What goes through your mind when you see a picture of yourself? Do you think, “Oh my gosh, I can't believe that's how I look! How horrifying!” If so, this episode is for you. This is a really common struggle, and I think it's safe to say that we've all seen photos of ourselves that we don't like, but for some of us this can be such a huge trigger for negative body image, self-hatred and desperately wanting to do something to lose weight or change our bodies. I don't want you to erase yourself from your own life by not being in photos, or by deleting every photo of yourself because you're so triggered by the image. You CAN work through the body shame that you feel when you see images of yourself. In fact, you can also desensitize yourself to how triggering this is in the first place. And this is exactly what we're going to cover in this episode. I'm going to walk you through: Steps you can take to work through a body image spiral when you see a photo of yourself that you're horrified by The work you can do NOW to prevent these shame spirals from happening in the future (without skipping out on being in the gosh darn photos) What to do if you feel like you don't recognize yourself in photos - if that image doesn't match the mental image you have of your body (this is actually really common) How to stop comparing your body to past pictures of yourself and to others Go ahead and hit play and give this episode a listen. In this episode, I go solo and talk about my ethos of coaching, the most important aspects of training all masters cyclists should focus on, discuss nutrition and why its important that masters cyclists need to eat differently than the mainstream media suggest and have a rant about some of the dumbest things I've seen on the internet recently Body composition changes associated with fasted versus non-fasted aerobic exercisehttps://www.ncbi.nlm.nih.gov/pmc/articles/PMC4242477/Under an hour training s not affected either way by fasted or a fed state. However, over an hour there are performance advantages of feeding pre-training.https://pubmed.ncbi.nlm.nih.gov/29315892/The current RDA for protein is 0.8g per kg per day. This is aimed at the general sedentary population and doesn't take into consideration athletes or older populations exercising to the level we do.My suggestion is that we should aim for total protein between 1.8g per kg of body weight and as much as 2g per kg of body weight for older athletes. Ryan: Hello, I have been wondering if you know anything about the approximate ratio of black pepper to turmeric is needed to get the benefits of the bioperine? I don't want to get too much into the micro here but also would like to know practically if a tiny grind from the shaker is enough or if larger amounts are needed? Thanks in advance. Cindy: Dr Cabral, I listened to Episode 760 on killing viruses. It talks about the need to ramp up your immune system, however, I have an autoimmune thyroid,(Hashimotos) and was told to avoid things that ramp up my immune system, because the immune system has already gone rogue, and it could cause the body to attack itself even more. For instance, I read to avoid echinacea and Holy Basil. Can you clear this up because I certainly would like to support immune system without causing issues with my autoimmunity. I listen all the time, so thanks for your help on this issue. Cindy Liz: Hi Dr. Cabral, Thank you for putting so much time and effort into your podcasts; it's clear that you have a drive and a need to help people help themselves! I've learned a lot over the past couple of years. My question is, what do you think of fucoxanthin supplements to aid in fat loss? I haven't been able to find a lot of information on it, but what I have seen seems promising. It's hard to find though, and I question the quality of the supplements I have been able to find. Is fucoxanthin legit? And if so how do I find a source I can trust? Thank you! Bob: Hi Dr. Cabral. Love the podcast and your unbias approach to everything you cover. I know you have a lot of content on Intermittent Fasting so I apologize if my question has already been addressed, but I'm wondering what breaks a fast when doing Intermittent Fasting? I'll start my morning w/ some organic non-caffeinated herbal tea (typically some Roasted Dandelion Root or Rose Hips & Hibiscus by Traditional Medicinals, or Lemon Ginger plus Probiotics by Bigelow). Would that break my fast? Also curious about supplements. Would taking a supplement pill first thing in the morning or right before bed break a fast? Crystal: Hi Dr. Cabral! I am 25 and have struggled with my weight my entire life. When I was in high school, I got very slim but then gained over 100 pounds between ages 19-20. I was then diagnosed at 20 with PCOS which has resulted in chronic dry eye syndrome and hypothyroidism. I went vegan shortly after I was diagnosed for ethical and health reasons. I was able to lose the majority of the weight, but in the past year have put a lot back on. Even with listening to your previous podcasts on PCOS, doing slow weighted workouts and steady cardio with balanced, high-protein meals, the weight does not want to shed. I'm at a loss for what to do and was wondering if you could recommend a certain diet/protocol? I want to mention that because I'm vegan and aiming to eat higher protein meals, I often use faux meats to meet my macro goals. I sway towards cleaner products, but many have PUFAs, which I know you say causes inflammation. Could this be a reason I'm not losing weight? Do I have to give these up to reach my goals? What are some better protein sources for vegans besides beans, tofu, and tempeh? Thank you so much! We're planning another group detox soon and wondering if we can use the activated multivitamin instead of the DNS because the DNS flavors were not a hit for us. Other than adding our own hemp protein, are we mostly just missing the glutathione? Thank you for everything you do! -Nancy KP: Is it possible to be a Vata dosha but still gain weight fairly easily? I’m super tall, very small frame and was always in a healthy weight range but quite over fat. By switching to a whole food plant based diet I was able to lose the weight and get down to the correct body fat percentage, but I find that if I eat to full satiety of even these whole plant foods the scale and body fat will start to climb if I’m not careful. I’ve been listening to your podcasts about dosha’s and was curious as to if you can fit the physical description of one but be more metabolically like a kapha (like I think I am). If so, should I be eating to support my vata, or kapha, or do I just choose which one is my current goal? Karen: Hi Dr. Cabral! I'm 48 years old and have noticed that over the years I tend to get cold sores more frequently than I used to even though I eat a much healthier diet than I did when I was younger and am much better at managing stress than I used to be as well. This may be coincidental but it seems like anytime I supplement with creatine I end up with a cold sore. Nothing I have found online would suggest that creatine can increase the incidence of cold sores but am interested in hearing any insight you might have. I have listened to your other podcasts related to cold sores but didn't hear anything related specifically to supplements that might trigger them in some people. Thanks for being such a great resource - it is incredibly appreciated. Georgina: Hi Dr Cabral! I am about to enroll in the IHP program, super excited! I’m interested in your thoughts on the blood type diet? Thank you so much for all that you do, you’re awesome. Georgina Leslie: Thanks for your show..Q: I've been using Fatlossity, and find if I take the evening dose with dinner, it keeps me awake. How can I manage that? I've done a 21-day, then a 14-day and now 2) 7-day detoxs over the lockdowns since last March. I sleep great during the detoxes. I would like to lose another 20 lbs. I also use DNS, Adrenal soothe, and magnesium. I find it difficult to stay on such a restricted diet, so I add back in the blueberries and a sweet potato at lunch. And I no longer lose weight. If I don't eat the sweet potato, I feel strong food compulsions around mid-afternoon and I am often unable to make it until dinner without snacking. Fruit snacks sometimes put me into a tailspin where I am more hungry and want to start grazing through the pantry. I have started to use hypnotherapy to help.. and lost some weight. I would like to use that second dose of Nighttime Fatlossity and still be able to sleep well. Thanks for your ideas. My question is I have a friend in his late 50s treated with oxaliplatin Chemo for stage 3 colon cancer. He has been off chemo for at least 1.5 years but suffers severe debilitating peripheral neuropathy in his lower extremities. His Physicians say there is nothing they can do. Mason: (00:06) Okay. Nirala Jacobi? Nirala Jacobi: (00:07) Very good. Mason: (00:08) Yeah, yay. Nirala Jacobi: (00:12) Yes. Mason: (00:13) Okay. Guys, got to do it in person today, which is- Nirala Jacobi: (00:17) What were the chances of that? Mason: (00:19) Considering you live in Wilson's Creek, I think they're pretty good. But in terms of the chances of doing it, two people, that's a party but I think that's a legal party at these times, isn't it? Nirala Jacobi: (00:29) These days, it is. Mason: (00:31) Oh, pretty legal. Goji is sitting in the room if you hear Goj wrestling around, but dogs don't count. Guys, we're talking about SIBO. We got the SIBO Doctor here. I'm following you on Instagram for, I think, like three years. Nirala Jacobi: (00:51) Wow. Okay. Mason: (00:51) Yeah. I've been aware of your work. SIBO has been one of those things I used to say, facetiously, that it got trendy about three years ago in terms of I don't know where you see the mass awareness come about in the naturopathic and medical circles or whether it's even really accepted in the medical circles but, obviously, you would have watched the trend occur and then the mass misdiagnosis and then realisation that we're actually able to test and find out that it is this SIBO, which we'll find out from you what it is. Why did it, all of a sudden, hit mass consciousness? What I see a few years ago anyway. Nirala Jacobi: (01:42) I'm going to go back nine years. I've been a naturopathic doctor for about 22 years now. I have practised in Montana and saw everything from heart disease to urinary tract infections to actual IBS or irritable bowel syndrome. We had really good result rates, but there was always a subset of patients that just did not improve. Then fast forward nine years ago, I sat in a lecture at one of our conferences and heard about SIBO. It was like a light bulb went on because it explained those cases that just didn't improve with conventional naturopathic approaches even to irritable bowel syndrome. Nirala Jacobi: (02:27) Then, I started to become an expert in SIBO. I moved here about 15 years ago, Australia, but I became an expert and started lecturing for other supplement companies and to practitioners and started a breath testing company because there was just nothing here at all about SIBO. I think one of the reasons why it has really exponentially grown the interest is if you think that about 11% of the world's industrialised nation has IBS. IBS, according to conventional medical texts and the conventional medical approach, has no real cure. Nirala Jacobi: (03:18) To find something that actually is the cause of IBS that is so profoundly responsive to treatment, I think, really gave hope to a lot of people. Now, of course, with that comes the fact that SIBO is often, as you mentioned, I do think there is an element of overdiagnosis. Everybody just basically treats according to the symptoms, which is not what I recommend at all. Because in that case, you can use antimicrobials and things like that for far too long. Nirala Jacobi: (03:53) I think it has to do with the fact that there really wasn't other options for people. They really improved when they began to treat SIBO, or I saw a dramatic improvement in my patients when we finally treated the cause rather than just giving probiotics and giving fibre and giving all of the stuff that we know how to do, and people were actually getting worse, not better with those approaches. That was really my journey into this. Mason: (04:22) That was probably about a time when I think naturopathic medicine got a little bit more integrated even. There were all these different pockets. All of a sudden, naturopaths, even though they were specialising in particular areas, became aware of just all these different specialisations, became I did say trendy for that reason, because it was about the end of that era where people were really trusting health coaches who would read up about the symptoms of SIBO and, therefore, put their clients onto an antimicrobial or whatever it was and just flying blind. You've got the breath test of your business where I see it's like if you're in Chinese medicine, you are doing pulse and tongue and the questions diagnosis. If you're in naturopathic medicine, you need that testing most of the time, I'd imagine. Nirala Jacobi: (05:25) Yeah. I'm a gastrointestinal specialist. I don't just do SIBO. I specialise in functional gastrointestinal disorders, so I do a number of tests. This, I think, is a big shortcoming of practitioners where they consider the finances of ordering a test for a patient. I always tell practitioners that I teach, "You're not their accountant. You don't know if they want to test or not want to test, but it's your job to give them the best options and the diagnosis," because if you're just reading, you're not going to get better because SIBO is a really distinct condition that requires a really strategic approach. There's different kinds of SIBO. Mason: (06:10) That's always what happens. Yeah. It's the same with PCOS or whatever it is. There's different arms. Obviously, there's different sources. There's mainly four major causes, is that right? Nirala Jacobi: (06:25) There's four major groups of causes. Mason: (06:27) Okay, okay. Nirala Jacobi: (06:29) But maybe what we should do is backtrack and really define what SIBO is, right? Mason: (06:33) Yeah, good idea. Well leading, you can tell you have a podcast. Nirala Jacobi: (06:35) Yeah. All right. Let's talk about ... so that people can really understand that it's not just bacterial overgrowth, and as soon as you kill the bacteria, boom, that's it, you're cured. In some instances, that's the case, but it's actually the exception rather than the norm. But SIBO stands for small intestinal bacterial overgrowth. It's a condition where bacteria that are typically usually found in the large intestine are, for some reason, found in the small intestine. Nirala Jacobi: (07:03) Now, the surface area of your small intestine is about the surface area of a double tennis court. Imagine having a massive bacterial overgrowth right where you absorb your nutrients, where you release your enzymes, where you do all of these different important digestive functions and, all of a sudden, that surface area is just chock-a-block with bacteria. These bacteria ferment the food that you're eating into hydrogen gas. There's a group of bacteria or a phylum called proteobacteria. The