The Biden administration, which endlessly professes its commitment to democracy, has been supporting its ideological allies' efforts to block the sorts of governmental checks-and-balances foundational to our own constitutional Republic. It supports those who want the left-dominated Israeli Supreme Court effectively to make elected officials irrelevant, and would clearly welcome their toppling of Benjamin Netanyahu's government. Such interference in the internal affairs of our most important regional ally is not only reprehensible. Declining Sperm Count: Much More Than You Wanted To Know

Levine et al 2017 looks at 185 studies of 42935 men between 1973 and 2011, and concludes that average sperm count declined from 99 million sperm/ml at the beginning of the period to 47 million today. Levine et al 2022 expands the previous analysis to 223 studies and 57,168 men, including research from the developing world. It finds about the same thing. Source: Figure 3 here The “et al” includes Dr. Shanna Swan, a professor of public health who has taken the results public in the ominously-named Count Down: How Our Modern World Is Altering Male and Female Reproductive Development, Threatening Sperm Counts, and Imperiling the Future of the Human Race. Is Declining Sperm Count Really "Imperiling The Future Of The Human Race”? Swan's point is that if sperm counts get too low, presumably it will be hard to have babies (though IVF should still work). How long do we have? This graph (source) shows pregnancy rate by sperm count per artificial insemination cycle. It seems to plateau around 30 million. An average ejaculation is 3 ml, so total sperm count is 3x sperm/ml. Since sperm/ml has gone down from 99 million to 47 million, total count has gone down from ~300 million to ~150 million. 150 million is still much more than 30 million, but sperm count seems to have a wide distribution, so it's possible that some of the bottom end of the distribution is being pushed over the line where it has fertility implications. But Willy Chertman has a long analysis of fertility trends here, and concludes that there's no sign of a biological decline. Either the sperm count distribution isn't wide enough to push a substantial number of people below the 30 million bar, or something else is wrong with the theory. Levine et al model the sperm decline as linear. If they're right, we have about 10 - 20 more years before the median reaches the plateau's edge where fertility decreases, and about 10 years after that before it reaches zero. Developing countries might have a little longer. It feels wrong to me to model this linearly, although I can't explain exactly why besides “it means sperm will reach precisely 0 in thirty years, which is surely false”. The authors don't seem to be too attached to linearity, saying that “Adding a quadratic or cubic function of year to meta-regression model did not substantially change the association between year and SC or improve the model fit”. People are bad at naming things When people ask what I've been working on and I say that I'm looking into declining global fertility rates, they sometimes think that I'm talking about rising rates of infertility. Which makes sense as an interpretation – fertility is a very overloaded term! But the way that I (and most economists and biologists) usually use the term fertility is to discuss the rate at which children are actually born, rather than their potential for being born. Infertility refers to the inability of a heterosexual couple “to achieve a pregnancy after 12 months or more of regular unprotected sexual intercourse". So if there were widespread infertility in some group, this group would have low fertility rates, but low fertility rates are not necessarily indicative of widespread infertility. Note that fertility in this sense is usually talked about on a population level, and there are different measures that try to capture different relevant features of it. Two popular measures of it are the total fertility rate and completed cohort fertility. See my discussion of those in my earlier piece here .The term that gets closest to the opposite of infertility is fecundity, the “physiological potential to bear children” or “the biologic capacity to reproduce”. Once I clarify that I'm working on studying the decline in the rate at which people are having children, I get the follow up question: but is some of the decline in global fertility rates due to a decline in fecundity and an increase in infertility? TL;DR: Decreasing fecundity? This is a tricky question to answer, because there isn't one marker that can be used to evaluate fecundity, and so we have limited information on whether physical ability to conceive and bear children has changed over time. I won't address all possible measures of reproductive health here, and will instead focus on sperm, because when the question comes up, people are often referring to the results of this paper: Temporal trends in sperm count: a systematic review and meta-regression analysis (Levine et al, 2017) and the subsequent book by one of the authors on the paper, “Count Down: How Our Modern World Is Threatening Sperm Counts, Altering Male and Female Reproductive Development, and Imperiling the Future of the Human Race”. The paper does not draw a connection between falling sperm counts and global fertility decline, but the book does, claiming that "the dramatic decline in sperm counts is an important factor in the fertility slump that's being seen in many parts of the world". My conclusion from reading the paper, skimming the book and looking through the related literature, is that: there is evidence of a decline in global sperm counts, but the data is less clear cut than is often implied Levine et al. meta-analysis finds decline, but has issues I will discuss the evidence for sperm count and quality declines is strongest among sperm donors and among those seeking fertility treatments there are very limited studies comparing different points in time that don't fall into one of those two groups, but the ones that I've been able to find all find no decline this potential decline in sperm count is unlikely to have been a substantial driver of the global fertility decline the Levine et al. paper finds a decline in sperm counts specifically among Western men, but the countries from which the Western sample was drawn already experienced most of their fertility decline by the start of the sample, at a time when, according to the paper, sperm counts were high evidence that age-adjusted rates of infertilit...

