The blood-brain barrier (BBB), while essential for protecting the brain from toxins, has long been one of the greatest obstacles in treating brain diseases, particularly aggressive cancers like glioblastoma. Most chemotherapy drugs simply cannot reach the brain in effective concentrations, leaving patients with limited treatment options and poor outcomes.  In this episode of Curing with Sound, we speak with Graeme Woodworth, MD, Chair of the Department of Neurosurgery at the University of Maryland School of Medicine, whose work is transforming the landscape of brain tumor treatment through the use of focused ultrasound–mediated BBB opening. Dr. Woodworth discusses the role of microbubbles, his efforts to develop a real-time monitoring and dosing strategy for BBB opening, and the exciting results from his multicenter glioblastoma clinical trial. Discussion highlights: Glioblastoma Clinical Trial: Results published in Lancet Oncology report, for the first time, a possible survival benefit among newly diagnosed glioblastoma (GBM) patients treated with focused ultrasound + temozolomide (or TMZ, a chemotherapy drug). Researchers used focused ultrasound to noninvasively open the BBB in GBM patients before administering TMZ. When compared with a matched control group, a 40% increase in overall survival was observed. Published Findings for Measuring and Predicting BBB Opening: Researchers established a real-time, ultrasound-based “dose” measurement—using acoustic emissions from microbubble oscillations—to accurately predict when focused ultrasound will open the blood-brain barrier in glioblastoma patients. They discovered a non-linear dose–response range where blood-brain barrier opening is maximized, enabling safer, more precise, and more effective treatment monitoring. Le 2 octobre, le gouvernement d' « union des droites » de Giorgia Meloni a approuvé les dernières prévisions financières pour fin 2025 : le déficit serait ramené autour de 3% du PIB, ouvrant la porte à une sortie de la procédure pour déficit excessif engagée par la Commission européenne. L'Italie est devenue la quatrième puissance exportatrice mondiale se classant derrière l'Allemagne, elle devance désormais la Corée du Sud. En 2014, elle était classée au septième rang.Ces résultats ne doivent pas occulter une dette italienne restée à des niveaux très préoccupants (140% du PIB). De plus, le redressement financier de l'Italie est facilité par les effets du plan de relance européen décidé durant la crise sanitaire de 2020-2022, particulièrement généreux pour Rome (avec 194 milliards d'euros perçus, le pays est le premier bénéficiaire du dispositif). Une dette souveraine qui n'a pas empêché le 19 septembre l'agence de notation Fitch d'en relever la note à BBB+, quelques jours après avoir abaissé celle de la France à A+, donnant l'impression que les courbes, entre les deux pays, étaient destinées à se croiser, le redressement italien ne faisant que mettre en lumière, par contraste, l'affaiblissement des positions de la France.L'économie italienne conserve de grandes fragilités structurelles, notamment une productivité atone, une croissance en berne et une crise démographique qui ne cesse de s'aggraver, notamment par le départ à l'étranger des Italiens diplômés. Le pays connait une productivité du travail faible, estimée par l'OCDE à 65,60 € par heure en 2024, soit beaucoup moins que la France (90,86€). Deuxième puissance manufacturière d'Europe, l'Italie arrive seulement quatorzième dans le classement des pays membres les plus innovants en 2025, selon la Commission, tandis que le climat social se détériore. Le recul du chômage de 7,8% en 2022 à 6% en 2024 cache une augmentation du nombre de travailleurs pauvres et un temps partiel contraint qui frappe d'abord les femmes. Dans ces circonstances, le nombre de familles en situation de pauvreté absolue se maintient au-dessus de 8 millions, quand le taux de population à risque de pauvreté est de 23,1% en 2024. De plus, une étude d'octobre 2024 menée par plusieurs universités analysant les données de la Banque centrale européenne a mis en évidence le