main gram-negative bacteria in that group are Klebsiella, Proteus, E. coli, those types of bacteria that are the biggest culprit for causing SIBO. Nirala Jacobi: (07:47) Why is this happening? This is how we get into the underlying causes. I think one of the main driving cause of SIBO is, imagine you've gone to Bali, you had a case of food poisoning, or if your listener is in America, you've gone somewhere else and you had food poisoning. You came home, it resolved, but then you still have ongoing digestive symptoms. Actually, over time, they become worse, you go to the doctor, they diagnose you with IBS. That is the classic scenario. Nirala Jacobi: (08:19) What happens there is the bacteria that caused the food poisoning are not the bacteria that are causing SIBO, but they're the bacteria that are damaging to the enteric nervous system, which is really the motility, the brain of your gut. You are meant to have this innate ability to clear bacteria from the upper gut, because the body doesn't want them there. You're supposed to sweep them all towards the small intestine. When you've had a case of food poisoning that results in this damage, you actually cannot effectively clear these bacteria from your upper gut. Mason: (08:55) What is it that's affected in the small intestine and it stops you from having the motility to move it out? Nirala Jacobi: (09:03) This part of the nervous system is called the migrating motor complex. It's a part of the enteric nervous system. Enteric just means digestive or your gut. It's basically the brain in the gut. This particular section of the small intestine is meant to clear these bacteria out every 90 minutes on an empty stomach. Imagine that you've had this food poisoning and it damaged that section or that particular part of what clears the gut out in the upper gut. Nirala Jacobi: (09:32) That actually can be tested with a blood test. We're trying to get it to Australia. Because of COVID, we've had some issues. But we do want to offer this test for people to test for these antibodies, because if you know that's the cause, the proper treatment for SIBO for you would be to have antimicrobials, whether that's the conventional antibiotics that are indicated for this or herbs. Then you must follow it up with something called a prokinetic, which is a medicine that aims to reset this migrating motor complex. That's probably the biggest group of people that have this as an underlying cause. Nirala Jacobi: (10:10) But then you also have people that just were totally stressed out for a long time. Chronic stress, as you probably have discussed this before, causes you to be in this chronic fight or flight. If you're in chronic fight or flight, you're not in rest and digest, it turns off your digestion. These natural antibiotic fluids, like hydrochloric acid, bile, digestive enzymes that are meant to kill bacteria are very poorly produced and, therefore, you suffer not just from maldigestion, but then also bacterial overgrowth. That's a different kind of cause of SIBO that then wouldn't necessarily require the prokinetics. Mason: (10:50) Like a stealthy, slow-grown... Nirala Jacobi: (10:54) Yeah, yeah. Mason: (10:54) I like that you're just actually bringing up those antibacterial fluids. I was going to ask you, and you did it straight away. Nirala Jacobi: (11:04) Yeah. Then the other one, there's more, the fourth group ... The first one would be a matter of a problem with motility. That is not just this, what we call, post-infectious IBS. It can also be hypothyroidism, other autoimmune diseases, mould exposure. All kinds of things can cause this problem with motility. Then you have these digestive factors, and not a big one because a lot of people don't think about this, but previous abdominal surgery that causes scar tissue known as adhesions that actually attach to the small intestine in the abdominal cavity and cause like a kink in the garden hose. That prevents bacteria from leaving the small intestine. Also for that, you would need prokinetics. You can see how it's so much more intricate than just, "Here are some antibiotics," or "Here's berberine and here's Allimax." Nirala Jacobi: (11:59) One last thing I'll say about SIBO before the next question is that there are two groups. I've mentioned the proteobacteria that produce hydrogen. There's another group of ancient organisms. They probably live on Mars, too. Honestly, they're like extremophiles. They live on the bottom of the ocean. There are these ancient archaea. They're not even bacteria. They produce methane. Methane, we know, causes constipation. If you're somebody that's been diagnosed with SIBO methane or SIBO-C or SIBO constipation, it's likely that your methane is high. That's a different kind of treatment. That's starting to be thought of as actually a separate condition. That's advanced SIBO discussion. Mason: (12:46) I like that. We always got this travelling of these bacteria up through the ... Is it the ileocecal valve? Nirala Jacobi: (12:55) Ileocecal valve. Mason: (12:57) Ileocecal valve. Is that a constant occurrence of reality? Nirala Jacobi: (13:00) No. Mason: (13:00) No? Nirala Jacobi: (13:01) No, that is not how it happens. These bacteria, they are normal in very, very small amounts. Nothing in your body is really sterile. Nothing really, even though we think it is, but it's not really. Mason: (13:17) But we've been told it is. Nirala Jacobi: (13:17) Yes, exactly. Mason: (13:17) Programmed. Nirala Jacobi: (13:18) Yeah. It's like modern medicine at the time thought that's what it was, but it turns out that one of the most famous bacteria that survives the stomach is H. pylori. We know it can survive very well there. But you have maybe 1,000 bacteria or colony-forming unit per mil in the upper gut, just below the stomach, the duodenum. Then as you progress towards the large intestine, actually, the diversity and the sheer number of bacteria increases. That's normal. Nirala Jacobi: (14:00) These bacteria, even though gram-negatives that cause SIBO, are actually not pathogens. They're called pathobionts. Pathobionts are organisms that you normally find in low amounts. But when they get overgrown, they become pathogenic. I often tell people, my patients, I say, "Your gut is like a white supremacists neighbourhood. It's just one kind of bacteria, and you need diversity and you need low numbers of those organisms." That's what we're aiming for. Mason: (14:36) There's, I guess, an as above, so below, we've sterilised everything in our environment, in our house, and we have low bacterial biodiversity there, we're going to see low bacterial biodiversity internally. Is there a particular macro or even micronutrient cycles that that gram-negative bacteria ... What did you say? What was the group? Nirala Jacobi: (15:02) The group is called proteobacteria. Mason: (15:03) Proteobacteria. Is there anything that would feed them excessively? Nirala Jacobi: (15:08) No, it's basically food. Those bacteria are usually found in higher amounts in the large intestine. They're normal there. A pathobiont becomes problematic when it outgrows its environment or the other bacteria in that location. They've actually just did a microbiome assessment study on the small intestine. I think the other reason, just to briefly sidetrack to get back to your first question, why is this such a big deal now, is because we know so much more. Nirala Jacobi: (15:44) The Human Microbiome Project that's undergoing, it's like discovering the universe, because what happened before we were able to actually understand what was happening in the small intestine, we couldn't culture out these organisms because they would die. They were anaerobes. They couldn't be cultured out. Now that we have this different technology that uses RNA and DNA, we can understand far more. Now we actually understand the normal microbiome of the small intestine a lot more. It's totally fascinating to be in this field of microbiome research. Mason: (16:26) Of the large intestine bacterial testing and analysis of the biome, testing has got a little bit more efficacy with that, is that right? Nirala Jacobi: (16:33) Oh, way more. Mason: (16:34) Way more? Nirala Jacobi: (16:36) Way more, because it used to be culture-based, it turns out it's like fairy dust of what actually is in the large intestine as a representation of the ... We know about Lactobacillus and Bifidobacterium. That is literally just 2% to 5% of your entire microbiome. There are so many more species that do fascinating things. Mason: (16:59) We've had the chat on the podcast a couple of times of why just throwing a probiotic in the gut is ... Quite often, you can get a little bit more sophisticated. Nirala Jacobi: (17:09) I think we're at that place now where ... I'm somebody who used to just do a probiotic. "Yeah, just a couple of Bifido, couple of Lacto, you're good." But now, I'm way more strain-specific. I would use Bifidobacterium lactis HN019 if you're constipated. I'm not going to necessarily give a whole combination of products, or I give you Lactobacillus rhamnosus if you have leaky gut and eczema, for example. It's a lot more fun now than it used to be. Mason: (17:46) Yeah, I can imagine. It's like rather than just having your shotgun, you got the Men in Black chamber. You walk and there's all different types of guns and grenades all over the wall, but in a more life-giving kind of ... Like a seed gun. Nirala Jacobi: (18:01) I like that. Actually, this brings up a really important point, is that even when I went to naturopathic medical school, it was taught to us that we could reseed the gut. Remember that? Mason: (18:15) Mm-hmm (affirmative). Nirala Jacobi: (18:15) We cannot do that. These Lactobacillus and Bifidobacterium, they are response modifiers. They actually do something that is not involving reseeding. If you've lost a lot of your native species because you were on chronic antibiotics for acne, or Lyme disease, or whatever that may be, if you've lost a lot of your species or have really reduced them, probiotics will not reseed what you've lost. You can't do that. It's a really important point because some practitioners still preach this method, but I had to really switch my thinking. I tell my patients, "I'm going to give you this probiotic for this symptom. I'm not going to give it to you because you're reseeding." You can't do that. Not if it's 5% or 2% of the gut. Mason: (19:01) In terms of it being for the symptoms specifically, is that because the probiotic is able to do it like having a short-term effect within the gut and then it's on its way? Nirala Jacobi: (19:11) Yes, exactly. Exactly. That's what probiotic research is really good at, is seeing what symptoms a particular strain can alleviate. Mason: (19:24) Obviously, we've touched that medication and antibiotics can be another reason why we could lead to SIBO and IBS. Nirala Jacobi: (19:33) Medications like proton pump inhibitors that stop stomach acid, there's some debate whether or not, but I have seen people definitely have a problem with SIBO after using chronically proton pump inhibitors, and others that are more slowing the gut down. Medications like opiates and things like that, morphine will really slow it down. But then that's pretty temporary, you're not going to see chronic SIBO with that. Mason: (20:05) Yeah. The stress factor, you're looking at a combination, mould exposure, stress, and antibiotic here and there, it's kind of a cocktail of reasons, I imagine. Nirala Jacobi: (20:18) This is always the overwhelming part for people. It's like, "Oh, my God, where do I even begin?" But this is where a really skilled practitioner can ... I actually have a questionnaire that you can get on thesibodoctor.com. That is a questionnaire about finding the cause for SIBO. You can download it, it's free, as is the diet that I've devised for SIBO. You can take that to your practitioner and it can whittle it down to what the possible causes are. It goes through these four groups of causes. Mason: (20:52) That's cool. Something that I really like about your approach is I'm hearing just on your website right here, you've got the patient course, practitioner course. Obviously, you're a practitioner and you've got a focus on the patient being able to understand it and get to the source themselves, getting, for lack of a better word, empowered around it, getting informed, and then bridging the way that they can then take that questionnaire and they can create a dialogue between them and their practitioner. Mason: (21:25) It's something we always ... You go there automatically. It's why I like your work. It's something we always try to do and talk about on the podcast when we're chatting with practitioners as well, because it diffuses it. You've even got great resources there of like once you've treated yourself, how are you going to stay out of that practitioner office, which it's overlooked quite heavily. I don't know what your thoughts are on that. Nirala Jacobi: (21:49) The SIBO Success Plan, which is the patient course, it's an eight-hour course that goes through everything from leaky gut to all these different things, it really was born out of a necessity. In a perfect world, everybody would have a practitioner that is SIBO savvy that can nail this thing for you. But I got calls from people or emails from people in Finland and from all over the world that just said, "There's no one here. No one can help me." This is the course that really had to be made for people like that. They don't have a practitioner. Mason: (22:26) You go straight to sibodoctor.com/sibo-success-plan/. So good. Eight hours? Nirala Jacobi: (22:35) It's eight hours because it's eight modules. One of the reasons I shouldn't say I love SIBO, because SIBO is a medical condition, but if a practitioner is listening to this, if you can master SIBO, you got the gut down. You understand practically most of the things that can go wrong with the gut, bearing in mind that there are other issues that are more anatomical problems and stuff. Nirala Jacobi: (23:06) But everything from, like I mentioned, leaky gut, the effects of stress on the gut, what to do when you're constipated, how to help yourself with different home treatments, I have an online dispensary guide that guides you through all the major products that are out there that are for SIBO, and pros and cons and stuff like that, and food sensitivities, histamine intolerance, salicylates, oxalates, SIFO. SIFO is small intestine fungal overgrowth, which often accompanies SIBO. There's a lot there that I had to cover to really make it comprehensive for people. Mason: (23:46) Do you do a leaky gut analysis on a patient as well? Is there always going to be a presence of SIBO and therefore- Nirala Jacobi: (23:55) Not always. No. Mason: (23:57) No? Nirala Jacobi: (23:57) The thing is SIBO can cause leaky gut. Mason: (24:00) Can cause... Right. Nirala Jacobi: (24:00) But just because you have leaky gut doesn't mean you have SIBO. But it is a major cause of it. They've even done research on, all right, well, one month after clearing SIBO, the intestinal permeability was also resolved. If you have the wherewithal and the fortitude to get rid of SIBO, then you can also get rid of leaky gut. Mason: (24:24) I think it's