Episode 224: Community Health Workers Dr. Arreaza: Today we will discuss a topic that, frankly, every single person listening, whether you're a medical student, a resident, a nurse, a family doctor, or any primary care provider, needs to really understand. We're talking about community health workers (CHWs). We are joined by our stellar medical student; you may be familiar with her voice from previous episodes about insomnia. Moira, welcome, please introduce yourself.  Moira: I want to be upfront about why Community Health Workers matter to you specifically. If you've ever felt frustrated that your patient with uncontrolled diabetes keeps missing appointments because they can't get a ride, or that your heart failure patient was readmitted because nobody checked whether they could afford their medications, then you already understand the problem that CHWs are designed to solve. Dr. Arreaza: We're going to give you the definition of a CHW, the evidence behind their effectiveness, how they fit into your care team, the return on investment, and practical steps for integrating them into your practice. We have pulled information from a lot of peer-reviewed sources, and we want to share them with you. So, Moira, let's start with the basics. What exactly is a community health worker? Moira: Great question, and it's one that even literature struggles with, because there are so many titles for this role. Community Health Worker is an umbrella term that encompasses more than 20 different titles including outreach workers, promotores or promotoras de salud, community health representatives, lay health workers, peer educators, patient navigators, and many more. The American Public Health Association defines CHWs as frontline public health workers who are trusted members of or have an unusually close understanding of the communities they serve. Arreaza: And that trust is so important in health care. CHWs are not physicians. They are not nurses. They do not diagnose or prescribe. But they are like a bridge connecting the medical environment, social services, and the community to reduce gaps in healthcare delivery.  Moira: Exactly. Ciao RAGAZZI liebe Radsportfreunde, MOININGER und ein herzliches Willkommen zum (hoffentlich täglichen) Stiefel-Update zum Giro d'Italia 2026 der JAN ULLRICH ULTRAS.Kurz und knapp für zwischendurch.Also rein in die Pedale und ab über den Brenner doooooo!***Stiefel-Update 5.0 - Regen in Italien***Die 5. Etappe des Giro d'Italia 2026 war eine Chaos-Etappe dieser Rundfahrt - und gleichzeitig eine der spektakulärsten der bisherigen Saison. Über 203 Kilometer ging es von Praia a Mare nach Potenza, mitten durch das bergige Inland Süditaliens. Mit rund 4.100 Höhenmetern, Regen und einem extrem schwierigen Finale war schon vor dem Start klar, dass diesmal nicht die Sprinter um den Sieg fahren würden. Schon früh setzte sich eine große Ausreißergruppe ab. Das Hauptfeld ließ die Gruppe lange gewähren, weil viele Teams ihre Kräfte sparen wollten und die Gesamtfavoriten keinen offenen Schlagabtausch planten. Dadurch wuchs der Vorsprung der Fluchtgruppe auf mehrere Minuten an. Besonders aktiv präsentierten sich dabei der Spanier Igor Arrieta vom Team UAE Emirates-XRG und der Portugiese Afonso Eulálio von Bahrain Victorious.Die Etappe wurde vor allem durch das schlechte Wetter geprägt. Immer wieder gab es Stürze auf den nassen Straßen, viele Fahrer hatten Probleme in den Abfahrten und selbst erfahrene Profis wirkten am Ende komplett erschöpft. Sowohl Arrieta als auch Eulálio gingen im Verlauf des Rennens zu Boden, kämpften sich aber jedes Mal wieder zurück. Der entscheidende Abschnitt begann am schweren Anstieg zur Montagna Grande di Viggiano – dem härtesten Berg der ersten Giro-Woche mit Rampen bis zu 15 Prozent. Dort zerfiel die Spitzengruppe endgültig. Arrieta attackierte mehr als 60 Kilometer vor dem Ziel und fuhr zunächst allein an der Spitze. Später konnte Eulálio zu ihm aufschließen. Gemeinsam bauten die beiden ihren Vorsprung auf das Feld immer weiter aus. Das Finale in Potenza war dann kaum zu glauben. Zunächst stürzte Eulálio rund sieben Kilometer vor dem Ziel, konnte aber weiterfahren. Kurz danach verlor wiederum Arrieta in einer Kurve beinahe die Orientierung und bog auf den letzten zwei Kilometern sogar kurz falsch ab. Dadurch entstand eine kleine Lücke, und es sah