“We came up with the idea of testing a ketogenic diet… which in mice, it's basically a lot of Crisco… giving a keto diet to mice with CGD… led to a decreased susceptibility to colitis.” —Dr. Emilia Liana FalconeThe microbiome isn't something sitting on the sidelines. It's part of the immune system, interacting with the barrier, shaping responses, and, in IEI, reflecting the underlying defect. On this episode, Dr. Mariam Hanna is joined by Dr. Emilia Liana Falcone, physician-scientist and director of the Microbiome and Mucosal Defense Research Unit at the Montreal Clinical Research Institute, to walk through how these host–microbe interactions drive disease. From early-life immune programming to microbial signals that activate inflammatory pathways, this is a shift from association to mechanism. And a step toward therapies that target both sides of the equation.Key Points:The microbiome reflects the underlying immune defect in IEIFocus on what microbes are doing, not just which ones are presentMicrobiome changes both result from—and contribute to—diseaseEarly life is a critical window where these interactions are setLoss of protective microbial functions matters more than specific bacteriaMicrobial signals can directly drive inflammation (including inflammasome activation)Future treatments will likely combine immune therapy with microbiome-targeted approachesThis is not about adding a probiotic. It's about understanding how immune defects reshape the microbial environment, and how that environment feeds back into disease.Get that interaction right, and you're not just managing symptoms. You're changing the system driving them.Have an idea for the show or a comment, send us a text!Visit the Canadian Society of Allergy and Clinical ImmunologyFind an allergist using our helpful toolFind Dr. Hanna on X, previously Twitter, @PedsAllergyDoc or CSACI @CSACI_caThe Allergist is produced for CSACI by PodCraft Productions

"So the future is one IV infusion, likely no chemotherapy, and that'll cure our IEIs." — Dr. Nicola WrightFor children born with inborn errors of immunity, bone marrow transplant has long been the closest thing medicine had to a cure. It works — but it comes with chemotherapy, graft-versus-host disease, and a donor search that doesn't always end well. Gene therapy is changing that calculus. Dr. Mariam Hanna speaks with Dr. Nicola Wright, a pediatric hematologist and clinical immunologist at the Alberta Children's Hospital and holder of the Barb Ibbotson Chair of Pediatric Hematology, whose research focuses on developing gene editing platforms for blood and immune disorders. On this episode, they discuss:Why bone marrow transplant is, in Dr. Wright's words, "almost a poor man's gene therapy" and what gene therapy offers insteadThe spectrum of technologies: lentiviral insertion, CRISPR, base editing, and prime editing. What each does, and where each falls shortImmune reconstitution outcomes across diseases, including over 90% good immune reconstitution in ADA-deficient SCID treated with lentiviral therapyHow to counsel a family when gene therapy might be an option and why most patients still can't access itThe "valley of death": why therapies that work in trials are failing to reach patients, and what it will take to cross itCAR T-cell therapy in IEI, including a Canadian trial underway for refractory autoimmune diseaseWhat long-term follow-up looks like and why 15-year post-trial monitoring is now an FDA requirementThe science is outpacing the infrastructure. Dr. Wright's vision of shipping cells instead of patients, in vivo delivery via lipid nanoparticle, no chemotherapy required isn't speculative. The runway is being built.

We spoke with the Institute for Education Innovation (IEI) Founder and CEO, Doug Roberts, about the problem and possible solutions. We also got a better understanding about the IEI, a network of over 100 K-12 superintendents committed to improving student outcomes. IMAGE CREDIT: iStock / Getty Images Plus