important that you said you do love SIBO because it's, as I mentioned before in the podcast, we're at that point where my mum, she's nine years post-aneurysm, 24-hour care, in a wheelchair all the time. We've done well to keep her off medications and keep her going well, but it's just this bloating that's been there and it finally got to the point where we're like, "Right, we got to test for SIBO," and so we've got there. We're doing the breath test thing. Is it five days? Nirala Jacobi: (24:52) No, it depends on if you're constipated. Mason: (24:55) All right. Nirala Jacobi: (24:55) If you're constipated, it's a 48-hour prep for this test because what we want to do is have bacterial fermentation really down, really reduced before you then start the test, which is a three-hour test where, first, you get up in the morning, you drink this very sugary drink. That's a prebiotic substance that promotes the growth of those bacteria that you've starved over the past two days, one or two days. Then you're measuring your breath every 20 minutes. If we see a rise of hydrogen or methane before 90 minutes, that's the window of SIBO. Mason: (25:36) Yeah. If you get the methane, then we're going into that real nerdy, new sector of SIBO. Is that right? Nirala Jacobi: (25:46) Yeah. [Laughing]. Mason: (25:47) For that instance, my stepdad, he's managing that and he's just looking at like, "All right, test, okay, we can handle it," and trying to get a bunch of carers to all unite and align on that and then looking at having the management of the diet. I think the SIBO diet is the thing. That's why I say I appreciate you saying that you love SIBO because ... But I am curious when you're approaching, how do you keep the excitement up with your patients when you're- Nirala Jacobi: (26:21) That's a really good question. I think that even just this morning, I spoke with somebody who has been ill for so long, and I'm not saying that just curing her SIBO is going to be the be-all, end-all. People are complicated. There's no one approach to it. You can have somebody who has childhood trauma. We know from studies that even childhood trauma can cause what they call adverse childhood events. It can cause a major shift in the microbiome, for example. Nirala Jacobi: (26:53) You can have somebody like that that you work with in finding a good practitioner around trauma and regulating their own nervous system. Then you have somebody who just discovered that their house was full of mould, or you have somebody who has an autoimmune disorder or chronic viral infection. It always is different presentations. It forced me to really become really good at all these different conditions, and that's why I think if you can really not just look at SIBO, but the underlying causes for me is where it's really at where I continue to learn also. Mason: (27:32) Yeah. I guess that's the exciting part, is knowing that you're not just going to have another random go at figuring out what's wrong with you, but you're actually ticking things off to be like, "Look, if it's not this, great. We know it's not this. We know it's not this. We know it's not moulds. That means you're getting closer." I think just the trouble is finding a good practitioner. Nirala Jacobi: (27:56) We have an answer for that. On thesibodoctor.com, we have also the SIBO Mastery Program for practitioners. After they've completed all three levels, they're eligible to be listed free as a SIBO doctor approved practitioner, so all the people that are listed in there. We had to purge a whole bunch. We had to start fresh from scratch this January. As we go along, this list will get bigger and bigger, but they all have taken these very extensive training courses that covers all of these topics. I think you're pretty safe. A lot of them do Zoom calls. I will say that. Nowadays, we're forced to do more and more virtually. Mason: (28:37) Which is amazing. Nirala Jacobi: (28:39) It's amazing. It has its drawbacks. I do, as a practitioner, a hands-on practitioner that does physical assessment and certain manoeuvres, I miss that part but- Mason: (28:50) Can you explain what the physical assessment and manoeuvres are? Nirala Jacobi: (28:54) In America, we're trained like physicians. We're actually like naturopathic GPs, if you will. We're trained in physical exams. I always enjoyed that part of my practise, too. Some people have things like the ileocecal valve problem, which is the valve between the small and the large intestine and it can be stuck open, and then you have this backflow problem with bacteria. You can easily manipulate that with using different manoeuvres, or the hiatal hernia manoeuvre, which is part of the stomach moving into the thorax. It's those kinds of things, as well as physical exam and stuff like that. You get a lot of information from looking at somebody's body, for sure. Mason: (29:39) Yeah, 100%. I can get the drawback, if we can get back to getting in-person as much as possible, great. Otherwise, if you're in Finland and you don't have a practitioner, "Oh, well, that's wonderful." Nirala Jacobi: (29:49) Honestly, well, 90% of my practise is virtual, and then sometimes I'm like, "Okay, stand up, lift your shirt, press there." That will have the work. Mason: (29:58) Yeah, you do what you have to do. Nirala Jacobi: (29:59) Yeah. Mason: (29:59) Do you ever recommend for people to be physically manipulating their own gut with massage as treatment? Nirala Jacobi: (30:07) That's a great question because let's hypothetically say ... Well, let me rephrase it. Yes, if it's for just the ileocecal valve. I do have a little video on my Facebook page, The SIBO Doctor, where I go through how to do it, how to actually release the ileocecal valve yourself. It's not going to be as great as when a trained practitioner does it, but it's good. The massaging of the gut, let's hypothetically say that you're a patient that's listening to this and you're like, "Oh yeah, I may have SIBO." Nirala Jacobi: (30:45) You may have had abdominal surgery for things like you may have had caesarian or you may have had your appendix out or you may have your gallbladder out or the myriad of other things that would be considered routine surgeries, and you have adhesions. That is not a good thing to massage your own belly because it can trigger more scar tissue formation, but light touch, we're just talking light touch. For that scenario, I usually refer to a visceral manipulation practitioner. Mason: (31:17) What's that? Nirala Jacobi: (31:19) Visceral manipulation, so the viscera are the organ up in the abdomen. It's extremely light touch but they are trained to actually feel the rhythms of these organs. Don't ask me what that is. Mason: (31:31) Actually, Tahnee, my fiancée, she's a Chi Nei Tsang practitioner. Do you know that? It's Daoist abdominal massage. Nirala Jacobi: (31:37) Oh, okay. Yeah. Mason: (31:38) We've talked a little bit about it. I was wondering whether that's what you were talking about. Nirala Jacobi: (31:41) Right. No. Visceral manipulation, as far as I know, there's a group from The Barral Institute and they have a very specific technique to very gently break down scar tissue or break up scar tissue. Mason: (31:58) Okay. That's good to get that resource because there's people listening to the podcast, like Tahnee's not practising and she gets asked a lot about doing abdominal massage, so to be able to tune in with another group of practitioners that are doing this I think will help a lot of people. All right. Well, that's going to be in the show notes, gang. When we do get to treatment and, obviously, the dietary charts, there's different phases of healing of SIBO? Nirala Jacobi: (32:27) No, so what happened is, okay, so in a nutshell, the food that promotes or that feeds the bacteria are foods that are high in fibre. That makes sense. Those are healthy foods that feed our own microbiome. That's why we want to eat them. In a case of SIBO, the bacteria are like miles further up so they're fermenting in the wrong place, and so you want to minimise those foods. Those foods are known as from FODMAPs, so Fermentable Oligosaccharides, Disaccharides, Monosaccharides, and Polyols. Did I miss it all? Did we miss one? Anyways, so these are fermentable fibres. Mason: (33:06) Forgot the A? Nirala Jacobi: (33:09) And. Mason: (33:09) Oh, okay. Nirala Jacobi: (33:13) I know, right? Those are the foods that typically are to be avoided when you suffer from SIBO. What I did is I took that diet from Monash. Fantastic work that they did over there to really pinpoint this. Before then, we didn't really know. I took that and put my own spin on it because I found even with that, people were reacting. I made it more restrictive, also added in SCD stuff and that type of thing, because I'm a very structured person and I don't like wishy-washy, vague treatment plans, and so I needed to structure it for myself. Nirala Jacobi: (33:49) What I found is that I got very good results by having a diet that was in two phases. Then I called it the bi-phasic diet. Phase one was the most restrictive part where you have almost no grains and no fruit and really the high-fermenting foods, and you are basically getting tested for SIBO and you're waiting for your test result. I was already seeing dramatic improvement by the time they came back and yes, indeed, the test says it's SIBO, so then we initiated antimicrobials. That then prevented a massive die-off reaction of just throwing in antimicrobials in a system that was still really activated. Mason: (34:33) Okay. You've got them going for, what, a couple of weeks now? Nirala Jacobi: (34:35) A couple weeks, yeah. Yeah. I really did it for practitioners so that they could also tailor it. It still has different food ... Some people are very sensitive to histamines when they have SIBO, and that means no fermented foods, so no sauerkraut, those kinds of- Mason: (34:55) That was very confusing for people, I think, about 10 years ago when capers and sauerkraut and body ecology diet and all that were going off real big time, and then some people will just get these intense levels of bloating every time they'd eat sauerkraut and kimchis and they wouldn't get it. They're like, "What's going on? This is a healthy food." Nirala Jacobi: (35:14) "Persist. Persist. Keep it up. It's just your body detoxing." No, it's your body reacting. Mason: (35:18) Yeah. Herxing became the ultimate. Just, "Oh, it's just a Herx." Nirala Jacobi: (35:20) Yeah, yeah. Exactly. Mason: (35:24) Is that normally enough to reduce the die-off from being too hard for someone just recently getting on the diet for a couple of weeks? Nirala Jacobi: (35:32) It depends. The other thing is if somebody's really constipated, I don't start antimicrobials until at least even with the aid of magnesium oxide or something, I get their bowels moving, because if you add in antimicrobials in a really constipated system, you really are begging for a Herxheimer reaction because the river is not flowing, there's algae growing, it's muddy, it's not moving, it cannot clear out these toxins. I get the system ready before I add in antimicrobials. Mason: (36:06) That's the most important part, is getting the river flowing. Nirala Jacobi: (36:11) If you're constipated. Mason: (36:11) If you're constipated. Nirala Jacobi: (36:12) Yeah, I would say that that's often when people feel really horrible, when they start something like that and it's just not working. It's ironic because with these archaea or these methanogens as we call them, these organisms that produce methane, once it's actually reduced by the help of antimicrobials, you can expect that the bowel returns to normal, but you can't expect it if you're using some form of garlic extract to combat your methanogens. It will take you a couple of months, or if not longer, to really reduce that level to such an extent that you can have spontaneous bowel movements. Mason: (36:53) What antimicrobials are you normally using? Nirala Jacobi: (36:57) When we talk about treatment, there's three kinds. You have your herbs. You have your conventional antibiotics, and these are very specific antibiotics that are not for ear infections or sinusitis. Then you have a third treatment called the elemental diet. Herbs are usually berberine-containing plants, some essential oils like oregano, clove, those kinds of things. There's a bunch of herbs that I use and an extract or a low-fructans kind of garlic. Garlic typically is a FODMAP food, but if we use it with a high-allicine content, we can use quite a lot of it without a problem for these archaea, and we know that they're really effective for that. Then when you look at antibiotics, you're looking at rifaximin, which is a type of antibiotic that stays in the small intestine, doesn't get absorbed, and it's bile-soluble so it works in that perfect environment. Mason: (37:57) Like the way doxycycline works, I think. Nirala Jacobi: (38:00) No, doxy is way more broader and you will absorb some of that. Rifaximin is not absorbed. It stays in the upper gut. Then you have neomycin for the methanogens. Some people use metronidazole or Flagyl, and I shy away from that because I think as practitioners, we're the custodians of our patient's microbiome and we have to really respect that. Some people, I have seen some shocking microbiomes, let me tell you, by just looking at stool tests and things like that. Stool tests will not give you any information about the small intestine but, very often, it's not like it's only in the small intestine. Problems continue on with the large intestine. Mason: (38:44) You're going to have an overgrowth most likely in the large intestine? Nirala Jacobi: (38:45) Yeah. I've seen microbiomes that are completely denuded, like a clear-cut rainforest, and you're trying to regrow it and no wonder they're so reactive. You had actually mentioned my last podcast guest on my show was the guy who wrote Fibre Fueled, Dr. B., Dr. Will B. Mason: (39:10) Dr. Will B. Yeah, that's what I call him instead of [mumbling]. Nirala Jacobi: (39:11) Bulsiewicz. I think it's Bulsiewicz. Mason: (39:15) Yeah, I can never... We were in Arizona. Nirala Jacobi: (39:19) Oh, right. Mason: (39:20) I met him at the mindbodygreen weekend. We had a we called it dads gone wild night... Nirala Jacobi: (39:28) Oh, do tell. Do tell. Mason: (39:29) Yeah, it wasn't that exciting. It was just me, the DJ, and Dr. B just having chats about the gut and veganism and getting on the gluten-free beers. Nirala Jacobi: (39:44) All right. That sounds like a hell of a party. Mason: (39:49) Yeah, it actually was. I think tequila made its way at some point, which is wonderful. Nirala Jacobi: (39:54) You were in Arizona after all. Mason: (39:55) Exactly. Nirala Jacobi: (39:56) Anyway, I really appreciated having him on the show because here he was, a gastroenterologist, epidemiologist, highly, highly trained specialist, and he had a sort of "Come to Jesus" moment when he really started to study the microbiome and started to work on it for himself. Now, he's like a complete convert about protecting the microbiome and regrowing it. I just think he's done a really good job with that book. Mason: (40:25) Is that where his book is coming from? From that angle or- Nirala Jacobi: (40:28) Fibre