My guest this time is Dr Nicky Keay: Nicky is a medical doctor with expertise in the field of exercise endocrinology and a lecturer at University College London. Her clinical and research work is focused on women experiencing peri/menopause and with exercisers, dancers and athletes. Her book Myths of Menopause was published at the end of 2024 and Nicky has taken a novel approach to the subject, using the theme of Greek mythology, with Athena, goddess of wisdom as the cover star!     The book itself brings together chapters from a wide range of experts in their individual fields, from brain health to bone health to exercise. Nicky and I discuss some of the most prevalent myths of menopause in this episode, not least the negativity around menopause and the ‘narrative of decline' that links in to the pervasive gendered ageism in society (or Western society at least!). Another incredible podcast episode ready for your listening pleasure this week - Lara Briden was so generous to gift me an hour of her time to talk about metabolism with women in perimenopause and through menopause. We discuss what effects metabolism - from genetics and weight resistance, to the impact of epigenetics, the relationship of oestrogen therapy and insulin resistance, and how evolutionary mismatch means modern women approaching menopause are not recalibrating our metabolism like our ancestors.  Lara is a dream to listen to - she effortlessly describes the influence of oestrogen on satiety, what metabolic inflexibility is and provides a really logical approach to the latest intermittent fasting craze. I honestly cannot recommend this episode enough for practitioners and anyone who was interested in Lara & Nicky Keay's talk last year.

In this episode of the Female Athlete Nutrition, I talk with a returning guest, Dr Nicky Keay. Listen to Episode 38: Understanding Hormones for Female Performance for my original conversation with Dr Nicky Keay. Dr Nicky joins us again to share her love of hormones and the new book she has just written on this topic “Hormones, Health and Human Potential. A guide to understanding your hormones to optimise your health and performance”. Working in this field for over 30 years, Nicky has amassed a wide variety of different experiences and advice that she shares in stories throughout the book. Zooming out, Dr Nicky introduces listeners to what hormones are and how they work in the body. Nicky explains the importance of hormones for health, as well as the role our behaviors have on our hormones, things like sleep, nutrition, exercise, and stress levels. Nicky ranks sleep as the most important factor for healthy hormones, advising listeners to prioritize sleep in the hours before midnight. Second, Nicky discusses the importance of varied exercise, avoiding either extremes of doing too much or not enough. Thirdly, Nicky touches on nutrition for hormones: going long periods without eating increases cortisol and should be avoided. We discuss hormones throughout the lifespan, starting before we're even born. We talk about how our exercise and nutrition needs differ as we age in response to fluctuation hormones. Dr Nicky and I talk about being proactive to get the most out of our hormones rather than being reactive when problems arise. We touch on the menopause and debunk the myth that hormone replacement therapy (HRT) is dangerous. Nicky speaks to the benefits of HRT during the menopause and for athletes experiencing RED-S. This week we're talking hormones. Well, first of all we're talking about creepy blokes in spinning classes, swiftly followed by Wordle tactics. It's all going on. At one point, we even start talking about running. Then after a bit, I have a chat with Dr Nicky Keay from Forth who offer blood tests to track your hormones. In this episode, I talk with a client of mine, Anna Grace, who recently graduated from our Female Athlete System of Transformation program (aka the FAST Track). Anna Grace shares about how her drive to perform at her best in soccer led her down a path of disordered eating and over-exercising that zapped her energy and impaired her performance. Anna Grace explains how the comparison trap to professional athletes and desire to “look like an athlete” fuelled her drive to train harder and restrict her diet more. This “discipline” backfired leaving Anna Grace frustrated at her lack of athletic improvement, ultimately injured and not achieving her goal of participating in NCAA D1 sports. Listening to podcasts like the Female Athlete Nutrition Podcast, coupled with the low of being injured, led Anna Grace to recognizing signs and symptoms of RED-S in her own life, including: low energy, mood fluctuations, menstrual irregularities and amenorrhea. We discuss how her internalized image of what an athlete looks like wasn't a picture of health. Anna Grace's journey to getting healthy wasn't a quick fix, nor is it for anyone recovering from chronic under-fuelling and overtraining, but with patience and persistence she has made huge progress. Anna Grace reflects back on the challenges of recovery, and speaks on the despair she felt from an overload of information when the severity of RED-S really hit home for her. Seeking help from specialists like Dr Nicky Keay, who I spoke with on Episode 38 of the Female Athlete Nutrition Podcast, and myself at Rise Up Nutrition, Anna Grace shares the progress she made regaining her menstrual cycle, changing her thought patterns and mindset, and building up her training in a health-supporting way.   We discuss the emotional rollercoaster of RED-S recovery: first the frustration of not achieving athletic results and the denial of anything being wrong, to acknowledging disordered habits and then working to change them, to the fears of body change and feeling  trapped and powerless to change. I enjoyed the podcast episode with Dr Nicky Keay so much last season that I had to invite her back, this time to focus specifically on the endocrine system and shining a light on the menstrual cycle - absolutely essential for all women! This episode really hones in on understanding the menstrual cycle, how the hormones in our body adjust throughout the system and nutrients to support us each month. Dr Keay also so eloquently explains what's happening in women's bodies in relation to endometriosis and PCOS, including the clinical testing that's involved for a true diagnosis. I recommend this episode highly for women who notice irregularities with their menstrual cycle, a good starting point to understanding what going on in your body.