Along with the ecological devastation expanding industrialization has wrought on the global environment, another looming catastrophe has emerged in the past century that threatens the human species: rapid decline in fertility. The reproductive systems of females and males in industrialized, particularly Western, nations have seen a precipitous drop in the ability to conceive and bear children. From falling sperm counts and testosterone levels in males, and early menopause and cysts in the ovaries of females, the reproductive systems in human beings have dramatically changed for the worse over the past several decades. Dr. Swan has identified several of the main culprits in this ongoing trend, namely the absorption and ingestion of various endocrine-disrupting chemicals found in countless materials we interact with daily: phthalates from plastics and numerous hygiene products; Bisphenol A found in hard plastics and electronics; flame retardants in furniture; and pesticides in foods — just to name a few. It is speculative to extrapolate, but there is also no evidence that it is tapering off. This means that most couples may have to use assisted reproduction." Is the human race on the verge of extinction? That's the jaw-dropping claim made in Professor Shanna Swan's new book ‘Count Down: How Our Modern World Is Threatening Sperm Counts, Altering Male and Female Reproductive Development, and Imperiling the Future of the Human Race'. According to the book, sperm counts have dropped almost 60% since 1973 and suggests that they could reach zero by 2045, which would mean no more reproduction and no more babies.This is a worrying discovery, particularly as one of the central drivers of this trend is modernity itself; Swann argues that chemicals, ranging from ATM receipts to Tupperware plastic, in the modern environment are altering —and endangering — human sexual development, and is getting worse by the year.Even if you find this argument a little too extreme (as UnHerd columnist Tom Chivers does), Prof Swan is certainly worth listening to. Having devoted over 20 years of her career to the study of sperm, the epidemiologist is about as well-credentialed as they come and has made a fascinating contribution to the debate. We thank Prof Swan for her time and hope you enjoy the discussion. Key quotes below:On plummeting sperm count:What we found was that sperm count had to climb dramatically over the preceding 40 years, and was at a point where nearly half of men would be entering that range of sperm count, which is associated with sub-fertility at least. We didn't see any indication that the slope of that line had levelled off, so that when we looked at the data restricting it to the past 30 years, 20 years, 10 years, you might hope that it would be flattening out. But we didn't see any indication of that, which is alarming, because if it were to continue on its present course — that's a difficult thing to project — but just mathematically, if you've extended the line, it does hit zero in 2045. So that's the median sperm count, that means half of men would have no sperm. On declining population figures:It is true that the being below replacement, the value of 2.1 children per couple is more common in Western countries. And in Asian countries, that's where it's most severe. The lowest point I believe, has been reached at 1.0 children in Korea, just recently. But, that's the rate of decline. Since 1960, of the most populous countries has been greatest, they are also declining, and declining faster. So it's going on everywhere and it's a problem. On phthalates (a group of chemicals):Phthalates are interesting because they have the ability to lower testosterone. They're called anti-androgens: they lower the androgens and testosterone is one of those. And so if the pregnant woman is exposed to phthalates in early pregnancy, these phthalates can reduce foetal testosterone at a critical time for male genital sexual development. This was first shown in rodents, and it was so striking that it was named the phthalate syndrome. On where these chemicals are found:These chemicals affect the body's hormones. Phthalates affect androgens, and bisphenols, which makes plastic hard and line tin cans — they are oestrogenic, which also affects reproductive function. There are other chemicals; for instance, we showed in our study that men with exposure higher levels of pesticides, in mid Missouri had much poor semen quality. Other chemicals set in flame retardants in your Teflon pan, in lots of things in your home, your wall coverings and floor coverings. And food is a really major source. On transitioning:It feels like there's an increase, but we don't have that data. So in the past it could have been there and completely unrecognised and un-talked about. What we do say is that disorders of sexual development, the actual alterations in the genitals — those are caused by environmental chemicals. And there's many examples of that in rodents, fish, and frogs. So many species have, for example, ovaries and testicles in the same organism, right? So there's no question that chemicals can change and make these developments atypical, but what the animals would prefer, what gender they would choose, if they could — how would we know that? On how to limit chemical exposure:You could choose products that are stored in glass. And I would recommend that you not store them in your home in plastics.