Fueled, yeah. I'm not his publicist, but I have the book and I read it and it's pretty good. Half the book is recipes, so vegan recipes, and how to regrow it. One word of caution, don't start with SIBO with that. We had this conversation. You can listen in on The SIBO Doctor podcast if you want to listen to the Feeding Your Microbiome. That shift is slowly happening. There's a lot more respect for the microbiome. I know of a lot of physicians who look back on medicine, on what it's done with antibiotics with real regret of like this was the wrong thing to do to just prescribe amoxicillin for every child's ear infection, or to prescribe for sinusitis, for these types of things. Still, to this day, it's happening day in, day out not just here but across the world where it's just way over prescribed, and it will catch up with you. Mason: (41:33) It's an important part of any practitioner's arsenal to be able to reflect on what they're doing and not be too concrete and make sure you don't have too much morality and judgement of yourself if you did just follow the doctrine at the time, but make sure you've got the capacity to... motility to actually move on to what's important, because I know I wasn't really up on the conversation on testing the microbiome and I think we were chatting about that. Mason: (42:01) Since then, I've got a naturopathic friend who he's basically moved a huge amount of his practise over to testing the microbiome and talking about how it takes out a lot of the guesswork, not only is it the antibiotics and seeing exactly the effect that they're having, which is great as well because you know what you've wiped out, but just dietarily as well, if it's vegan, high-carb, if it's carnivore or just high ... Whatever it is. Nirala Jacobi: (42:32) Yeah, carnivore, I would never promote. Never because it is so hard on the microbiome. It just is. That's basically just meat, and unless you live in countries where, for centuries, that's what you did and I just ... Anyways, that's digressing but they are, and we agreed on that. We totally agreed that most diets, really if it already has a diet, then it's a fad mostly. What we know is where people live the longest and, to me, that's evidence and that's the Blue Zones. Nirala Jacobi: (43:14) That's Dan Buettner's work. He wrote a book called the Blue Zones where people lived to be the oldest in the world, fully functional, still doing their daily work, very cognitively attentive, and very happy. There were seven hotspots in the world. They all had different things, but what they all had in common was 80% plant-based diet. For me, I'd go by that. I'd go by that. If people do well on veganism, then do that because the more plants you can eat, the more diverse your bacterial blueprint will be. Mason: (43:56) That's always with the Blue Zones. Yeah, I first heard about it ... The book I got was Healthy Till 100, I believe it was. That book included a couple of other places. I'll put it in the show notes, guys, the scientifically proven secrets of, I think, the world's longest living people. Vilcabamba was in there in Ecuador, which I think isn't in there with Dan's work but, otherwise, it's like Okinawa, Sardinia. Nirala Jacobi: (44:26) Yeah, and Loma Linda which is like eating processed vegetarian food. Mason: (44:33) I think that their faith gets involved. Nirala Jacobi: (44:33) Yeah, I don't know, but they got there in there, Sardinia. Mason: (44:37) Maybe they're just right. Nirala Jacobi: (44:38) Maybe. Well, who knows? Mason: (44:40) Maybe their prayers are just better than health. Nirala Jacobi: (44:41) Yeah, respect Loma Linda, California. Mason: (44:44) Oh, that's right, John Robbins was the author of that book I was talking about. I like him. He balances out, because I think the thing with Dan's work which always I'm like, "So good," then he's like ... because I'm only talking about his behalf and it's like because it's 80% to 90% plant food and then 100% is the obvious conclusion, which I don't find to be the obvious conclusion. Nirala Jacobi: (45:09) No, because I think and I will say if you look at the standard bi-phasic diet just to keep it in the SIBO spectrum, the standard bi-phasic diet is very animal protein heavy. Then I created a vegetarian bi-phasic diet, which is very amenable to vegans, and it's not just about taking the meat. That was a lot of work that I co-authored that with our clinical nutritionist, Anne Criner, here at our clinic. Then we have a third one which is the histamine bi-phasic. But there is something. Nirala Jacobi: (45:41) A lot of people have tried veganism and it's just like, constitutionally, they just couldn't do it. I don't know what the answer is for those people because there are some people that just they get weak. Dr. B would probably argue that he thinks that everybody can live like ... I think, I shouldn't speak for him, but I don't know. I find that everybody is a bit different. Mason: (46:09) Yeah, I'm with you as well. I find if you take one part of the body and solely focus on it, same if you're only focusing on the large intestine and the microbiome and not cellular, in particular cellular markers, then I can see how it would be really easy to justify a vegan diet. I was vegan and raw foodist for quite a while and then moved away from that direction and just was really questioning my need to eat a certain amount of domesticated vegetable and fruit matter. Mason: (46:46) Then once I got back into the microbiome, I've really come to peace and to terms with the fact that, "No, you know what, that's ..." I was really rocking. I was rocking with that majority of my well-being, and even moving back into lentils and legumes and beans, which had a huge chip on my shoulder about. But then just staying open to ensuring there's potentially ... Like in the Blue Zones, meats are normally a side dish, and I like that. Nirala Jacobi: (47:15) Yeah. I do, too. I know myself, I haven't eaten red meat in 40 years probably but I eat chicken occasionally. That's my one and only animal that I eat because also the carbon footprint. It's whole 'nother conversation, Mason. Nothing to do with SIBO. But in a nutshell, the diet is a therapeutic diet. It's not a stay-on-it forever diet. Mason: (47:40) Greaaaaat distinction. I'm going to have to get excited about the diet. I'm going to have to get my mum, because I'll let you all know how. Maybe if I can have a chat again, get you back on here after, I'm going to use all your resources, all the listeners are going to ... I'll keep you in the loop of where mum's at, especially. Nirala Jacobi: (48:02) Sure. Mason: (48:03) Yeah, I'll let you know on an intro at some point where she comes back with in the test. If it's positive, then we'll go on that journey together. With meat and impact. Have you tried a wild, invasive deer or anything from around here. It's like- Nirala Jacobi: (48:21) No, but I'm not opposed to it. I trust my body and I just have no affinity towards those things. Red meat, just no. Mason: (48:35) Yeah, that's fair enough. Nirala Jacobi: (48:38) We're really covering a lot of ground, but there's something about the whole blood type thing that I can tell you as a practitioner, that's been nearly a quarter of a century in practise that there's something about that. Blood type As tend to have a little bit harder time with digesting animal protein. Mason: (48:58) Is that just going back to the classic book, The Blood Type Diet? Nirala Jacobi: (49:02) Dr. D'Adamo. Mason: (49:03) Yeah, D'Adamo. That's right. Nirala Jacobi: (49:06) Yeah. Look, it's still got work to do, but I think there's elements that I certainly have seen be proved in practise. For me, I don't just need theories, I actually need evidence. For me, evidentially, I have seen that in practise, that people that are blood type O, they fade sometimes on a vegan diet because I don't know. I never got so fully into it that I can rattle off the science right now, but it has to do with rhesus factor and different ... Well, the theory was really that when we originated ... See, an evolution story. Nirala Jacobi: (49:47) When we originated in Africa, everybody was blood type O because you needed to be able to eat dead animals and stuff. You had a very forgiving type of blood type that was not very reactive. Then as we moved north and into Europe, it wasn't really economical to eat your animals, and so you became more farmers and started to grow things, and that was blood type A. Then as you move further north, you had natural refrigeration, and that was the AB type or the B type, which can handle dairy really well. That's the theory anyways. I can tell you that much. Mason: (50:22) It's a good theory. Nirala Jacobi: (50:23) It's a good theory. Mason: (50:23) That was always the thing with The Blood Type Diet. Nirala Jacobi: (50:26) It checks out. Mason: (50:27) It checks out. I remember The Blood Type Diet was a funny one because every practitioner I've talked to has said there is something to this- Nirala Jacobi: (50:35) Yeah, there's something to it. Mason: (50:36) But the science was never rock solid so it was open for criticism, yet anecdotally, it was on point. I love it. It's good to know. It's good to go into that world because as soon as you get into, as you said, you made that decision, it's why it's hard sometimes to listen to a practitioner talk about diet long-term because you know that the mindset is based on healing. Then as you said, this is a healing- Nirala Jacobi: (51:03) Therapeutic. Mason: (51:03) Therapeutic diet. Huge distinction because, otherwise, you stay in a "I'm sick" mentality long-term. Nirala Jacobi: (51:11) Right. Look, I always tell my patients when you travel ... Well, it's a different world now, but if you were going to see Paris, I don't want you on this diet. I want you to eat baguette and dip it in the cafe au lait. I want you to eat things that you enjoy. Most of the time, when people travelling and they suffer from food sensitivity, it actually miraculously goes away. Of course, celiac disease is a different story, but there is this element of you just having just more endorphins and your secretory IgA goes up and all of that, and people can tolerate a lot of foods that they would not normally tolerate in a happy setting and a happy live-your-life, I want you to drink wine if you're in Italy. Why restrict ourselves to this myopic thinking, it has to look this way? Mason: (52:03) It's refreshing. I like the way that you're bridging over there. It's something that I've always liked about your accessible approach because it's like bridge into what's actually going on and then I'm going to see your bridge out over there to live your life because, obviously, people do get addicted to being sick and something being wrong and then the fear of if I do something outside of the therapeutic- Nirala Jacobi: (52:23) There's a lot of fear. There's a lot of food fear and there's this whole new term of orthorexia. Mason: (52:29) Yeah, exactly. Nirala Jacobi: (52:30) That's a real thing. A lot of people are so concerned about having made some small error on the bi-phasic diet. I'm like, "You've made no error. It's fine." Not just the bi-phasic diet but also anything, really. They get very, very hooked on that they did something wrong, and there's a lot of food fear and that. Imagine, you're sitting down to eat your meal and you're already worried about the food. Sometimes, I tell people sit for two minutes and just appreciate the food, just take a moment and get into a rest and digest before you eat. Mason: (53:13) That's where the prayer comes in, the grace. Nirala Jacobi: (53:16) It used to be prayer, it used to be grace, all of that. That's all. It's a thing. Mason: (53:21) I feel like we go down this rabbit hole, that's probably another podcast talking about the orthorexia. I know it very well. I've had to go. I was so down the rabbit hole of raw foodism. I had to go and start eating things that I swore I would never eat again to start cracking myself out of just like that scrubbing myself clean with my diet. It's hardcore, and it isn't orthorexia, and it isn't eating disorder in varying degrees. But thanks for bringing it up, because especially when you're promoting a therapeutic diet, I always think the duty of care comes with making sure that people and patients are aware not to get stuck in it. About the Expert Peter D'Adamo is a naturopathic physician who is also an author, researcher-educator, and software developer. He is a world expert in glycobiology, principally the ABO (ABH) blood groups and the secretor (FUT2) polymorphisms. In 1996 Dr. D'Adamo wrote the NY Times Bestseller Eat Right For Your Type. D'Adamo is a distinguished professor of clinical sciences at the University of Bridgeport College of Naturopathic Medicine where he directs the new University of Bridgeport Center of Excellence in Generative Medicine. He is also an adjunct clinical professor for both the Southwest College of Naturopathic Medicine, Tempe, AZ, and the National College of Naturopathic Medicine, Portland, OR. This is episode 431. I am so excited for today’s guest. We have, this is going to be such an interesting interview, Dr. Steven Gundry. Your book is The Plant Paradox, and from everything I’ve seen, people either love it or hate it. There’s no in-between. I’ve not met someone who’s lukewarm about your book. People are either passionately for what you do or passionately against what you do. I think it’s going to be fun to have you on the show today and share what happened for you to discover and create The Plant Paradox, and how your system is helping people reverse disease and heal their body. Welcome to the show. [00:02:03] Dr. Steven Gundry: Thanks for having me. [00:02:05] Ashley James: Absolutely. I have interviewed several whole food plant-based doctors that reversed disease with plants. They are up in arms about what you do. They say it’s just ridiculous that someone would cut out legumes, beans, and plants that contain lectins. They think that these are very healthy foods. I’d love for you to start by sharing. Obviously, there’s so much controversy around that, especially with doctors who claim that the opposite of what your diet is healthy. How do you handle that kind of criticism? [00:02:50] Dr. Steven Gundry: I just show them the data that I show in my book and it’s subsequently published. There’s actually no getting around the fact that the harm of lectins has been known about for actually well over 100 years. In fact, just to use an example, there are three papers in the literature in monkeys showing that the lectin in peanut oil is a major cause of the hardening of the arteries—of coronary artery disease in monkeys. When you remove that lectin from peanut oil and give it to monkeys they don’t get coronary artery disease. Recently, I published a paper in circulation showing that lectins are a major cause of an autoimmune attack on the inside of blood vessels. That when you remove lectins from human’s diets, that they’re markers for an autoimmune attack on their blood vessels minimizes. We can say I’m telling people falsehoods but in fact, this is all published data that I write about. [00:04:20] Ashley James: How does one go about removing lectins from their