Alta DeRoo, MD, MBA, FACOG, DFASAM, joins host, Zach, to talk about her experience in the US Navy (USN) and her path to practicing OBGYN and addiction medicine. She discusses the intersection of general medicine or primary specialty practice and addiction and shares her perspectives on stigma related to addiction. Dr. DeRoo explains the potential challenges of transitioning from active duty to veteran status and how these can sometimes lead to substance use, addiction, or mental health issues. Finally, she shares what inspired (and continues to inspire) her sense of service and offers advice to the next generation of addiction medicine specialists. Dr. DeRoo's father was active-duty Navy when she was born on Midway Island in the South Pacific. A sense of service was branded in her character, and after earning a BA in psychology from Connecticut College, she joined the Navy and served for 24 years. She was selected for combat Naval Aviation and was among the first cohort of women to fly in combat. Dr. DeRoo was the first female in her community of the E2C Hawkeye, a carrier airborne early warning plane. (They have a short cameo in Top Gun, but you would not see Mav flying her plane.) After 8 years of aviation and over 40 combat missions, Dr. DeRoo was awarded a scholarship to attend medical school. After graduating from the University of Florida College of Medicine, she completed residency in OBGYN. She completed several missions, including Guantanamo Bay, Cuba, and 5 deployments on the USNS Mercy performing surgeries in Singapore, Vietnam, and the Philippines, and she was the mission commander of a Papua, New Guinea mission. Dr. DeRoo became very aware of the alcohol and drugs that sailors and soldiers used on deployment (for various reasons). Her father was also one of these sailors who eventually developed alcohol use disorder. From his stories and the stories of her shipmates and battle buddies, she became very interested in the science of addiction. Her Commanding Officer at the time allowed her to moonlight at a nearby methadone clinic, which was the beginning of her career in addiction medicine. Dr. DeRoo later became board certified in addiction medicine and practiced both OBGYN and addiction medicine. She retired from the USN to Culpeper, VA, where she started the county's first ever OBOT with their Community Services Board, started treating pregnant women with SUD, and incorporated this into a clinic in her town. Through aggressive naloxone distribution and OUD treatment, they were able to decrease their opioid overdose rate between 2016-2018. It was during this time Dr. DeRoo also became interested in the Virginia Chapter of ASAM. She served as secretary for two years, then president elect in 2020. The only thing that drew her away from serving as president in 2020 was a job opportunity that, to her, was a calling. She was asked to be the medical director of the Betty Ford Center and all California sites. She made dramatic changes there and was promoted to chief medical officer of the Hazelden Betty Ford Foundation and their 16 sites across the US. If she arrives according to the schedule, she could pass Astoria around 7:30am Friday.  She's headed to the shipyard in Portland at Swan Island. STORY 2: Gwinnett will see major representation shakeups after judge upholds new congressional, legislative maps A federal judge upheld the redrawn district maps for Gwinnett County, splitting it between four congressional districts and adding an extra seat to the county's state House delegation. The new maps were created during a special session after the previous ones were found to violate the Voting Rights Act. The changes include the dismantling of coalition districts, affecting Gwinnett's representation. Notably, state Reps. Sam Park and Gregg Kennard are now in the same district, and the District 105 seat, represented by Democrat Farooq Mughal, becomes more Republican-friendly. Critics, including Democrats, argue that the new maps preserve Republican majority, but the judge deemed them legal. STORY 3: McBath will run for her old 6th Congressional District seat after judge upholds redistricting maps U.S. Rep. Lucy McBath will no longer represent part of Gwinnett County after a federal judge upheld the recent redrawn congressional and state legislative maps. McBath, who moved to the Gwinnett-centric 7th congressional district after the 6th district was redrawn in 2021 to favor Republicans, announced she will run for her old 6th district seat in 2024. The judge ruled that the redrawn maps met the standards set and ordered by him, addressing concerns about the dilution of Black voters' strength. Gwinnett County is now split between four congressional districts, impacting the representation and political landscape in the region. Draper, a Navy veteran of 