bereits so aus, als würde Eulálio die Etappe gewinnen. Doch Arrieta mobilisierte seine letzten Kräfte, schloss die Lücke auf den letzten Metern tatsächlich noch einmal und zog im Sprint kurz vor der Ziellinie vorbei. Für den 23-jährigen Arrieta war es der erste Etappensieg bei einer Grand Tour und gleichzeitig der erste spanische Tagessieg bei diesem Giro. Nach dem Ziel war er völlig entkräftet und sichtlich emotional. Viele Zuschauer und Fans bezeichneten die Schlussphase sofort als eines der verrücktesten Giro-Finals der letzten Jahre. Trotz der Niederlage im Sprint hatte Eulálio am Ende ebenfalls Grund zum Feiern: Durch den großen Vorsprung auf das Hauptfeld übernahm er das Rosa Trikot des Gesamtführenden. Der bisherige Leader Giulio Ciccone verlor über sieben Minuten und fiel in der Gesamtwertung deutlich zurück. La quinta etapa del Giro de Italia 2026, que unió Praia a Mare y Potenza a lo largo de 203 kilómetros, quedará grabada como una de las jornadas más caóticas y surrealistas de los últimos años debido a la intensa lluvia y los constantes incidentes. El joven navarro Igor Arrieta (UAE Team Emirates) se alzó con una victoria heroica tras sobrevivir a una caída a 13 kilómetros de meta y a un error de trazada a falta de solo dos kilómetros, donde se equivocó de desvío y tuvo que rectificar frenéticamente. Pese a perder su ventaja, Arrieta logró dar caza y superar en los últimos 100 metros al portugués Afonso Eulálio (Bahrain Victorious), quien también se había ido al suelo poco antes; aunque Eulálio no pudo llevarse el triunfo parcial, encontró consuelo al enfundarse la Maglia Rosa de nuevo líder tras un hundimiento generalizado del pelotón de favoritos, que llegó a más de siete minutos de distancia.

This episode features Dr. Marie-Claire Arrieta PhD from the University of Calgary (Canada), speaking about development of the early life gut microbiome, both in preterm and non-preterm infants. Across the field, it has been established that the early days and months of an infant's life are very determinant of immune system development as well as chronic disease later in life. In this period, environmental cues are important, with some of these cues coming from the gut microbiome – both bacteria and fungi. Preterm infants show a very different gut microbiome than non-preterm infants. Ample evidence shows probiotics given to preterm infants can bring clinical benefits such as a reduced risk of necrotizing enterocolitis, but this is separate from investigations into the infants' gut microbiomes. Dr. Arrieta's work has shown that probiotics can guide the gut ecosystem of preterm infants toward approximating the non-preterm gut microbiome. One gap in the research is to know more about the effects of specific strains; their work found that although bifidobacteria were more effective at colonizing in the gut, lactobacilli drove some aspects of microbiota maturation. Dr. Arrieta speculates that the case for probiotic use for preterm infants will become stronger as trials increasingly focus on health outcomes not just during the neonatal intensive care unit stay, but also later in life. Overall in healthy infants, different patterns of gut microbiome and immune development can lead to the appearance of diseases later in life. The latest insight is that disease is linked not to specific microbes or metabolites, but to the pace of gut microbiome development. Misalignment of gut microbiome development (too early or too late) with stages of immune development is associated with later emergence of allergic disease. Several factors such as C-section birth and antibiotics may contribute to this misalignment, but breastfeeding seems to mitigate it. Dr. Arrieta has an ongoing longitudinal study on the early life microbiota and disease associations in preterm and non-preterm infants that is likely to reveal more details. De acuerdo con los Centros de Control y Prevención de Enfermedades, los casos de sífilis congénita han aumentado más de tres veces en los últimos años, con más de 3,700 casos notificados sólo en el 2022. Esta es la cantidad más alta notificada en un año desde 1994. Nuestro invitado del día de hoy, el Dr. Antonio Arrieta, nos compartirá por qué es importante hablar sobre sífilis congénita, cómo se presenta y cómo se diagnostica, entre muchos otros aspectos de esta enfermedad.
