In late 1941, as the Japanese invaded Malaya, British forces battled against overwhelming odds. Led by General Yamashita, the Japanese exploited weaknesses in the British lines, achieving significant victories at the Slim River and Muar. At Slim River, General Paris faced devastating losses as tanks breached his defenses, triggering a chaotic retreat. Scorched earth tactics demoralized locals, while ambushes by Australian forces at Gemas provided brief hope, quickly overshadowed by mounting setbacks. In Muar, British troops retreated to Bakri, where they regrouped under fire and faced a relentless siege. Ambushes and counterattacks ensued, but casualties mounted, and the inexperienced soldiers struggled. General Nishimura's Imperial Guards Division launched a brutal assault, capturing prisoners who faced horrific treatment. Amidst atrocities, survivors like Lance Havildar Benedict escaped, but many were not so fortunate. Despite initial resistance, the Japanese advance continued, forcing the Allies to withdraw to Singapore. In a desperate attempt to hold positions, the remaining British forces faced dire circumstances, battling through ambushes and heavy losses. As they fought for the Parit Sulong bridge, the Australians executed a daring plan to break free but suffered greatly, ultimately leaving wounded behind. General Percival reluctantly ordered a retreat as Japanese roadblocks emerged. Amid chaos, the unwavering spirit of the defenders shone through valiant actions, though many fell victim to the horrors of war. Eventually, the remnants of the Malayan campaign faced the looming siege of Singapore, marking a tragic chapter in the annals of World War II, a tale of bravery, sacrifice, and unimaginable suffering.

The campaign is demanding an end to price gouging by Australian energy retailers and immediate debt forgiveness for consumers experiencing financial hardship in the face of over a decade of increasing energy poverty in the country. In our last episode we tackled random questions with specific answers. After that episode went live, one of our nurses asked about the value of antibiotics with a Bakri balloon in place. This conversation happened as we were placing the balloon for PPH in a patient on Mag-Sulfate for Preeclampsia with Severe Features. That's antibiotic question is another random question that needs to be answered! So in this episode we will examine the data on prophylactic antibiotics with Bakri balloon for metritis prevention. Is there data for that? The ACOG has a statement on that very issue from back in 2018. Additional information has come to print since then, and we will review it. PLUS, as our patient was on Mag-Sulfate, we will also throw in this additional question as a FREEBEE: Is Mag-Sulfate an independent risk factor for PPH? The answer may not be what you think. Contributor: Kiersten Williams MD, Travis Barlock MD, Jeffrey Olson MS2 Summary: In this episode, Dr. Travis Barlock and Jeffrey Olson meet in the studio to discuss a clip from Dr. Williams' talk at the “Laboring Under Pressure, Managing Obstetric Emergencies in a Global Setting” event from May 2023. This event was hosted at the University of Denver and was organized with the help of Joe Parker as a fundraiser for the organization Health Outreach Latin America (HOLA). Dr. Kiersten Williams completed her OBGYN residency at Bay State Medical Center and practices as an Obstetric Hospitalist at Presbyterian/St. Luke's Medical Center in Denver, Colorado. During her talk, Dr. Williams walks the audience through the common causes and treatments for post-partum hemorrhage (PPH). Some important take-away points from this talk are: The most common causes of PPH can be remembered by the 4 T's. Tone (atony), Trauma, Tissue (retained placenta), and Thrombin (coagulopathies). AV malformations of the uterus are probably underdiagnosed. Quantitative blood loss is much more accurate than estimated blood loss (EBL). The ideal fibrinogen for an obstetric patient about to deliver is above 400 mg/dl - under 200 is certain to cause bleeding. Do not deliver oxytocin via IV push dose, it can cause significant hypotension. Tranexamic Acid is available in both IV and PO and can be administered in the field. The dose is 1 gram and can be run over 10 minutes if administered via IV. It is best if used within 3 hours of delivery. When performing a uterine massage, place one hand inside the vagina and one hand on the lower abdomen. Then rub the lower abdomen like mad. A new option for treating PPH is called the JADA System which is slimmer than a Bakri  Balloon and uses vacuum suction to help the uterus clamp down.* Another option for a small uterus is to insert a 60 cc Foley catheter. In an operating room, a B-Lynch suture can be put in place, uterine artery ligation can be performed, and as a last resort, a hysterectomy can be done. *EMM is not sponsored by JADA system or the Bakri balloon. I met Hebba Bakri a few years ago when I was visiting my dear friend Nihal in Cairo. I was immediately drawn to her fascinating personality.For many years, Hebba was running the Longchamps Hotel in Zamalek/Cairo which she had inherited the from her German mother. As a well educated Egyptian, whose father had been a pillar of the Egyptian tourism industry, Hebba had worked at several of the finest hotels in Egypt, Bahrain etc, such as the Mena House, and later served Lufthansa, the German airline for many years. Well traveled and well versed in the tourism industry, Hebba speaks a number of languages, including English, German and French, fluently. She also insisted that her staff be multilingual. She certainly leans a bit towards a European attitude about most things, and her strong German ties reveal themselves in the cleanliness of the Hotel Longchamps. Today, her skills as a hotel manager and operator have also been rewarded with positions on various Egyptian Hotel Association committees.Unfortunately, when we recorded this episode, Hebba announced, that after seventy years under two generations the time has come to say goodbye and retire gracefully while the Longchamps is recognized as the best small Boutique Hotel in Cairo . She remains a legend in the Egyptian tourism industry and will share her time between Germany and Egypt.

