The Chief Executive Officer of the Austin Film Festival and a co-founder since it's inception in 1993, Barbara has shepherded the festival from a single weekend of films and panels into a week-long celebration of storytelling through film, television, and other emerging forms of narrative media. Recognized as the prominent festival that champions screenwriters and media creators, the festival also boasts over a dozen internationally acclaimed film and screenplay competitions, diverse youth education programs, and a sprawling calendar of year-round events. Leveraging the vast amount of footage recorded at these events since its creation, Morgan created the festivals On Story Project in 2009, and serves as the project's Executive Producer. On Story includes a PBS television show, radio show (distributed by PRI and NPR affiliated stations), podcast series, a three-part book series (published in partnership with UT press), and extensive public archive. Barbara has received 2 Lone Star Emmy's for On Story episodes, Breaking Bad: A Conversation with Vince Gilligan and for the Season 8 episode Script to Screen: Arrival in 2018. Morgan is an independent film producer, whose films have included Natural Selection, Antone's: Home of the Blues, Portrait of Wally, and Spring Eddy. Jana's background in medicine and health stems all the way back to starting with pediatric hemotology and oncology acute care. Today she has found her calling with functional and integrative medical care for children. We loved learning more of her story and her passion for health. About Jana Roso Jana obtained her baccalaureate degree in Human Development and Family Sciences from the University of Texas at Austin. She then went on to obtain her nursing degree from UT Austin and shortly after began working as a pediatric hematology and oncology acute care registered nurse at Dell Children's Medical Center. She then returned to UT to obtain her master's degree in nursing with a pediatric nurse practitioner focus. She is board certified as a pediatric nurse practitioner from the Pediatric Nursing Certification Board. After working many years in a primary care setting, her love for nutrition and health led her down the functional medicine path. She helped open and own Neuronutrition Associates for many years. She also co-wrote and published a book on a functional medicine approach to treating autism. Jana is a fellow through the Medical Academy of Pediatric Special Needs and has been trained through the Institute for Functional Medicine. She is passionate about providing holistic healthcare to children with chronic maladies through an integrative and functional medical approach. She has a love for children with chronic complex issues- specifically autism, PANDAS/PANS, and learning challenges. The Change Healthcare breach, as discussed in the podcast episode with Benjamin Reigle, has had a profound impact on the healthcare industry. The breach resulted in challenges with claim submission, cash flow, and data security, affecting not only the operations of Change Healthcare but also having a ripple effect on healthcare providers, payers, and patients. The breach led to issues with claim submission, as some payers who relied solely on Change as their intermediary were unable to process claims. This created a backlog of claims and disrupted the revenue cycle for many healthcare organizations. Some providers resorted to manual processes, such as printing and mailing claims, which added complexity and delays to the billing process. Cash flow was significantly impacted by the breach, especially for smaller healthcare entities like physician groups and community hospitals. With claims not being processed efficiently, these organizations faced financial strain due to delayed payments and uncertainty about when the situation would be resolved. The lack of cash flow threatened the financial stability of these smaller entities, highlighting the importance of having contingency plans and backup systems in place. Data security concerns arose as a result of the breach, with questions about the extent of the data compromised and the potential implications for patient privacy. The breach raised questions about the adequacy of security measures in place and the vulnerability of healthcare systems to cyber threats. The need for enhanced data security protocols and proactive measures to prevent future breaches became a pressing issue for healthcare organizations. Experian Health Client Communication February 23, 2024 Experian Health is aware of Change Healthcare's (Optum's) announcement on February 21 about a cybersecurity incident affecting its systems. Experian's Cyber Incident Response team and Cyber Threat Intelligence teams have been engaged and are closely monitoring the situation. We have not seen evidence of a threat to Experian. Out of an abundance of caution and consistent with Experian's security practices, Experian has temporarily severed connectivity to Change Healthcare. During the period of this severed connectivity, there will be an impact to batch and EDI authorizations, claims and remits, claim status, eligibility, and notice of care transactions that Experian Health sends to Change Healthcare. Experian will resume connectivity to Change when it receives assurances from Change Healthcare that it has adequately contained the threat. Payers work with a variety of clearinghouses to process data. In many cases, multiple payer connections are available although sometimes payer connectivity may be limited or even exclusive to a single clearinghouse. In the majority of cases, transactions from Experian Health route directly to the requestedpayer. When connectivity is limited or certain routes are unavailable, however, it