Health Correspondent, Fergal Bowers reports on the main findings of the independent clinical audit into dysplasia of the hips (DDH) surgery in children at a number of hospitals.

Due to the timing this hole will still make it into the updated Resource Estimate due out in mid-year in July. These standout intercepts come from step-out hole # DDH 25-024, which encountered exceptional gold and silver mineralization just beyond the previously defined eastern margin of the conceptual Oculto open pit, in what has been dubbed the Oculto East area. These results underscore the strong potential for continued growth of high-grade mineralization east of Oculto, making this a high-priority exploration target for follow-up drilling in the ongoing Phase V drill program. Highlights include of DDH 25-024 include: 31.0 metres (“m”) grading 10.0 g/t gold and 16 g/t silver, including 6.0 m at 41.9 g/t gold and 22 g/t silver Additionally, the same hole returned a separate interval of 13.0 m grading 307 g/t silver from 216 m depth and within the conceptual open pit, including 8.0 m at 446 g/t silver in the upper silver-enriched zone With three drill rigs now active across the broader Diablillos land package, and the potential to add a fourth rig in the future, the Company is entering another exciting new phase of exploration growth. In addition, the Company is doing all the derisking work programs in parallel with exploration for their ongoing Definitive Feasibility Study due out in early 2026. John Miniotis, President and CEO and David O'Connor, Chief Geologist of AbraSilver Resource Corp (TSX.V:ABRA – OTCQX:ABBRF), join me to review the next batch of drill assays returned from the ongoing 20,000 meter Phase 4 diamond drill campaign on their wholly-owned Diablillos property in Salta Province, Argentina. These assay results we review are from four initial reconnaissance drill holes at the porphyry complex, located approximately 3.5 km northeast of the Oculto deposit. Results indicate that drilling has tested the upper part of a major porphyry system that is estimated to span up to approximately 3 km in length and 2 km in width, hosting zones of gold, silver and copper, including some high-grade intervals. The porphyry mineralization appears to have a high-grade gold epithermal overprint. Dave takes us through the key takeaways from the initial reconnaissance drilling at the Cerro Viejo target in the Diablillos porphyry complex. This included three holes confirming widespread gold mineralization; most notably drill hole DDH 24-056, which intersected 36.0 m at 1.91 g/t Au, starting at a downhole depth of only 87 m, with a higher-grade interval of 5.0 m at 7.22 g/t Au. This intercept highlights a shallow, high-grade gold zone easily accessible for future exploration. Dill holes DDH 24-059 and DDH 24-071, both intersected broad zones of gold, silver, and copper porphyry-style mineralization. The drilling at the Cerro Blanco target was limited to one hole, DDH 24-081, which was not ideally positioned and intercepted only short intervals of gold and copper mineralization. However, historical and recent surface sampling revealed high-grade zones with copper-rich intervals, offering promising targets for future exploration. Next John outlined the ongoing Phase IV drill program at both JAC and Occulto, which is focused on expanding the existing Diablillos Mineral Resource estimate across the broader Diablillos land package. The Company has two drill rigs operating and, to date, has completed approximately 19,000 m of drilling in 96 holes, and with drill likely turning for another month into February, it will likely end up being more than the 20,000 meters initially planned. Several assay results are awaited from the lab and will be released on an ongoing basis pending review. This data from the Phase 4 drilling will feed into an updated Resource Estimate later this year as the next key milestone. Additionally, the upcoming resource estimate in 2025 will feed into the Definitive Feasibility Study (DFS), which will be managed by a team of experienced engineers and consultants, supported by the Company's technical team. The Company will undergo a competitive tendering process to select qualified Engineering, Procurement, Construction and Management firms, with the award anticipated by no later than April 2025. Regular progress updates will be provided by the Company as the DFS advances forward for release in 2026, with results expected to further showcase the Company's potential to become a key low-cost silver-gold producer. Many questions may cross your mind: Is this a permanent damage? What is going on here? Where is the pain located? Do I need to send this child to the hospital? Today, hopefully, we can help you ease some of your fears. Case: This is a 14-year-old boy with no past medical history, no trauma, presents to the family medicine clinic with a complaint of left-sided hip pain. Mom notes that her son has been limping for the last week and complaining of pain in his left hip and knee when he walks. He has never experienced this pain before