caractère profondément inégalitaire du système fiscal italien. Les principaux avantages fiscaux y seraient ainsi concentrés entre les mains des 7% des plus riches, dont le taux moyen d'imposition se révèle inférieur à celui des classes moyennes moins favorisées.En dépit de ces faiblesses, 39% des Italiens, selon le baromètre publié en février 2025 par le Centre de recherches politiques de Sciences Po, déclarent faire confiance à la politique, contre 26% des Français. L'Italie, il est vrai, connait depuis trois ans, contrairement à la France, une stabilité gouvernementale inédite.LE PLAN DE PAIX DE TRUMP POUR L'UKRAINELe 18 novembre, un plan de paix en Ukraine, détaillé en 28 points a sidéré les Ukrainiens et les Européens, tant cette esquisse inacceptable à leurs yeux était alignée sur les positions russes. Il stipule notamment que la Crimée (annexée par la Russie en 2014) ainsi que Louhansk et Donetsk seront « reconnues comme russes de facto, y compris par les Etats-Unis ». Il évoque aussi la création d'une « zone tampon démilitarisée » et neutre dans la partie de l'oblast de Donetsk contrôlée par l'Ukraine, qui serait « internationalement reconnue comme territoire appartenant à la Fédération de Russie », mais où les forces russes n'entreraient pas. Il y est énoncé encore que l'Ukraine devrait renoncer à son projet d'adhérer à l'OTAN et réduire à 600.000 soldats ses forces armées. Il ouvre la voie à une amnistie générale, y compris pour les crimes de guerre dont est accusé le dirigeant russe, Vladimir Poutine, ainsi qu'à une levée progressive des sanctions occidentales, voire à un retour de la Russie dans le G8.Les négociations entreprises dimanche à Genève, entre les conseillers à la sécurité nationale américains, ukrainiens et européens, ont permis de remodeler le plan Trump et de le nettoyer de ses aspects les plus problématiques. Sur les 28 points initiaux, il n'en restait plus que 19 lundi. Certains paragraphes ont été tout simplement rayés, comme la suggestion de réintégrer la Russie dans le G8, ou d'offrir la possibilité pour les Etats-Unis d'utiliser les actifs russes gelés, majoritairement détenus dans des pays européens, afin de financer l'effort de reconstruction. Les mentions concernant directement les intérêts européens, comme les sanctions, la perspective que l'Ukraine adhère à l'Union européenne ou qu'elle n'intègre jamais l'OTAN ont été remises à plus tard. Ce nouveau plan en 19 points n'est pas définitif. Divers ajustements et précisions doivent encore être négociées dans les coulisses, avant d'être soumis à la Russie. Mais la copie paraît désormais acceptable à la fois aux Ukrainiens, aux Américains et aux Européens. Il n'est plus question pour Kyiv de renoncer aux territoires conquis par la Russie, ni d'établir une zone tampon sur son sol mais de proposer un armistice sur la ligne de front actuelle qui serait gelée. Il n'est plus envisagé de réduire par deux la taille de l'armée ukrainienne. « Beaucoup de choses justes ont été prises en compte » à Genève, a observé Volodymyr Zelensky, lundi, indiquant qu'il négociera lui-même avec Donald Trump les points les plus sensibles laissés de côté, lors d'une rencontre dont la date est incertaine. Terwijl de formerende partijen onder informateur Buma nog steeds druk bezig zijn, gebeurde er deze week ook nog van alles in en om de Tweede Kamer. Joost en Marleen nemen de week door en vertellen wat hen daarbij opviel. Zo is er opnieuw een senator van BBB die overstapt naar D66, een landbouwdebat waar stoelen moesten worden bijgezet en een hoop te doen rondom de publieke omroep.  Er was ook nog tijd om langs te gaan bij een minister in ‘Binnen zonder kloppen', waar onder andere werd gesproken over de voorbeeldfunctie die de landelijke politiek heeft als het gaat om wat men zoal zegt. Extra actueel in een week dat steeds meer lokale besturen laten weten onder druk te staan door onduidelijkheid over de spreidingswet. Burgemeesters geven aan dat zij steun uit Den Haag missen. Daar wordt ondertussen door minister Van Weel haast gemaakt met ‘het strengste asielbeleid ooit'. Gaat dat de formatie helpen?