diet? [00:04:25] Dr. Steven Gundry: That’s actually pretty easy. Recently, I had, on my podcast, Dr. Joel Fuhrman who actually I am a big fan of. A few years ago, when The Plant Paradox, came out, I believe he was pretty vocal that how dare I take beans away from people. I don’t take beings away from people, I merely ask them to destroy the lectins with the pressure cooker. While I had him on the podcast and we were chatting, it turns out that he actually pressure-cooks his beans. He does not eat his beans unpressured-cooked. Sometimes, the noise gets in the way. I had beans three times last week. Believe me, they were pressure cooked. I have nothing against beans, but we have to know our enemy, and we have to know how the plant decided to protect itself. The good news is, for the most part, you can neutralize the enemy by some simple tricks. [00:05:41] Ashley James: You brought up a really good point that plant protects itself. Explain what lectins are. How does the plant create it, and how does our body react to lectin? [00:05:51] Dr. Steven Gundry: Lectins actually were discovered almost 150 years ago now. We use lectins to blood type. There was a very famous lectin diet, it was well-hidden, called the Blood Type Diet. Lectins are used by plants as a defense mechanism—one of the defense mechanisms—against being eaten and of having their babies—their seeds—from being eaten. Lectins are proteins. They’re what are called sticky proteins because they actually look for sugar molecules to stick to—specific sugar molecules. Those sugar molecules line the inside of our gut, they line the inside of our blood vessels, they line the inside of our joints, and they line the space between our nerves. The theory is if lectins can break through the wall of the gut and they’re very good at this, Dr. Fasano from Johns Hopkins a few years ago proved that gluten, which is a lectin, causes leaky gut by binding to the sugar molecules in the gut and breaking the tight junctions. There’s no question that this happens. Why do they do that? Because quite frankly if you can cause an animal to not feel well, to not thrive, then a smart animal says every time I eat these particular plants or these plant babies I don’t do very well. I think I’ll go eat something else. The animal wins, the plant wins, and everybody’s happy. Then humans arrived. As most of us know, we’re not very smart. When we eat something that bothers us, let me give you an example, heartburn is caused by lectins. Instead of avoiding lectin-containing foods, like for instance, a hot pepper, which is loaded with lectins, we instead take Prilosec or Nexium and we keep eating these things. That’s really dumb because, actually, there’s a beautiful paper in humans that shows that in normal human beings who were asked to take one of these proton pump inhibitors for one week—one week only—they had dramatic changes in their ability to remember things, process—one week. I can’t believe Larry the Cable Guy thinks that not being very smart is worth a corndog. [00:09:03] Ashley James: How do lectins cause heartburn? [00:09:05] Dr. Steven Gundry: Lectins actually break down the mucus lining, that mucus is a mucopolysaccharide—a sugar molecule, and exposes the covering of our esophagus. The mucus is used up by a lectin attack and then the acid irritates it. I used to have horrible heartburn. I used to eat Tum’s left, right, and every day. I haven’t had heartburn in 20 years now. [00:09:36] Ashley James: Amazing. Tell us a little bit about your story. Your background is so extensive. Reading your biography, it’s amazing what you’ve accomplished through the years and what you have given to the medical space. You have helped so many children and so many cardiac patients around the world, but you yourself had health issues. You uncovered this particular way of eating to heal yourself. Tell us a bit about your personal journey with recovering your health. [00:10:15] Dr. Steven Gundry: I was a very famous heart surgeon, children’s heart surgeon, and transplant surgeon. Very famous for inventing devices to protect the heart during heart surgery, but I was 70 pounds overweight despite running 30 miles a week, going to the gym one hour a day, and eating a healthy low-fat, primarily, vegetarian diet. I had high cholesterol, pre-diabetes, and arthritis. I used to operate with migraine headaches doing baby heart transplants, I don’t recommend it, but somebody had to do it. I thought this was normal because I was assured it was normal because my father was very much the same way. It wasn’t until I met a fellow I described in my books by the name of Big Ed from Miami Florida who in six months’ time following a diet, very much like what I described and taking a bunch of supplements from a health food store, he cleaned out 50% of the blockages in his coronary arteries in six months’ time. He had inoperable coronary artery disease. I was totally shocked that that could happen. I was taught, as most doctors are taught, that coronary heart disease is progressive. Yes, we could maybe slow it down, but eventually, it’s going to get you. To watch an individual—now many individuals—reverse coronary artery disease with food and supplements changed my life. I was ready to discover this. Believe it or not, as an undergraduate at Yale, I had a special major in human evolutionary biology—basically epigenomics. I had a big thesis that my parents had. I’m actually staring at it in my bookcase right now. I actually put myself on my thesis and I lost 50 pounds in my first year, another 20, and kept it off for over 20 years. I started treating patients that I operated on with my program to try and keep them from visiting me again. Lo and behold, not only did they not visit me again, but their diabetes went away, their arthritis went away, their high blood pressure went away, and their autoimmune diseases went away. That’s what I’ve been doing for the last 20 years. [00:13:10] Ashley James: I love it. Lectins, are they proteins? [00:13:15] Dr. Steven Gundry: Yes, they’re proteins. [00:13:16] Ashley James: They’re proteins. Everyone’s heard of gluten at this point. I’ve been gluten-free since 2011. So many people have heard that gluten can cause leaky gut. There’s also this new thing that we’re hearing about called leaky brain. Have you heard of this? [00:13:37] Dr. Steven Gundry: Have I heard? What do you mean? I’ve been studying and writing about it. [00:13:40] Ashley James: Tell us more about leaky brain. [00:13:43] Dr. Steven Gundry: It turns out, there is an incredible gut-brain connection that is being elucidated, actually, with every passing day. I wrote quite a bit about it in The Longevity Paradox but my next book, which is entitled The Energy Paradox, gets even more into that. What we found, we now have some pretty nice sophisticated tests that look at the breakdown of the blood-brain barrier. The blood-brain barrier is basically a barrier that keeps everything out of the brain. For instance, if you had a malignant brain tumor, we can’t give you chemotherapy by swallowing it or through your veins because the chemotherapy won’t get past the blood-brain barrier. We actually have to inject chemotherapy into your spinal fluid. That’s how impenetrable the blood-brain barrier is. In people with leaky gut, a great number of people actually have a breakdown of that blood-brain barrier. There is more and more and more evidence that conditions like Parkinson’s, like Alzheimer’s, and like plain old everyday dementia is in large part coming from leaky gut and causing leaky brain. You don’t have to look very far to realize we have an epidemic of dementia, and we have an epidemic of leaky gut. In fact, this certainly was known for a very long time. Hippocrates 2,500 years ago, the father of medicine, said all disease begins in the gut. He didn’t have a human microbiome project, but he knew this. The fascinating thing he was absolutely right. All disease does begin in the gut, and the good news is, all disease can be reversed by reversing leaky gut. [00:16:03] Ashley James: How do we heal the barrier for our brain? How do we heal leaky brain? By focusing on the gut? [00:16:11] Dr. Steven Gundry: Correct. Leaky brain comes from leaky gut, not the other way around. [00:16:15] Ashley James: Got it. How do we heal the gut? Is it as simple as removing lectins from our diet? [00:16:20] Dr. Steven Gundry: It’s not as simple as that. For instance, I just gave a paper at the American Heart Association Lifestyle and Epidemiology meeting in March where we looked at people who were gluten intolerant, who did react to gluten. These people were gluten-free and yet they still had leaky gut. We found that 70% of people who are gluten-sensitive also react to corn vigorously. Sadly, most gluten-free products have corn in them. We also found that in this report, taking away other major lectin-containing foods like brown rice, like peas for instance, like legumes, like the nightshade family—tomatoes, peppers, eggplants, and potatoes—then and only then did these people stop their leaky gut. It was not only just being gluten-free, but it was also actually being lectin-free. In fact, as I wrote about The Plant Paradox, you can take people with celiac disease, which is the extreme form of gluten intolerance, and you can put them on a gluten-free diet for a year and a half and 70% of those people on a gluten-free diet will still have celiac disease by biopsy, which is the gold standard, even though they’re on a gluten-free diet. My premise is it’s because most of the gluten-free foods that they’re eating have lectins other than gluten. This is what I just showed in that paper I gave in March. [00:18:11] Ashley James: So many people who are gluten-free don’t avoid oats and oats getting gliadin, which is such a similar protein. I always tell people to try to avoid oats and then try to avoid them for a month. Avoid them for a month and then add them back and see what happens. Very quickly people notice a difference once they’ve been abstinent from it. [00:18:31] Dr. Steven Gundry: Yeah, there’s no such thing as gluten-free oats because you’re right, they could cause reactivity of those proteins. As my daughter who’s a horsewoman always reminds me, the only purpose of oats is to fatten a horse for winter. [00:18:50] Ashley James: Wow. That’s interesting. It reminds me of looking into the idea of eating chicken. People who want to bulk up at the gym are told to eat chicken. People who want to lose weight are told to eat chicken. That’s paradoxical in and of itself. It’s like wait a second, one person wants to bulk up and they’re told eat a bunch of chicken like the bodybuilders, and then people who are on Weight Watchers and stuff are told to just eat chicken, it’ll help you lose weight. But it doesn’t. That’s a paradox. There are foods that people think are really healthy, but they have to look deeper like you do. What about resistant starch? So many of these foods you’ve mentioned, which contain lectins, are also full of resistant starch, which helps to feed the good gut bacteria. This is the paradox. It’s something that could help us but also is harming us at the same time? [00:19:45] Dr. Steven Gundry: The good news with most of these resistant starches is that you can destroy lectins with the pressure cooker. The only lectin that has not been capable of being destroyed is gluten. You can pressure cook gluten for an hour, two hours and it will not break. All the other lectins, in general, will break. Oats is also a problem. We’ve had a number of people try it and it won’t work, but there are two grains that don’t have lectins because they don’t have hulls and that’s sorghum and millet. I have a lot of sorghum of millet recipes. The other great news is that the best resistant starches are tubers like sweet potatoes, taro root, yucca, or green bananas. They’re fantastic sources of resistant starches, and they don’t have any lectins in them. [00:20:47] Ashley James: I noticed that you didn’t mention any nightshades. What are nightshades, and why are they so bad for us? [00:20:55] Dr. Steven Gundry: Two reasons, the nightshade family that we think of have pretty impressive lectins in their peel and their seeds, the flesh doesn’t. Traditional cultures have always peeled and deseeded tomatoes or peppers before they eat them. The nightshade family came from America and, believe it or not, even goji berries are nightshades. They actually came from America and were taken to China and trade. They were called wolfberries in America. Even goji berries are pretty nasty little lectin sources. If you go over to Italy and talk to chefs, which I do all the time, they will tell you that you cannot make tomato sauce without peeling and deseeding your tomatoes. If you go talk to the Southwest American Indians, you know that you have to peel and deseed peppers before you eat them or grind them in chili powder. In fact, what’s really hilarious, is those chili pepper flakes and seeds that we put on our pizzas we’re actually the byproduct of making peppers safe to eat. [00:22:14] Ashley James: Oh my gosh. [00:22:15] Dr. Steven Gundry: Oh, yeah. That’s why they exist because they were thrown away. [00:22:19] Ashley James: That’s so funny. What about peeling a potato, just a regular Yukon potato, would that make it safe? [00:22:27] Dr. Steven Gundry: Yeah but there has recently been discovered a new class of lectins, which are called aquaporins. There is an aquaporin in potatoes, there is an aquaporin in green bell peppers, there’s an aquaporin believe it or not in spinach. I have, sad to say, a number of, particularly, women who have autoimmune diseases and leaky gut who we’ve been befuddled as to why they get better but not all the way. These women, for the most part, react to the aquaporin lectin in spinach. When we take their spinach way, and they’re usually big spinach eaters, they finally get better. Why haven’t I had a podcast on that? Because I don’t want to have mass panic. Most people don’t react to the aquaporin in spinach, but those that do it’s pretty impressive. [00:23:34] Ashley James: It’s like if you’ve tried everything and it’s not working, try this one thing, but not everyone. Does aquaporin become destroyed by pressure cooking it? [00:23:47] Dr. Steven Gundry: Probably, nobody has actually done the experiment but I do have people that eat potatoes and I do ask them to pressure-cook it. So far so good. [00:24:00] Ashley James: Because they’re discovering new lectins, what can we do to stay on top of this information? Is your book updated? Does your book have the aquaporin information in it, or should they follow your podcast? What’s the best way to make sure we stay on top of this information? [00:24:21] Dr. Steven Gundry: My podcast covers these subjects. For instance, between my Plant Paradox book and The Plant Paradox Cookbook, it was discovered that pecans have a lectin that in some people it actually causes an autoimmune attack on the pancreas. We put that into The Plant Paradox Cookbook that probably pecans are not your best not to eat, particularly if you have an autoimmune disease or if there’s any question of diabetes. [00:25:03] Ashley James: Are there any nuts or seeds that are safe? [00:25:07] Dr. Steven Gundry: Oh, yeah. For instance, walnuts