this week. He does not take any medications. Physical exam: He is afebrile and all of his vitals are within normal limits. On exam, you note that his BMI is at the 90th percentile (overweight), and has an antalgic gait where he is favoring the right side and has tenderness on his left groin. His left foot is turned outward while standing up straight. His left knee has negative findings on specialized tests, but he has restricted movement of the left hip. Discussion: This is a common topic that you will see on board exams or limping into your office. Although pediatric hip pain may seem like a benign musculoskeletal concern, taking the time to take a complete history and perform a thorough physical exam is critical to assess the severity of the patient's concern.Physical Exam for Pediatric Hip Pain.Observation: Every physical exam begins the moment you first see the patient. This allows you to gauge the patient's comfort level, the natural stature, length, and positioning of the patient's extremities, skin changes, gait, and ability to bear weight. Palpation: In medicine, our hands are one of our greatest tools for evaluating patients, especially those with musculoskeletal concerns. This is the time to palpate the area for any tenderness or gross deformities of the pelvis, hip, knee, or leg. Special Tests: In the world of MSK, we have all sorts of tests to evaluate the range of movement of our joints and tendons. When specifically evaluating the hip, the most common are the FABER(flexion, abduction, external rotation),test to assess the sacroiliac joint, Ober's Test to assess the iliotibial band, and Straight Leg Raise to assess for lumbar radiculopathy.Legg-Calve Perthes Disease-Legg-Calve Perthes disease is an idiopathic avascular necrosis of the femoral head. -It is most commonly observed in patients between the ages of 2-12 years and in a higher ratio of males to females 1. -It often manifests as an atraumatic limp with limited movement in abduction and internal rotation. -X-ray imaging may demonstrate a widening of the joint space and sclerosis of the femur, and MRI will confirm osteonecrosis of the femoral head. -Early diagnosis is key to minimizing the risk of developing osteoarthritis of the hip. -The goal of treatment is to maintain the shape of the femoral head and the range of motion of the hip. -The first-line treatment includes managing pain with NSAIDs, limiting weight-bearing activity, and physical therapy for range of motion.-If the disease progresses, bracing and casting can be used to retain the femoral head within the acetabulum to keep the shape and integrity of the femoral head. In more serious cases, a surgical osteotomy may be done to cut and realign the bones. Developmental Dysplasia of the Hip (DDH)-Developmental Dysplasia of the Hip (DDH) is a pediatric condition that results in unilateral or bilateral instability of the hip due to the abnormal development of the acetabulum or femur. -This is most commonly seen in newborns, especially those which develop in a breech position. -These patients often present with a shortened leg or asymmetric gluteal creases and a Trendelenburg gait when walking. -The Trendelenburg gait is an abnormal gait caused by weak hip abductor muscles. The person's trunk shifts over the affected hip during the stance phase of walking and away from it during the swing phase, making it look like the person is missing steps or limping. -On physical exam, hip joint laxity can be evaluated with the Ortolani and Barlow maneuvers to apply pressure to the proximal femur to assess dislocatability of the hip joints. These maneuvers would both be considered positive if a “clunk” is felt over the hip as this means that the hip is dislocated with pressure. Due to the patient's age usually being under 6 months old, ultrasound is the most common imaging modality to confirm the diagnosis, otherwise, an X-ray can be used. -The treatment in patients under 18 months old, a Pavlik Harness is often used to treat patients to maintain the placement of the hip within the acetabulum. -Patients between the ages of 18 months and 9 years old, are most often treated with open or closed reduction of the hip. -There is generally less success in reduction treatment of children older than 9 years old as they have likely developed femoral head deformities and are at greater risk of osteonecrosis. -Children with DDH should continue to be monitored with regular imaging to evaluate for complications. These patients should also be made aware that they are also at increased risk of requiring a hip replacement, especially if their treatment included a reduction. 