This is the first of a special podcast series on obstetric critical care.  I am joined on this series by Dr. Elizabeth Garchar, MD, FACOG.  She is an OB/GYN and Maternal Fetal Medicine (MFM) specialist who has a special interest in obstetric critical care.  She is also unique in that she flies regularly with our critical care transport teams and acts as one of our Assistant Medical Directors for the flight program.  So, Dr. Garchar has unique insight into managing this population in transport. Over the past two years, Saddleback Hospital, in Laguna Hills, Calif., has implemented a robust ERAC (early recovery after cesarean) protocol that includes a transversus abdominis plane block with liposomal bupivacaine for post-op cesarean delivery pain. Results have shown that the protocol has reduced the use of narcotics for the post-op period by 80%, and has improved the overall satisfaction with the post-op period considerably by reducing nausea and vomiting, itching, and constipation. In addition, hospital length of stay has been reduced by one day. According to Steve Garber, MD, a practicing obstetric anesthesiologist for more than 30 years, moms are eating earlier, up and around quicker, and ready and willing to go home earlier. The patient in this case study, who had a myomectomy 20 months earlier, essentially had a very similar surgery to a cesarean delivery. The incision was the same, the uterus was operated upon and the patient remained in the hospital. Her post-op course, as related to Garber and her husband, was vastly different in regard to her narcotic usage and how she felt. She was “out of it” for two to three days, and these were days she was home already. She felt dizzy, wasn't able to concentrate and was in moderate pain, which is to be expected for the extent of her procedure. So now, we get to compare the experience for the same patient, having essentially a very similar surgery, with a different post-op pain protocol: the ERAC protocol. Of note is the extent of the hemorrhage that occurred, the overnight stay in the ICU, Bakri balloon intervention and the patient didn't require any narcotics for pain throughout her stay. The patient was amazed how well she felt post-op day 1, back in her private room with her newborn. “Cesarean delivery is the most commonly performed major surgery in the world, with 1.3 million performed in 2019,” Garber said. “Unfortunately, every year 26,000 women become persistent opioid users after cesarean delivery. With a robust ERAC protocol, the reduction of opioid use is quite significant. “If you look at our patient, if no ERAC protocol was used, she would have felt the same, or probably worse, than after her first surgery. Lethargic, out of it, dizzy, and would have been expected to bond with and take care of a newborn. The first hours and days with a newborn are so precious, and I believe all the moms should have the ability and opportunity to experience it with little to no narcotic use.”

Rampersad and Shanks explain the definition of PPH based on the American College of Obstetricians and Gynecologists (ACOG) guidelines. They further describe the differences between early / acute versus late blood loss, in addition to the most common etiologies using the “Four T's” pneumonic: tone, trauma, tissue, thrombin. Drs. Rampersad and Shanks then describe their approach to the workup and management of PPH. The group discusses topics such as uterine massage, oxytocin, hemabate / methergine, tamponade (e.g. Bakri balloon, the JADA System), embolization, and hysterectomy. The physicians then describe the role of cross-specialty collaboration between OBGYN and IR, specifically in the management of PPH. When highlighting the role of IR, Dr. Beck describes how he counsels patients for uterine artery embolization (UAE), and he provides an anecdote regarding a repeat UAE. He also shares his perspective with utilization of gel foam versus coils. The group then transitions to describe diagnosis and management of placenta accreta spectrum (PAS), its association with PPH, and the role of radiology in this disease process. Lastly, Drs. Rampersad and Shanks allude to what the future may hold for PPH, including more personalized medicine and potential technologies to prevent PAS. A theater student from a Palestinian village without electricity, Mohammad Bakri became a renowned actor and film director. He has stirred controversy in Israel with his movie Jenin, Jenin—a documentary about the Israeli invasion of the refugee camp in Jenin as told entirely from the Palestinian residents themselves. In 2002, Israeli military prevented journalists and human rights organizations from entering Jenin. Rumors of a massacre began to surface. Mohammad Bakri himself sneaked into the camp to ask the question, "What happened?" In this interview, Miko Peled talks with Mohammad Bakri about his first introduction to theater and film in his village and being a young Palestinian theater student in the foreign world of Tel Aviv.
