Episode 224: Community Health Workers Dr. Arreaza: Today we will discuss a topic that, frankly, every single person listening, whether you're a medical student, a resident, a nurse, a family doctor, or any primary care provider, needs to really understand. We're talking about community health workers (CHWs). We are joined by our stellar medical student; you may be familiar with her voice from previous episodes about insomnia. Moira, welcome, please introduce yourself. Moira: I want to be upfront about why Community Health Workers matter to you specifically. If you've ever felt frustrated that your patient with uncontrolled diabetes keeps missing appointments because they can't get a ride, or that your heart failure patient was readmitted because nobody checked whether they could afford their medications, then you already understand the problem that CHWs are designed to solve. Dr. Arreaza: We're going to give you the definition of a CHW, the evidence behind their effectiveness, how they fit into your care team, the return on investment, and practical steps for integrating them into your practice. We have pulled information from a lot of peer-reviewed sources, and we want to share them with you. So, Moira, let's start with the basics. What exactly is a community health worker? Moira: Great question, and it's one that even literature struggles with, because there are so many titles for this role. Community Health Worker is an umbrella term that encompasses more than 20 different titles including outreach workers, promotores or promotoras de salud, community health representatives, lay health workers, peer educators, patient navigators, and many more. The American Public Health Association defines CHWs as frontline public health workers who are trusted members of or have an unusually close understanding of the communities they serve. Arreaza: And that trust is so important in health care. CHWs are not physicians. They are not nurses. They do not diagnose or prescribe. But they are like a bridge connecting the medical environment, social services, and the community to reduce gaps in healthcare delivery. Moira: Exactly. In this episode, Therese Markow and Dr. Daniel Buchman discuss chronic pain, affecting over 20% of the population in the US, Canada, and Europe. Chronic pain (persisting pain lasting over three months) includes conditions like arthritis, lower back pain, fibromyalgia, and phantom limb pain, among many others. Daniel highlights the challenges in pain assessment, the role of AI in pain management, and the ethical considerations, including bias in pain reporting. He notes that chronic pain increases suicide risk, and the conversation also touches on the stigma surrounding pain and opioid use. Key Takeaways: Pain can come from an identifiable condition, but sometimes doesn't have a clear cause. Primary pain is the type of pain that is not explained by another disease. Secondary pain is pain that arises because of or is related to an underlying condition - perhaps injury, illness, disease, or other reason. As AI is used more in medicine, we do need to be aware of bias in the data sets that the AI is utilizing to learn and then use to make predictions. Pain is a warning sign. "There isn't a blood test or a brain scan or anything that can determine how much pain somebody's in, or what it's like to experience that pain. This is subjective because it depends on the person, and it depends on a whole host of factors, not just their biology. Accurately defining the population of patients with short bowel syndrome (SBS) and intestinal failure has long been a challenge in gastroenterology. Ehab Bandar is a sought-after product design consultant for startups and fintech. What's amazing about his career is that he's managed to do it while being both an outsider and an insider in remarkably distinct ways. Ehab attributes credit to his early life as an immigrant to the US from Lebanon at the age of six. His family moved yearly until he was in seventh grade. Ehab was a shy kid with a stutter, yet he was also a natural observer and listener, taking in different cultural norms and personalities. He recalls endearing himself to fit in with new people by throwing a joke into random conversations, noting how fortunate he was to be warmly received as he started the year at each new school. Ehab describes these characteristics as shy self-reliance while being quietly engaged. Ehab had to invent his own career to become a design leader himself. Educated as a city planner, Ehab started his career as one of the youngest technology managers at Wells Fargo. He then left corporate and went on his own to advise and lead design at fast-growing startups in Silicon Valley, digital agencies, and corporate giants. Ehab uses city planning tools and curiosity to hone his craft in the tech world. He explains how a quarterly print newsletter he started in grad school out of boredom led him to work in the dot-com boom and how being an outsider as an immigrant from Lebanon made the act of observation and discovery a daily habit. Today, he's the founder of , an experience-led design agency that partners with product teams to build human-centered designs at scale. (Original Airdate: January 12th, 2018) Jurassic Park III is a 2001 American science fiction action film[3] directed by Joe Johnston and written by Peter Buchman, Alexander Payne and Jim Taylor. It is the third installment in the Jurassic Park franchise and the final film in the original Jurassic Park trilogy, following The Lost World: Jurassic Park (1997). It is also the first film in the franchise not to be directed by Steven Spielberg, as well as the