(Adam@TheWorldview.com) By Adam McManus European Union delegation warns Pakistan about human rights abuses On Friday, a European Union delegation said that Pakistan's preferential trade status could be jeopardized if it does not address human rights concerns, including controversial Islamic blasphemy laws, forced marriages and conversions of Christian girls, and freedom of religion, reports Morning Star News. According to Open Doors, Pakistan is the 8th worst country worldwide for the persecution of Christians. In John 15:18, Jesus said, “If the world hates you, know that it has hated Me before it hated you.” Reagan Airport's control tower was understaffed Since an Army Blackhawk helicopter crashed into an American Airlines jet last Wednesday night, killing all 67 people aboard both, authorities are taking a closer look at what went wrong. Staffing at the air traffic control tower at Ronald Reagan National Airport was “not normal for the time of day and volume of traffic,” reports The New York Times. The controller, who was handling helicopters in the airport's vicinity Wednesday night, was also handling planes that were landing and departing from its runways. Those jobs typically are assigned to two controllers, not one. The Reagan airport tower was nearly one-third below targeted staff levels, with 19 fully certified controllers as of September 2023 instead of the recommended 30 controllers, according to the most recent Air Traffic Controller Workforce Plan. Alarms were raised about ‘congested' airspace before fatal D.C. crash In related news, Daniel Driscoll, Donald Trump's nominee for Secretary of the Army, questioned why military helicopters needed to conduct training exercises near such a busy commercial airport, reports The Guardian. Driscoll told lawmakers that the incident seemed “preventable” and vowed to review Army practices. Martin Chalk, a former British Airways captain who retired in 2020, said, “The military tend to have a bit of a law-unto-themselves approach,” explaining that military pilots do not have to follow all civil aviation protocols. Senator Kennedy urges Kash Patel to reform the FBI, not burn it down Kash Patel, President Donald Trump's nomination for director of the FBI, was at the center of a tense and highly charged Senate hearing Thursday in which he defended his calls for significant reform at the beleaguered agency. Patel, age 44, worked as a counterterrorism prosecutor for the U.S. Justice Department under the Obama administration. Then, he served in the first Trump administration as chief of staff for the U.S. Department of Defense, deputy director of National Intelligence, and senior director for counterterrorism at the National Security Council. Republican Senator John Kennedy of Louisiana had a fascinating exchange with Patel. KENNEDY: “You're not saying that everybody at the FBI is bad, are you?” PATEL: “Never said that. I've overwhelmingly said, multiple times, that 98% of the FBI is courageous, apolitical, warriors of justice. They just need better leadership.” KENNEDY: “Who put away the Unabomber?” PATEL: “The FBI.” KENNEDY: “Who put away Timothy McVeigh and his [Joseph] Stalin's stomach for blood?” PATEL “Agents at the FBI.” KENNEDY: “Who helped investigate Jesse Smollett, who, in an effort of self-aggrandizement, set back the fight for minority rights for years? The FBI had opened an investigation, wasn't it.” PATEL: “Eventually, yes, Senator.” KENNEDY: “There have been or were some bad people at the FBI and the Justice Department. Mr. Hunter Biden cheated on his income taxes. Didn't pay $1.4 million in income taxes over four years. And he filed fraudulent returns. He tried to deduct his hookers as a business expense, and they hit him with two misdemeanors before there was a public outcry. Don't you think the American people are entitled to know the details of that?” PATEL: “The American people are entitled to a singular form of justice and the details to every public corruption investigation.” KENNEDY: “You, you remember Peter Strzok?” PATEL: “I sure do.” KENNEDY: “One of the lead investigators for Mr. [Robert] Mueller's Russian collusion hoax.” PATE: “Yes, Senator.” KENNEDY: “You remember when the Inspector General, not his colleagues, turned him in? Inspector General found that Mr. Peter Strzok sent an email to his girlfriend. ‘Just went to a Southern Virginia Walmart. I could smell the Trump support.' Remember that?” PATEL: “He