"May God be gracious to us and bless us and make his face shine on us." "God our God blesses us. May God bless us still." I'm sure that they aren't the only ones who use the phrase, but a family that used to attend Calvary, the Norwood's are the first ones who I heard use it, --BBB--  "Blessed Beyond Belief." It's not a bad phrase to carry around in our hearts as we make our way through the Christmas season, as we shop and sing and bake and eat...and eat...and eat...It's a good thing to be reminded that in so many ways that don't require a credit card or a receipt, we have been blessed beyond belief.  In this episode and the next I just want to give us a pause for gratitude!

Marnix van Rij heeft een hele loopbaan in politiek en bestuur achter de rug. Raadslid, wethouder, CDA-partijvoorzitter, Eerste-Kamerlid, nog een keer partijvoorzitter ad interim, staatssecretaris fiscaliteit en Belastingdienst in Rutte IV en nu plaatsvervangend bewindvoerder bij het IMF in Washington. Zijn nieuwe boek Reflecties van een politieke optimist gaat over de voorbije vijf jaar en de vele perikelen die hij in die partij en dat kabinet van zeer dichtbij meemaakte. Zend ons een mailtje en wij zoeken contact. *** Begin 2021 viel hij in als CDA-voorzitter nadat Rutger Ploum de verantwoordelijkheid voor de missers in de verkiezingscampagne op zich had genomen. Een verlies van 19 naar 15 zetels bleek nog het minste pijnpunt. Van Rij ontdekte dat de partijtop en de organisatie overladen waren met een reeks conflicten, frustraties en pijnpunten. Het leiderschap van Wopke Hoekstra was aangeslagen, bewindslieden communiceerden niet meer met elkaar, Mona Keijzer werd door Mark Rutte zelfs ontslagen. De achterban roerde zich, niet in het minst de mensen en afdelingen die zich vooral thuis voelden bij Pieter Omtzigt. Maar die was overspannen afgehaakt, hield hof in Enschede en richtte uiteindelijk een eigen partij op. In retrospectief ziet Van Rij dat Omtzigt zijn partij gijzelde door vaag te blijven over zijn intenties, zijn mate van loyaliteit en de behoefte om zijn gekwetste gemoed te laten strelen. Daarbij merkte hij dat Omtzigt niet was wie hij meende te zijn. Bij hem was sprake was van een radicalisering waarin hij ontkoppeld raakte van de christendemocratie. Ook Van Rij slaagde er niet in een dialoog zonder wantrouwen te laten ontstaan - ook al omdat Omtzigt zich ondanks zijn slechte toestand in hoge mate bleef mengen in het Haagse en interne CDA-debat. Toen uit zijn kring van medestanders een geheim evaluatiedocument gelekt werd, was zijn rol uitgespeeld, ondanks alle pogingen hem 'bij de club te houden'. Terugblikkend stelt Van Rij dat het vertrek van Omtzigt – en ook van Mona Keijzer, die naar BBB overstapte – uiteindelijk bevrijdend werkte. Een nieuwe generatie met een eigentijds wereldbeeld kon het CDA nu nieuw perspectief geven. Terwijl hij in Rutte IV als staatssecretaris zijn handen vol had, werd hij in de kabinetsformatie van 2023-2024 juist door Omtzigt en diens NSC intensief geraadpleegd. Geert Wilders polste hem daarop als informateur, om Ronald Plasterk op te volgen. Van Rij vond Plasterks werkwijze onzuiver en de uitkomst tamelijk funest. Niettemin werd hij door NSC alvast gepolst of hij niet zou willen aanblijven op Financiën, gelet op de grote problemen daar en de afhandeling van de toeslagenellende. Hij zei niet meteen nee, zo blijkt. In de volgende fase van de kabinetsformatie tussen PVV, VVD, NSC en BBB werd het nog gekker. Omtzigt nodigde Van Rij uit voor een gesprek en vertelde ‘dat dit een dag zou worden die ik nooit zou vergeten’. Want nadat Plasterk zich wegens een integriteitskwestie onmogelijk had gemaakt, zou Wilders hem gaan polsen voor het premierschap. Hoewel de nieuwe CDA-leider Henri