2Slipped Capital Femoral Epiphysis (SCFE)-Slipped Capital Femoral Epiphysis (SCFE) is one of the most common pediatric hip pathologies in which the capital femoral epiphysis is anterolaterally displaced from the femoral neck. -Although slightly more common in males than females between the ages of 10 to 16, the greatest risk factor for an SCFE is childhood obesity 3. -Common symptoms include an insidious onset of unilateral hip pain and a change in gait due to the displacement of the hip from the acetabulum. In some instances of chronic SCFE, some patients will experience ipsilateral knee pain due to compensation. -A SCFE can be evaluated with an AP radiograph which will demonstrate a widened physis in the early stages or the classic “slipped ice cream cone sign” which is the posterior displacement of the femoral epiphysis. -Management of a SCFE includes limiting weight-bearing activities as well as screw fixation by an orthopedic surgeon to stabilize the hip.Patients should consider pinning the contralateral hip due to increased risk of developing a future SCFE. Early diagnosis is critical as untreated SCFE can lead to osteonecrosis.Osgood-Schlatter-Osgood-Schlatter is a repetitive-use pediatric condition as a result of traction to the growth plate of the tibial tubercle. -This pathology is most common in male children between the ages of 9 to 14 years old 4. -Active athletes or children with rapid growth spurts are at greater risk of developing Osgood-Schlatter than non-active children.-These children often present with an achy knee pain that can lead to a unilateral limping gait. On physical exam, these patients often have a bony prominence over the tubercle that is tender to palpation with greater tenderness over the patellar tendon. -The knee will have full range of motion and stability, but will likely have a warmth and erythema over the knee. Imaging of the knees can have nonspecific findings and diagnosis is made clinically. -For management, it is recommended that children continue their regular activities and rest with NSAIDs for pain management as needed 5. Physical therapy can be prescribed to prevent deconditioning as this can result in recurrence or additional injuries.Arreaza: It seems like the pain is more localized to the knee, but it can be referred to the hip. If you have tenderness on the tibial tubercle, you got the diagnosis. Juvenile Idiopathic Arthritis (JIA)-Juvenile Idiopathic Arthritis (JIA) is a systemic rheumatologic condition in children that often presents as a polyarticular pain. The onset of disease is often bimodal with peaks between 2 to 5 years old and 10 to 14 years old. 6-Patients will often complain of minor symmetric joint pain and stiffness until an infection causes an inflammatory reaction that exacerbates the joint pain or can increase joint involvement. Small joints are the most likely to be involved, but hips and knees can also be affected. -Lab evaluation will demonstrate inflammation with an elevated ESR, low hemoglobin, and a positive ANA. -Disease management starts with NSAIDS for pain control and can escalate to immunosuppressive measures for moderate disease7.Toxic Synovitis-Toxic synovitis, also known as transient synovitis, is the leading cause of acute hip pain and limping in children aged 2–12, more commonly affecting boys. -This self-limited inflammatory condition, often confused by its name as "toxic," has no relation to a toxic state. It typically arises after an upper respiratory or other viral infection (e.g., rubella or coxsackie virus).-Children with toxic synovitis may show mild to moderate hip pain, limp, and keep their hip in abduction and external rotation. Movement is usually possible within a limited range, and weight-bearing is often maintained.-Evaluation: A thorough history and physical exam are key, as laboratory tests like CBC, ESR, and CRP are often normal, mainly used to rule out other conditions like septic arthritis. X-rays typically show no abnormalities, although small changes may appear. Ultrasound can help detect joint effusion and rule out septic arthritis if no effusion is present.Arreaza: DDX: DDH, SCFE, Osgood Schlatter, and toxic synovitis.Osteopathic Manipulative Treatment in Pediatric Hip PathologiesSacroiliac Articulatory Technique- this is a technique in which you move the joint into an out of its barrier to reduce restriction and improve movementCounterstrain of Tender points (psoas, piriformis, hip adductors)- in this technique we shorten the muscle to decrease tension. This allows the muscle to increase blood lymphatic flow to reduce nociceptive and proprioceptive activity of the muscleBalanced Ligamentous Tension of the Innominate- with this technique, we manipulate the joint in a way that moves the ligaments into neutral position so that there is balance in all planes of motion. The goal is to again release tension within the muscles and the jointClinical Decision Making Now that we have covered the most common differential diagnoses for pediatric hip pain, let's revisit our patient presentation and identify the key characteristics to determine which diagnosis he most likely has.The patient is 14 years old. This makes DDH and Legg-Calve Perthe less likely, and SCFE more likely.He has been complaining of symptoms for 1 week, which indicates that is not likely a chronic condition. This makes DDH and Osgood-Schlatter less likely.The patient has never experienced joint pain like this before. This makes JIA, DDH, and Osgood-Schlatter less likely.The patient is overweight. This