14 years, serves as the security officer responsible for antiterrorism and force protection. The mission focuses on disaster response preparedness and capacity-building while enhancing partnerships with participating nations. The USNS Mercy will visit the Republic of the Marshall Islands, Solomon Islands, Palau, and the Federated States of Micronesia, conducting civic-action projects, medical symposiums, and disaster response training activities. STORY 5: Osprey crashes could endanger funding for new tilt-rotor Army aircraft Bell is developing Fort Worth-based Bell Textron, contracted to develop the Army's next long-range assault aircraft, faces potential challenges due to recent fatal crashes of its V-22 Osprey. The V-280 Valor, Bell's tilt-rotor design, was chosen by the Army to replace the UH-60 Black Hawk. Concerns about safety and the award process have been raised, especially after a Nov. 29 Osprey crash. Despite opposition, Bell's contract stands, and the company continues progress, including new facilities. Congressional concerns and the Osprey's safety history may impact the Future Long-Range Assault Aircraft program, but experts suggest the investigation's results will guide decisions. Doctor Explains Combining cardio and resistance training in one session is beneficial for overall health, reducing cardiovascular and metabolic risks. Research indicates that the order of aerobic and resistance exercises doesn't significantly impact fitness improvements. While consistency is crucial, personal preferences can guide the order of exercises. Concurrent training, involving both forms of exercise, yields generalized benefits in aerobic capacity and muscular strength. This approach is particularly advantageous for individuals with chronic disease risk factors. With 38.4 million Americans having diabetes and nearly 70% being overweight or obese, blending exercises contributes to improved health outcomes. STORY 7: Health Alert: ADHD Meds in Childhood Do Not Increase Future Drug Use Risk. Doctor Explains Children treated with prescription stimulants for ADHD do not face a higher risk of substance use disorder (SUD) later in life, according to a study involving rigorous diagnosis and long-term assessment until an average age of 25. This finding dispels concerns about ADHD medication's long-term effects. Recognizing nontraditional ADHD symptoms, such as sleep issues and irritability, is crucial. Prior to coming to PAU she was a professor of psychology at John Jay College of Criminal Justice, City University of New York (CUNY) for 16 years, during which time she was instrumental in the development of a new doctoral program in clinical psychology with an emphasis in forensic psychology and served as the program's first Director of Clinical Training. Prior to her time at CUNY, she was on the psychology and law faculty at the University of Alabama. In 2009, Dr. Zapf founded CONCEPT Professional Training with the mission of elevating the level of practice in psychology and related professions. In 2018, Dr. Zapf brought CONCEPT Professional Training to Palo Alto University to further its mission of continuing and professional studies in partnership with Palo Alto University. Dr. Zapf is a past President of the American Psychology-Law Society (AP-LS; Division 41 of the American Psychological Association). In 2006, in recognition of her outstanding contributions to the science and profession of forensic psychology, she was named a Fellow of the American Psychological Association (APA) and a Distinguished Member of the American Psychology-Law Society (AP-LS). His leadership roles include Department Head of the Embedded Mental Health Department and Mental Health Advisor to the Commander. Additionally, he serves as faculty in the Tripler Army Medical Center Clinical Psychology Postdoctoral Residency Program, supervising an interservice clinical rotation serving surface warfighters. In 2022, LT Grant was selected by the Office of the Chief of Naval Operations' 21st Century Sailor Office as a Fellow in the International Career Advancement Program.  LT Grant completed his first tour at Naval Medical Readiness and Training Command (NMRTC) San Diego from 2019 to 2021 where he served as the Division Officer of the Mental Health Unit of the Marine Corps Recruit Depot, San Diego. In addition to this role, he co-led the base's Psychological Health Committee. He additionally served as the chair of the Grand Rounds Committee for NMRTC San Diego's Directorate of Mental Health and co-chair of the Naval Medical Forces Pacific Transgender Care Team. In the fall of 2019, LT Grant deployed to the USS ABRAHAM LINCOLN during its time in the Fifth Fleet. He served as the lead for a Mental Health Augmentation Team, which spent the final three months of the ship's record-breaking deployment supporting the crew. In the spring of 2020, he deployed again, this time serving as lead mental health clinician aboard the USNS MERCY during its defense support of civilian authorities mission to provide hospital relief to Los Angeles in response to COVID-19.  