In that capacity she oversees a staff of over 150 professional social workers and support staff that provide a wide range of social work services in inpatient acute, outpatient and residential settings. Social workers at VANJHCS provide case management, discharge planning, program coordination, psychotherapy and educationservices. Prior to being appointed to her current position, Emily was the Program Manager for the Transition and Care Management (TCM) program at VANJHCS which serves Post 9/11 Veterans, providing case management and other support services. She began her career with the VHA in 2008 when she was hired as a CBOC Social worker at the Hudson Valley VA Health Care System. In that capacity she provided concrete services, case management and crisis intervention to Veterans in a Primary Care clinic. This allowed her to learn the VHA system inside and out rather quickly. Prior to joining VA, Ms. Feiner enjoyed a varied career in Social Work spanning over two decades. She worked with adolescents and young adults in schools and an outpatient clinic, homeless pregnant women in a maternity shelter, and was the Director of an Outpatient Substance Abuse Counseling Center. Ms. Feiner was also an adjunct professor of Social Work at Fordham Graduate School of Social Services where she taught courses in Advanced Practice, Human Behavior and the Social Environment, Substance Abuse Treatment and Clinical Practice seminar. In addition, she also taught in the Human Services program at Westchester Community College. She has maintained a private psychotherapy practice since 1988. Emily has always had a strong commitment to her community and has served on the boards of several community agencies including HeadStart of Rockland and Planned Parenthood Hudson Peconic. She was twice elected to her local Village Board of trustees. Ms. Feiner holds a Bachelor of Arts degree from Hamilton College and a Masters of Social Work from Hunter College of the City University of New York. She has completed the coursework for a PhD in Social Work at New York University. She is the proud mother of two young adults, and enjoys going to hear live music, skiing and hiking in her free time. Research suggests that LGBTQ adults have higher rates of substance use and mental health issues, compared to their heterosexual cisgender peers.  These health disparities are often related to minority stress—the chronic psychological and emotional strain that people experience due to being part of a marginalized social group. For example, some LGBTQ folks may use alcohol and drugs to cope with internalized homophobia or to self-medicate for anxiety and depression. On the other hand, some people use drugs in the context of social settings and connecting with others in their community. Since not all substance use is unhealthy, how do providers know how to identify when substance use is problematic?  And what can physicians do to best support their patients who may be at greater risk for harm or even overdose?    On this episode, Antonio Urbina, MD, Medical Director for CEI's HIV Primary Care and Prevention Center of Excellence, speaks with Eric Kutscher, MD, MSc. Dr. Eric Kutscher is an Assistant Professor of Medicine at the Icahn School of Medicine, where he practices as a primary care and addiction medicine physician at Mount Sinai's Internal Medicine Associates and REACH Program. He currently serves as the Medical Director of the Mount Sinai Hospital Opioid Overdose Prevention Program and Primary Care Lead for the Mount Sinai Center for Transgender Medicine and Surgery. His clinical work and research focus on harm reduction techniques for patients with substance use disorders, particularly within the LGBTQ+ community. Drs. We have pre-diabetes and pre-glaucoma, diagnosis intended to prompt early treatment in order to prevent worse disease.  Should we be diagnosing and treating Pre-Addiction? Thomas McLellan, Ph.D. has been a career researcher in addiction treatment and policy for 40 years, working primarily at the University of Pennsylvania and at the Treatment Research Institute (TRI) which founded and served as CEO for 25 years. From 2000-2009 he was Editor-in-Chief of the Journal of Substance Abuse Treatment.  From 2009 to 2011, he was Deputy Director of the White House Office of National Drug Control Policy (ONDCP) where he formulated President Obama's National Drug Control Strategy and helped include addiction treatment into the Affordable Care Act.  In 2015-2016 he served as Senior Editor on the US Surgeon General's Report, Facing Addiction. In his career he has published over 550 articles and chapters on addiction research and was awarded over 150 NIH research grants.  He has been among the top ten most cited researchers in the addiction field for 15 years. Dr. McLellan is the recipient of several distinguished awards including the Lifetime Achievement Awards of the American, Italian, Australian, Egyptian and British Societies of Addiction Medicine; the Robert Wood Johnson Innovator Award; Distinguished Contribution to Addiction Medicine from the American Public Health Association, the American Medical Association; and jointly from the National Institutes of Drug Abuse and Alcoholism and Alcohol Abuse. He currently serves on the boards of Indivior LLC (makers of Suboxone) and Recover Together; and lives with his wife Kerry in Sarasota Florida. Today’s podcast discusses the impact of foster placement on children. According to researchers Sarah Font and Elizabeth Gershoff, at any given time in the United States there are approximately 700,000 children living in out of home placement away from their parents due to substantiated child abuse or neglect. While foster placement is an effort to protect vulnerable children, there is no question that placement itself brings its own level of trauma. Removal from a family home to an already traumatized child adds another layer of trauma and loss. As Font and Gershoff note, however, “the alternative to foster care may be continued abuse, neglect and traumatization of vulnerable children. Leaving children in homes with caregivers investigated for maltreatment can have dire consequences”.   