did that while employed at the FBI, while working on that investigation.” KENNEDY: “Do you remember when the IG revealed another email by Mr. Strzok to his girlfriend? Mr. Strzok said, ‘I am riled up. Trump is an idiot. Is unable to provide a coherent answer.'” PATEL: “Peter Strzok said that.” KENNEDY: “You remember when his girlfriend texted Mr. Strzok? [She said,] ‘Trump's not ever going to become president, right?' And Mr. Strzok replied, ‘No. No, he won't. We'll stop it!'” PATEL: “I do remember their insurance policy.” KENNEDY: “And then Mr. Strzok testified, in front of God and country, that never had his political beliefs impacted his work. You believe in the tooth fairy?” PATEL: (laughs) “When I was a kid.” (crowd laughs) KENNEDY: “You believe Jimmy Hoffa died of natural causes?” (laughter) Then Mr. Strzok got fired, and next thing we know, the FBI and the Department of Justice, after he sued, gave him $1.2 million. Don't you think the American people are entitled to know the details of that?” PATEL: “Absolutely!” Senator Kennedy gave Kash Patel this advice once he's confirmed to be the new FBI Director. KENNEDY: “Now, I want you to think hard before you answer my next question. Do you believe in the adage that ‘two wrongs don't make a right, but they do make it even'”? PATEL: “Senator, I think if anyone commits a wrong in government service, the American public deserve to know the absolute detail of that corrupt activity.” KENNEDY: “When reforming the FBI and the Justice Department, ‘two wrongs don't make a right, but they do make it even' is the wrong approach, isn't it?” PATEL: “That's correct.” KENNEDY: “And we're going to hold you accountable for that.” PATEL: “I hope you do.” KENNEDY: “There's some good people at the FBI.” PATEL: “Lots.” KENNEDY: “And there have been, and may still be, some bad people there. And you've got to find out who the bad people are and get rid of ‘em in accordance with due process and the rule of law. And then you got to lift up the good people. Don't go over there and burn that place down. Go over there and make it better! Do you commit to us today that you would do that?” PATEL: “I commit to you, if confirmed, Senator, every single day, 24/7, 365, the FBI will be the premier law enforcement agency in the world!” Christian teen hockey player suffered spinal cord injury And finally, on Friday night, January 24th, while playing hockey, 17-year-old Jackson Drum of Parkers Prairie, Minnesota, sustained a traumatic neck injury during a hockey game in Vancouver, Canada. When he hit the boards head first, the 6'4”-tall young man with Fellowship of Christian Athletes Hockey endured a devastating spinal cord injury and had a cardiac arrest. Fox 9 in Minneapolis talked with his aunt, Emily Nouyen. NOUYEN: “There's really no words to describe how our family is feeling.” He was stabilized and rushed to the Vancouver General Hospital. On Saturday, he underwent surgery to fuse C1 and C2 together to stabilize his neck. NOUYEN: “My sister has said a few times, ‘I feel like I'm in a nightmare!' And that's exactly how I would describe it: just devastation.” Jackson remains on a ventilator, unable to move. NOUYEN: “The doctors in Vancouver have prepared my sister and Jason for the possibility that Jack may not walk again, may not be able to use his arms, and may not breathe on his own.” Jackson has a long road ahead in recovery with considerable medical bills accruing. It's the family's hope to move Jackson to the Mayo Clinic in Minnesota as soon as he's stabilized. Please pray that God would miraculously heal Jackson Drum. Pray that the inflammation comes down, that he breathes on his own, and gets feeling back in his arms and legs. So far, $66,280 has been contributed to pay for his medical care including a medical transport from Canada to Minnesota. Earlier this month, the Canadian Association of Physicians for the Environment (CAPE) launched a campaign this month highlighting how the fossil fuel industry in British Columbia is increasing healthcare system costs and reducing access to care. The campaign shared billboards and transit ads, held a press conference in front of Vancouver General Hospital and shared an open letter from over 300 doctors and nurses calling for immediate action to protect communities and the healthcare system. Aaron Volpatti is an Author, Cognitive Performance and Injury Coach, Speaker, retired NHL player, and burn survivor. He is