PS: Paul Sweeney is an MSP for the Glasgow region and is the spokesperson for mental health for Scottish Labour. Formerly the MP for Glasgow North East, he served as Shadow Minister for Scotland from 2017-2019. Paul is a proud trade unionist and a member of both Unite and GMB trade unions, and worked closely with Unite to try and save 200 jobs at St Rollox Caley Railway Works in 2019 and in attempts to avert the closure of the McVitie's factory in Tollcross in 2021. More recently, Paul continues to work with trade union colleagues on issues such as social care pay and conditions and to protect the roles of community link workers in Glasgow. MR: Dr Mike Ramsay is a senior lecturer in mental health nursing, part-time practitioner, researcher and writer. He is professional lead for mental health nursing at University of Dundee. Mike's NHS career spanned 20 years prior to academic life as both RGN and RMN across various roles and specialties before 8 years as a senior nurse in hospital and community settings. An active member of the Mental Health Nurses' Association, he has served on the editorial board of Mental Health Nursing since 2006, including over 6 years as Chair. Mike's academic interests centre upon therapeutic communication, sociology of mental health and mental health in later life and research interests include carers for those with dementia, MHN identity and nursing education. His doctoral thesis explored graduateness in nursing and educational influences in compassion in nursing and social work. In 2017, Morgan started a project where she invites people to submit their stories to her website. From there, she creates art as a response to their stories and sends it to them before sharing the work publicly. All stories and names are kept private. The concept of “accountable care in the safety net” was introduced in a Dartmouth Study published by the Commonwealth Fund back in 2013. In that study, there were 4 critical success factors outlined for a coalition-based Medicaid ACO: 1) aligned leadership through a shared vision, 2) strong governance, 3) a unified strategy for using data, and 4) a sophisticated care coordination infrastructure. Abstract: Spatial- and object-related signals are preferentially processed through the medial and lateral hippocampal networks (MHN and LHN), respectively1,2. MHN comprises interconnected medial entorhinal cortex, proximal CA1 (pCA1), and distal subiculum (dSub), whereas the LHN comprises interconnected lateral entorhinal cortex, distal CA1, and proximal subiculum3,4. Previously, we showed that Teneurin-3 (Ten3) has matching expression in all interconnected regions of the MHN and is required in both CA1 and subiculum for the precise pCA1->dSub axon targeting through homophilic attraction5. Can matching gene expression in interconnected nodes of the LHN also contribute to hippocampal network assembly? Here, we discovered that latrophilin-2 (Lphn2), an adhesion GPCR known to bind teneurins6-8, has matching expression in the LHN that is complementary to Ten3. Viral-genetic perturbations in vivo revealed that Ten3+ pCA1 axons are repelled by ectopic expression of Lphn2 in dSub, and ectopically invade proximal subiculum deleted for Lphn2. Simultaneous subiculum deletion of Lphn2 and Ten3 causes Ten3+ pCA1 axon mistargeting reflecting loss of both repulsion and attraction. Our findings demonstrate that Lphn2 acts as a repulsive ligand for Ten3+ axons, identify Ten3 as a receptor for both repulsive and attractive ligands in the same axon during target choice, and reveal how a "Ten3->Ten3, Lphn2->Lphn2" rule directs the precise assembly of functional hippocampal networks. Logan chats with Cheryl Lulias, President and Executive Director of Medical Home Network, based in Chicago, Illinois, about how MHN strives to improve care for their most vulnerable patients in Cook County utilizing social determinants. MHN focuses on how care is delivered to to Medicaid recipients and unites healthcare systems and community organizations in providing that care. This comprehensive care model is based on behavioral, medical, and social factors explains Lulias. MHN coordinates care across the continuum, providing seamless communication in patient care, and mobilizing resources for the right patients at the right time, she adds. The result: Costs have been cut by more than $50 million in the last four years. This huge savings is derived from shortening inpatient hospital stays, reducing emergency room utilization, increasing primary care involvement, improving communication, and connecting hospitals to patient’s homes to enhance real-time alert activity. In addition, their screening process focuses on “11 impactable barriers” that include access to food, security, and transportation, which are predictive of medical care costs. When COVID-19 arrived, MHN was able to use social determinants and AI to create effective treament plans. Understanding that this pandemic would strongly affect the elderly, poor, and socially isolated patients, MHN took preventative measures to reduce risk. The MHN care management team has now completed more than 6,000 outreach calls in the COVID-19 era in the hope that ERs would not be overrun and to help patients recognize symptoms and figure out the best treatment. He has served as an internal consultant at AltaMed; senior consultant at Cattaneo & Stroud, Inc.; executive vice president and COO at HealthSpring; president and CEO at MHN; senior vice president at Health Net; senior vice president, benefit administration, at Kaiser Permanente; partner at NorthShore LLC; and regional vice president, Pacific Rim at North American Medical Management/Phycor. Thomas Lee is an MD with One Medical Group. He specializes in primary care internal medicine with an emphasis on preventive health, complex cases and quality improvement. Lee graduated from Yale University and the University of Washington School of Medicine, and completed his residency at Harvard’s Brigham and Women’s Hospital before serving as editor-in-chief for the widely used drug reference application Epocrates. He then founded One Medical Group as a step toward improving primary care delivery. Paul Wallace is director of the Lewin Center for Comparative Effectiveness Research. A board certified physician in internal medicine and hematology, he is a renowned lecturer on topics including evidence-based medicine practice and policy; performance improvement and measurement; clinical practice guideline development; population-based care and disease management; new technology assessment; and comparative assessment. He serves on advisory committees at the Institute of Medicine (IOM), and is a member of a number of healthcare-related boards. Arnold Milstein is professor of medicine and leader of Stanford University’s Clinical Excellence Research Center. His career and ongoing research are focused on acceleration of clinical service innovations that improve the societal value of health care. He serves as the medical director of the Pacific Business Group on Health, the largest regional health care improvement coalition in the U.S. He also guides employer-sponsored clinically-based innovation development for Mercer Health and Benefits. Dr. Leroi Hicks is an Associate Professor in Medicine in the Division of General Medicine and Primary Care at Brigham and Women's Hospital and a Lecturer in Health Care Policy at the Harvard Medical School. His research has been related to three areas: (1) The effects of patients' racial and cultural background on the treatment and clinical outcomes of chronic disease; (2) the development and assessment of interventions aimed at improving quality of medical care and the reduction of disparities in care; and (3) community-based participatory research to identify and address healthcare disparities. In addition to his research roles, Dr. Hicks is co-Director of the Health Disparities Program and research faculty to the Community Health Innovation and Research Program for the Harvard Catalyst. Dr. Hicks received his M.D. from Indiana University and completed his M.P.H. from the Harvard School of Public Health. Theresa S. Morrow is the Director of Marketing and External Relations for Men's Health Network (MHN) located in Washington, DC. MHN is a nonprofit, educational health organization dedicated to improving the health and well being of men and their families. Through her work at Men's Health Network, Theresa has co-founded the newly formed, Women Against Prostate Cancer (WAPC), a national organization working to unite the voices and provide support for the millions of women affected by prostate cancer and their families. She has successfully organized concerned women advocates from across the country around the importance of prostate cancer efforts and lead efforts around the First Annual WAPC Advocacy Day in July 2010.
