Kathryn Solie is widely known for her work as a "poisonous" plant educator. She feels that her current mission is to foster a greater cultural understanding of certain aspects of plant consciousness and, ultimately, to help build a community of unique practitioners who engage in dialogue, learn from, and support one another, both in their commonalities and differences.She believes that together, we can recover the most ancient human lineage—our relationship to the wisdom of the plant kingdom.Kathryn studied painting at Naropa University, a Buddhist-inspired school in Boulder, Colorado, completed a sacred plant medicine apprenticeship with the Gaia School of Healing in Putney, Vermont, and underwent training in Hakomi, a mindfulness-based, body-centered psychotherapy. We are proud to bring to you Paige Von Hess. She is the daughter of the legendary Kurt Von Hess. Paige has helped keep the memory of her dad alive and well within the wrestling community over the years. She's also a lifetime member at the annual CAC reunion and attends other wrestling events. The Von Hess name was one of the most feared by the fans because they knew Kurt was always a threat to their favorite wrestlers. He had many tag partners and held many championships during his career which was cut short due to illness. Why do I want an air-fryer so badly? And why won't my aging loved one ask for help when they need it? How can these things possibly be connected? And maybe most importantly, why are we talking about this on a podcast? At least once or twice a week we talk to someone who made a promise to a loved one that they can't keep. “I promised my dad I would never put him in a home,” for example. They had the best of intentions to keep their promise, but they didn't have all of the facts before making a commitment like that. Aside from legitimate constraints, like finances, that may make keeping promises like that impossible, home may not actually be what your loved one wants. In many circumstances people are not thriving at home and that is not what they are clinging to, instead it's the feeling of comfort and safety that being home represents for them. And change is hard at any age. In this episode, Alyson and Michelle discuss ways to successfully get your loved one's buy in to participate in planning for their safety in the future. Three key takeaways from this episode: We are all in a developmental phase, the purpose for which is a search for independence, control and meaning. Our aging loved ones are no exception. Air fryers are cool. The best way to get our loved one's buy in is to listen to them and make sure they feel that they are being heard. Even if their expectations are unrealistic, success really comes best from understanding exactly what they envision as possible for themselves. David Solie, in Unlocking the Communication Code of Seniors, states that developmental stages in life are characterized by sets of oppositional tasks that need to be completed so the individual can move on to the next stage. Their identification and impact on the development in old age has only recently been understood. When we fail to recognize the need to maintain control and identify legacy, Solie states, "Good advice may be rejected in favor of illogical or shortsighted choices because from a developmental perspective the need for control is greater than the need for medical, financial, or social correctness." In this episode of Long-Winded, the guys sit down with Braidy Solie, a former classmate of Cyrus' at NDSU who is now a practicing physical therapist. Braidy received his undergraduate degree in Exercise Science from North Dakota State University, and his Doctor of Physical Therapy degree from the University of North Dakota. Following the completion of his education, Braidy has specialized in working with athletes, and more specifically on injury prevention and the rehabilitation of athletic injuries pertaining to the knee and ankle. In this episode, we discuss how to best approach receiving care in physical therapy, how Braidy's education has influenced his own health and training, and the very overlooked benefits/underutilization of eccentric contractions in many people's training regimens. SHOW NOTES: Journal Article 1: "Diagnosis of Pulmonary Embolism with d-Dimer Adjusted to Clinical Probability" PEs and DVTs can be elusive. There are rising numbers of chest pulmonary CTAs being done with lower yields. These can result in increased cost and health risks. In this study, the adjusted d-dimer was looked at to see if the number of CTs being ordered can be reduced. The Wells criteria was used to place patients into low, moderate or high clinical pretest probability for venous thromboembolism of VTE. In the podcast, "clinical pretest probability" is referred to as risk. Of the entire 2000 patients enrolled, the diagnosis of VTEW was only made in 7%. Participants that qualified as low risk