Greetings Glocal Citizens! This week we commemorate Africa Day across the African Diaspora. Reflecting on the journeys of liberation that Black people have endured in trial and triumph, I am ever reminded that those that laid the groundwork will never in my heart, mind or acts have done so in vain. As the pillars of imperialism are finally weakening--though, not without reverting to its tried and tested battle techniques--the new next is taking shape in all manner of action and practice across the globe. So today, as I do in some way every day, I salute us all for staying the course and redirecting the path as necessary on the #PanAfricanProgress mission that is imparative to human evolution and a sustainable existence here on earth. Speaking of existence here on earth and the ways that we inhabit it, I came across my guest this week via a connection that I made last year at the Charter Cities Summit in Nairobi. Remi Adeleke, a former U.S. Navy SEAL and entrepreneur, serves as Chairman and Co-Founder of the Small Farm Cities Africa Resilience Fund, a for-profit venture developing sustainable, agricultural communities to combat poverty and human trafficking. He is actively scaling this model across Africa, including projects in Malawi, Zambia, and other regions, focusing on economic empowerment through infrastructure and land ownership for local farmers. Following a successful career as a Navy SEAL, in 2016 he found himself pursuing a careers in writing, speaking, and acting, including the 2017 franchise film Transformers: The Last Knight, SEAL Team CBS, and the 2022 Universal thriller Ambulance. He served as a consultant on Netflix's 6 Underground and Apple+ TV series Invasion. As an educator he co-founded Muskegon Michigan's OPS Academy which opened to inner city students K-8th grade in Fall of 2022. Episode 224: Community Health Workers Dr. Arreaza: Today we will discuss a topic that, frankly, every single person listening, whether you're a medical student, a resident, a nurse, a family doctor, or any primary care provider, needs to really understand. We're talking about community health workers (CHWs). We are joined by our stellar medical student; you may be familiar with her voice from previous episodes about insomnia. Moira, welcome, please introduce yourself.  Moira: I want to be upfront about why Community Health Workers matter to you specifically. If you've ever felt frustrated that your patient with uncontrolled diabetes keeps missing appointments because they can't get a ride, or that your heart failure patient was readmitted because nobody checked whether they could afford their medications, then you already understand the problem that CHWs are designed to solve. Dr. Arreaza: We're going to give you the definition of a CHW, the evidence behind their effectiveness, how they fit into your care team, the return on investment, and practical steps for integrating them into your practice. We have pulled information from a lot of peer-reviewed sources, and we want to share them with you. So, Moira, let's start with the basics. What exactly is a community health worker? Moira: Great question, and it's one that even literature struggles with, because there are so many titles for this role. Community Health Worker is an umbrella term that encompasses more than 20 different titles including outreach workers, promotores or promotoras de salud, community health representatives, lay health workers, peer educators, patient navigators, and many more. The American Public Health Association defines CHWs as frontline public health workers who are trusted members of or have an unusually close understanding of the communities they serve. Arreaza: And that trust is so important in health care. CHWs are not physicians. They are not nurses. They do not diagnose or prescribe. But they are like a bridge connecting the medical environment, social services, and the community to reduce gaps in healthcare delivery.  Moira: Exactly. Anny Jenkins, MSN, RN, grew up in a small, rural town in West Virginia, the first nurse in her family. Her father, a fireman who everyone in their community knew by name, showed her what it looked like to drop everything and help people. That instinct led her to nursing, though the road was long and nonlinear. She began as an LPN, earned her associate degree from Blue Ridge Community and Technical College, then returned to school during the height of COVID in 2022 to complete her BSN at Western Governors University, where she was honored as a commencement speaker for her own graduating class. She earned her MSN in 2024 and is now pursuing her doctorate, with a clear purpose: to bring advanced nursing education back to the rural communities that need it most.  She believes that you can't be what you can't see and strives to set an example for nurses to achieve more than they might think they can. This hit home for her when, on the verge of leaving nursing altogether, Anny attended a Washington, D.C. conference hosted by the Black Nurse Collaborative and found herself in a room with 250 Black nurses and advanced-degree holders. It was the first time she had ever experienced that and it was a lightbulb moment. Today, she serves as Executive Secretary of the Black Nurse Collaborative, chairs the WV Nurses Association Membership Committee, and travels the Eastern Panhandle recruiting the next generation of nurses. Desmond Patton is the 31st PIK University Professor at the University of Pennsylvania, with joint appointments in the School of Social Policy & Practice and the Annenberg School for Communication, where he is the Waldo E Johnson Jr. Professor of Communication. He also holds secondary appointments in the Department of Psychiatry at Children's Hospital of Philadelphia & Perelman School of Medicine. He is founding director of SAFElab, founding faculty director of the Penn Center for Inclusive Innovation & Technology, and Chief Strategy Officer for the School of Social Policy & Practice.Professor Patton's groundbreaking research examines the relationship between social media and gun violence, grief, and loss, focusing on how online communities influence offline behavior. His work has made him the most cited and widely recognized scholar in this critical area of social science. Early research focused on detecting trauma and preventing violence on social media has evolved into broader investigations of language analysis and algorithmic bias in artificial intelligence. He currently serves as a member of Spotify's Safety Advisory Council, the Ethics and Equity Advisory Council (EEAC) at Axon, TikTok's U.S. Content Advisory Council, and is a trusted advisor to several AI startups.As a social work scientist, Patton identified that traditional data science methods often fail to capture the cultural and linguistic nuances of predominantly Black and Hispanic youth. In response, he developed the Contextual Analysis of Social Media (CASM) framework, which integrates culture, context, and inclusion into machine learning and computer vision analysis.