Dr Putnam-Hornstein studied children born in California between 1999 to 2006 and found that children who were reported to the system for maltreatment, substantiated or not, were 6 times more likely to die from intentional injury by their caregiver and 2 times more likely to die from unintentional injury. Further, Drs. Kim and Drake found in 2019 that one in four child victims of confirmed maltreatment were revictimized. Foster care may not be ideal, but nor are the alternatives. In this episode, Patricia speaks with Wendy Carpenter, Chief Executive Officer of Penny Lane Centers. Founded in 1969, Penny Lane today offers preventative services, Substance Abuse Treatment, residential care, transitional and permanent housing for former foster youth, foster care and adoption services as well as outpatient mental health care. Wendy began her career at Penny Lane as a clinician, finding a professional home where she has served since then. Recent leaps in trauma research have transformed the fields of addiction treatment and mental health. Awareness of the neurobiological impacts of trauma and violence has shifted our understanding of how to work effectively with individuals in addiction recovery, and innovative trauma-informed practices are now being implemented in communities globally. Unfortunately, these updated, evidence informed models are not making their way into medical settings as quickly, leaving trauma survivors at risk of being retraumatized due to the lack of training in trauma-informed care for most healthcare professionals. Today's episode focuses on best practices in addiction care from a trauma informed lens. Dr. Valerie Kading is the CEO of Sierra Tucson, where she provides the strategic vision to ensure that the facility's patients and their families receive the most innovative, compassionate, and leading medical and psychiatric treatment for trauma, addiction, and other mental health conditions from a world-class staff of clinicians and behavioral health professionals. Dr. Kading joined Sierra Tucson in 2015, where she rose to become chief medical operations officer, then interim chief medical officer, chief operations officer, and ultimately Group Chief Executive Officer for the entire 258-bed Sierra Tucson Group – Sierra Tucson in Arizona, along with Sierra Southlake in Southlake, Texas and Sierra Scottsdale in Scottsdale, Arizona. Dr. Kading brings to Sierra Tucson a special interest in women's mental health including perinatal mood disorders, maternal well-being, and postpartum depression. In addition, she has deep expertise in addressing the trauma of Adverse Childhood Experiences (ACES) that can lead to substance use and other issues in adulthood. Dr. Kading has written articles for multiple professional journals. She has led a variety of webinars and CE events on topics ranging from adverse childhood experiences to compassion fatigue and provider resilience for organizations including: Recovery Plus Conference, London, United Kingdom; Southern Arizona Advanced Practice/Nurse Practitioner's Primary Care Conferences; Tucson Postpartum Coalition; C4 Conferences; Philippine Nurses Association; and the University of Arizona. Dr. Kading graduated with honors from New Mexico State University at Las Cruces, New Mexico, with a Doctor of Nursing Practice degree. Her doctoral studies focused on optimizing health care systems, translational methods, and evidence-based practice, health care organization innovation, epigenetics, utilization of informatics in healthcare, and interdisciplinary collaboration. Her dissertation focused on increasing health care provider knowledge on perinatal depression, and she created a web-based educational program for health care providers aimed at facilitating evidence-based treatment to perinatal women. Dr. Kading graduated with honors from the University of Arizona with a master's degree in nursing, a Bachelor of Science in Nursing, and a bachelor's degree in psychology. Dr. Kading also received her Master of Business Administration degree from the University of Arizona. How long do New Year's resolutios last? Most people quit soon after many of them make the resolution, but on January 1, don't keep them into February. New Year's  resolutions are the very definition of best-laid plans. Most of us ring in the new year with ambitious intentions to lose weight, exercise more, or quit smoking, but our freshly minted goals are likely to fall apart even before February. That's according to 2018 data from Strava, a social network for athletes. The company analyzed more than 31.5 million fitness records from its users and found that the second Friday in January is the fateful day when most of our annual commitments starts to crumble. So, for 2020, that's January 10. Sticking to resolutions is hard and we all know there's a lot of talk and pressure in January about getting fitter and being healthier. Granted, Strava's research only draws from its own user base, so it's slightly skewed, but an oftcited 1988 study from the Journal of Substance Abuse Treatment produced similar findings: only 55 percent of participants stuck to their resoluitions for a full month. To make a resolution and stick with it: 1. Set a concrete goal that you truly believe you can achieve; 2. Don't fret the occasional slip-up; 3. Reward yourself; 4. Change your surroundings; and 5. Use the buddy system.

Today on Burning Issues Dr. Mitch Earleywine is joined by Marcel Bonn-Miller. Dr. Marcel O. Bonn-Miller received his B.A. in psychology and Ph.D. in clinical psychology from the University of Vermont. He completed a pre-doctoral clinical psychology internship at the Palo Alto Veterans Affairs Health Care System and a postdoctoral fellowship at the Center for Health Care Evaluation at the Veterans Affairs Palo Alto Health Care System & Department of Psychiatry and Behavioral Sciences at Stanford University School of Medicine. After completing his VA Clinical Science Research and Development (CSR&D) Career Development Award (CDA-2), he transitioned into his position as Research Health Science Specialist at the VA Center of Excellence in Substance Abuse Treatment and Education (CESATE), National Center for PTSD (NCPTSD), and Center for Innovation to Implementation (Ci2i). Dr. Bonn-Miller's research interests are broadly rooted in the examination of associations between substance use and anxiety symptoms and disorders. His current work involves the examination of the interrelations between cannabis use and its disorders and posttraumatic stress disorder (PTSD), including moderating and mediating processes underlying their associations.