numbered 1742 and 1200 of these had d-dimer less than 1000. No VTWE was found in these patients for the next 90 days. For those with a d-dimer between 500 and 999, none had a VTE at 90 days. In moderate-risk groups with d-dimer less than 500, none had VTE at 90 days. Combining low-risk patients with a d-dimer less than 1000, non of these patients had evidence of VTE at 90 days. Even in the 467 patients with a d-dimer greater than 1000, only 87 had a VTE. Moderate- or high-risk patients are not applicable for this study. According to the article, if the d-dimer is greater than 1000, and the patient is low-risk, there was a 20% incidence of VTE. While it is an impressive study, it is one peice of data and should not replace clinical gestalt and decision making when truly concerned about the presence of VTE. Journal Article 2: "Bedtime Hypertension Treatment Improves Cardiovascular Risk Reduction: The Hygia Chronotherapy Trial" HTN is difficult to manage in many patients. This was a large study out of Spain of approximately twenty thousand patients. Patients were selected to take their medication either in the AM or nighttime and 48-hour blood pressure monitoring was performed. Patients were followed for 6 years. Night time dosed patients had significantly lower cardiovascular event rates than the daytime group, as well as better blood pressure management. There is little evidence to not advise nighttime antihypertensive dosing for patients, unless there would be compliance concerns. Medications that would not be tolerated, or specific medications, like diuretics, that can disrupt sleep. This was an impressive study that demonstrates a rather simple maneuver to effect a remarkable change in cardiovascular risk. Bear in mind, diet and lifestyle may also contribute to the results, but those were not assessed in this study. Journal Article 3: "Early or Delayed Cardioversion in Recent-Onset Atrial Fibrillation" A-Fib is a common presentation in primary care practice and in Emergency Departments. In this study of early cardioversion strategy vs delayed, 437 patients, aged 18 and above, were reviewed. Necessary criteria included A-fib bit less than 36-hours and hemodynamic stability. The conclusion was that neither strategy delayed or early cardioversion was an inferior approach. A large number of patients in this study spontaneously converted to normal sinus rhythm without demonstrating higher rates of stroke. However, this study was not powered to assess risk of long-term stroke, and this remains unknown. Though based on other studies referenced today, it's known that a patient cardioverted after 12-hours of A-fib has an increased risk of stroke. Psychologically, being in A-fib can be disturbing for the individual, and remaining in A-fib is not always desirable from the patient perspective. There are also potential logistical and cost considerations with delayed approach including numerous repeat clinics and ER visits for a small number of patients. Journal Article 4: "Short-term Use of Oral Corticosteroids and Related Harms Among Adults in the United States: Population-based Cohort Study" An impressive review of three hundred thousand patients was performed. Corticosteroids were given for mostly musculoskeletal, respiratory and allergic issues. Sepsis, VTE and fracture were monitored for over a 90-day time period and statistically significant higher rates of all of these were noted. Bear in mind, this was a study without true placebo, and patients essentially compared their experience on steroids to their experience not on steroids. It should probably be followed up with a prospective trial to help further validate these concerning findings. Still, this study only looked at 3 different complications and the numbers here are pretty striking, with 205 of adults receiving steroids. There are a number of studies which have shown no evidence of benefit in the use of steroids for a variety of indications, including conditions, such as urticaria and even anaphylaxis. Journal Article 5: "No Association Between MRI Changes In The Lumbar Sone and Intensity of Pain, Quality of Life, Depressive and Anxiety Symptoms in Patients With Low Back Pain" In this study, out of Poland, patients were referred for a lumbar spine MRI by neurologists, surgeons or other specialists, but not by primary care. These MRIs were graded in the study based on criteria derived from the reading radiologists. The endpoint of the study was to compare the severity of MRI findings with the patient's self-assessment and scoring of pain, quality of life, etc. The study ultimately showed there was no correlation. However, age and BMI, and total MRI scores did correlate. Physically active patients had better scores. Learning new ways of coping with pain and helping our patients with this reality can equal a more efficient use of time and money. Per this study, medications or a reassuring MRI, does not correlate