are quite safe, pistachios are safe, macadamia nuts are safe, for the most part, hazelnuts are safe, and Brazil nuts are safe. You notice I’m not mentioning almonds. There is a lectin in the peel of almonds that a number of my patients with rheumatoid arthritis react to. That’s why we recommend either blanched almond flour and/or Marcona almonds—the peeled almonds. Again, it’s very interesting that there are a number of cultures, particularly Spain and Portugal, where the mothers teach the daughters how to soak and peel almonds before they’re eaten. Again, you start looking at cultures and say why do they do this? Because it’s kind of a lot of work. Why not just eat the skin? [00:26:13] Ashley James: When I was six years old my mom took me to a Naturopath. He put us on a specific diet, and it turned out that Naturopath was Dr. D’Adamo. I grew up on the O blood type diet. It changed my life. I was very sick. I was just sick all the time, and my mom was sick too. He had a practice in Toronto. Overnight, my life changed. It was phenomenal. One day I was sick, the next day I was healthy. That’s how quick it was shifting my diet to the O blood type diet. Then, of course, when I was 13 I got incredibly rebellious and started eating everything my mom didn’t want me to eat. I got to develop sickness again. Then through my 20s, I was very sick, and in my late 20s and through my 30s I spent trying to get my health back and reversing all the diseases I gave myself by eating the wrong foods. I’ve lived this several times. Eat the wrong foods, get sick. Eat the right foods, get healthy. The waters can be muddy for many of us especially those with autoimmune because, like you said, some people react to this but not to this, some people react to this, not to this. Where do we start? Should one do an elimination diet? What’s a good place to start? Because not everyone reacts to all the lectins like you mentioned. [00:27:41] Dr. Steven Gundry: I’ve published a paper of 102 people with biomarker-proven autoimmune diseases whether they’re lupus, rheumatoid arthritis, Crohn’s, ulcerative colitis, or MS who were put on my program for six months. At the end of six months, 95 out of 102 or 94% were biomarker negative and off of all immunosuppressive drugs. That’s not a bad result. My first principle, The Plant Paradox, is it’s not what I tell you to eat that matters, it’s what I tell you not to eat. It really is. That’s an elimination diet. Interestingly enough, the carnivore folks, have taken my recommendations to the extreme. Since all plants are out to get us one way or another, that total elimination of plants is a rather impressive elimination diet. I happen to think that they’re going to be sadly mistaken because there are some really great things, particularly the soluble fibers in plants that our gut microbiome is dependent on. As people found in The Longevity Paradox, and they’re going to learn more in The Energy Paradox, we are absolutely positively dependent on messages and transmitters that our gut microbiome makes that keeps our mitochondria working properly, that keeps our brain working properly. Taking away the known causes of leaky gut, and that includes more than just changing the type of plants you eat. It’s primarily trying to eliminate, for the most part, antibiotic overuse, which is rampant, not only in us but in the animals that we eat. Eliminating the non-steroidal anti-inflammatories like ibuprofen for instance like Naprosyn. One ibuprofen is literally like swallowing a hand grenade. Eliminating the antacid drugs like the proton pump inhibitors like we mentioned earlier. They totally changed the bacterial flora. Eliminating artificial sweeteners like Splenda, just as an example, completely changes your gut bacteria. Something that many of us are passionate about trying to get glyphosate roundup out of our lives. It’s probably not doable, but glyphosate is a major disruptor by itself of our gut ball—really good at causing leaky gut. [00:30:59] Ashley James: You said that ibuprofen is like swallowing a grenade. Can you elaborate on that? [00:31:06] Dr. Steven Gundry: I could give you an hour lecture. Long ago, the original nonsteroidal anti-inflammatory was aspirin. Aspirin was used extensively in our arthritis and rheumatoid arthritis. Of course, people know that it cause stomach bleeding. A class of drugs in the same family were developed that wouldn’t cause stomach bleeding, but the drug companies knew that the bleeding wouldn’t actually be caused in the small intestine. You couldn’t see down into the small intestine with gastroscope so you would never know it was there. Believe it or not, when these drugs came out, they were prescription only. Things like Motrin was a prescription, things like Naprosyn was a prescription. There was an FDA black box warning that you could only use these for two weeks at a time because they were so dangerous. Now, of course, they are the largest over-the-counter drug there is. There is children’s Advil for instance. What these do, and this is documented and google it sometimes, great fun. These are drug company research that shows that these destroy the lining of the small intestine causing leaky gut. I can’t tell you the number of people that I see, young women and men who were athletes, who suffered an injury and were put on high-dose non-steroidal anti-inflammatories by very well-meaning orthopedic surgeons, and they, in turn, developed autoimmune diseases. I write about some of them in my books. When we stopped these medications and sealed their gut, lo and behold, their autoimmune disease went away. [00:33:07] Ashley James: How quickly can one recover their gut? How quickly does it take to seal the gut once they have eliminated the foods that have been causing holes in it? [00:33:22] Dr. Steven Gundry: Great question. We’ve seen it turn around as early as three months. One of my more troublesome patients with multiple autoimmune diseases all her life took about nine months, but there are other things that are part and partial with healing the gut. The vast majority of people have very low vitamin D levels. As I write about in The Longevity Paradox, vitamin D is essential to tell stem cells that help repopulate the gut to grow and divide. Without vitamin D, they just kind of sit there and twiddle their thumbs. The vast majority of people I see with autoimmune disease and/or leaky gut, they have very low vitamin Ds when I see them. I’ll give you an example of a woman I just saw today from Southern California. She’s in her 40s. She developed ulcerative colitis five years ago, out of the blue. We think we know why but she was put on an immunosuppressant and then came to see us a year ago. She was positive for antinuclear antibody, which is an autoimmune disease marker that most people associate with lupus—very positive form. She had a very low vitamin D. We’ve now been seeing her for a year. She stopped her immunosuppressant a year ago. She’s had no episodes of ulcerative colitis since. She is negative for antinuclear antibody and has been actually since we started the program. She’s a pretty happy camper. [00:35:28] Ashley James: I love it. The idea, for those suffering from autoimmune, that they can completely go into remission—I mean, that’s a dream come true. There are so many people suffering. They’re told by the average doctor that they’re going to be on medication for the rest of their life and this is their new norm. It’s so frustrating that so many doctors are still in this old way of thinking that once you’re in a diseased state you’re going to always be in the diseased state. They don’t look to nurturing the body through food and shifting diet and lifestyle to heal the body. You must be really waking a lot of doctors up showing them that there’s a way to heal. You’ve obviously published so many articles on helping people to reverse autoimmune. Are you starting to see that doctors are listening and prescribing your diet? [00:36:29] Dr. Steven Gundry: Particularly in functional medicine, I don’t do functional medicine, I do what I call a restorative medicine. I’m not quite sure what functional medicine means. I have good friends like Mark Hyman and Jeffrey Bland in functional medicine, and that’s fine. But I think there are more and more people interested in the fact that Hippocrates was right. That all disease comes from the gut, and that we really ought to be looking at the gut as to where we need to do our work. Somebody tell Kelly Clarkson that you can’t reverse Hashimoto’s thyroiditis. Kelly found my book. She had Hashimoto’s thyroiditis, she was on thyroid medication, now she doesn’t have Hashimoto’s thyroiditis, and she’s off of medication. Her doctors told her hey, you’re going to be on thyroid for the rest of your life. [00:37:35] Ashley James: I love it. I love hearing stories of success of people being able to reverse diseases and get off of medications. [00:37:44] Dr. Steven Gundry: I’ll tell you another funny Hashimoto’s story. Usually, we’re so busy that the first visit in our office they see my PA and then the next visit they see me. That’s usually three months or six months after the first visit, depending on the severity. I’m seeing a woman in her late 50s for the first time after she had seen my PA. I say, “Why’d you come here? She said, “Well, I have Hashimoto’s thyroiditis.” I said, “Oh is that a fact?” She said, “Yes, I’ve had it all my life.” I had seen her new results and when we first saw her she in fact did have both markers for anti-thyroid antibodies and Hashimoto’s. This time they were negative and I said, “Well, that’s interesting you should say that because you don’t have Hashimoto’s.” She said, “What kind of quack are you? Of course, I have Hashimoto’s that’s why I’m here.” I said, “Well, yeah. You used to Hashimoto’s but you don’t now.” I flipped her lab results open and she said, “Oh my God. It is true. You can get rid of this.” I said, “Yeah, look at that.” [00:38:59] Ashley James: I love it. Once her antibodies go down do the hormones restore themselves? I know of some people who have completely eliminated—the antibodies are virtually non-existent in their labs but their thyroid is still not functioning optimally. Are the lectins causing harm still to their thyroid levels? [00:39:24] Dr. Steven Gundry: A lot of times, people who have had it for a long time, they’re immune system has destroyed—their thyroid gland. For instance, type 1 diabetes, the immune system destroys the beta cells in the pancreas. But having said that, we always, once we get these antibodies turned off, start weaning off thyroid medication. We actually just started this weekend with a woman from San Francisco, a fairly young woman, who we now have negative for anti-thyroid antibodies and we’re starting to wean her thyroid know, and we’ll see. The good news about thyroid medication is that you can take both T4 and T3 and do a good job of replacing what the thyroid does, but that doesn’t mean we should actively allow Hashimoto’s thyroiditis to continue. Because one of the things that got me interested in this in the first place is this autoimmune attack takes many forms, and you could have multiple autoimmune diseases at the same time. Recently, there’s increasing evidence that Parkinson’s disease is actually an autoimmune disease, which to me makes a great deal of sense since I and others are convinced that Parkinson’s disease comes from the gut, not from the brain. [00:41:05] Ashley James: Fascinating. I have heard there’s a relationship between MS and Parkinson’s and MS is autoimmune, so that doesn’t surprise me. Have you seen someone reverse Parkinson’s through healing their gut? [00:41:19] Dr. Steven Gundry: We’ve seen it stop. My father had Parkinson’s for 20 years without changing his medication at all. If you know anything about Parkinson’s that’s impossible, but we got to him early. My mother was a very good person about denying him the foods that he loved. He made it to 91 and then actually died suddenly of a bladder infection. Getting to 91, 20 years with Parkinson’s pretty doggone good run. [00:42:01] Ashley James: Very cool. We’ve talked about gluten and lectins, are phytates or phytic acid, I know they’re anti-nutrients, are they also lectins? [00:42:11] Dr. Steven Gundry: No. Again, the plant has lots of tools to prevent itself from being eaten, or to warn the predator that you really don’t want to eat me, or try to make the predator not thrive. Phytates are one of these methods. I actually think and agree with some of my vegan colleagues that there’s a lot to like about phytates, but this is all part of the anti-nutrient system. One of the things that we have to realize, for instance, since lectins are proteins, rats and rodents are primarily grain predators. Rats and rodents have 10 times the amount of proteases that are enzymes that break down proteins than we have. They’re very well equipped to go after these lectin proteins in the food that they eat. When people point out we look at these great rat and mice experiments where whole grains are really good for them. That’s great. They have a great protease system that breaks down these proteins, we don’t. [00:43:50] Ashley James: Are you suggesting that we shouldn’t get our protein primarily from plants? [00:43:58] Dr. Steven Gundry: Oh, no. Are you kidding? I am a plant predator. I tell my patients that we should actually be gorillas who live in Italy. By that I mean we should be eating huge amounts of leaves and pouring olive oil on them. [00:44:21] Ashley James: Very interesting. Why consume olive oil? [00:44:26] Dr. Steven Gundry: Great question. As I wrote in The Longevity Paradox, there are three groups of people who live in blue zones, and blue zones are those areas of the world that Dan Buettner, the journalist, described as having incredible longevity. I’m actually the only nutritionist who has ever spent most of his career living in a blue zone and that was Loma Linda, California where I was a professor. When people talk to me about blue zones and say I don’t know anything about blue zones, I said, “Well, I guess I didn’t live in one for most of my life.” Anyhow, three blue zones use a liter of olive oil per week. That’s about 10 to 12 tablespoons a day. There are some fascinating head-to-head studies done in Spain called the PREDIMED study making 65-year-old people use a liter of olive oil per week versus a low-fat Mediterranean diet. Lo and behold, at the end of five years, people who used the olive oil had actually gained memory compared to when they aged 65, while the low-fat group lost memory, the people in the olive oil group had a reversal of our disease, whereas the low-fat diet group increased their heart disease, and we could go on and on. [00:46:02] Ashley James: Could we gain the same benefit from eating olives instead of drinking or consuming olive oil? [00:46:10] Dr. Steven Gundry: Yeah, and I actually ask people to do both. I actually have a product that I sell that is the combination of incredibly high polyphenol-rich olive oil, olive leaves, and olives that are pressed into little pearls that look like caviar. They’re called polyphenol olive oil pearls. It turns out, interesting fun fact, there are far more polyphenols in the leaves of fruiting