Bontenbal helder had gesteld geen CDA'ers in het nieuwe kabinet te willen zien, ging Van Rij toch praten. Het gesprek met formateur Richard van Zwol en de vier fractieleiders duurde een uur. Duidelijk werd dat een premier van buiten de coalitie meer een etalagepop zou worden dan een leider. Het hoofdlijnenakkoord was bovendien zo anti-Europees dat Van Rij daar toch wel buikpijn van kreeg. En: “Als democraat vond ik dat Wilders zélf premier had moeten worden." Toen werd het Dick Schoof, de laatste in het rijtje potentiële premiers. Voor de huidige kabinetsformatie komt Van Rij met nuttige wenken. En uit zijn werk in Rutte IV heeft hij twee cruciale agendapunten waar een nieuw kabinet direct 'meters kan maken'. Het saneren van ondoelmatige fiscale regelingen is helder in kaart gebracht. Hier is een ruimte van tientallen miljarden belastinggeld die veel effectiever ingezet kunnen worden. Als wezenlijke les uit de enquêtes en rapportages over het falen van overheidsinstanties zou een nieuw kabinet een Nederlandse versie van het Amerikaanse 'Handvest voor de rechten van belastingbetalers en toeslagengerechtigden' kunnen vaststellen. Burgers en uitvoeringsorganisaties weten dan wat zulke rechten zijn en hoe burgers met respect en helderheid bejegend moeten worden. Depuis le mois dernier, le pays n'est plus considéré à risque en matière de blanchiment d'argent. Le Gafi (Groupe d'action financière) l'organisme international de lutte contre ces pratiques a sorti Pretoria de sa fameuse liste « grise » au nom de ses réformes pour plus de transparence.  Une bonne nouvelle n'arrivant jamais seule, vendredi 14 novembre, c'est l'une des trois grandes agences de notation S&P qui a relevé d'un cran la note du pays. Une première en 20 ans. Concrètement la note de crédit à long terme — sorte de note de confiance" sur la qualité de la dette du pays — est passée de "BB-" à "B.B". Pour donner une idée, c'est un peu comme si l'Afrique du Sud passait de 8 à 9/20. Certes, ce n'est pas encore la moyenne, mais c'est un signal fort, un gage de confiance pour les investisseurs. Et cela devrait permettre à Pretoria des taux d'intérêts moins élevés et donc d'emprunter moins cher sur les marchés.  Redresser les finances publiques  La confiance des investisseurs s'explique en partie par les efforts de la coalition au pouvoir pour redresser les finances publiques. Non seulement l'Afrique du Sud collecte plus d'argent via l'impôt, mais elle le collecte aussi mieux. Le budget présenté la dernière semaine prévoit une hausse des recettes. Autre point positif : le plan de libéralisation de l'économie lancé par le président Cyril Ramaphosa qui vise encourager les partenariats publics/privés dans des secteurs clés: les trains, les ports ou encore l'énergie. L'agence de notation S&P cite une réussite. Le cas de la compagnie d'électricité publique Eskom qui a enregistré le mois dernier son premier bénéfice net en huit ans. Les coupures de courant sont moins fréquentes dans le pays, ce qui pénalise moins les entreprises et favorise la croissance.  Pays le plus inégalitaire au monde Après une décennie de croissance atone, les prévisions ont été revues à la hausse. Le PIB pourrait progresser de près de 2% en moyenne sur les trois prochaines années (1,8% en moyenne sur les trois prochaines années selon les prévisions du Trésor) Et l'inflation, reste maitrisée. La banque centrale table sur une hausse des prix de 3% en moyenne, contre à 3% et 6% auparavant.  Pourtant, tout n'est pas rose. Le principal défi de Pretoria reste son taux de chômage qui dépasse les 30% Et c'est encore pire chez les jeunes : un sur deux est sans travail. L'Afrique du Sud ne créé pas assez d'emplois par rapport à sa population, et les inégalités se creusent.