makes SCFE more likely.The unilateral hip tenderness and no knee pain. This makes Osgood-Schlatter and JIA less likely.The patient has antalgic gait and limited internal rotation of the foot. This makes Legg-Calve Perthes and SCFE more likely. Now when we take the epidemiological factors, the history of the present illness, and the physical exam findings into account, this patient's presentation best aligns with a SCFE. We would order a bilateral AP and Frog-leg views of the hips. If either imaging shows a widened physis or the classic “ice cream cone sign”, this is when we would start the referral process for an orthopedic surgery consultation for internal fixation. As family medicine physicians, we would give instructions for strict non-weight bearing activities and analgesics or anti-inflammatories for pain management.Keep in mind some of the DDX: Calve Legg-Perthes disease, Developmental Dysplasia of the Hip (DDH), Juvenile Idiopathic Arthritis (JIA), Osgood Schlatter, toxic synovitis, and Slipped Capital Femoral Epiphysis (SCFE). We also discuss the new TITAN geophysical survey results and targeting for upcoming drilling of some porphyry targets at Cerro Blanco and Cerro Viejo. We start off having John and Dave recap the recent wide-intercept silver mineralization returned from the next batch of drill holes released from expanding the Oculto and JAC areas.Holes DDH 24-017, DDH 24-021 and DDH 24-024 were drilled in the Oculto northeast area and designed to expand the existing known shallow mineralization. Holes DDH 24-020 and DDH 24-023 were step-out holes in the JAC southwest area that were drilled to extend the existing Mineral Resources beyond the current conceptual open pit boundary. Next we discussed the TITAN geophysical survey results have come back helping to target a couple of deeper drill holes to explore for an underlying copper and gold porphyry system at both the Cerro Blanco and Cerro Viejo targets. This area is located approximately 3.5 km northeast of the Oculto deposit, and drilling in this area is expected to commence by the end of September. Additionally, another target Cerro Bayo, nearby the Oculto deposit, will be drilled, looking for more near-surface high-grade silver and gold oxide mineralization to expand and extend the front-end economics of a project development scenario. Wrapping up, John outlines the company is still very cashed up to finish the ongoing Phase 4 drill program, and complete the upcoming Pre-Feasibility Study, all while heading into next year with plenty of funds left over to continue working on expanding and derisking the Diablillos Project. John Miniotis, President and CEO of AbraSilver Resource Corp (TSX.V:ABRA – OTCQX:ABBRF), joins us to review more drill holes that have been released from the ongoing 20,000 meter Phase 4 diamond drill campaign expanding the JAC Zone to the SouthEast, and the Occulto Zone to the NorthEast on their wholly-owned Diablillos property in Salta Province, Argentina. We also discuss the mapping and targeting for upcoming drilling of some porphyry targets at Cerro Blanco and Cerro Viejo. We start off having John recap the recent wide-intercept silver mineralization returned from the next 3 drill holes exploring the SouthWest extension of JAC. Drill hole # DDH 24-018 was a step-out hole to the south of JAC which intersected a high-grade silver intercept of 31.5 m grading 277 g/t Ag, including 13.7 m grading 455 g/t Ag. This intercept confirms a new significant high-grade mineralized structure at an angle to the main JAC zone, which is now a top-priority exploration target. The southwest extension of the main JAC trend and the structure heading towards the Alpaca zone are currently being drilled. Next we discussed Drill hole DDH 24-011 located in the Oculto northeast zone, between 150 - 200 meters above the previously known dominant level of mineralization. The core is well mineralized to the end of the hole, and this section again demonstrates the potential to expand the currently defined open pit and with it the Mineral Resources and Reserves. More drilling at the NorthEast Extension of Occulto is planned in order to test the various levels of mineralization in the area for shallower mineralization and a potentially higher-grade gold zone beneath. Additionally, the nearby Cerro Bayo will be drilled, looking for more near-surface high-grade silver and gold oxide mineralization to expand and extend the front-end economics of a project development scenario. Wrapping up, we discussed the electromagnetic survey currently being conducted to help site a series of deeper drill holes to explore for an underlying porphyry system at both the Cerro Blanco and Cerro Viejo targets. This area is located approximately 3.5 km northeast of the Oculto deposit, and drilling in this area is expected to commence by the end of September. Dr. Melissa Allen, a Pediatric Orthopedic Surgeon, joins 3rd year Pediatric Resident Erica DeMaagd and 4th year medical student Jacob Weiser to discuss the evaluation and management for DDH. John Miniotis, President and