Ambulatory Surgical Centers (ASCs) have been around in concept for the past fifty years, but their recent explosion has caught the attention of healthcare systems and, frankly, patients. Why? Today's guest, Adnan Qureshi, is a Managing Director with the Mergers and Acquisitions practice at Kaufman Hall. He provides strategic advisory services for healthcare providers and investors around the merger or acquisition of ASCs. The benefit he's seen in partnership with his clients perhaps explains the answer to this question. The “DNA”, as Adnan puts it, of the ASC is rooted in independent physicians who, as an extension of their practice, saw the benefit of doing lower acuity surgeries in an outpatient setting. As pain management and technology improved over time, the use case also evolved to the point where there are now few specialty areas where uncomplicated surgeries cannot be performed in an ASC. Without the overhead and operating costs of a hospital, ASCs allow for far more transparent pricing, lower costs, greater efficiency, and often better outcomes, all driving towards higher patient satisfaction. And that's a win we should all be paying attention to. Adnan Qureshi has over fifteen years of healthcare transaction experience. Prior to joining Kaufman Hall, he was a Director of Development at SCA Health, a subsidiary of Optum/UnitedHealth Group. In that role, Mr. Qureshi led market entry strategy across several geographies, and sourced, structured, and executed ambulatory surgery center acquisitions.

Garrett Klassy has been the AD at Fresno State for almost 2 years and is leading the Bulldogs into the Pac-12 this summer. Expect to hear a lot in this interview, beginning with the evolving profile of the FBS AD position. I asked Garrett if he thinks FBS ADs will always be the key decision-maker for head coaching hires throughout the department or if that will maybe fall off to a deputy AD in the future as the job becomes less day-to-day involvement and more of a big picture role. Klassy is also one of several former ADs that left the chair for a P4 deputy AD job that eventually launched him back into the AD chair at a different level. Has this trend become the norm? Seventy percent of the protein your herd uses for lactation is derived from the microbial population in her rumen. Researchers from UC Davis and Feedworks USA sought to learn more about how different substrates might impact rumen microbial efficiency. They did this with the ultimate goal of increasing the flow of protein available for absorption to help offset feed costs, recognizing protein is the highest cost of the ration, and likely the largest fraction to the cost of production on most dairies. “We know that efficency of growth in the rumen varies dramatically, nearly 2 fold. Microbes can use 1/3 of their energy for growth or as much as 2/3,” Hackmann described. His lab is using invitro cultures to attempt to determine why this variation exists. “If we can pinpoint cause then we can accomidate and make microbes grow more efficiently and deliver more protein to the ruminant at a lower cost.” Past models, used in ration formulation software such as CNCPS, claim rumen bacteria perform digestion more efficiently when fed cellulose, over glucose. However, featured work by Dr. Tim Hackmann's invitro lab suggests a different result. “We found there will be a larger mass of microbes that grow on glucose then cellulose but they also digest more, so the efficiency is not differnet.” Listen-in to this episode for in interesting dive into the expected changes in the rumen under these conditions.
