The blood-brain barrier (BBB), formed by brain endothelial cells, pericytes, and astrocytes, is organized into a neurovascular unit that regulates the exchange of proteins between blood circulation and brain parenchyma. Human stem-cell-based models using brain endothelial cells are a powerful tool to investigate how disease-related conditions might affect the blood-brain barrier integrity. However, the cell type composition is critical to faithfully model transcytosis across the blood-brain barrier. In this episode of Curing with Sound, we explore a groundbreaking approach to one of medicine's toughest cancers, glioblastoma (GBM), with Michael Canney, PhD, Chief Scientific Officer at Carthera. GBM is the most aggressive form of brain cancer, notoriously hard to treat because the blood-brain barrier (BBB) blocks many life-saving drugs from reaching tumor cells. With survival rates of just one to two years, new approaches are urgently needed. Dr. Canney shares how Carthera's SonoCloud-9, an implantable, therapeutic ultrasound device, temporarily opens the BBB and enhances the delivery of circulating drugs to the brain. He also discusses the SONOBIRD clinical trial, comparing the use of Carthera's SonoCloud-9 device combined with chemotherapy to standard-of-care therapies in patients with recurrent GBM. Discussion highlights: Phase III SONOBIRD Trial: The largest clinical trial in Carthera's history, enrolling 560 patients across the United States and Europe. De campagne gaat richting eindstreep. Hoe zetten de partijen hun lijsttrekkers in? Welke thema's en welke uitnodigingen naar anderen vallen op? Wie zit klem en wie krijgt onverwachte kansen? Jaap Jansen en PG Kroeger verdiepen zich in het positiespel van VVD, D66, GroenLinks-PvdA en CDA. Een zelden vertoonde politieke strijd. Niet vaak was er een campagne waarin de zittende premier politiek zó afwezig en gecastreerd was, waardoor verrassende kandidaten voor die functie zich kunnen aandienen. *** Deze aflevering is mede mogelijk gemaakt met donaties van luisteraars die we hiervoor hartelijk danken. Dilan Yesilgöz moet de kiezers de Schoof-coalitie doen vergeten. Het beleid kan zij zo niet verdedigen en de wens van een centrumrechts kabinet komt niet uit de verf. CDA en D66 laten zich niet tot willoze vazal maken. En hun geheugen is actiever dan het liberale. Door de PVV én GroenLinks-PvdA uit te sluiten maakt de VVD meteen elke partij die met de linkse fusiepartij de dialoog aan wil besmet. Erg aanlokkelijk of 'gaaf' maakt de VVD de stembusgang zo niet. Het optimisme en de vrolijkheid van Mark Rutte lijkt geheel verdwenen. D66 kan zijn geluk niet op. Rob Jetten lijkt van de Rutte-toverdrank te hebben gedronken. De strategische bewegingen van VVD, GroenLinks-PvdA en Volt hebben zijn speelveld fors vergroot. Met het CDA bezet D66 een groeiend deel van het middenveld, waarbij zij uitstralen elkaar dat te gunnen. Dat positivisme is een scherp contrast met de oude Schoofcoalitie waar de VVD aan vastgekleefd blijft. Toch moet Jetten oppassen. In het laatste RTL-debat maakte zijn vrolijkheid hem kwetsbaar voor de scherpte en ervaring van Frans Timmermans. Er ontstond een moment dat de allure had van Jan Peter Balkenende tegen Wouter Bos in 2006 en Rutte tegen Emile Roemer in 2012. En ook nu over de zorgkosten! Dat liep toen lelijk af voor die twee. Tot nu toe stagneert GroenLinks-PvdA. Het Den Uyl successcenario van 1977 wil maar niet opdoemen. Van Timmermans’ eigen sterke thema’s wordt geen gebruik gemaakt. Europa, klimaat en energie en hoe dit in de toekomst van onze economie doorwerkt, ze blijven ongenoemd, terwijl hij hier ongeëvenaard staatsman zou kunnen zijn. Ook lijkt GroenLinks-PvdA de kiezer nauwelijks te willen herinneren aan de wanprestaties onder Schoof. Een zekere oubolligheid zit Timmermans ook nog dwars. Onbedoeld vergroot hij daarmee het culturele en mentale contrast met de twee concurrenten voor het premierschap Bontenbal (42) en Jetten (38) die een generatie jonger zijn. Het CDA is net een zombie die nu voor de derde keer in vijftig jaar herrijst, nadat - zoals Henri Bontenbal zelf zegt - de vraag was 'wie bij de partij het licht uit zou doen'. Hij valt de Schoofcoalitie wél aan op 'gepruts' en 'chaos'. VVD en BBB hebben het daar heel lastig mee. Bontenbal belooft onbekommerd saaiheid, zelfs in gesprekken met internationale media. Ruttes oproep 'normaal doen' en Lubbers' Rotterdamse doenerigheid weet hij te combineren. In de debatten blijkt kwetsbaarheid van zijn onervarenheid op het grote toneel. In het recente RTL-debat bleek hij niettemin een razendsnelle leerling. Een zelfspot over zijn kansen doet daarbij goed. Geert Wilders’ PVV staat buitenspel en de meeste kleine partijen slagen er niet in met inhoud aandacht en respect te winnen. En soms vallen zij op door onderlinge heibel. En hoe gaat de partij van Keijzer dat voor elkaar krijgen?   Vladimir Kara-Murza was samen met Aleksej Navalny de belangrijkste criticus van de Russische president Poetin. Kara-Murza werd twee keer vergiftigd en werd in 2022 gearresteerd en veroordeeld tot 25 jaar strafkamp. Ruim twee jaar zat hij gevangen in Siberië. Vorig jaar kwam hij vrij na een gevangenenruil. We spraken hem eerder deze week over Rusland, oppositievoeren vanuit het buitenland, autocratie en over hoe verandering in Rusland altijd onverwacht komt.   En tweeënhalf jaar geleden bracht Ilja Leonard Pfeijffer het boek Alkibiades uit, dat gaat over het verval van de democratie in het oude Athene. Het moest een waarschuwing zijn, want Pfeijffer zag parallellen met het heden. Binnenkort komt de schrijver met de essaybundel Absolute Macht. Hoe staat onze democratie er nu voor volgens Pfeijffer?

Vorige week escaleerde een demonstratie tegen een asielzoekerscentrum in Doetinchem, waarbij politieagenten werden bekogeld met vuurwerk, stenen en eieren. Burgemeester Mark Boumans noemde het ‘terrorisme' en ‘rechts-extremisme' en waarschuwt dat ‘Den Haag gemeenten in de steek laat.' Hoe blijft het lokale bestuur overeind in tijden van polarisatie? En de Sociaal-Economische Raad (SER) bestaat 75 jaar. Reden voor een feestje of is het vooruitzicht somber? Polderen vraagt om rust en compromissen. De tijd lijkt daar momenteel allesbehalve geschikt voor. De polarisatie groeit, het vertrouwen in de politiek is gekelderd en de daadkracht lijkt flink onder druk te staan.