At the time of recording, Indigenized Indigenous Energy Initiative was named Indigenous Indigenized Energy Initiative.Indigenous Energy Initiative:Indigenous Energy Initiative (IEI) is a native-led non-profit and Earth Island Institute-sponsored project that is developing solar infrastructure within tribal communities. Native communities disproportionately lack access and pay higher costs for utilities, especially electricity, which significantly impacts access and opportunities for remote work, education, and more. In 2014, the U.S. Energy Information Administration reported that 14% of native households lack access to electricity, which is ten times the national average. IEI is working to increase indigenous energy ownership and access to help address the social, economic, and environmental injustices that burden native communities. IEI's focus is on building energy independence as a way to support indigenous communities and eradicate energy poverty. IEI is currently working within the Northern Cheyenne Reservation community in Montana to guide the reservation's $4.1 million solar initiative. IEI's first project was Muddy Hill Microgrid which was a small solar system to support a multi-use community center, fire station, and water filtration system on the reservation. The project was built by tribal members who received solar training through IEI. In 2021, IEI worked on a residential project for elders in the Standing Rock Reservation. Another project IEI is currently working on is the White River Community solar project which is developing residential solar for specific tribal elders and commercial solar for several schools. The Department of Energy is providing $3.2 million of funding for the project and the reservation, and IEI must match twenty percent of this funding. Chéri Smith:Chéri Smith is the founder and CEO of the IEI. Smith is the descendent of the Mi'kmaq tribe of Maine/Canadian maritime and her mission is to use her expertise in climate change and economics to educate, build solar capacity and bring independence to indigenous communities. Smith has two decades of experience in energy and environmental sustainability in multiple sectors from the public, private, NGOs, higher education, consulting, and tribal communities. Before IEI, Smith worked as a coordinator at Tesla-SolarCity, Director of Education and Outreach at the American Council on Renewable Energy, she served as an advisory board member for Masters in Renewable Energy program at Pennsylvania State University, worked as an advisor at the Yale School of Business and the Environment, and worked on Core Education Advisory Committee for SEIA's Solar Power International. Additionally, Smith founded and was the Executive Director of the nonprofit Solar Campus Initiative and there worked as Climate Leader trained by Al Gore. Smith also was awarded the Cordes Fellowship for recognition for developing solar projects for native communities. Through her work at IEI, Smith works to honor her native heritage and use energy knowledge to make native communities more resilient, independent, and sustainable sovereignties. We've all learned so much from the pandemic, and school leaders and superintendents are no exception. In addition, things that we knew from being inside the schools have been spotlighted to the public, thanks to social media and the news media. While that might be good for conversations around change, there's still a lot to unpack and many hurdles and red tape to get around. This week on the podcast, I'm talking with Doug Roberts, an educational consultant who works with education entrepreneurs and district superintendents. He's recognized the importance of connecting leaders across state lines to help bridge the gaps that are all too evident now. We're talking about changes on the horizon thanks to 1:1 models that were realized during the pandemic, why leaders are hitting their limit of what they can give, and the hardest part of being an educational leader. We need to continue having conversations like this as we strive to give leaders the tools and support they need to continue the impact they desire. About Doug Roberts: Doug has worked with leading ed-tech entrepreneurs and district administrators for almost 20 years, developing partnerships that improve outcomes for students and help nascent organizations get their "sea legs." As Founder and President of Educational Solutions Consulting, Doug found that there was a piece of the puzzle missing, a barrier between those who run school districts and those who start companies to help school districts. He partnered with some of the nation's most innovative educational leaders to form IEI to bridge that gap. A Princeton graduate, Doug is a former public high school social studies teacher and ed-tech business development executive who earned his Ed.M. in Teaching and Curriculum from the Harvard University Graduate School of Education. (16:16)Do you feel like the awards program is elevating your profile and do you think it is making an impact in the industry? (24:07)What final advice or insights would you like to share? (25:55)In This Episode, You Will Learn:About the Institute for Education Innovation (IEI) (2:16)How to conceptualize an awards program (4:45)Considerations before launching an awards program (7:19)Starting an awards program (11:26)An overview of what happened in the Supe's Choice Awards for 2021 (16:22)The long-term branding benefits of an awards program (26:02)Quotes:“The people making the decision have to be authentic about who wins, so you have to really believe in what you're doing, and it has to come from the right place.”“I would say the setup part was the most difficult. Once it was set, as far as getting the judges in there, getting the scorecards—that ran very smoothly, but there was a lot of front-end work. So I would also say to anybody who's starting off, don't get discouraged if you feel very overwhelmed in the beginning, because really doing all that hard work, in the beginning, is what pays off in the end. Then it becomes a smoother ride.”“I think our members care to be part of this conversation about quality. One of the things they tell me all the time is that they appreciate that the companies that get in front of them are usually either ready for prime time or ready to roll up their sleeves and listen and work with them and let them provide input. I think that's an important part of what we do, positioning these discussions to be successful between them. And I think Supe's Choice now could be a way to introduce concepts that can turn into larger discussions.”“Even if you don't win, you are still seen by so many district leaders that you could get potential leads that way. It really is a conversation starter. And it's a great low-cost way to be seen.”“You have to have some standing in your industry, you have to have a good network and a list to message to in the beginning to grow from. So [starting an awards program] shouldn't be the first thing you do; you should have something going already. I think that's really important. We were able to build off of our existing brand network membership to grow this.”Connect with Doug Roberts and Alexa Escobedo:Institute for Education Innovation (IEI) WebsiteDoug Robert's LinkedInAlexa Escobedo's LinkedInStay in touch with Sarah Williamson:Free Case Study GuideWebsiteFacebook