a pioneer in Cinematic Visualization and performance mindset. After a horrific campfire accident in 2005, Aaron was airlifted to Vancouver General Hospital. He suffered second and third-degree burns to 40% of his body and was told his hockey career was over. He faced insurmountable odds. After several surgeries and a gruesome recovery, he returned to the ice four months after his injury and signed with the Vancouver Canucks five years later. In his 5th season with the Washington Capitals, Aaron was hit from behind and underwent a spinal fusion surgery that forced him to retire. In this episode of the IJGC podcast, Editor-in-Chief Dr. Pedro Ramirez is joined by Drs. Jessica Mcalpine and Amy Jamieson to discuss p53abn and no myoinvasion endometrial cancer. Dr. Jessica McAlpine is a surgeon-scientist at the University of British Columbia and BC Cancer and a Tier 1 Canada Research Chair. Dr. Amy Jamieson is a gynecologic oncologist at Vancouver General Hospital and BC Cancer and an Assistant Professor at the University of British Columbia. Highlights: • p53abn endometrial cancers are aggressive tumours, responsible for the majority of recurrences and death from this disease. The optimal management of patients with stage IA p53abn endometrial cancer without myometrial invasion is debated. • We demonstrate the recurrence rates in p53abn endometrial cancer stage IA without myometrial invasion were high at 16% and higher still with residual endometrial tumor (19%). • Recurrence rates in p53abn stage IA without myometrial invasion did not differ compared to p53abn stage IA with myometrial invasion, where adjuvant treatment is routinely recommended. • Most recurrences in patients with p53abn endometrial cancer stage IA without myometrial invasion were distant (89%) and fatal. • High rates of distant and unsalvageable recurrences in stage IA p53abn endometrial cancer without myometrial invasion justify a discussion about adjuvant therapy.
Bio: David Borkenhagen is a Post Doctoral Fellow at the Mathison Centre for Mental Health where he researches community-based interventions for mental health. He completed his PhD in Psychology at the University of Waterloo under the supervision of Dr. Colin Ellard and holds a Bachelors in Neuroscience from McGill University. He uses the methods of behavioural science and research psychology to generate meaningful insights for designers about the effects of urban and architectural design on human health and wellbeing. Examples include developing a data-driven methodology to measure liveability in inner-city communities, conducting user evaluations of aging-in-place laneway housing, and conducting mock-up evaluations of a universal operating room design for the Vancouver General Hospital. In addition to his research work, David also writes theory articles that combine recent insights from philosophy, cognitive science, and neuroscience with contemporary discourses in architecture to better account for the profound effects space has on thought. His writing has appeared in Aeon, Psyche, and the blog of the Journal of the History of Ideas. I'm Stacey Simms and every other Friday I bring you a short episode with the top diabetes stories and headlines happening now. XX In the news is brought to you by Edgepark simplify your diabetes journey with Edgepark XX Our top story this week… XX A recall of infusion sets. This is the VariSoft infusion set used with Tandem Diabetes Pumps. The recall here isn't new.. but the FDA has changed the rating to Class 1, its most serious. The VariSoft version is flexible and can be put in place at an insertion angle anywhere between 20 and 45 degrees, it's usually recommended for people “who are thin or who have scar tissue or limiting potential insertion sites.” The problem is that the connector can detach from the set – which means no insulin is going in. To date, according to the FDA notice, there has only been one report of injury related to the recall. https://www.fiercebiotech.com/medtech/fda-gives-class-i-recall-rating-infusion-sets-used-tandem-insulin-pumps XX An already existing drug may help preserve beta cell function in people with new onset type 1. It's got a very long name, so it's usually referred to as DFMO. It inhibits a pathway, which plays a role in the inflammatory responses in autoimmune diseases, including type 1 diabetes. It's sold under a few names for different conditions, including Vaniqa which is a cream