run the risk of starting out 2017 “OPTIMISTIC”, then wondering why by the end of the year you are once again, “PESSIMISTIC”. I devoted 3 full days to this Meeting. Me and my team worked Saturday, Sunday - and all day Monday to make this happen. Add in the fact that I allocate 10% of every working week to staff training and workshop style meetings - you can start to get a feel for how much I believe in this type of stuff… The reason I do? Simple: “You can only leverage two things in business”. One is People - and the other is Systems. And during this Annual Planning Meeting I am able to learn more about both - and how to leverage each one better to help me achieve my business goals. In this Episode I walk you through the 3 days I spent with my team, share with you some of the questions I asked my team, and many of the answers that we collected… The outcome of the planning event? I'll tell you this time next year when I check the numbers! But I know that whatever happens in 2017, if I didn't do this planning, the odds of me being as successful as I want to be are massively STACKED AGAINST ME. On the flip side, sacrificing a weekend to do this gives me the best possible shot of becoming more successful. I start the podcast with a re-cap of my last few weeks here in the clinic - and talk to you about how I was able to maintain business success DESPITE the fact that “4” of my key members of admin staff announced that they were pregnant - and all 4 within the space of 6 months!… When you lose nearly a third of your key members of staff - all at the same time, you really do find out how solid your business is - and if indeed, you actually have a business or a highly stressful job! Episode 14: KEYNOTE: Recorded Live Inside a Top Las Vegas PT Clinic In this 14th Episode of the Physical Therapy Business School Podcast I share with you a recording of a KEYNOTE that I did for the incredible staff of “Fyzical Therapy and Balance Centres” of Las Vegas. "RJ Williams" is the Manager of the clinic and he asked me to talk to all of the staff - admin and PT - so that they could better understand the changes that the our profession is living through - with the goal of being in a better position to take advantage of those changes. I went deep - and spent the whole morning on the topic of how the healthcare profession is in a “world of hurt” right now and one of the reasons is because patients don't make decisions like they used to... Why would they? While providers of health care think that they have been hard-done by with the changes, just image how the consumer feels at having to pay twice as much for health care coverage - and then again to actually access it... In this Keynote, I talk candidly about why most PT clinics are suffering and I identify two specific reasons why: 1. Cost has risen - and so to *resentment* to having to pay And, 2. Options for “look-a-like” providers are everywhere and easier than ever to access and that leads to confusion over who to choose (…and a confused buyer, well, buys nothing). Two cash-based practice owners walk into a bar … And in this podcast episode you get to hear their conversation about hiring and training cash-practice staff and filling their schedules with private-pay patients. I just recently gave a presentation at the Referral Ignition conference and it was an awesome experience. My presentation went very well but the biggest highlight for me was meeting the other practice owners, a number of whom were my podcast listeners and eBook readers. One of the many people I got to finally meet in person was Paul Gough from my podcast episodes 36 and 37. One afternoon, we found ourselves at the hotel bar watching football and I decided to record our conversation. Click here to download this episode The public's perception of physical therapy and especially the “Value” they see in it has become a hot-button topic in the past few years. I recently joined Periscope and in my first live broadcast, I read some excerpts from a fantastic book called Marketing is Everything by Fred Joyal. I discussed value perception in our patients and prospective patients, how we can increase it, and I wanted to share what I said and expand on it a bit further in this episode. [If you're not yet on Periscope, search for it on your phone in the app store and follow me @DrJarodCarter … I'll be doing occasional live broadcasts giving cash-based business advice and answering questions.] In preparation for this episode, I began with some stream-of-consciousness style note writing and all of a sudden I had written a decent amount. So today I decided to share both a blog post and an episode. With so much going on leading up to the release of the Medicare e-book, I didn't have time to make this a perfectly written article… So make sure you also listen to the episode to fill in the gaps and learn my in-clinic value building strategies and post discharge strategies as well. Getting Patients in the Door by Tapping into what's Most Important to them Regardless of your business model you continually need to ask yourself, “Why would someone place a high enough value on my services that they would choose my practice over the competition?” (Note that when the cost of your competition is far less out-of-pocket, the perception of your service's value must be substantially higher to convert a prospective patient into a paying customer) Since I and many other cash-based practitioners tout longer one-on-one care and advanced manual therapy training, let me set the record straight: People do not inherently value your hands-on techniques or longer one-on-one time with their healthcare provider. They value the way those techniques make them feel and ESPECIALLY what limitations those techniques resolve for them. If you think that only rich people have any surplus of funds that could be spent on out-of-network healthcare expenses, go to Vegas. Do you think all those people blowing thousands of dollars in a weekend are rich? Go to a crappy bar that isn't really frequented by people with lots of money, and notice that there are still probably at least a few women with breast implants. Not trying to be crude here, just making a point. Vegas has done an incredible job of convincing the general public that what it offers will make their lives better. Beauty magazines, Hollywood, et al. have done the same when it comes to appearance and what plastic surgery can do for someone's quality of life. In my opinion, the field of Physical Therapy and other related healthcare fields have done a terrible job of this. I personally value time and experiences much more than I value things. I have a pretty crappy old car and pretty small TV, and I only go clothes shopping about once every 2 years; but I just spent 3 weeks in Italy, and last year I spent a month in Australia. So someone who wanted to target me for a product or services would be wise to tap into the idea that the product or services would allow me to be in a position to travel more and have more of the experiences that really get me excited (like surfing). Your market for a cash-based practice is NOT everyone who has pain or everyone who could benefit from your services. It is NOT everyone who has a large financial surplus. You need to get the attention of those who place a higher than average value on the results you specialize in providing to your clients. Click to Tweet What you say online, on the phone, in person, and to those who may be speaking to your prospective patients (i.e. your referral sources) should be skillfully crafted to convert that attention into interest and that interest into the decision to choose your practice over others. This doesn't mean giving anyone a hard sell. It means getting them to focus on what their pain or other subjective findings are keeping them from doing what they feel is important in their life. Paul Gough mentioned in the episode 36 that the biggest fear/concern for prospective patients is not their pain or that they'll always have pain, it's the fear that they will lose their mobility and independence (he was speaking a little more directly about those with back pain). So if you have or would like to develop a niche market, you need to identify the primary fear of someone in that market. Maybe you're in women's health … perhaps your patients most fear that they'll always have an aversion to sex and it will negatively affect their partnership/marriage (yes, the aversion comes from having pain during sex, but your focus is on the resultant fear that the pain creates in their mind). Or perhaps they fear that they'll never be able to confidently go out with friends without pads or diapers due to the possibility of incontinence. That fear has nothing to do with pain, but it's a fear that could compel them to pay cash for PT if you convince them you're practice has the best answer to their problem. Runners run through pain all the time. It's when that pain becomes enough that they can't run as much as they want to, that sparks the fear trigger in their mind. What you say in your marketing needs to: 1)   Stoke that fear a little bit by directing their attention to what may happen if they don't do anything about the problem, 2)   Then provide hope by letting them know their problem is treatable and that many others with the same limitations have found a solution to their problem. 3)   Explain why your solution is different and better than what they will get if they choose to visit your competition. If you do the above correctly, your prospective patient will place a high enough VALUE on your services to book an appointment. Even though it will cost more to see you than your in-network competition down the road. But building the perception of your services as being highly valuable has only begun at this point!!