to resolution of pain. Of course, MRIs are often indicated in the setting of significant neurologic findings and emergencies, but outside of those settings, some patients may not be convinced that an MRI is not necessary. Using articles like this one can assist to better counsel patients and reduce unnecessary MRIs. Journal Article 6: "Contrast Associated Acute Kidney Injury Is A Myth: Yes" IV contract is often blamed for acute kidney injury, or AKI. It turns out, like many time honored beliefs in medicine, this is not likely the case. While attempting to research and write a paper on this subject, the investigators quickly discovered that ample data already exists that shows CIN or contrast induced nephropathy, appears to be more a myth than truth. One senior author of this paper demonstrated in a pool of thousands of patients in two other separate studies that there's no association between contrast and AKI. Another investigator who is a cardiologist demonstrated actually less incidence of AKI in a cohort of patients. Hello and welcome! Today, I am so excited and honored to be interviewing the amazing Kathryn Solie. Kathryn does a lot of amazing different work, but primarily she teaches some meditation classes and workshops on poisonous plants and plant medicines. An Interview with Kathryn Solie: What poisonous plants can teach us about healing. Interview transcript: Kathryn Solie: Thank you for having me here! Ashley: Oh, it is my pleasure and my honor. I have really been getting into this journey with plants and plant medicines and I actually signed up for your Poisonous Plant Medicine, Class 1 and Class 2. And I know that you are going to be starting a new round of that very soon. I was wondering if you could introduce yourself to everyone? Tell them a little bit about your story, and how you got started on this path, and what you are up to now? Kathryn: That is always an interesting question to ask because it is hard to know what exactly brought me here. There has been so many different things, but essentially, I work with plant medicine. If I am just talking to a regular person, I just say, I am an herbalist. Probably, not a lot of regular people are listening, so I can kind of go into a little bit more detail in the way that I work with them. I work a lot with plant consciousness, or what a lot of people call plant spirit medicine. It's really similar to the way that you work with crystals in that they have this energetic medicine. I have been really pulled towards poisonous plants. Or I kind of want to do air quotes as poisonous, because it is the dose that makes the poison. Everything is poisonous, even water can kill you if you have a big enough dose. Those plants have just really called me in the past few years. And their medicine is really needed on the planet at this time. They help bring us into the shadows and to our deeper psyches and to our unconscious into places where we are discouraged from exploring. Our current society only focuses on being happy. Be happy all the time. And it is great if we can be happy all the time, but there is also likely an undercurrent underneath that has all our life stories and moons and fears and that is always going to be there. That is normal and healthy and part of being human. I feel like the poisonous plants can help guide us through those spaces that might be normally kind of uncomfortable. Ashley: I feel so grateful that you are bringing this up. In my personal journey with these medicines, this is exactly when they showed up in my life. When I really started to consciously dig into that shadow aspect. I found that I needed some support, and this was what came through. For me, this was what my body, my mind, my spirit all needed. To kind of do this work on multiple levels and find that energetic support and that medicine for this, really deep dive inner journey, personal healing, transformative, shadow aspect work. Was it something similar for you personally that drew you in? Or did this just evolve naturally in your exploration of herbalism? Kathryn: I think that it was always kind of just part of my makeup as a person, just even as a little kid I was always kind of, not, I would never use the word morbid, I feel like other people might use that word, but just kind of interested in the aspects of life that people try to keep hidden. Even for many years before I really fully stepped on the plant path, I was really on a path of meditation and non-duality and kind of eastern philosophy in eastern religion. I still am on that path, but a big part of that path for me was working with the archetypes of the wrathful deities. In Hinduism and Buddhism you get Kali, which is symbolic of transformation and of going into these difficult times. Or if you have worked with this astrology -- Saturn, Pluto -- that kind of energy. I feel like that kind of aspect of life has always felt natural and comfortable to me,