for unwanted hair growth in women. It also has orphan designations for treating various cancers, including neuroblastoma. The new safety study by Sims and colleagues, which was published November 1 in Cell Medicine Reports, enrolled 41 people with type 1 diabetes that had been diagnosed within the previous 8 months, including 31 children. Participants were randomly assigned to undergo oral treatment with DFMO at one of five doses or placebo for 3 months, with 3 additional months of follow-up. Following a mixed-meal tolerance test at 6 months, the C-peptide area under the curve ― a measure of beta-cell function ― was significantly higher with the three highest DFMO doses compared to placebo (P = .02, .03, and .02 for 125 mg/m2, 750 mg/m2, and 1000 mg/m2, respectively). https://www.medscape.com/viewarticle/998671?form=fpf XX Despite earlier promising findings, it seems unlikely that cinnamon can improve blood sugar levels in people with type 1, or insulin-dependent, diabetes, researchers report. Previous research has shown that cinnamon appears to help fat cells recognize and respond to insulin. In test tube experiments and in animal studies, the spice led to a noteworthy increase in the processing of glucose. Moreover, in a previous study of people with type 2, or non-insulin dependent, diabetes, those who incorporated a small amount of cinnamon each day for 40 days into their normal diets experienced a healthy drop in blood sugar levels. But a new study of teenagers with type 1 showed cinnamon made no difference after 90 days . In fact, "In essentially all outcomes...the trend favored the placebo group, although did not achieve statistical significance," the researchers report. https://www.reuters.com/article/us-cinnamon-diabetes-idUSSIB65463020070406/ XX New results from ViaCyte clinical trials – that's a stem cell-based treatment for type 1 diabetes. The therapy aims to replace the insulin-producing beta cells that people with type 1 diabetes lack. Dubbed VC-02, the small medical implant contains millions of lab-grown pancreatic islet cells, including beta cells. The devices—approximately the size of a Band-Aid and no thicker than a credit card—are implanted just beneath a patient's skin where it is hoped they will provide a steady, long-term regulated supply of self-sustaining insulin. The clinical trial was conducted at Vancouver General Hospital, with additional sites in Belgium and the U.S. Ten participants, each of whom had no detectable insulin production at the start of the study, underwent surgery to receive up to 10 device implants each. Six months later, three participants showed significant markers of insulin production and maintained those levels throughout the remainder of the year-long study. These participants spent more time in an optimal blood glucose range and reduced their intake of externally administered insulin. One participant, in particular, showed remarkable improvement, with time spent in the target blood glucose range increasing from 55% to 85%, and a 44% reduction in their daily insulin administration. In another ongoing trial, the UBC-VCH team is investigating whether a version of the device containing cells that have been genetically engineered to evade the immune system, using CRISPR gene-editing technology, could eliminate the need for participants to take immunosuppressant drugs alongside the treatment. https://medicalxpress.com/news/2023-11-stem-cell-based-treatment-blood-sugar.html XX A new look at sleep quality in people with type 1 finding that more time in range means better sleep. No surprise here, but important to quantify. Interestingly, recurrent high or low blood sugar, rather than constant or prolonged higher levels seems associated with worse sleep. A study in adults with type 1 diabetes showed that 17.7% wake up every night, and 53.5% wake up once or twice a week1. This was a small pilot study – the researchers want to now move to a larger one. https://www.nature.com/articles/s41598-023-47351-x XX New study looks at women, sleep and insulin sensitivity. This is a small study, only 40 women, which found that particularly post menopausal women who sleep less 7 hours per night may have impaired insulin sensitivity regardless of body fat. Nearly 40 women were randomly assigned to either restricted sleep or adequate sleep for 6 weeks, then crossed over to the other sleep condition. During sleep restriction, women slept an average of 