is common to route traffic through another clearinghouse. Below is a list of potentially impacted payers. Our teams are actively evaluating and rerouting transactions when an alternative route is available and will provide an update to this list as soon as possible. Hello Creative Friends, When you exist inside an artwork, for better or worse, everything that gets added to that work gets compared to the original work as a reference point. If you're happily painting a pink canvas for 30 years, and then suddenly decide it's more in alignment for you to change it to blue, those first strokes of blue will look "wrong." They won't "fit." They might look "disordered." Even if those strokes are heading in the direction you want to go. In this episode I expound upon this metaphor as a way to reflect on our approach to disorders in society. Specifically in regards to my own diagnosis in 2007 of ADHD. Over the years, I began to revise my relationship with that label, and how knowledge of artistry related to those narratives. While not everyone listening to this episode has ADHD, we are increasingly living on a planet where everyone is experiencing being "disordered." In this episode I explore that story. Not only is Duriel brilliant, he has also qualified for the last two Olympic Marathon Trials and done so in his 30's while working an incredibly demanding job. He is also the husband of Christine Ramsey Hardy who has qualified for three Olympic Marathon Trials and helped Duriel jumpstart his marathon journey in 2019. Dr. Duriel Hardy is a pediatric neuroimmunologist in UT Health Austin Pediatric Neurosciences at Dell Children's and an adult neurologist in UT Health Austin's Multiple Sclerosis and Neuroimmunology Center within the Mulva Clinic for the Neurosciences. He specializes in multiple sclerosis and neuroimmunology and related inflammatory disorders. Additionally, Dr. Hardy is an assistant professor in the Dell Medical School Department of Neurology. Dr. Hardy earned his bachelor's degree in neuroscience from Brown University and his medical degree from Duke University School of Medicine. Hello Creative Friends, The crux of this podcast, my art, my business (and heck, my life) is making in tandem with something that is bigger. I first posted about this in 2022, and it was the most listened to episode of that year. I have talked about this on and off since, entirely in a positive way. In this episode I want to talk about the challenges that arise when you begin to prioritize creating in this way. We all are living in a world that still creates using external metrics. When you begin making art with something bigger, rather than making with the market or economy, with what your colleagues, friends, or family think, or with what institutions teach, you will (for better or worse) look "crazy" to them. In this episode I share a few ways I've navigated this, and continue to try to remain true to something bigger in the face of confusion from others. While I do believe this process is never-ending, I hope this episode helps you keep from doing the most insidious thing of all when making with something bigger: gaslighting and doubting yourself. Hello Creative Friends, This episode is a follow up to last week's Art as a Threshold, however as always it's also a standalone episode. In this episode I talk about one of the fundamental aspects of my work as an artist... My Art Kid. Not my human kid. Not an imaginary friend. The part of me that is a kid. The part that never leaves. The part that caretakes one of the most precious things I have as an artist... the gateway to 'something bigger.' She is the part of me that knows exactly what I'm here to do, and exactly how I want to do it. She is also the part of me that I told buzz off as a teenager, desperately trying to fit into the "responsible adult world." I think most of us dump on our Art Kids a lot as we grow older. Hello Creative Friends, This episode is, in some ways, piggy backing off "Art Will Save Us" posted a few weeks back. But in other ways, it's a standalone episode. Creating is often a largely solitary act. Even those of us who create in groups, like perhaps a choir or theater, have a very solitary experience that can make us wonder: Does this even matter? Like, does my ability to paint a bowl of apples function in any larger system? Does it do anything outside of making a wall more colorful? In some ways, a bowl of apples might just be about apples. It might be purchased by a restaurant, and people will regard it simply as a a painting of food. But there are many times where the things we make go out into the world and enter the Secret Sauce. The Soup. And the soup is doing its own thing. And in those moments our work is part of something else. As we enter 2024, can our awareness of the Soup... even if it's amorphous and mysterious... help us feel less alone and more meaningful in the ways we make things. Dana Henning is mother to Evan Henning, who is a 12-year-old Heart Warrior born with a critical congenital heart defect. Evan has has multiple procedures and open-heart surgeries. His life is very full of therapy appointments, homeschool activities, and extra-curricular activities such as Special Olympics and track.Dana is musically gifted. She is a former music teacher and choir director. These days she homeschools Evan and works as the Mended Little Hearts Austin Coordinator. She also works with the Parent Faculty Advisory Counsel at Dell Children's Hospital in Austin, Texas. This is the Weight and Healthcare newsletter! If you like what you are reading, please consider subscribing and/or sharing!At their base, the guidelines recommend intentional weight loss for higher-weight children via “Intensive