plants than there are in the fruit. For instance, there are far more apple polyphenols in apple leaves than in apples, there are far more polyphenols in black raspberry leaves than there are in black raspberries, and so on. [00:47:03] Ashley James: It’s nettle season right now so we can go out in nature and harvest nettles, which the leaves are rich in polyphenols. I learned recently that if you grow sweet potatoes or yams, you can harvest some of the leaves and eat them much like spinach. That is such a delicious thing to grow in your own backyard. I know a lot of people are looking into growing their own food given that they’re all at home and want a new hobby and that they’d like to have some sustainability and have some healthy food. I know you tell us what not to eat, what are some of your go-to foods to eat that are very supportive of our gut and just are very healthy overall? [00:47:51] Dr. Steven Gundry: Great question. Avocados are a great choice to start with. The family of chicories: radicchio, chicory, Belgian endive, and curly endive are some of the best foods you can possibly eat to help your gut buds. Jerusalem artichokes and artichoke hearts are just loaded with a type of sugar molecule that we can’t digest called inulin but our gut buddies think it’s the best food that they could possibly eat. That along with the cruciferous vegetables: broccoli, cauliflower, arugula, bok choy, and Swiss chard. All of these actually have some pretty fascinating compounds that actually tell the immune system in your gut to calm down and relax a little bit. They’re called the AHR receptors if anybody wants to look them up, but pour olive oil on them, please. [00:49:10] Ashley James: Interesting. I think the olive oil thing—it’s so controversial depending on who you talk to. There’s a handful of doctors that say that we shouldn’t consume any oil. That any oil, no matter what kind of oil, even if it’s cold-pressed virgin olive oil, raw cold-pressed coconut oil or all the kind you could buy in the supermarket, that they’re all bad for you and that they cause damage to the endothelial lining of the cardiovascular system. Once the oil is exposed to oxygen, it creates free radicals, so you’re actually absorbing free radicals into the body. What do you say to that? Are the benefits of olive oil outweigh those negatives? [00:50:01] Dr. Steven Gundry: All I say is why don’t you look at the actual human controlled trials where that has been tested, and the results are exactly the opposite. One of the most famous trials was the Lyon Heart trial where they looked at a diet that was supplemented with alpha-linolenic acid oil from purslane and compared that to the low-fat American Heart diet. It was a 5-year study, it was randomized. They stopped the study at three years because the group given the Mediterranean diet with the alpha-linolenic acid oil, and large amounts of it, did so much better in new episodes of MIs and unstable angina that it was not fit to continue the trial. Anybody can look it up—the lean Lyon Heart diet—and anybody can look up the PREDIMED trial and see a head-to-head of high-oil versus no-oil or low-oil and the results always come out that it wins. The olive oil wins, the alpha-linolenic acid wins. People say this becomes rancid. Yeah, olive oil could become rancid. That’s why you want to buy it from a high-volume source, and you want to use it quickly. Interestingly, olive oil is the least capable of oxidizing of any oil. It even beats coconut oil as not being oxidized with cooking. We’ve had two of the world’s oil experts on my podcast and they both confirm that olive oil is the safest cooking oil. People say no, no. It smokes and that means it’s oxidizing. That’s not true at all. It has a low smoke point but smoke has nothing to do with oxidation. People have been cooking with olive oil for over 5,000 years and the results speak for themselves. [00:52:16] Ashley James: I don’t think when you burn olive oil it tastes that good anyway. If I were to eat olive oil, I’d eat it raw anyway. The idea of cooking with oil just concerns me, especially it doesn’t particularly taste good when you burn it. Very, very interesting. Where would one buy oil that is very high quality? Where’s the place to buy oil in the highest quality form? [00:52:45] Dr. Steven Gundry: They can come to my website Gundry MD. We have an olive oil list that has 30 times more polyphenols than any oil that’s ever been tested. Having said that, you’re not all going to come to gundrymd.com. Believe it or not, Costco has an excellent olive oil. It comes in at a tall square bottle. It says Toscana on the label, and I use it as my everyday oil. That’s a good source. There’s another very good company out of California called Bariani. Again, I have no relationship with these companies. They just have a very high-quality oil. There’s a company out of Napa and Sonoma Valley called O, just a big O olive oil company. All small producers, all organic. Those are good choices. [00:53:38] Ashley James: Very cool. You had touched on mitochondria. Obviously, you’re coming out with your book The Energy Paradox. Can you give us a little bit of preview into this book? How does your method help our mitochondrial health? [00:53:55] Dr. Steven Gundry: Well, it turns out, we’ve done just the best job of destroying our mitochondrial function. You couldn’t design a better lifestyle, a better diet than the standard American diet for destroying mitochondria. In the book, I talk about how—and I talked about this actually in The Plant Paradox as well. Mitochondria are the energy-producing organelles in our cells. They’re actually ancient engulfed bacteria. They carry their own DNA, but they are bacteria that live inside our cells. If you think of them as workers on an assembly line, they have periods of time where they’re going to do one shift, but currently, in the United States, we’re asking them to do three shifts with no time off. They actually produce a labor slowdown because they don’t want to work that hard. That labor slowdown is the cause of pre-diabetes, insulin resistance, and cancer. We got to have them have some time off. That’s why fasting, intermittent fasting time, restricted feeding actually gives our workers some much-needed rest and recreation. The more we do that the more work they’re able to do for us and do it better. That’s a little teaser. [00:55:52] Ashley James: I am such a big fan of fasting, and I love that you brought that up. Since mitochondria are bacteria, when we take antibiotics, do antibiotics harm our mitochondria? [00:56:11] Dr. Steven Gundry: Indirectly because it turns out, as I write about in The Longevity Paradox, that the bacteria in our gut actually talk to their sisters in the cells. They actually tell the mitochondria how things are going in the outside world. The more diverse those bacteria are and the happier those bacteria are, then the better the mitochondria function. We used to conjecture that there were text messages that we hadn’t been able to measure, but they had to be there. It turns out, we’ve discovered a large number of those text messages—they are real things that we can measure. The book is about we ought to have a diverse group of bacteria in our gut, we ought to give them what they want to eat, and they need to tell their mind everything’s great. [00:57:21] Ashley James: Since researching and writing The Energy Paradox, what changes have you made to your lifestyle or diet? [00:57:31] Dr. Steven Gundry: Actually not a whole lot because The Energy Paradox is where The Longevity Paradox would naturally take me. If you actually look at the people who enter my office, fascinatingly, fatigue is one of the biggest complaints. At least 50% of the people I see are fatigued for apparent no reason. The Energy Paradox grew out of that. Believe it or not, there are really good reasons why most normal people are fatigued. [00:58:25] Ashley James: Since the last few months have been really crazy with the COVID-19, have you done anything to change your lifestyle or diet to give extra support to your immune function? [00:58:41] Dr. Steven Gundry: As most people know, I’m a huge fan of vitamin D. I’ve never seen vitamin D toxicity, neither is my friend Dr. Mark Hyman. There are now four human papers showing that people with low vitamin D are much more susceptible to the COVID-19 virus and are likely to do poorly with it—that is die. Whereas if you have an adequate or high vitamin D level, you’re much likely not to contract it and/or much likely not to die. That’s number one. Number two, sugar dramatically suppresses our white cells’ ability to engulf bacteria and viruses. I’m releasing a podcast about that. The less sugar or things that we turn into sugar the better. What’s really fascinating is a paper from 1973 showed that white blood cell function dramatically increased daily during five-day water fast and that the longer you fast the better your white blood cells work and aggressively ate bacteria and viruses. Additionally, that same study showed that even orange juice, even fructose would dramatically suppress your white cells ability to engulf bacteria for up to six hours after you had that beverage—even orange juice. The idea that drinking orange juice right now is going to help you fight these viruses is actually exactly wrong. [01:00:35] Ashley James: You said that foods that convert to sugar, which would be plants, would be carbohydrates. Are you saying that we should look at more of a low glycemic diet right now? [01:00:48] Dr. Steven Gundry: Right. Here’s the problem with a lot of my well-meaning patients. You can take a resistant starch in its natural forms like say yucca, taro root, turnip, or even almonds for instance. You can grind it up into a fine powder and make a flour out of it. Unfortunately, and I’ve seen this much too much, those will rapidly turn into sugar even though what you originally started with isn’t sugar. For instance, I had Dr. David Kessler who was the head of the FDA when the food labeling law came into effect back in the Reagan era. The food labeling law was incorrectly made because of food lobbyists and not tell the truth about how much sugar. One of the things I have people do is on the back of the label read total carbohydrates, subtract the fiber, and that will give you the amount of grams of sugar per serving in that product and it will shock you. It will have nothing to do with what they put has the sugar content on the label. For fun, since there are four grams of sugar per teaspoon of sugar, divide the number by four and you will see the teaspoons of sugar per serving. You will shudder when you see it. [01:02:36] Ashley James: Right. You can take a healthy food, if you dehydrate it and turn it into flour, it reacts totally different with the body. You could eat chickpeas—pressure-cooked chickpeas—or you could eat something like a chickpea pasta. Chickpea pasta is going to convert much quicker to sugar, give you a larger sugar spike in the blood. Even for those who are not diabetic, they still will have that. Whereas if you ate pressure-cooked chickpeas, it’s a slow steady release of sugar. [01:03:08] Dr. Steven Gundry: Covered in olive oil. [01:03:10] Ashley James: Covered in olive oil. [01:03:13] Dr. Steven Gundry: And throw some mushrooms in. [01:03:15] Ashley James: Oh, man. Mushrooms are so great for the immune system. Get your vitamin D. Let’s just clarify that. What form of vitamin D is best, and how many international units should someone be taking a day? [01:03:30] Dr. Steven Gundry: Vitamin D3. The bare minimum should be 5,000 IUs a day. For me, my 5.000 gets me vitamin D level above 120 nanograms per ml, but during this season, I’ve doubled my vitamin D to 10,000. If I think I’m coming down with something, and I’ve said this before, I actually take 150,000 international units of vitamin D3 three days in a row. Now I’m not telling people to do that. I’m telling people that’s what I do. For instance, when this all started and I decided to keep seeing patients, even though I wasn’t feeling anything, I took on a Sunday, 100,000 international units, on a Monday 5,000, on a Tuesday 25,000, and then I continued on 10,000. I just load up with it. I just saw one of my patients in Santa Barbara last weekend. Her vitamin D level is 244. I assure you, she’s not vitamin D toxic, and she doesn’t have an elevated calcium level. I think we’ve underestimated what a normal vitamin D level is. The Cleveland Clinic Lab now says that a normal vitamin D level can be up to 150. [01:05:08] Ashley James: Amazing, amazing. I know people whose vitamin D level is 10. [01:05:15] Dr. Steven Gundry: I know, it’s really scary. [01:05:17] Ashley James: I have known MDs to say they don’t want to see it above 30. They’re scared if you’re above 30. They want you to stop taking your supplements. Whereas NDs want you at least to be 60, but many NDs I’ve met don’t want you above 90. They’re afraid that if you get above 90 that that could cause toxicity. You’re saying that you’ve never seen toxic levels. Because of course, the worst-case scenario is vitamin D toxicity could cause kidney failure. At that point, it’s almost too late. We definitely don’t want to harm ourselves with supplements, but you’ve never seen that happen in prescribing large amounts of vitamin D. [01:06:01] Dr. Steven Gundry: I measure vitamin Ds on people every three months. I’ve been doing this for over 20 years. I’ve never seen vitamin D toxicity. I have some patients who taught me in their late 70s they run their vitamin Ds in the high 200s and have all of their lives. When I first met these people I couldn’t believe they weren’t dead, they didn’t have kidney failure, or they didn’t have calcifications and kidney stones. They didn’t. When I was researching The Longevity Paradox, it turns out that people with the highest vitamin D levels have the longest telomeres. If you like the telomere theory of aging, and it’s a good one, why wouldn’t you want long telomeres? [01:06:54] Ashley James: That’s exciting. What form of vitamin D is best? Obviously, you said D3, but I’ve seen supplements where it’s like a dry capsule, and then I’ve seen the liquid form as drops. [01:07:06] Dr. Steven Gundry: Great question. Turns out that Dr. Michael Holick from Boston University, probably the most famous researcher in vitamin D, showed that vitamin D is absorbed whether or not there is any fat around. So a dry vitamin D is perfectly fine. Most vitamin Ds in capsules or gel caps. Interestingly, I see a number of people who use vitamin D drops. As a general rule, those people have much lower vitamin Ds than people who swallow the little gel caps. Most of my liquid folks I change over and I’m delighted to see that their vitamin D goes up. [01:07:53] Ashley James: I was using a liquid and my vitamin D went down and down and down and I kept using more and more and more. I got so frustrated I switched to a capsule even though I heard from a Naturopath how could that work? There’s no fat in it. I was like well I’m going to try this now. My vitamin D went up. I was told it’s not going to work but my labs say it worked. [01:08:18] Dr. Steven Gundry: Dr. Holick showed that it has nothing to do with fat even though it’s a fat-soluble vitamin. [01:08:25] Ashley