LT Grant was commissioned into the US Navy via the Navy Psychology Licensed Direct Accession program in 2019. He is a native of Southern California, and prior to joining the Navy, he worked as a Clinical Psychology Subject Matter Expert at the Defense Health Agency's Psychological Health Center of Excellence and as a clinical researcher in the Department of Medical and Clinical Psychology at the Uniformed Services University of Health Sciences supporting research on military suicide prevention.  Capt Deonte Williams is a Clinical Psychologist at Aviano Air Base Italy. In 2021, he received his doctorate in Clinical Psychology from Palo Alto University in Palo Alto, CA. He completed his pre-doctoral clinical residency at Wilford Hall Ambulatory Surgical Center (WHASC), Joint Base San Antonio-Lackland, Texas, and is a previous recipient of the United States Air Force Health Professions Scholarship Program in 2018.   Capt Williams specializes in trauma treatment, and has an added focus on treatment focused on couples/intimate relationships. His clinical experiences similarly include working with adults with a wide range of clinical presentations, including neuropsychological assessment, mood disorders, and behavioral-health-related conditions. His graduate-level clinical experiences included advanced practicums within the Palo Alto VA Healthcare System, where he focused on outpatient mental health, neuropsychological evaluation, inpatient care, and clinical research on opioid use within the veteran population. His primary research interests include personality factors and habits related to romantic relationship outcomes in young adults as well as the impact of stress/anxiety on human performance in stressful environments. Additionally, he hosts a weekly radio podcast titled the DNA of Love with Dr. Williams, where he uses humor and anecdotal personal experience to highlight research-informed aspects of healthy dating behaviors in hopes of helping listeners find the one they're meant to be with. In addition, she spent 3 years in Yokosuka, Japan, wherein, she worked with US military families. There, she initiated a pilot program with her therapy dog, Bear. He was a resounding success with the service members and the staff and the first, ever, for any Fleet and Family Support Center, in the world. A native of San Diego,  Yvette draws on her wide array of experience and contacts within the local culture of the city. Today we salute Hospital Corpsman 2nd Class Benjamin Ong of the United States Navy. Benjamin hails from Bremerton and right now he's deployed aboard the hospital ship USNS Mercy in Los Angeles Harbor. Mercy and her crew of just about 1000 Sailors are on the front lines of the battle against Covid-19. Corpsman like Ong are trained in a wide range of medical treatment and techniques, and the Mercy is better equipped than most land based hospitals. With Sailors and other military personnel assisting and supplementing civilian medical personnel, we will win this battle. I spoke incorrectly referencing the USNS MERCY. That hospital ship was enroute to Los Angeles. The hospital ship berthing in New York was the USNS COMFORT. Here is more information about the history of the USNS COMFORT. Laura mentioned turning off the water when brushing teeth. I found this statistic about turning off the faucet: two gallons of water flow from a faucet each minute. Something as simple as turning off the water while brushing your teeth can save up to three or four gallons of water per person per day. That’s more than 1,000 gallons of water per year or enough for almost 100 four-minute showers. We had a brief confused conversation about numerous authors. Iwas referencing Malcolm Gladwell’s Revisionist History and it’s episodes on higher education. I found this article. What followed was a list of other authors and books while I tried to explain Gladwell to Laura. These included Tim Ferris, author of The 4 Hour Work Week and then Simon Sinek, author of Start with Why which I incorrectly cited as The Power of Why and graduate of Northern Valley Regional High School like Laura and myself Finally Laura and I agreed she was referencing Eckhart Tolle, the author of Power of Now. Laura was surprised that the Bowery Ballroom was listing events rescheduled from May 18 forward. As the COVID-19 pandemic is a evolving situation, here is a link to the Bowery Ballroom website and calendar of events. Laura used the story of DJ D-Nice and his evolving free virtual dance floor parties “Club Quarantine” as an example of the power