Health Behavior and Lifestyle Training” starting as early as age two (2) with drugs as young as twelve (12) and surgeries starting as early as age thirteen (13).There is a lot to unpack here, and this will be a multi-part series but I have had a flood of requests to write about them since they came out, and I had to get very familiar with them because I presented Grand Rounds at Children's Minnesota this morning, so I thought I would get started with this first ever special edition of the newsletter so that I could get this info to you sooner and keep Saturday's newsletter from becoming a novel! I'm going to start with conflicts of interest because I believe these form the rickety scaffolding upon which the rest of these recommendations rest.When I think about conflicts of interest, I generally feel that having a conflict of interest is not proof of bias, but is certainly a red flag indicating the need for taking a deeper look. Failing to freely and openly disclose a conflict of interest, on the other hand, seems more to me like a red flag doused in gasoline and set on fire. There's a lot of the latter type of red flags in these guidelines.To address conflicts of interest, the guidelines themselves state:FINANCIAL/CONFLICT OF INTEREST DISCLOSURES: An Independent review for bias was completed by the American Academy of Pediatrics. Dr Barlow has disclosed a financial relationship with the Eunice Kennedy Shriver National Institute of Child Health and Human Development as a co-investigator.That's it. That is the only conflict of interest information provided. The same language is provided at the top of the guidelines and under the section “competing interests.” I cannot find any link to the actual review that they performed (if I missed it/you have it, please feel free to leave it in the comments!) I have to tell you that Dr. Barlow's disclosed relationship is pretty much the least of my concerns here.For example, based on their disclosure language you might not guess that, of the 14 authors who are medical doctors, at least 7 have taken money from companies that are developing or sell weight loss products that either directly benefit, or may benefit from these recommendations either through the development of a new drug, or approval of an existing drug for adolescents. The amount ranged from less than $20 for food and beverage (typically indicating that the doctor had attended one or more “educational” seminars by these companies,) to one author who took more than $50,000 primarily for consulting and speaking engagements on behalf of these companies.One of the companies that many took money from was Novo Nordisk, the pharmaceutical company which is aggressively marketing the drugs that are discussed in the guidelines (with an acknowledgment that they were included in the guidelines even though the research for them was published after the evidence review had already been completed.) Also, these numbers are only for 2015-2021 and where data is available. We know that Novo Nordisk has been pouring money into their effort to promote Wegovy and make good on their promise to shareholders that they would use the drug to double their “ob*sity* sales” by 2025, so it's possible that quite a bit more money has changed hands than is represented here. Regardless, as you can see, none of these payments are mentioned in the conflict of interest statement.Based on the American Academy of Pediatrics' conflict of interest statement, you also might not guess that, in fact, Novo Nordisk is a “Patron” of the American Academy of Pediatrics itself- donating somewhere between $25,000 and $49,999 to the organization.Other “patrons” of the AAP include:GlaxoSmithKline, maker of the weight loss drug alliGenentech, which sells the weight loss drug XenicalProlacta, which manufactures human milk-based nutritional products that they claim lowers the risk of ob*sityOf course, none of this is proof that they manipulated the guidelines to benefit these companies, but absolutely none of this is disclosed in the conflict of interest statement which, again…big red flag for me. Considering all of this, I am unconvinced that the American Academy of Pediatrics was in a position to conduct an “Independent review for bias” of guidelines that are incredibly favorable for their “patrons” and the companies from which the authors received money.You may also be surprised to learn that disclosure rules don't even require that they acknowledge that almost every author has a career that is based in the “higher-weight as lifelong chronic illness” model that these guidelines embrace and perpetuate. There are authors who run pediatric weight loss clinics, authors who run pediatric weight loss surgery programs, authors who are employed as pediatric weight management specialists. Below you'll find a list of the authors with information that I pulled from their various online bios about their past and current work in pediatric “ob*sity” as well as the payments I found to them on openpayments.cms.govFor almost all of the authors, these recommendations will increase the market for their services. That doesn't necessarily mean that's why they are making the recommendations, but it's still the literal definition of a conflict of interest. And yet, no disclosure of this is even required and, as you might imagine, none is made.There is also one oddity that I want to point out. Sarah C. Armstrong (who, of the authors who are MDs, took the most money from pharmaceutical companies that do or may benefit from these guidelines at $51,050.45) claims “I subscribe to a "health at every size" approach to supporting children, teens, and young adults living with ob*sity” on the same webpage on which her title is “Medical Weight