In the not so distant future, the sun is dying and the best scientists from around the globe are gathered along with their families, to inhabit a research station on the far side of the moon: Helios 1. Desperate to find a solution, lead research scientist Neil Collins is tasked with overseeing the facility and its projects. As an eclipse approaches, an anomaly disrupts the entire facility. In the nursery where Neil and his wife admire their newborn baby girl, another newborn exhibits terrifying behaviour. Concerned and determined to understand this child named Nero, Neil isolates the boy, thus beginning a life of experiments and loneliness for Nero. Reluctantly, Neil allows his young daughter, Selene, to become friends and connect with Nero. Their bond grows through the years and Selene becomes his only connection to the outside world. A love grows between them while her father seeks answers to Nero's condition, all while keeping his existence secret from Earth. Many years later, Helios 1 is forced to shut down and its inhabitants are required to return to Earth. Yet not all its inhabitants return. When media reports suggest that sunscreens “fail” SPF testing, GPs are often the first to field patients' concerns. In this episode of Life by a Thousand Cuts, A/Prof Tony Dicker takes a closer look at recent media controversies and applies a clinical lens to discuss whether sunscreens work and are safe to use. In the episode, he discusses SPF testing and how it is not absolute; small variations in testing conditions or sample batches can change results, even among regulated products. Despite this, Australia's regulatory standards remain among the most stringent globally; unlike in many other countries, sunscreens are classed as therapeutic goods and regulated by the TGA. A “failed” SPF test result doesn't necessarily mean a product is unsafe or ineffective. It often reflects marginal variation rather than complete loss of protection. Affected manufacturers have acted responsibly by voluntarily withdrawing products to investigate further, which is an important sign of quality control, not negligence. A/Prof Dicker reiterates that GPs play a vital role in patient reassurance and countering misinformation, since sunscreen remains a key element of skin cancer prevention, alongside other sun safety measures like shade and clothing. “Some sun protection is better than none. In this episode of Life by A Thousand Cuts podcast, A/Prof Tony Dicker sits down with Dr Ian Katz, a dual-trained pathologist and founder of several skin cancer clinics, to explore one of the most debated topics in skin cancer management: margins. Dr Katz shares his unique perspective as both a pathologist and a clinician, reflecting on why confusion around margins persists, and how GPs can better interpret pathology reports in practice. They discuss: Clinical vs histological margins: Guidelines are based on clinical margins, not what's measured under the microscope. A “1.5mm histological clearance” isn't the same thing as a “5mm clinical margin.” Measurement variability: Pathology reports aren't exact science. A “2mm margin” often means “2mm ± 1–2mm.” Numbers give guidance, but they're not absolute. What really matters: In many cases, being histologically one cell clear is enough. The reason we aim wider is due to uncertainty, because we only ever see a small fraction of the specimen under the microscope. Managing uncertainty: Skin cancer surgery comes with ambiguity. If you are someone who struggles to sleep without a precise answer, skin cancer work can feel uncomfortable. Understanding the limits of pathology helps. Communication counts: Clear request forms (with the prior diagnosis, margin status, and level of clinical suspicion) make it easier for pathologists to provide meaningful reports. This discussion is about bridging the gap between the clinical and pathology worlds, and recognising that margins are less about chasing numbers, and more about good clinical judgement, clear reporting, and thoughtful follow-up. While most skin cancers don't present neatly in a diamond shape, traditional rhomboid flaps are based on excising a diamond-shaped defect, which can sometimes remove more tissue than necessary. In this week's episode, A/Prof Dicker explores both the classic rhomboid flap and the modified rhomboid approach, which allows for effective closure of more natural, circular tumour excisions. He explores the geometry behind traditional rhomboid flaps (including 120- and 60-degree angles), why the modified rhomboid is often better suited for circular defects, how to design and execute a flap that aligns with natural skin tension lines (thus improving both functional and cosmetic outcomes), and a step-by-step walkthrough of a rhomboid flap on the cheek, demonstrating how tissue is rotated into place for optimal closure. In this episode of Capital for Good we speak with Maria Ressa, the globally celebrated free speech champion, journalist, entrepreneur, dissident, and winner of the 2021 Nobel Peace Prize for her work “to safeguard freedom of expression, which is a precondition for democracy and lasting peace.” Ressa is a co-founder of Rappler, one of the most influential media platforms in the Philippines. For her reporting on the administration of President Rodrigo Duterte, Ressa was threatened, arrested, tried, and convicted of cyberlibel, facing over one hundred years imprisonment. Today Duterte has been arrested by the International Criminal Court and awaits trial in the Hague, Ressa has been cleared of nearly all charges, and her work as a journalist and activist continues as she warns of the very real world challenges of online disinformation. We begin with Ressa's earliest days in the United States, when her family immigrated in 1973 after martial law had been declared in the Philippines. We discuss the importance of her education in those years, in elementary, high school, and at Princeton, and the support of those who “taught her to keep learning,” lessons that would inform her pursuit of journalism when she returned to the Philippines. “I fell into journalism,” Ressa says, as she found it to be critical “connective tissue between government and the people,” and a way to “hold power to account.” She and three fellow journalists launched Rappler in 2012; by 2016, when Duterte was elected President, Ressa found herself persecuted by the government — threatened, arrested, tried and sentenced to over one hundred years in prison — for reporting on its corrupt and increasingly authoritarian practices. We discuss Ressa's fight for her rights “as a journalist and a citizen” and her realization that technology could accelerate misinformation, distort truth, and blur the boundaries between the virtual and real world. “A lie told a million times becomes a fact,” she says. Ressa chronicles these experiences in her 2022 memoir and call-to-arms How to Stand up to a Dictator: The Fight for our Future. Ressa cautions about the dangers of and linkages between the weaponization of algorithmically driven disinformation — and illiberalism worldwide. “Without facts you can't have truth, and without truth you can't have trust. The only government that exists without trust is a dictatorship: you can't have journalism or democracy.” In her own work, she and Rappler are building upon the Matrix protocol, a secure, open-source decentralized platform that has the potential to become a global independent news distribution outlet. Although she is deeply concerned — “I feel like Cassandra and Sisyphus combined,” she says – Ressa also maintains her faith in the power of people to come together for change. “It's all about community,” she explains. “We are standing on the rubble of the world that was; we need to take responsibility for the world we want. We can build a world that is more just, more equitable, more sustainable; we can do this if we decide to come together, to demand better.” Thanks for Listening!