of online communities and how people need relief from the stress of the pandemic. U.S. Navy CDR JANETTE M. ARENCIBIA Health Security Cooperation, Readiness & Training Officer,  ENTOMOLOGY Center of Excellence, Jacksonville Naval Air Station Global Health Professional, Plans, Operations and Medical Intelligence Officer Janette Arencibia is a 1988 graduate of Mercer County High School in Harrodsburg, Kentucky. She graduated from Eastern Kentucky University in 1995 and in 1996 from the EKU MPA program where she was a Patricia Roberts Harris Fellow and Presidential Internship Management Candidate. She is a graduate of the U.S. Navy War College where she earned a Masters in National Security and Strategic Studies. LCDR Arencibia is, as well, a graduate of the Marine Corps Command and Staff, Marine Corps University and is currently enrolled in the Uniformed Services University Global Health Engagement program. In support of Operation Enduring Freedom, she was the first Medical Service Corps officer to deploy from a training command reporting as Chief, Joint Visitor’s Bureau with BCT 41 - Task Force Phoenix V to Afghanistan. Arencibia became one of the first female officers to instruct security and weapons training for females in the Afghan National Army ranks. She well represented the U.S. Navy among Army ranks earning the Keith L. Ware Award for her photography and additional written submissions throughout the course of her assignment in Afghanistan. She was awarded by the Afghan National Army for training and instruction provided and is also an honorable member of The Order of St. Barbara, a military honor society of the U.S. for both the U.S. Army and the U.S. Marine Corps Artillery, including Field Artillery and Air Defense Artillery as a result of her involvement in artillery field training. Arencibia’s initial sea tour was aboard USNS MERCY where she was a medical planner and patient regulator managing over 200,000 casualties during OPERATION UNIFIED RESPONSE –HAITI. Immediately following her redeployment from Haiti, she deployed onboard the USS IWO JIMA (CP 10) as the Lead Medical Planner. While serving aboard the IWO JIMA, she liaised with Central and South American embassies on behalf of FST-2. As the Plans, Operations and Medical Intelligence DUINS awardee, she reported to the Joint Staff Surgeon’s office in 2012. During her Joint Staff assignment, she was the Project Manager for the Joint Medical Planning Tool; now the required verification and validation analytical tool for the Joint Forces. She was requested to extend at the Joint Staff Surgeon’s office then accepted the Lead Medical Planner Position at NORAD-USNORTHCOM in 2013 where she served until 2016. Arencibia transferred to Marine Forces Central Command where she served as the Lead Medical Planner for exercises EAGER LION and NATIVE FURY and is currently the USCENTCOM Theater Security Cooperation Manager for the Hashemite Kingdom of Jordan. Her shore assignments include the Navy School of Health Sciences Security Manager, NNMC-Bethesda Officer in Charge of Contingency Operations, BUMED M3 Readiness Officer, and Medical Officer Recruiter.  Currently, CDR. Arencibia coordinates Global Health Initiatives, Force Readiness, Force Protection and Theater Security Operation specific to building Global Medical Force multiplier capacity. She is a Joint Qualified Officer. After 14 years studying and working in nursing, one of Lynette's most memorable career experiences was traveling aboard the USNS Mercy hospital ship to treat pediatric patients in the little-known country in the middle of the Pacific Ocean, Timor Leste. This first job led her to pursue a master's degree at the University of San Diego as a Family Nurse Practitioner, leading her to meet her husband. Their first baby boy arrived a couple years after practicing full-time as a Board-Certified Family Nurse Practitioner. While managing patients with advanced heart failure, heart transplants, and mechanical cardiac support, Lynette had her own change of heart.Feeling she was missing too many precious moments of her son's childhood, she backed off her corporate healthcare pursuits to have more control over her schedule and co-founded The Mama Ladder™. CDR JANETTE M. ARENCIBIA Global Health Specialist Plans, Operations and Medical Intelligence Officer MARINE FORCES CENTRAL COMMAND Janette Arencibia is a 1988 graduate of Mercer County High School in Harrodsburg, Kentucky. She graduated from Eastern Kentucky University in 1995 and in 1996 from the EKU MPA program where she was a Patricia Roberts Harris Fellow and Presidential Internship Management Candidate. She is a graduate of the U.S. Navy War College where she earned a Masters in National Security and Strategic Studies. LCDR Arencibia is, as well, a graduate of the Marine Corps Command