Management Specialist.” That is, quite literally, impossible as those two things are mutually exclusive. Health at Every Size™ stands completely opposed to the concept of weight management and the pathologizing of body size, including the use of the term “ob*sity.” There is no ambiguity or grey area here, Health at Every Size™ is the trademarked brand of the Association for Size Diversity and Health, there are literally rules for this. So, from my perspective either Dr. Armstrong (a medical doctor who is co-authoring guidelines for other doctors) doesn't understand the basic principles upon which she claims her practice is built, or she is co-opting the language of weight-neutral health in order to promote weight loss. Either way, to me this is concerning enough to point out.I want to be clear, I don't think that these authors are necessarily trying to create profit for themselves, or to harm children. There are a LOT of degrees and certifications held by these authors, and many hold positions at prestigious universities and healthcare facilities. I think they are probably well-intentioned and truly believe that they are doing what's best for children. (As someone who is both queer and fat, I can imagine that if, a few decades ago, you had gotten together a group of doctors who had centered their careers on treating “homosexuality,” you would have gotten guidelines very much like this.) I also think that they are deeply misguided, and that's not surprising given the billions that the weight loss industry has poured into conflating health with weight and weight loss, including in the research, and the (mis) education of healthcare practitioners. Still, I believe that, if adopted, these guidelines will do massive harm. In Saturday's regular edition of Weight and Healthcare there is a deeper dive into the guidelines themselves. When we think of someone living we think of them out and about, working, running errands, and traveling. The truth however, is that massive creative aspects of our art and lives are not active. They are quiet, internal... pregnant with something. The culture does a poor job of educating us on this aspect of making life and art. Whole societies of people completely skim over the quiet, internal creative work that is so important to making new things. This sounds inconsequential, until you consider it in other terms. Imagine going into active labor with a human child and then once it was over, going into labor again. And again. It sounds silly, and yet look around at the planet. There is evidence of human, social and systemic breakdown that absolutely results from unsustainable creative cycles such as perpetual labor without pause. Hello Creative Friends, In 2016 when I launched my illustration business I posted a light comparison to "what I imagined" was a similarity between birthing a business and birthing a baby on social media. Woo! Was that a touchy subject! A lot of parent friends had an opinion on that one... and they were spicy. Interestingly however, parents who also worked for themselves all agreed: Business and Babies were similar. This episode is about one of the similarities I've noticed now that I have firsthand experience with and Babies: They are catalysts for healing. The culture doesn't talk about this very often. In this episode I talk about a topic that's near and dear to my heart: Using art making as a model for making tough decisions. In April of this year I shuttered most of my art business, even at the height of its success, to focus on my new son. No one was more surprised than me, to make that decision. Big decisions often bring up a lot of turmoil. There have been large decisions in my life that have absolutely wrecked my quality of life. Over the years I've learned a lot about navigating those decisions in ways that preserves my well-being. Over time I realized those ways had a lot in common with being an artist. Artists have so much in their arsenal that they don't realize. We're all taught to focus on the things we lack and improve upon them. In this episode I made the case that certain parts of the artistic mindset can be a tremendous ally to us as we make large life choices. There is a significant mental health care epidemic going on that has only been worsened in the past few years with the COVID pandemic. Rates of anxiety and depression are sky high, not just for adults, but for adolescents and teens as well. Our current approach to mental health care places the burden of care upon Psychiatrists and therapists and the truth is there just aren't enough people to meet the incredible demand out there. When Psychiatrists or other mental health care professionals are seeing patients, there are often not able to do so in a holistic and collaborative way - an approach that will ultimately benefit the patient more. This type of a model with better access to care is so necessary and the topic of my discussion today with Dr. Hani Talebi. We discuss: His story and journey into the world of mental health. In his role as Senior Vice President of Health Systems Integration, Dr. Talebi's work focuses on advancing collaborative care and measurement-based care in primary care and health system settings. With more than 20 years of clinical experience, he has served in a variety of roles including Director of Pediatric Psychology at Dell Children's Medical Center, Director of Psychological Services at Del Valle Independent School District, Director of Clinical Programming in various community mental health centers, and as a clinical/consultative leader in the private sector. Dr. Landers graduated from Auburn University, in Auburn, Alabama, with BS degrees in Biology and Chemistry. At Auburn, she was elected to Mortar Board, the