first not to be based on a novel by Michael Crichton; however, the film features characters and ideas by Crichton. Sam Neill and Laura Dern reprise their roles from the first film. New cast members include William H. Macy, Téa Leoni, Alessandro Nivola, Trevor Morgan and Michael Jeter. The plot follows a divorced couple who deceive paleontologist Dr. Alan Grant into helping them find their son, who has gone missing on Isla Sorna. After the release of Spielberg's Jurassic Park, Joe Johnston expressed interest in directing a sequel, which Spielberg agreed to. Universal Pictures announced the third film in June 1998, with a release scheduled for mid-2000. A draft written by Buchman was rejected five weeks ahead of filming in favor of a simpler story idea suggested by David Koepp, the writer of the previous two films. Payne and Taylor were hired to rewrite the earlier script by Buchman, who made further revisions to their draft. Filming lasted five months, beginning in Hawaii in August 2000 before moving to California. A final draft of the script was never completed during production, a circumstance Johnston considered quitting over. Jurassic Park III features a combination of computer-generated and animatronic dinosaurs. A Spinosaurus replaces the Tyrannosaurus from previous films as the principle dinosaur antagonist. Jurassic Park III premiered on July 16, 2001, and was theatrically released on July 18. Despite receiving mixed reviews from critics, the film was successful at the box office, grossing $368 million worldwide to become the eighth highest-grossing film of 2001; however, it is the lowest-grossing installment in the franchise to date. Damian Buchman has defied one in a million odds not once but twice in his life. Diagnosed with a rare bone cancer at 13 years old and having endured 28 knee surgeries since, he embodies what it means to have grit and tenacity. She is a recipient of the Toby Devan Lewis Fellowship, and her work has been featured in Hyperallergic and The Hopper Prize Journal. As an extension of her practice, she runs an independent artists' books and publications project, Seaton Street Press, to collaborate with artists through publishing and distribution. Seaton St. Press is an artist-run, independent artists' books and publications project. Committed to exploring temporality, its name is a dedication to a former studio in Los Angeles, now based in Philadelphia. Rex Buchman is a christian horseman, Clinician & Rancher - "Staying Safe in the Saddle on the Trail."A third generation rancher from the Flint Hills of Kansas. A Christian, father to five, who with wife Teresa, works raising ranching heritage horses, registered red angus cattle and hosting dude ranch adventures. He created the “Trail of Billy's Last Ride”, a historic 170 mile trail ride tin New Mexico that lasted for 10+ years. Rex served on the board of the Ultimate Horsemen's Challenge Association and competed in that association for five years. He has shown in the Midwest Ranch Horse Association. Rex has attended, hosted, and even taught a few colt starting and horsemanship clinics. Rex has made a living starting colts for five or six years after returning to Kansas from 11.5 years as a county agent in New Mexico. He has coached 4H horse judging teams and colt futurities while in New Mexico. Rex's purpose at EquiFest is to offer people some help with their horses, to increase their level of enjoyment and safety as they live out their horsemanship journey.

Cosmos contributors recently released a new whitepaper that proposes major upgrades to the Cosmos Hub, the central blockchain of the Cosmos ecosystem. A community vote to determine whether or not the proposed changes are integrated into the Hub concludes on Monday, Nov. 14th. In this episode of The Scoop, Cosmos Co-Founder and Interchain Foundation President Ethan Buchman explains what will change if Atom 2.0 passes, as well as how these changes fit into his overall vision for how Cosmos's app-chain architecture will continue to evolve. According to Buchman, key to Atom 2.0 is the introduction of a security model which will allow new application-specific blockchains to bootstrap security by utilizing the Cosmos Hub's existing validator set: “The kind of change that's coming to the Hub specifically will allow new application-specific blockchains to launch that use the same validator set as the Cosmos Hub and use the same stake that already exists on the Cosmos Hub.” Not only will the proposed changes to Cosmos Hub make it easier and more secure for projects to launch their own chains, but Buchman anticipates it will increase the amount of development activity on the Hub itself: “This for the first time, opens the door to a sort of more active development environment and development velocity in and around the Cosmos Hub, whereas in the past, the Cosmos Hub was very slow to evolve.” Episode 111 of Season 4 of The Scoop was recorded live with The Block's Frank Chaparro and Cosmos Co-Founder and CEO of Informal Systems Ethan Buchman. Today we have two young superstars changing the sport of show skiing. Derek and Carter Buchman are brothers, star skiers, and true ambassadors. They are the architects behind the Buck Up Freestyle Jump Tour that is bringing freestyle jump back to the forefront. Derek and Carter grew up in Chetek on the Chetek Hydroflights, they've skied pro, and this season was one for the ages for them. Carter skied for Chetek and competed in every Buck Up freestyle event. Derek skied for Mad-City, was a standout in their show, and took 1st place in freestyle at Wisconsin State, D1 Nationals, and won the Buck Up season points title.