James: Should we take it on an empty stomach, with food, or does it not matter? [01:08:28] Dr. Steven Gundry: It doesn’t matter. [01:08:30] Ashley James: Love it. What did you eat in the last 24 hours? [01:08:35] Dr. Steven Gundry: Let’s see. When did I eat? Last 24 hours I had nothing for breakfast, I had nothing for lunch, and I had some sautéed calamari and a Chinese cabbage salad with olive oil and rice vinegar on it. That’s what we had. [01:09:08] Ashley James: Very interesting. Now your diet has helped people to reverse autoimmune conditions. You’ve also helped people to reverse cardiovascular issues and weight loss. Weight loss is the biggest industry out there. You look at all the diets and people are just yo-yoing. Every diet seems to work for a short amount of time. They blame themselves. The diet stops working, they blame themselves, they fall off the bandwagon, and they go back to eating the state American diet, but people are left feeling broken, right? They failed. What we’re looking at is it’s not their fault, it’s the diets fault because there are so many wrong diets out there. You’ve had great success in helping people with weight loss. Why does removing lectins help with weight loss? [01:10:09] Dr. Steven Gundry: As I write about in The Plant Paradox, there are actually some very interesting data looking at one of the lectins called wheat germ agglutinin (WGA), which is present in whole grains, whole wheat. It binds to the insulin receptor on muscle cells and also on fat cells. In muscle cells, it actually blocks insulin from letting sugars and protein into the muscle cell, but in fat cells, it actually continues to pump sugar into fat cells. I go into more of this in The Energy Paradox. We’re set up with our diet to literally starve our muscle cells and feed our fat cells with lectins are a big chunk of that. Also, it turns out, that it starves the brain and so your brain is constantly hungry despite how much you eat or not eat. [01:11:19] Ashley James: Wow. So when someone removes that, they’re all of a sudden not feeling so starved? They’re not feeling hungry all the time? [01:11:28] Dr. Steven Gundry: Yeah. [01:11:29] Ashley James: Very cool. [01:11:31] Dr. Steven Gundry: The other thing that we have to make sure we understand is that we took over the world because of our ability to go prolonged periods of time without eating. Where the fat ate for a good reason. It is absolutely normal to go periods of time without eating. In fact, do you really think our ancestors crawled out of their cave and said what’s for breakfast? There wasn’t any. There was no storage system. We had to go find breakfast. If breakfast occurred at lunch, or breakfast occurred at dinner that’s when we found food. I actually tell my patients, when we get into this, to embrace the hunger. There is nothing horribly wrong with going a period of time without eating. In fact, just the opposite. It’s really one of the smartest things that we can do for long-term health. [01:12:32] Ashley James: Actually, I haven’t eaten today, and I’m not going to eat today. I love fasting. Hunger is probably the hardest part about fasting, but hunger goes away. [01:12:44] Dr. Steven Gundry: It goes away quickly, very quickly. [01:12:45] Ashley James: It’s kind of like going to the gym. Just get your shoes on and go to the gym. The hardest part is actually getting your shoes on. Once you’re there, it’s easy. Starting a fast is the hardest part, and then following through is the easiest part. I love all the science that’s coming out about fasting. What resources could you point us to for those who haven’t really dived into fasting yet? [01:13:14] Dr. Steven Gundry: There’s a lot of good ways to learn about it. I certainly spend a lot of time talking about it in The Longevity Paradox, one of my New York Times best-selling books. My friend Jason Fung has some good books about it. I think Dr. McCullough and I would agree that for most people who have been following the standard American diet that a prolonged water fast of 3 to 5 days is probably the dumbest thing to do because heavy metals and organic pesticides are stored in fat. They’re released into our circulation and we do not have a good detoxification system. I’ve got a lot of podcasts out on that, so please be cautious. [01:14:05] Ashley James: Right. Always proceed with caution when fasting, especially if someone’s on medication. I agree. I like some of Jason Fung’s work—easy to digest. I particularly like—he has a video on YouTube called the 2 compartment syndrome. I think that’s a great place to start. Intermittent fasting is something where it’s gentle enough that people can ease into it. I actually did a whole series with a man who has invested his own personal money into labs doing fasting to show the heavy metals and pesticides being released in the body and how to best remove them while fasting. He found that, dramatically, if you were to use a sauna while fasting and also consume activated charcoal while fasting throughout the day, they saw a dramatic decrease in heavy metals and pesticides being released from the fatty tissue into the bloodstream. There are ways to combat it but we have to be aware of it. Thank you so much for coming on the show. It’s been such a pleasure having you on. Today it's all about health, and the focus turns to the Blood Type Diet, the fascinating field of Epigenetics, generative medicine and more. I personally benefitted from trying the Blood Type Diet, which went from a 30-day experiment of changing my food choices to my now ongoing lifestyle. I absolutely love it. What's even more exciting is that I had no idea you could go much, much deeper with it, and literally shift your own DNA in order to avert disease that could be lurking in your genes. My guest, Dr. Tara Nayak, a naturopathic practitioner in Philadelphia, PA and I discuss. Dr. Tara Nayak grew up knowing she wanted to be a doctor but not agreeing with our healthcare system, including overprescribing and lack of connection in patient care. She went on to Temple University to get my bachelor's degree in Biology but chose to concentrate heavily on genetics. At this time she took some of the FIRST courses ever offered at her university in the fields of epigenetic, genomics, and proteomics. (That tells you how new the fields of epigenetic, nutrigenomics, etc. really are!) She graduated in 2009, taught high school science for 2 years, then went back to medical school at the University of Bridgeport College of Naturopathic Medicine. It was here that she met Dr. Peter D'Adamo and forced herself under his wing. She took his classes, did clinical rotations with him, externships at his clinic, and was eventually selected as his resident physician at the Center of Excellence in Generative Medicine. This is a clinic where all patients are treated based upon blood type with the addition of further parameters to establish personalization including body measurements, lab tests, and history taking of their health and current symptoms. Dr. Tara has continued to work with Dr. D'Adamo to learn and contribute to his latest developed tools in personalized medicine: 1. "Opus 23" which allows doctors to evaluate specific genes to assess patient risk and determine the best plan of action in terms of diet, nutrients, herbs, etc. and, 2. "Utopia" a micro biome analysis program that analyzes gut bacteria trends and guides dietary/supplementation decision to promote healthy gut flora. Dr. Tara contributes to editing and mining data for the program as well as participate in teaching other health care practitioners how to use this software in their practice. We're back with the 2nd edition of the weekend Cabral #HouseCall ! Here are today's community questions I'll be answering: Jen: Hi! I have two questions I've been thinking about and thought maybe they could be good for the podcast. One only affects half of the population, as you'll see: 1) what are your thoughts on the blood type diet? 2) what are your natural recommendations for common vaginal infections such as yeast infections or bacterial vaginosis? Anonymous: Why does my stomach make so much noise?!??!!! It doesn't matter whether I have eaten or not it is always gurgling. This has happened for years. I have been plant-based for two years and thought that would have helped but, stomach still makes noise. I take probiotics. I drink kombucha. I have ACV... I can not tell that anything specific helps. Also, I can feel bubbling in the lower right quadrant of my abdomen, where I sometimes experience discomfort. (I could go on with more but I won't get too carried away. Wish I was closer...) Help with the noise, PLEASE, it is no fun in quiet work meetings. Secondly, I've had greying hair since mid-end of my 20s and now with 35, I have a ton of grey hair. Is there a natural way to stop and/or reverse greying of hair? Thank you. Fro: And last but not least, a dear cousin of mine has been dealing with grave's disease and various unpleasant symptoms thereof for a few years now and her doctor has been prescribing rather radical treatments (in my opinion), the latest to include the recommendation of eye surgery for the abnormal protrusion affecting one of her eyes...I cannot help but think that the treatments are short-term methods intended to mask the symptoms rather than addressing/treating the underlying cause for the disease. I'd love to hear your take on cold thermogenesis for general health and/or fat loss, as well as its potential use as a training/recovery tool. ~ In my response to Tom, I mention this recent Get-Fit Guy article, and my previous interview with Ray Cronise. Melanie asks: I know you had an interview regarding metabolic typing, but I wanted to know how much stock you put in the details of these type of "diets". I am a protein type and I know that is right for me, but I did the metabolic typing from the Wolcott group website and received a specific list of foods to eat and ones to avoid. Some of the ones for me to avoid are ones that are in the "eat" category in your food pyramid. For example, I'm supposed to rarely have yogurt, cabbage, berries, and completely avoid whey and broccoli. What is your opinion of the Wolcott metabolic type diet? Should I consider cross-referencing your diet and my metabolic type list and eat only what is a match? Also, along the same vein, can you quickly sum up your opinion on the Blood Type Diet? I'm a protein type on that one too, but again some of the foods you recommend would be a no for me on that one. ~ The movie Brock was thinking of is The Road To Wellville. Horacio asks: I'm about to run my first ultra running race in April, 84k Trail race in Patagonia. How fast should I run? I want to be fast but not bonk or have a cramp crisis. Last year I ran this same race but only the 42k version. I probably ran it near my anaerobic threshold or about 165-170 bpm... but for the 84k I plan to go much slower. This is my plan: I've been following Phil Mafftone's advice and I've trained mainly trying not to go over 180-age limit (that would be about 150 bpm) currently at that level I can run at 8 min/mile pace. My plan is to run with a heart monitor and not increase 150 bpm on the race. Does this makes sense? I feel that with proper fueling I should be able to make it. I'll try to eat all I can during race, about 300 calories per hour if my stomach can handle that amount of gels. Brian says: The reason I write you is I have a friend that is 23 years old and has just been diagnosed with a rare (for his young age) adeno carcinoma of the duodenum. He is going to have a short regiment of chemo before a very complex surgery to remove the mass that is about the size of two grapefruits. My question to you is, what are your thoughts on anything he can do nutritionally through food or supplementation that will give his body the support and healing power it needs. I realize you are not a medical Dr. but respect your knowledge in the nutrition realm more than most medical Dr.s :) Trevor says: I am concerned triathlete. I have ambitions bigger then what my dreams can handle but it is unfortunate that I have had my dreams squashed again. I was watching the news and saw yet another rider hit by a car from behind. I wonder, in your experience, how does the community we are a part of deal with this? I am love the sport and it has saved my life in many ways but I am worried that my obsession of being active is now becoming a threat to my life. It really sucks to see people get hit by cars and then mentally be off your game due to the vivid images of past accidents. I have been training for about two years and the first day I got on bike, a bike rider got killed in front of me. I just wanted to hear if you had anything on this. ~ In my response, I recommend the book Eyes Wide Open. Lia asks: I have signed up for my first ever event - a Tough Mudder, the inaugural Mudder in Vegas. I looked through archives on the site and didn't see any recommendations on how to train for an event like this. I have quite a bit of time because the event is not until October 6. The suggestion on the Tough Mudder website is pretty slim, and I'd love to hear what your approach would be. ~ In my response about the Tough Mudder, I reference this workout. Joe asks: I am 53 years old. In Triathlon for about 12 years (Sprint to IM). I have asked this question to many and I just get a shoulder shrug. When I started Tris, I couldn't swim, I had never biked, but was a good high school runner. I could run 51 secs for the 400m. My problem is I cant run. I can put in a solid swim and good bike but my run is poor. Stand alone times for a 1500m swim would be 25minutes for a 40km bike 65 min and for a 10km run 48 min. (non tris) I have done intervals, LT runs, hills etc, I have read all the books and have tried everything I even bought power cranks and stand alone run blocks. Why can't I run? Josh says: In a recent podcast you mentioned natural testosterone enhancement using D-aspartic acid with an aromatase inhibitor. Could you please speak a little more in depth on this subject? Including sources, dosing, detrimental effects, and whether you cycle on and off. ~ In my response to Josh, I recommend Hard Rhino D-Aspartic Acid and Myomin aromatase inhibitor. Diane asks: I'm a 53 year-old woman (120 lbs., 5 ft. 6 in. tall) and am doing an Ironman for the first time this summer. I've done two 70.3 triathlons (about 7 hours to complete), one in 2011 and one in 2010, and some sprint and Olympic distances. By my calculations I will be lucky if I finish the IM in the 17 hours, mostly because it's a hilly course (Mt. Tremblant). My question is about nutrition during the event (although I have many more questions). I anticipate taking 2 hours for the swim and I've read that it might be wise to take a gel at the half-way point during the swim. Do you think that's necessary? Because I'll be on the course for so long how should I manage my nutrition intake? Because I'm slow and working at a lower intensity should my total nutrition intake be the same as someone my size who finishes in 12 hours? Any information that you could provide in how many calories/carbs to take during a 17 hour event would be appreciated. ~ In my response to Diane, I mention these amino acids.