National Women's Honorary. In 1977, she received her MD degree from the Medical University of South Carolina, in Charleston, South Carolina. There she was a member of Alpha Omega Alpha (AOA), the National Medical Honorary. After graduation from medical school, she completed a pediatrics residency at the University of Texas Southwestern Medical School hospitals in Dallas, Texas, in 1980. She completed her neonatology fellowship at Baylor College of Medicine hospitals, in Houston, Texas, in 1983. Dr. Landers practiced academic neonatology for fourteen years and served on the faculty of two medical schools – Baylor College of Medicine, in Houston, and the University of Arkansas for Medical Sciences, in Little Rock. In this role, she conducted clinical research, published twenty-three peer-reviewed papers, and taught medical students, residents, and fellows. While caring for patients in private practice, she served as a speaker for the Texas Department of State Health Services from 1997 to 1998. She was Medical Director of the Mothers' Milk Bank at Austin, from 2000 to 2004, and served on the milk bank's board of directors from 2006 to 2009.Even though she practiced full-time, Dr. Landers continued to publish papers and work for the American Academy of Pediatrics (AAP). She was an expert in breastfeeding medicine and became a Fellow of the Academy of Breastfeeding Medicine (FABM) in 2002. She served as a physician educator at AAP and ABM national meetings for a decade. She served on the Executive Committee of the Section on Breastfeeding in the AAP from 2008 until 2014. In that capacity, she contributed to AAP policy statements and clinical guidelines, and wrote four more peer-reviewed publications and a book chapter. In 2008, she was recognized by Pediatrix Medical Group with a national award for “Outstanding Accomplishments in Quality Improvement.”Together with her husband, Dr. Phillip Berry, she raised three children, one son and two daughters, each to young adulthood. I am, by profession, an ad writer. I tell stories about people and products and services. You do, too. But because I get paid for it, I spend a lot of time considering – and measuring – the impact of stories. Some of the stories I've told have made people an enormous amount of money. But the most important stories I tell, by far, are the stories I tell about myself, to myself. Those stories are the source of my identity and the foundation of my purpose in life. But we've talked enough about me. I see something good in you and I'm calling it out.Is it okay for me to do that? Let us stare together into the eyes of the truth: Whether good or bad, your current circumstances are temporary. Success is temporary. Failure is temporary. Your future depends on your choices. Your choices depend on what you believe. What you believe is not determined by what you see and hear, but by how you interpret what you see and hear. How you interpret what you see and hear is determined by the stories you tell yourself, about yourself. Who do you believe yourself to be? What do you believe about this world we live in? What does the future hold? Your mood, your attitude and what happens to you next will be greatly impacted by your answers to those questions. “If you want your baby to die with a name, you need to pick one now.”The newborn had inhaled meconium during birth, the most the doctors had ever seen. His lungs were 95% full of it. The father and the baby rode with lights and sirens to Dell Children's Hospital 30 minutes away, with the grandmother riding the back bumper. The doctors at Dell looked at the x-rays and slowly shook their heads in disappointment. The grandmother stayed with the newborn while the father went back to see his wife. The mother was puzzled when the nurse showed her the baby's birth certificate. She and her husband had been torn between 2 names for their new son and had agreed to choose the name after they met him. The husband walked into the room. She said, “I thought we agreed to talk about it before we chose the name.” “Honey, Lincoln died. But Gideon overcame impossible odds. When they asked me his name, I said: ‘This boy isn't Lincoln. This boy is Gideon.'” When the specialist at Dell met with the parents the following day, he was holding two sets of x-rays. Holding up a film in his left hand, he said, “I have no explanation for it, but this baby…” Then he lowered that film as he raised the one in his right, “isn't this baby.” Gideon will be eight years old on March 15 and he suffers no after-effects at all. You may believe that what happened was going to happen anyway, and that belief in the power of a name is superstitious nonsense. That would be the logical, scientific belief, to be sure. But do you really believe that beliefs have no power? Beliefs are what separate Democrats from Republicans, Hindus from Muslims, stock market Bulls from stock market Bears, and scientists from storytellers. Your beliefs are what make you who you are.And your beliefs are determinedby the stories you tell yourselfabout yourself.You are not responsible for the beliefs of others. You are responsible only for your own. During his time at Walden pond, Henry David Thoreau observed, “The mass of men lead lives of quiet desperation.” And I agree with him. I also agree with Jack Kerouac. “But why think about that when all the golden land's ahead of you and all kinds of unforeseen events wait lurking to surprise you and make you glad you're alive to see?” Did you experience 5 years of life during the past 5 years? Or did you experience 1 year of life 5 times? Don't let 2018 be the 6th straight year of 1 years' experience. Do something new. Tell yourself a different story about yourself. And believe it. Roy H.