and Staff, Marine Corps University and is currently enrolled in the Uniformed Services University Global Health Engagement program. In support of Operation Enduring Freedom, she was the first Medical Service Corps officer to deploy from a training command reporting as Chief, Joint Visitor’s Bureau with BCT 41 - Task Force Phoenix V to Afghanistan. Arencibia became one of the first female officers to instruct security and weapons training for females in the Afghan National Army ranks. She well represented the U.S. Navy among Army ranks earning the Keith L. Ware Award for her photography and additional written submissions throughout the course of her assignment in Afghanistan. She was awarded by the Afghan National Army for training and instruction provided and is also an honorable member of The Order of St. Barbara, a military honor society of the U.S. for both the U.S. Army and the U.S. Marine Corps Artillery, including Field Artillery and Air Defense Artillery as a result of her involvement in artillery field training. Arencibia’s initial sea tour was aboard USNS MERCY where she was a medical planner and patient regulator managing over 200,000 casualties during OPERATION UNIFIED RESPONSE –HAITI. Immediately following her redeployment from Haiti, she deployed onboard the USS IWO JIMA (CP 10) as the Lead Medical Planner. While serving aboard the IWO JIMA, she liaised with Central and South American embassies on behalf of FST-2. As the Plans, Operations and Medical Intelligence DUINS awardee, she reported to the Joint Staff Surgeon’s office in 2012. During her Joint Staff assignment, she was the Project Manager for the Joint Medical Planning Tool; now the required verification and validation analytical tool for the Joint Forces. She was requested to extend at the Joint Staff Surgeon’s office then accepted the Lead Medical Planner Position at NORAD-USNORTHCOM in 2013 where she served until 2016. Arencibia transferred to Marine Forces Central Command where she served as the Lead Medical Planner for exercises EAGER LION and NATIVE FURY and is currently the USCENTCOM Theater Security Cooperation Manager for the Hashemite Kingdom of Jordan. Her shore assignments include the Navy School of Health Sciences Security Manager, NNMC-Bethesda Officer in Charge of Contingency Operations, BUMED M3 Readiness Officer, and Medical Officer Recruiter. LCDR Arencibia donned the Commander rank in September 2018.

Andy Barker is coming up on a nearly 30 year career in the U.S. Navy and is trained as an Emergency Room Registered Nurse. He has seen, done and heard about more things than many people could think of. He's been stationed in Japan, he was on the USNS Mercy during the first Gulf War, Global War on Terror and Operation Enduring Freedom Philippines, and there he's responsible for a large team that report directly to him, and he will go to bat for anyone on his team.  Andy Barker is a family man who has and always will be there for his family. He has been by his father's side during illness and seen him making a recovery, up to the point of just before we recorded this podcast episode. He is based in Virginia but we spoke while he was in St. Louis with his parents after his father returned home from hospital. April is Sexual Assault Awareness Month, and the Department of Defense is highlighting its campaign to end sexual assault in the military. Secretary of Defense Leon Panetta traveled to Brussels, Belgium, with Secretary of State Hillary Clinton for joint ministerial meetings with NATO allies and International Security Assistance Force partners. The meetings precede this summer's NATO summit in Chicago. The secretary and Joint Chiefs Chairman Gen. Martin Dempsey testified before the House Armed Services Committee regarding the ongoing situation in Syria. President Barack Obama announced April 16 that Army Spc. 4 Leslie H. Sabo Jr. will posthumously receive the Medal of Honor. Sabo was killed during an ambush after shielding an injured soldier from a grenade blast and destroying an enemy bunker. The explosion that destroyed the bunker also killed Sabo. He had been nominated by his unit for the award, but his paperwork had been lost until recently. The U.S. and a broad coalition of nations and non-governmental organizations are prepating for Pacific Partnership 2012, a humanitarian and medical exercise set to begin in May. The hospital ship USNS Mercy will deliver in-country aid to Indonesia, Vietnam, Cambodia and the Philippines. Gen. Martin Dempsey, the Chairman of the Joint Chiefs of Staff, spoke on a Sesame Workshop panel to discuss the "Talk, Listen, Connect: Deployments, Homecomings and Changes" campaign and other collaborations between the Sesame Workshop and Department of Defense to help military children cope with the unique stresses of military family life.