Dr Nicky's aim is to redefine optimal health and fitness for the individual. Her approach integrates all aspects of her professional clinical experience with her medical research expertise in Endocrinology. She conducts clinical endocrine research, frequently publishes in medical journals, presents at medical conferences and is currently writing a book about hormones. Dr Nicky Keay was awarded a BA in Medical Tripos and medical qualifications MB, BChir from Cambridge University. After passing post graduate medical examinations to become a Member of the Royal College of Physicians (MRCP), Nicky gained extensive clinical and research experience in Endocrinology. As a Research Fellow at St Thomas' Hospital, she was also part of the international medical team working to develop a test to detect athletes doping with growth hormone, supported by the International Olympic Committee. Nicky's other research studies investigated the effects of exercise and nutrition on the Endocrine system, body composition and bone mineral density, supported by medical research awards. These various research projects led to a number of publications and awards. With medical scholarships her international attachments have included North Sydney Sports Medicine Centre, Olympic Park Sports Medicine Centre Melbourne; Australian Institute of Sport; Faculty of Medicine, Discipline Human Physiology, University Newcastle Australia; and Department of Paediatrics, Division of Growth and Reproduction, University of Geneva. Back on the podcast today is Eric Helms, PhD. Eric is a research fellow at the Sports Performance Research Institute New Zealand at Auckland University of Technology, pursuing research in training, nutrition and psychology for strength and physique sport. He has a PhD in Strength and Conditioning with a research focus on autoregulating powerlifting, a masters with a research focus on protein and macronutrient manipulation for dieting bodybuilders, a second masters in exercise science and health promotion. Also an athlete, Eric earned pro status as a natural bodybuilder with the Professional Natural Bodybuilding Association in 2011 and is a powerlifter in the International Powerlifting Federation. Dave McNeill at time of recording this episode had recently wrapped up his Physiotherapy Studies but also just days earlier ran a 28:16 for a 4th Place at the Australian 10,000m Track Titles at the Zatopek Classic. By way of bio, in the 2012 London Olympic Games, Dave finished 28th in the 5,000m, at the 2016 Rio Olympic Games Dave finished 16th.  In the final of the 10,000m in July 2019 Dave marked his comeback with a 5000km National Cross Country Title but as you'll hear during this episode, Dave's career has certainly also take him to the wilderness years of injury. In fact, injury cost Dave Australian representation at the 2018 Commonwealth Games where he was set to take part in the 5,000m and 10,000m events. It was bone stress injury which sidelined Dave. As you will hear today that took Dave down the path of exploration where he was to discover that one of the causing factors of his injury had been training through the status of low energy availability or as we've been exploring in prior episodes of the show including last week's Expert Edition with UK based Sports and Endocrinologist Dr. Nicky Keay. Many endurance athletes inadvertently and unintentionally find themselves in a state of RED-S Relative Energy Deficiency in Sport. The flow on effects can be detriments in performance but also the effects can be found through a detriment of health. One particular flow on effect of relative energy deficiency for athletes is low bone mineral density. Dave had to confront that when his bone mineral density came back very low in his lumbar spine. During this episode we explore topics including how to navigate self doubt, appraising readiness to perform, overcoming the fear of the pain that we put ourselves through leading up to an event, Dave's concept of old man strength, the idea of not looking at the training in isolation or in a vacuum but rather over the long term considering the bigger picture, the mistake Dave made in mistaking the stable weight has been an energy balance.