CEO and David O'Connor, Chief Geologist of AbraSilver Resource Corp (TSX.V:ABRA – OTC:ABBRF), join us to review more recently returned high-grade silver drill intercepts from the JAC Zone at the Diablillos Project in Argentina. Today on July 12th, at the JAC Zone, results continue to encounter silver-rich intercepts outside the main mineralized zone, as drilling remains focused on determining the margins of the high-grade silver oxide mineralization. DDH 23-046 located in the northwestern part of the main zone encountered 3.0 metres at 2,070 g/t Ag and 0.27 g/t Au. DDH 23-050 intercepted 10.5 metres at 281 g/t Ag on the SE edge of the main zone. DDH 23-052 encountered 35.5 metres at 112 g/t Ag on the NW edge of the main zone from a downhole depth of only 41.0 metres. Dave outlines why the exploration team is so encouraged by the continued success of the expanded Phase 3 drill program, with 22,000 meters and 113 holes drilled to date out of 115 planned in the larger program. There are over 20 more holes to still report from this drill program that will feed into an upcoming maiden Resource Estimate on the JAC Zone targeted for September/October. There is also ongoing work incorporating other metallurgical data, environmental and permitting work, and other project derisking going on into a Preliminary Feasibility Study on the overall Diablillos Project encompassing both the Oculto and JAC depostis by year end. Current testwork confirms that the same process flowsheet can be used to process mineralization from the Oculto, JAC and Fantasma deposits. We also review that at the Company's earlier-stage La Coipita Project, that drill hole# DDHC 23-001 intersected a broad zone of continuous copper-molybdenum mineralization totaling 694m grading 0.16% Cu and 81 ppm Mo at the Yaretas target from a downhole depth of 548.0 metres.. This large intersection demonstrates the substantial size potential of the Yaretas porphyry system. John mentions that the strategy here moving forward will be to partner with a larger company to earn-in to this project to increase the exploration on such a vast land position. I spoke to Patrick Sullivan May 24, 2023. It was great to catch up on his latest work with Rescue Horses from across the United States and working with them at Liberty. The ability to grow confidence and empowerment through Liberty and the healing that can take place for both horse and people is amazing to hear about. It was wonderful to hear some of Patrick's insights from the path he is on with these horses. Kevin Patricio is the co-founder and CEO of Basqueland Brewing. Kevin emigrated from the United States to Spain in 2011 with his family and now resides in San Sebastian in the Basque Country. Kevin's professional career started in New York City at Food & Wine Magazine in 1998. After six years at the magazine he left to pursue a career in the kitchen. He continued to consult for media and spirit companies until the kitchen took him completely. He trained in NYC classics like The Red Cat, Blue Hill, as well as Arzak in San Sebastian. It wasn't until he was running a kitchen in San Sebastian in 2011 that craft beer came knocking. “In the USA, I took the availability of craft beer for granted. Almost everywhere you go, you're never far from good beer. That all changed when I got to the Basque Country in 2011. The craft beer culture was pre-embryonic. I really wanted to complete the experience for customers in and out of the restaurant which in this case meant having quality, local beer.” He opened Basqueland Brewery in 2015. The brewery is one of the largest independent breweries in Spain and has been named Best Brewery Spain four times in five years (2017, 2019, 2021, 2022) by the Concurso Nacional de Cerveceros, Fira Poblenou; as well as Best Brewery two years in a row (2021-2022) in the international competition, Barcelona Beer Challenge. It can be said that Basqueland is best known for their IPAs, from West Coast to DDH and session to DIPA. The team takes great care with all styles from its crisp, clean Santa Clara Helles Lager to massive pastry stouts and zingy sours. “We strive to make the best beer we can in whatever style.” Basqueland has recently opened Basqueland Izakaia in San Sebastian–a small tavern with Asian bar food and organic wine. It has become a beacon for Asian food in San Sebastian. Kevin hosts guest-chef pop-ups which have included Andy Ricker of Pok Pok and wine guest Nico Boise, the sommelier from Restaurante Elkano. You gotta love a show that challenges your expectations of style, and this one reinvigorates some haze appreciation on our part. We have two new breweries on the show. Both are from England. First up is S43 out of Middlesbrough. Maris the Dank Engine uses columbus, chinook, citra and simcoe and then double dry hopped with cryo citra, cryo columbus, chinook and centennial hops. It's a tropical flavor bomb of goodness. We also try Play Brew which has released a collaboration with S43. Love Note is also double dry hopped IPA but the hops remain a mystery. Both are worthy of great discussion.