6.2 hours per night vs 7-9 hours per night. Both fasting insulin levels and insulin resistance were significantly increased during sleep restriction with more insulin needed to normalize glucose. These researchers say if sustained over time, it is possible that prolonged insufficient sleep among individuals with prediabetes could accelerate the progression to type 2 diabetes https://www.medscape.com/viewarticle/insufficient-sleep-impairs-womens-insulin-sensitivity-2023a1000tlz XX XX Pregnant women with type 2 who use CGM saw improvement in blood glucose levels but only if they used the device more than 50% of the time during their pregnancy. This study involved high-risk women from regional and remote Australia all with type 2. No changes in diabetes metrics were seen in those who used the CGM only early or late in their pregnancies. https://www.healio.com/news/endocrinology/20231128/greater-use-of-cgm-linked-to-glycemic-benefits-for-pregnant-women-with-type-2-diabetes XX Commercial XX When it comes to walking and type 2 diabetes risk, it's not just how much you do it that helps — it's also how fast you move, a new study has found. Brisk walking is associated with a nearly 40% lower risk of developing type 2 diabetes later in life, according to the study published Tuesday in the British Journal of Sports Medicine. “Previous studies have indicated that frequent walking was associated with a lower risk of developing type 2 diabetes in the general population, in a way that those with more time spent walking per day were at a lower risk,” said the study's lead author Dr. Ahmad Jayedi, a research assistant at the Social Determinants of Health Research Center at the Semnan University of Medical Sciences in Iran. The study authors reviewed 10 previous studies conducted between 1999 and 2022, which assessed links between walking speed — measured by objective timed tests or subjective reports from participants — and the development of type 2 diabetes among adults from the United States, the United Kingdom and Japan. After a follow-up period of eight years on average, compared with easy or casual walking those who walked an average or normal pace had a 15% lower risk of developing type 2 diabetes, the researchers found. Walking at a “fairly brisk” pace meant a 24% lower risk than those who easily or casually walked. And “brisk/striding walking had the biggest benefit: a 39% reduction in risk. Easy or casual walking was defined as less than 2 miles per hour. Average or normal pace was defined as 2 to 3 miles per hour. A “fairly brisk” pace was 3 to 4 miles per hour. And “brisk/striding walking” was more than 4 per hour. Kathrin Mentler wasn't in a good space and went to Vancouver General Hospital to get some help. She'd been thinking about harming herself and the doctor asked if she had ever considered asking for medical assistance in dying. MAID is a program in Canada that allows terminally ill people the option of dying in a controlled environment. Kathrin was shocked. She had come to the hospital for help in staying alive, not help in planning her death. To think that death was being offered as a choice along with aspirin and bed rest seemed not only uncaring but also defeatist. The hospital offered an apology, along with an explanation that shows health care systems worldwide are all a bit messed up.

A riveting story of defying all odds, three miracles, and the power of love. Danielle Kaplan is my guest, and we tell her story in three parts. We begin by talking about her roots, a South African growing up in a life of privilege but living with the guilt of apartheid, and why she and her husband came to Canada. The second part is when Danielle Kaplan's thrill-seeking husband, Steve Kaplan, crashes his motorcycle on a solo trip that has taken him from Toronto to a deserted road in the Yukon. Steve is thrown off his bike, breaks his spine, and damages his heart. Hungry Grizzly Bears are nearby. No one knows where he is, and he has zero chance of survival. What happens next is spellbinding and genuinely miraculous. The third part is about how Steve survives the crash, but at the Vancouver General Hospital, his organs begin to shut down. Danielle credits the team there, EMCO, a miracle healthcare invention that took over his heart and lungs until he could get stronger and the healthcare workers who made it all happen. She also reminds us how fortunate we are to have universal health care.
