Research suggests that LGBTQ adults have higher rates of substance use and mental health issues, compared to their heterosexual cisgender peers.  These health disparities are often related to minority stress—the chronic psychological and emotional strain that people experience due to being part of a marginalized social group. For example, some LGBTQ folks may use alcohol and drugs to cope with internalized homophobia or to self-medicate for anxiety and depression. On the other hand, some people use drugs in the context of social settings and connecting with others in their community. Since not all substance use is unhealthy, how do providers know how to identify when substance use is problematic?  And what can physicians do to best support their patients who may be at greater risk for harm or even overdose?    On this episode, Antonio Urbina, MD, Medical Director for CEI's HIV Primary Care and Prevention Center of Excellence, speaks with Eric Kutscher, MD, MSc. Dr. Eric Kutscher is an Assistant Professor of Medicine at the Icahn School of Medicine, where he practices as a primary care and addiction medicine physician at Mount Sinai's Internal Medicine Associates and REACH Program. He currently serves as the Medical Director of the Mount Sinai Hospital Opioid Overdose Prevention Program and Primary Care Lead for the Mount Sinai Center for Transgender Medicine and Surgery. His clinical work and research focus on harm reduction techniques for patients with substance use disorders, particularly within the LGBTQ+ community. Drs. In der Postkutsche geniest er trotzdem die schöne Landschaft, auch wenn seine Mitreisenden einige komische Worte aussprachen, die nichts Gutes verhiessen. Am Treffpunkt angekommen ist die erwartete Kutsche nicht da, was den Mitreisenden Hoffnung macht. Zu früh, da kommt rasch eine schwarze Kutsche mit einem unheimlichen Kutscher und nimmt Jonathan auf. Es war eine kalte, neblige Nacht im Winter 1803, als ein Kutscher seine Pferde im Laternenlicht durch die engen Landstraßen von Hammersmith in England führte. Zu dieser Zeit war der Londoner Stadtteil nur ein Dorf am Rande der Stadt, das in letzter Zeit von seltsamen paranormalen Problemen geplagt worden war. Die Uhr schlug eins, als die Kutsche gerade an der hohen Turmspitze von der St. Pauls Kirche vorbeifuhr. Die Kirche war in Nebel gehüllt und zeichnete sich als Silhouette im Mondlicht ab. In diesem Moment brachte der Kutscher die Pferde plötzlich zum Stehen. Die Pferde machten einen vorsichtigen Schritt zurück. Etwas befand sich mitten auf der Straße. They've had clients including Gilette, Roman, Shake Shack and Smart Water.   Before founding his own agency Ryan was a creative director at Crispin Porter Bogusky where he worked on a lot of influential work including the Whopper Freakout campaign. Ryan also worked as Chief Creative Officer at J. Walter Thompson.  In this episode we talk a lot about starting an ad agency. Why Ryan wanted his own shop, how he found his first clients, his experience pitching for business, developing a company culture, creating an in-house production arm and so much more about the process of running a small agency. The Mountains, the Catskills, the Borscht Belt, upstate, the country. Monticello, Liberty, Woodridge, Swan Lake, Woodbourne, Fallsburg. A place of many names with one thing in common: a place rich in Jewish history and lore. As a summer destination, the Catskills would be home to countless bungalow colonies as city Jews organized their communities for a mountain air getaway. Generations of children attended the many summer camps that dotted Sullivan County. Camps included Mesivta, Agudah, Munk, HILI, Kol Rinah, Torah Vodaath, Ohr Shraga, Sternberg and many more including the Betar Jewish self defense camp where Vladimir Jabotinsky died in 1940. It was also famously known as the Borscht Belt, and the legendary hotels/resorts/country clubs like Grossinger's, Kutscher's and the Concord made their mark as vacation sites. Most Jewish comedians of the time commenced their careers there. In addition to the summer crowd, the Catskills were home to many year round Jewish communities over the years. These towns built shuls, mikvas, hired Rabbis and even Jewish owned farms burgeoned for a time.
